INDUSTRIAL FORKLIFT Deere Waterloo worker hurt in forklift accident (employee remains hospitalized after an accident – he was struck by a forklift while working on third shift – no other details) FATAL FALL from DRILLING RIG Worker dies in Alberta rig accident (employee, 32, died from injuries suffered in a fall on a drilling rig – he died of injuries he suffered in a fall on a drilling rig – operations at the rig, which was drilling a surface hole at the time of the accident, were immediately suspended) FATAL FALL @ DRILL RIG Rig worker dies in Kansas accident (one worker was killed and another injured at a drilling site – one man was crushed when a derrick basket fell – he was pronounced dead at the scene – the man who was in the basket at the time also sustained unspecified injuries and was flown to hospital for treatment) FORKLIFT FATALITY Factory worker killed by forklift (a factory worker has died from his injuries after being crushed by a fork lift truck – he is said to have become trapped beneath the forklift at the factory, which makes aluminum products and components, and died from his injuries at a Hospital) FORKLIFT FATALITY Steel plant worker crushed to death by steel roll at USP Structural Connectors (a steel plant worker, 25, was crushed to death when a large steel roll fell on him – he was standing on top of a forklift trying to unpack several of the rolls and cut a steel band that had been holding them together – loosening the band may have caused the roll of steel to spring forward, pinning him to the forklift) CRUSHING Man Pinned, Hurt in Industrial Accident (a worker, 35, was injured at 3:21 a.m. – he was pinned between a large piece of piping that rolled off a machine in the assembly line and material that was standing up, off the floor – accident was not witnessed by any other employee – once on scene the five-year employee was no longer stuck and was receiving care by an internal emergency response team – the accident happened in the building where steel lighting poles are made – he was taken to a Medical Center in critical condition – the more severe injuries appeared to be internal including possible broken ribs or damage to organs) ARC FLASH FATALITY Injured DVC worker dies (a labourer succumbed to his injuries – he was one among seven workers who were admitted to a Hospital – he had suffered 100 percent burns in the accident – seven persons, four contract workers and two engineers sustained burn injuries when an electrical flash occurred in a breaker – the other patients are out of danger) NITROGEN LEAK Chemical Leak at Intel Fab Injures a Dozen Workers (a chemical leak at a semiconductor manufacturing plant sent 12 employees to the hospital – 43 people in all were evaluated by medical personnel following the accident – the accident, reportedly a nitrogen gas leak, occurred at around 6 a.m. in Fab 32, one of three silicon fabrication plants – the leak was caused by a problem with “a single manufacturing tool” in Fab 32 – the tool “has been taken offline and operations at the plant have resumed as the company tries to determine the cause of the malfunction) CONFINED SPACE FATALITY – ONE ENTRANT – ATMOSPHERIC Son dies, father critical after exposure to tanker truck fumes (one worker died and another is in critical condition after losing consciousness and being rescued from inside an empty asphalt tanker truck – son, 24, dies, father, 60, critical after exposed to tanker fumes – his father is in critical condition after losing consciousness and being pulled from inside an empty asphalt tanker truck – a worker was inside the tanker when he was overcome with fumes, and another worker entered the tanker to help – the second worker also was overcome by fumes, and a third person called 911 – crews found two people unconscious inside the empty tanker and set up a “confined space” rescue -the father and son were helping a friend that drives a tanker that was parked at that location – the owner of the tanker thought that there had been an obstruction, possibly a bucket, inside the tanker that was causing it to drain slow – the men went down into the tanker to check on the obstruction when they were overtaken by fumes) STRUCK BY TRUCK FATALITY Man killed in accident at Romulus trucking company (a man, 40, was killed at a trucking depot after he was hit by a truck driven by a fellow employee – accident happened at 12:10 a.m. as the man was walking across the parking lot – a fellow employee driving a semi-trailer truck struck the man – alcohol and drugs are not suspected) LOTO/GUARDING FATALITY Man dies after being trapped in machine at Frankfort company (worker, 55, died after an accident at his workplace – he was trapped in a machine about 3 p.m. – when crews arrived on the scene he was out of the machine, apparently having been removed by co-workers – he was taken by helicopter to a Medical