INDUSTRIAL MACHINERY FATALITY Worker dies following industrial accident in Swadlincote (a worker, 24, has died as a result of injuries sustained during an industrial accident – he was rushed to the hospital following the accident – he is believed to have suffered crushing injuries following an accident involving machinery) FATAL FALL @ MINE Miner who died in Alabama coal mine accident identified as Pleasant Grove man (a miner, 36, died in an accident at a coal mine – he was a supervisor at the mine, where he had worked for five years – he was working on or near a conveyor belt when the accident happened – he died when he fell from a walkway onto equipment that was in operation) CONFINED SPACE FATALITIES – One entrant and 4 would-be-rescuers 5 Dead in E China Factory Accident (five workers died and another was injured in a factory accident at 1:30 p.m. in a vinegar plant – a factory worker fell into a sewage tank while applying anti-rust paint to its rail – four others jumped in to try and save the worker, adding that they perished later after medical treatment failed to save them – another worker was injured in the accident but is in stable condition) JCB worker injured in Rocester factory accident (a man, in his 50s, was flown to hospital by air ambulance following the accident just before 2.30pm – his injuries are not thought to be life threatening – no other details) LINE BREAK and LOTO Two workers injured in accident at Tarapur industrial unit (two workers suffered 90 per cent burns when hot oil gushed out from a pipe – the incident occurred at a printing and packaging factory – incident occurred when the pipeline leak was being repaired – there is a boiler next to it to control the temperature of the oil in pipeline, which is about 200°C – the valve was not closed properly and the hot oil came gushing out – the three workers, 20, 28, and 50 were rushed to a local hospital) ELECTRICAL EXPLOSION in MANHOLE CL&P workers injured in explosion (three workers were injured in an explosion while working in a manhole shortly after 10 a.m., – the three workers have been taken to a Hospital – the exact extent of their injuries is unknown at this time, but company spokeswoman said none of the injuries are life threatening) FORKLIFT FATALITIES 2 workers killed in Dallas forklift accident (two workers have been killed in a forklift accident at a plastics company – the unmanned forklift rolled into the men – the victims were tending to a load at the back of a truck when they were crushed) HOT ASH from PRECIPITATOR Two die of burns at private power plant (two workers died and three others suffered severe burns after hot ash engulfed them at a private captive power plant – around 8am, more than 15 workers had gathered to open a precipitator chamber to remove the ash generated after production of iron sponge when the accident occurred – the moment it was opened, the chamber threw out hot ash that swept over five workers standing closest to it – workers, 23 and 28, died later of severe burns at a hospital, while three others are battling for life at a private hospital) LATHE Alberta machinist crushed while working lathe (a worker, 59, sustained life-threatening injuries after being crushed while operating a machinery – the incident took place at around 5 p.m. – he was operating a computer-controlled lathe the size of a truck when his upper torso was crushed between the lathe turret and a rear door – he was sent to hospital where he remains on life-support) HOTWORK @ POWER PLANT Fire Burns Xcel Power Plant’s Conveyor Belt System In Pueblo (firefighters say a blaze on a conveyor belt system at a coal-fired power plant caused an estimated $1 million in damage – the fire happened on a conveyor belt system that is about 180 feet above ground – it moves coal from trains to storage areas – according to initial reports, a subcontractor was doing welding work on the conveyor system when sparks ignited the conveyor belts, which had coal and coal residue on them – burning pieces of conveyor belt fell into a stack of coal that also caught fire – estimated damage at $1 million – no injuries were reported) CRANE FATALITY Man dies in accident at wood processing plant (worker, in his 20s, was working near a crane which was loading logs at the time when he was hit by the crane, causing fatal injuries) AMMONIA Firefighters called to ammonia leak in Winnebago County (shortly after 7 p.m., a truck struck and ruptured a pipe connected to a roughly 26,000-gallon tank containing anhydrous ammonia – the gas spewed into the air on the grounds releasing 8,000 pounds – no evacuations were ordered, and no injuries were reported – County Sheriff’s Department asked residents downwind (within one to two miles) to stay sheltered in place) AMMONIA PSA investigating accident at natural gas processing plant (a work accident injured two persons who were exposed to ammonia in connection with maintenance work on an ammonia tank valve – one of the persons was reported to have been sprayed with ammonia, while both were exposed to ammonia vapors – they were both sent to a hospital) NITRIC ACID Chemical Accident In Los Alamitos Injures Two, Leaves Area Evacuated on Friday (a chemical spill at an industrial business has resulted in two people being sent to the hospital and several others being check for minor injuries and the evacuation of a number of businesses – the incident took place at approximately 1:37 when a 55 gallon drum containing nitric acid over pressurized and began off gassing into the air – the situation was “stabilized” at approximately 3:15 p.m.) CONSTRUCTION WORKZONE FATALITY from CONCRETE CUTTER Identity of worker killed in I-29 construction zone accident released (a worker, 58, was killed in a construction zone accident on Interstate 29 – he was operating a concrete grinder about 8 a.m. on northbound I-29 – he became trapped under the front of the machine and died on scene) Injured painter rescued on Newburgh-Beacon Bridge (second accident in less than month halts work – work on a lead abatement and repainting job on a Bridge was halted after a worker was injured in the second accident there in less than a month – a worker suffered undisclosed injuries in the containment system, an enclosed area on the underside of the north span that keeps paint chips and other contaminants out of the river – a construction crane lowered rescuers in a large basket to the scene – rescuers loaded the worker on a stretcher and into the basket and the crane hauled them to the top of the bridge – injuries were serious enough that westbound traffic on the bridge was halted for 30 minutes so a helicopter could land on the roadway and transport the worker to a Medical Center – Thursday’s injury came just over two weeks after two workers on the job were left dangling over the river in their safety harnesses after temporary supports beneath them gave way on May 