I used AI, specifically Gemini Plus and Super Grok, to create this image from an OSHA Case File, and I was impressed. Now the image is going to be shocking, but this is a very good depiction of what was described in the case file. And I’ll bet a burger and a beer that this was not the first time this stunt had been pulled off. Unfortunately, this stunt claimed the life of one (1) worker, 45, and seriously injured two other workers, 58 and 66..

The 250-gallon LPG tank was inverted and suspended from the service truck boom so that the valve and filter could be used to pump vapor into the tank and push liquid from the tank to the bobtail. The pressure relief valve vented shortly after pumping started, releasing liquid from the tank. The liquid vaporized and exploded upon contact with an ignition source. Employee #1 was killed as a result of the explosion, and Employee #2 was hospitalized with burns on multiple parts of their body. Employee #3 was treated and released on the same day for a sprained finger and fractured heel bone that occurred when Employee #3 jumped a fence to escape the explosion and fire.