Center in critical condition and died at 4:17 p.m. – an autopsy scheduled will determine the cause of death, but preliminary information listed it as a crushing injury) OXYGEN CYLINDER EXPLODES in GARBAGE TRUCK Sanitation worker injured when oxygen tank exploded speaks out (two sanitation workers were injured when a discarded oxygen tank exploded in their truck – driver, 53, was hit in the face by the debris – he marked his 13-years on the job the same day of the explosion – co-worker was hit in his eyes by all the debris – his eyes were severely burned and his vision is blurry) UPDATE on ELECTRICAL ACCIDENT Burnt GPL worker loses another section of his arm (the employee who was burned while working on a utility pole last Tuesday, had another part of his left hand amputated – he is said to be in a stable condition – on Friday evening, a part of his hand was amputated – his mother had told News that her son would be greatly affected if his hand were amputated because he was a very active person) FATAL GRAIN DUST EXPLOSION Overheated motors blamed in deadly Union Mills silo blast (investigators say an overheated motor in a grain silo ignited dust and caused a blast that killed a worker at a business – investigators from ATF and the state Fire Marshal’s office found scorched motors in the basement and on the decking of the silo at the Co-op – both motors were experiencing drops in voltage before the explosion, and one or both appear to have overheated) 61-year-old Chrysler worker dies at Warren Truck Assembly Plant (a man, 61, working at the plant was found dead – he was found slumped over while working at a table about 10:30 a.m. – he was pronounced dead at the plant after he could not be revived – autopsy will be performed to determine the man’s cause of death – the man was performing a “weld-check process,” a type of quality control, at a table that rotates 180 degrees when he was found) FATAL TANK COLLAPSE Water tank kills worker, injures another in A’Ibom (a worker was killed while another was injured when a mini water tank under construction collapsed – the two workers were working on the water tank when it suddenly collapsed and killed one of them – the tank fell from the top and smashed the man upstairs before crashing to the ground) CHEMICAL REACTION Evacuation lifted after Plainwell chemical explosion, fire (an evacuation order has been lifted following a fire and explosion at a chemical processing business – it happened Monday afternoon and the fire rekindled overnight forcing more evacuations – evacuation was lifted Tuesday afternoon after air and water quality samples came back safe – evacuation impacted a nearby industrial park with about 20 businesses – the fire started during a routine chemical treatment process, which unexpectedly overheated – the company says it expects to reopen on July 1) UPDATE Williams Cos.: Explosion at Louisiana plant may have originated in fractionator area (company officials believe they know where the explosion originated at the company’s plant on June 13, but the cause has not been determined – the natural gas liquids plant remains closed and under investigation by federal agencies as crews develop repair plans and continue the expansion project that was halted after the explosion – the explosion may have originated in the natural gas liquids propylene fractionator area of the plant – the fractionator is where olefins are reported head injuries and another was reported to have an arm injury – two other men were treated for shock) HOTWORK Blaze sparked by welding equipment (welding equipment at a health care products plant sparked a fire at about 1:40 p.m. – fire crews were able to locate and extinguish a fire inside the facility – employees were evacuated, and no injuries were reported – no major property damage and the company was able to resume operations) HOTWORK w/ BURNS Welder injured when blow torch ignites fire on Boomtown Casino boat in Harvey (a welder was injured when a fire broke out in the underbelly of the Casino boat – he was taken to a hospital with second-degree burns on one of his legs – the boat’s sprinkler system extinguished the fire before firefighters responded – workers were repairing several water control valves when a hydraulic control line broke – sparks from a blow torch ignited the fluid) UPDATE on REFINERY TANK FIRE Preliminary report says spontaneous reaction to blame for fire at Marathon refinery in Detroit (a preliminary investigation says a spontaneous reaction is to blame for a fire in a tank at the refinery – a fire in a tank at the refinery forced the temporary evacuation of a number of area residents – no one was injured in the fire, which burned a tank that the company said held wastewater produced in refining processes – the ignition source was attributable to a spontaneous reaction of iron sulfide deposits with oxygen in the roof