21 – those workers were able to climb to safety and were not injured) WORKZONE FATALITY Contract Worker Struck, Killed on I-81 (a contract worker, 40, died when he was hit by an 18-wheeler on I-81 – he died at the scene – his vehicle was parked on the side of the road, and he was cleaning up debris when he was hit) CRANE FATALITY Worker killed in crane accident on Martin Grove Road (a man in his 50s has been killed at an industrial accident involving a crane at 11:38 a.m. at a construction site – the man was unconscious, bleeding and was rushed to hospital without vital signs) ELECTRICAL and CRANE Two workers hurt in White Rock electrical accident (two electrical contractors have been rushed to hospital after injuring themselves in an electrical accident – the men were installing a green lamp standard using a mobile crane – the workers were guiding the standard into place when the crane contacted some high voltage power lines – witnesses said there was a huge flash and a bang, and the two workers were knocked to the ground – paramedics and the fire department responded quickly and treated the men on site before being airlifted to hospital – they were conscious and talking – their current condition is unknown) DEMOLITION FATALITIES 2 Salvation Army workers die in Pa. bldg collapse (two employees were among the six people killed when a building collapsed on a thrift store – the four-story building collapsed, trapping workers and others inside the thrift shop next door – the dead employees include a 35-year-old woman working her first day at the store and a 68-year-old immigrant – UPDATE: Crane Operator Had Drugs in His System After Building Collapse …) WORKZONE and BUCKET TRUCK ODOT worker toppled from truck (a Department of Transportation worker landed in the hospital after a fall from the elevated bucket of a “cherry picker” truck, but is expected to make a full recovery – the worker’s fall was caused in a traffic accident, when a Penske rental truck struck the ODOT truck shortly before 3 p.m. – DOT was doing maintenance work on the intersection’s traffic signal when the eastbound Penske truck struck the bucket truck – as he was working on the light, the lanes next to him were not blocked off – the bucket of the cherry picker was not in line with its vehicle, and vehicles had been passing under it, including a different Penske truck that cleared the bucket – the bucket was hanging over the lane of traffic – the worker was outside his safe zone – the worker, 29, fell 16 feet into the roadway – he was taken to a Medical Center with “incapacitating” injuries) FATAL FALLS Three construction workers plunge 150 feet to death (a collapsing metallic framework of an under-construction skyscraper sent four labourers plunging 150 feet to their deaths, while three of their coworkers were injured in the accident – the accident was caused by a displaced shuttering at an under-construction tower and led to at least seven labourers falling from a tremendous height – two workers, 30, died instantly when their bodies smashed against the metallic pipes on the ground below them, while a third labourer died at a Hospital – four other labourers were also injured in the mishap – all of them were being treated for moderate to serious head injuries and broken bones) FALL from ROOF Worker badly hurt in 30-foot fall at Gilbert work site any internal bleeding) FATAL FALL from BRIDGE Local construction worker dies after fall from bridge (a construction worker, 25, who fell from a bridge died from injuries at a hospital Saturday – he was injured when he fell 23 feet from the bridge – he was working on constructing a wooden platform when he fell and landed on rocks – because he was an organ donor, his kidney, heart and lungs were saved – “He’ll probably save five people’s lives,” the hospital spokeswoman said) FATAL FALL from ROOF Worker dies from Westport fall (a man, in his late 20s, who fell off a roof while working has died – he suffered severe head injuries while working on a home) FALL into HOLE Man rescued after fall into hole at Monroe construction site (firefighters were called to a construction site after a worker fell into a large hole – the accident happened when a worker stepped off or fell from a backhoe and went head first into the hole, which is 10-12 feet deep – the victim was transported to a Hospital with non-life-threatening injuries) UPDATE on HOTWORK FIRE State sues contractors for igniting Taylor Bridge wildfire (the state of Washington is suing to recover some of the millions of dollars it spent to fight a wildfire, started at the site of a bridge repair project – the fire destroyed 61 homes and burned 23,000 acres last August – the Department of Natural Resources is suing after a state investigation found that the fire was human-caused and that the contractors were negligent in their welding operation – the report concluded it was most likely that sparks from the welding ignited dry brush near the bridge – the state is suing for more than $5 million – dozens of landowners have also sued the contractors for negligence – the total cost of fighting the fire is estimated at more than $11 million) MISCELLANEOUS CRANE FATALITY Crane accident kills Canadian GP worker (a Formula One track worker has died after being run over by a crane removing a car from the course after the completion of the Canadian Grand Prix – he died after being taken by helicopter to a hospital – the man was walking beside a crane as it moved a car that had gone off the track during the race – the worker tripped and the fell in front of the crane’s wheel as he was escorting the vehicle – the crane operator couldn’t see the man and drove over him) EXPLOSION Laurel body shop explosion burns two workers, critically injures one (a worker at an auto body shop is in critical condition and suffered burns over nearly half his body after an explosion inside the facility – the suspected vapor explosion took place inside a body shop at about 8 a.m. – one victim, who worked at the shop, suffered second and third-degree burns over 50 percent of his body and was taken to a burn unit for treatment – a second blast victim also suffered less severe burn injuries – fire officials believe the explosion was caused by a buildup of vapors where cars are air-dried after being maintained – the facility’s sprinkler system doused a small fire that broke out after the blast – the explosion did about $20,000 in damage) REFRIGERANT Blast of refrigerant injures two at UF (two men working on a chiller unit were hospitalized after a pipe burst near where they were working, blasting them with refrigerant – one worker was treated and released while the other was expected to be released the following day – the accident occurred around 1:30 p.m. when a ball valve broke – the men, employees of a contractor hired to make repairs, both suffered skin burns and one inhaled the
