Story of Interest HAZCOM LABELING FATALITY Beer Store fined in washer fluid death (a worker died after drinking windshield washer fluid in a bottle mislabeled as liquor – two workers washing delivery trucks at the distribution center found a liquor bottle filled with blue liquid – the workers took turns drinking from the bottle – which held methanol windshield washing fluid used in the trucks – with one of them then taking the container home and finishing it off – that worker later died from methanol poisoning) INDUSTRIAL FATAL EXPLOSION Worker killed in blast at W.Va. gas well pad (a worker was killed in an explosion at a natural gas well pad – the victim was working alone at the time – he was attempting to transfer briny wastewater from a tank into a truck – what sparked the explosion is unclear and will be the focus of the state’s investigation – the victim was an employee of a gas company contractor) LOTO/GUARDING Wheeling worker hospitalized after accident (a worker was flown by helicopter to a Hospital after his hand was caught in a machine at 8:09 p.m. – a worker at the factory, described as a man in approximately his early 30s, was working with an industrial roller, when his hand became entangled in the machinery – rescuers worked extrication equipment and the plant employees to disassemble the machine and detach the hand from the equipment – the entire extrication operation took approximately an hour) STRUCK by TRAIN FATALITY Man killed in accident at Cleveland steel plant of ArcelorMittal (an employee died in an accident at the steel plant – he reportedly fell off of a railroad car and into the path of an oncoming train where he sustained fatal injuries) ORGANIC PEROXIDE CHEMICAL CONTAINER EXPLODES AT MELBOURNE FACTORY (eight people have been treated for exposure to chemicals and fumes after an explosion at a steel recycling factory – a container of organic peroxide exploded at about 7.30 p.m. – it blew a hole in the roof of the storage container – a large plume of chemicals drifted onto about eight workers at the plant – the victims appeared fine but were decontaminated and taken to hospital for observation) CHLORINE Possible chlorine gas leak reported at Detroit wastewater treatment plant (plant workers were evacuated from the area as the fire department’s HAZMAT team responded to the incident at 9 a.m. – a worker was changing out equipment when he noticed the leak – no injuries have been reported) TANK LINER FIRE Fire Breaks Out At Columbia Chemical Plant (a fire at a chemical plant was brought under control quickly and is not expected to pose any health threats – the fire started when the outer lining or insulated shell of an outdoor tank caught fire – the 1,000-gallon tank, which stores a chemical used in paint mixtures, was not ruptured – the fire was brought under control in less than 30 minutes – there were no injuries or evacuations at the plant or in the area – fire marshal is investigating) AMMONIA Ammonia Leak Prompts Plant Evacuation (employees at a beef processing plant were evacuated from the building and sent home after an ammonia leak – some employees were taken to a nearby hospital for evaluations – it was unclear how many employees were being evaluated – no injuries were reported) CHLORINE 4 people treated and released, cleanup underway after chlorine spill at Bermco Aluminum causes evacuation (a chlorine spill caused an evacuation of the building – four people were treated and released but were not transported – the spill was reported at about 9:40 a.m. – a vendor was making a weekly delivery of liquid chlorine used for cleaning purposes when the spill occurred) CONSTRUCTION FATAL FALL from CRANE MELBOURNE CRANE OPERATOR KILLED IN FALL (a crane operator, 59, has been killed in a fall at a construction site – he is believed to have fallen 35 metres from a crane platform around 7am – several workers reportedly went to his aid but were unable to resuscitate him) PFAS SAVES ANOTHER LIFE! Two construction accidents hospitalize workers (a construction worker was taken to the hospital as part of the the second construction accident in the city – two injured construction workers were transported to the hospitals from separate building sites after falling at their respective locations – a worker slipped off a beam at a construction site at 11:30 a.m, however his harness prevented him from hitting the ground – another man fell about five feet onto a roofing area at a construction site at approximately 10 a.m. – neither injury was life threatening) FATAL CRANE COLLAPSE One killed, 3 injured in accident at SAIL’s Bhilai plant (a worker was killed and two others were injured in a mishap at a Steel Plant when an iron beam, being lifted by a crane in the under-construction coke oven battery 11, collapsed – one worker standing on the ground was killed, and three others injured – the injured were taken to a hospital where the condition of one of them was reported to be critical) FALL from LADDER Construction worker rescued after falling into 20-foot hole (a construction worker is recovering after he fell 20 feet down a hole – the worker was climbing down a ladder when he fell into an estimated 20-foot deep hole – he was unable to climb up the ladder on his own and rescue crews had to lift him out – the man is believed to have non-threatening injuries) MISCELLANEOUS HOTWORK Employee Accidentally Starts Fire At OKC Business (around 8:30 a.m. an employee accidentally started a fire in an office when he left a soldering gun on, and unattended – most of the damage was contained to the office, but other parts of the repair shop did sustain smoke and water damage – the business sustained about $30,000 in damage) |