area of the tank – an investigation continues, but that the company has modified their process for managing oxygen content in tanks) REFINERY FIRE Fire breaks out at Slovnaft refinery, one person hospitalized (a fire occurred at the oil refinery – refinery started an ethylene unit, and there was a smaller fire during this start, but it was soon extinguished by refinery firemen – one man was examined at the hospital, but his condition did not require further hospitalization – he complained about problems with breathing, but examinations finally didn’t reveal intoxication by combustion gasses) SOLVENT TANK FIRE Explosion, fire at Voltaix started in chemical tank (no one was injured when an explosion and fire caused damage and a chemical leak at the plant – no evacuations were necessary – the fire started just after 10 p.m. in a 10,000-gallon tank, but it was contained to the tank – employees fought the flames with water until firefighters arrived – the fire was doused by 12:45 a.m., and fire crews were able to leave by 3:45 a.m. – the fire started after less than 1,000 gallons of excess solvent that is used to make those chemicals was put into a 10,000 storage tank – the tank, which is fully automated and controlled by employees working in a building nearly 200 feet away, is supposed to be airtight – air somehow got into the tank while the plant was using a new process to store the solvents – that caused an explosion and dislodged the lid of the tank, causing a fire – employees doused the flames and cooled the tank with water until firefighters arrived to extinguish the fire with foam – the damage was contained to the tank) AMMONIA FATALITY Auburn man dies after ammonia leak on fishing boat (a worker, 82, exposed to ammonia on a processor-tender ship tied up at a dock has died – he was flown to the hospital after the accident – he and his son co-owned the ship – he received “a full blast of ammonia” from a leak of the refrigerant in the hold of the vessel – his 20-year-old grandson also was on the boat and helped lead his father and grandfather away from the hazard) AMMONIA Gas leak at Auckland ice-cream factory (firefighters were called to deal with a gas leak at the ice cream factory – the leak set off a siren at the factory about 9pm, which was heard throughout much of the surrounding area – three fire engines were sent to shut off a valve that was leaking ammonia – they liaised with engineers on site, ventilated the area and left after about an hour – no one was hurt) CHLORINE Man hospitalized after chlorine leak on Leith ship (an engineer has been taken to hospital after inhaling chlorine on board a ship – the man in his mid-30s inhaled a mixture of chemicals – he was helped from the ship by fire crews and taken to an Infirmary after he inhaled the chlorine in the engine room while he was mixing chemicals to clean sewage pipes – crew are investigating how a cap on a chemical drum was removed, causing substances to leak) FORKLIFT FATALITY Man dies in Iowa forklift accident (a 28-year-old man was killed in a forklift accident at about 5 p.m. at a Feed and Grain business – the Sheriff’s Office didn’t release any details about how he was injured) CONSTRUCTION WORKZONE Accident on I-96 near Merriman in Livonia sends MDOT worker to hospital (a Department of Transportation worker has been released from the hospital after being injured on I-96 after an accident involving a vehicle and a cleanup crew – crews were working all week cleaning and sweeping the side of the freeway, which resulted in small, rolling lane closures – at about 2 p.m. a car driving eastbound on I-96 struck the arrow-board truck in the right lane, injuring one of the workers – the worker was taken to a nearby hospital and did not have life-threatening injuries) MANLIFT FATALITY Orange worker killed in Port of Port Arthur accident (a man, 40, was killed in an industrial accident at the Port – he was killed when he was struck by a piece of angle iron – a manlift became stuck against a silo while it was about 40 feet in the air – while they were trying to free to manlift, a 10-foot piece of angle iron was catapulted from the top of the manlift to the ground, striking the worker in the upper-body – witnesses at the scene say the victim was hit in the head – he was transported to a Hospital where he was pronounced dead) WORKZONE MDT worker hit by trailer in downtown Bozeman (a Department of Transportation worker was reportedly hit by a trailer – the worker was clipped by a truck hauling a trailer and taken to hospital with non-life threatening injuries at around 9:30 a.m.) FATAL FALLfrom LADDER Worker for Local Company Dies in Roof Mishap (a man died after falling 30 feet off a ladder at about 9:30 a.m. – the man suffered undisclosed injuries when he fell approximately 30 feet from the ladder, hitting] a deck railing before hitting the ground – the man later succumbed to his injuries at the hospital) WORKZONE FATALITY Man arrested in Northeast El Paso hit-and-run involving TxDOT worker (a 23-year-old man has been arrested in connection with a hit-and-run that killed a Department of Transportation worker – he was driving a black Chevrolet Camaro when he hit the worker, 41, at about 1:37 p.m. – at the time, the worker was outside of a work vehicle conducting road maintenance – he died as a result of the accident – shortly after the accident, the car was found and the driver was identified) THROWN from MANLIFT Worker Hurt in Navy Yard Fall (a construction worker, 27, is injured after falling from a cherry-picker lift while doing demolition work at a warehouse shortly after 11 a.m. – the machine apparently malfunctioned and threw the worker in the air – he landed at the foot of the machine – he’s been taken to a hospital after complaining of pain – his condition hasn’t been released – wasn’t immediately clear what caused the machine to malfunction) CRUSHED by COLLAPSED WALL Two killed in work accident (two construction workers, 40 and 55, died after they were crushed by a wall they were trying to repair – the accident took place around 1 pm when the two workers were repairing the four metre high by 15 metre long wall – a brother was also working on the site when the wall gave in but he escaped without injury – a fourth worker driving a bulldozer tried to warn the workers but only the brother had time to get away – the two victims died instantly while family members were distraught when they arrived at the scene to find the men buried beneath the rubble) WORKZONE FATALITY Construction worker killed in hit and run (a construction worker died following a hit and run on I-65 around 4:45 a.m. – a blue sedan struck and killed the construction worker – he was working on Interstate 65 on a repaving project was removing traffic control signs, and the last sign of the morning, and was struck by an automobile going northbound – the vehicle did not stop – he had been with the company approximately a year) FATAL LIGHTING STRIKE while climbing SCAFFOLDING Construction worker in Naples dies after being struck by lightning (a construction worker has died after being struck by lightning while he was climbing down scaffolding – he died at the scene – he and other construction workers working at a home saw bad weather approaching and decided to seek shelter – that’s when he was struck) GAS EXPLOSION Worker injured in Caringbah gas explosion (a man, 44, has been taken to hospital with serious burns after an explosion about 11am – a worker from a gas company suffered second degree burns – during the course of work, natural gas has ignited and there was a small explosion – he was taken to a Hospital in a stable condition) SCAFFOLD COLLAPSE Construction scaffold collapse injures workers (several construction workers were taken to the hospital after an incident at a construction site – a hydraulic construction scaffold collapsed and one end of the platform fell approximately 20-25 feet before striking the ground – no one was able to comment on the extent of the workers’ injuries, but several witnesses to the accident said that the injuries did not seem severe – the injured workers were transported via ambulance to a Regional Hospital) MISCELLANEOUS MAILMAN Postal worker dies in accident in Baltimore County (a mail man has died after a traffic collision – the bumper of the mail truck was clipped by another vehicle while slowing down to make a left turn – the collision caused the mail truck to eject the postal worker and roll over – the mail man died at the scene) Worker Killed Changing Tire On I-95 (an automotive service worker, 57, was killed while helping a disabled disabled tractor trailer on Interstate 95 shortly before 3:00 a.m. – he was changing a tire on the tractor trailer truck parked on the shoulder of the northbound lanes when another tractor trailer drifted onto the shoulder and struck his service truck – the driver of the disabled vehicle, who was outside his truck at the time, was hurt – accident remains under investigation) Idaho Fish and Game Worker Found Dead in River (Department of Fish and Game is mourning the death of one of its seasonal employees, 26 – his was discovered at a salmon trap – he died while working at the trap – he was found by another employee, who pulled him out of the water, called for help and began CPR – paramedics were unable to revive him) ELECTRICAL Tree worker is hospitalized after electrical shock in Elgin (a tree-surgery worker, 22, was in a Medical Center after being nearly electrocuted at 1:14 p.m.) HOTWORK Hot Work Operation Sparks Brush Fire near Buellton (firefighters have contained a small brush fire – it was a result of a hot work operation shortly before 2pm – it scorched about two to three acres of dry grass, but was contained in
