INDUSTRIAL FORKLIFT Worker injured by forklift at GM plant (one person was injured in an accident inside the Metal Center when an employee driving a forklift accidentally struck another worker who was walking – that worker suffered a serious leg injury and was taken to a hospital) FATAL FALL from FORKLIFT Worker falls 30 feet in fatal accident (a worker, 61, died in a fall at an industrial warehouse – the employee was outside the warehouse building, on a raised platform attached to a forklift-type of equipment – the platform shifted and the worker tumbled off – he fell about 30 feet onto the pavement, and the scaffold then fell on top of him – he suffered major head injuries and died at the scene) FATAL FALL from OILFIELD TANK (Potentially H2S related) Man Killed In Winkler County Oil Field Accident (an oilfield worker has died while inspecting a tank battery – he was on top of a tank battery when he told a fellow worker that he smelled gas – investigators say moments later, the co-worker saw him fall to his death – officials aren’t sure if H2S gas played a role in this accident – an autopsy has been ordered) FORKLIFT Worker treated after accident at Lipton factory (a man was hurt in an accident – his foot was run over by a forklift – the man failed to look in both directions when walking through an intersection at the factory, and his toe was injured – he was taken to a Hospital for treatment) LOGGER STRUCK BY LOG FATALITY Man killed in forestry accident (worker, 45, was killed about 5.20am after being “hit by a large log” – the death comes after another forestry worker, thought to be in his 60s, suffered serious leg injuries while working) GAS PLANT FATALITY Gas Plant Worker Dead (worker, 32, dies after being struck by a piece of pipe – he had been working as part of a pipeline crew when he was struck by a large piece of pipe – he died at the scene) UPDATE now FATAL DUST EXPLOSION Worker Dies Following Springdale Recycling Plant Fire (one person has died following a fire – the employee passed away at a burn unit – three were injured in a fire at the factory, including one man with second to third degree burns over 85 percent of his body – one other associate is reported to be stable in the burn center while the third who was injured was treated and released – company creates composite wood using recycled plastic and fine sawdust – maintenance was occurring inside the facility, the dust associated with the type of process that goes on here resulted in some type of a flash fire) UPDATE Y-12 operator exposed to lithium hydride received second-degree burns, returned to work (an operator injured after being exposed to lithium hydride powder in early April sustained second-degree burns, primarily on the face and scalp, but has returned to work – he also received some first-degree burns – the employee’s injuries were healing as expected without complications, and the worker was expected to make a full recovery) ELECTRICAL EXPLOSION 5 injured in electrical explosion at Iowa plant (authorities are trying to determine what caused an explosion that injured five people performing electrical work at a manufacturing plant – the explosion happened while workers were switching some power lines to a newer transformer – five men sustained burns in the accident – some of the burns were extensive – two of the injured workers were employed by the business and the other three were employed by the electrical contractor) Troopers say oil services worker dies at job site (a worker, 26, has died at a job site – he was reported dead by an operations supervisor – he died while walking between drilling sites – there were no obvious signs of foul play) DIESEL TANK FIRE Diesel tank explosion in Mussaffah leaves workers stranded (three diesel tanks exploded leading to power cuts and evacuations – firefighters spent more than five hours putting out the fire – the blaze started at 11.30am after the first tank, stored under an umbrella, exploded – while investigations are continuing, the Emergency and Public Safety Department said the tank exploded from the heat) HOTWORK Westhampton man injured in welding accident (a man was seriously injured in a small explosion and taken to the hospital by helicopter – accident happened at about 2:30 p.m. – the man is believed to have been welding when he was hurt – extent of his injuries are unknown – Fire Marshal’s Office is investigating) UPDATE HOTWORK Roof work caused Ore. seafood plant blaze (a two-alarm fire that destroyed a seafood plant last month was caused by maintenance being done with a propane torch – the fire in the seafood plant was reported at 10:30 a.m. on June 4 – all 80 employees inside evacuated the building while crews from six different agencies battled the blaze – the fire started while vents were being installed on the roof of the building – the vents were being sealed using a propane torch – the building was destroyed – no injuries were reported) FATAL EXPLOSION @ QUARRY Explosion at rock quarry kills one, 11 injured (worker, 23, was killed and 11 others injured in a 12.50 p.m. blast at a quarry – the massive explosion caused rocks and boulders to rain down on the light industrial park – he was killed after he was crushed by the boulders) CONVEYOR FIRE Conveyor catches fire at Lehigh Cement Co. (forty firefighters extinguished a grease fire in a conveyor at the cement plant – the bearings in the conveyor overheated and the grease caught fire – the fire was controlled and put out in 15 minutes – there were no injuries) AMMONIA Ammonia Leak At Michael Angelo’s Plant Contained (an ammonia leak at a frozen food manufacturer had nearby residents either sequestered indoors or unable to reach their homes for about five hours – Fire Department and County hazmat teams responded and discovered a large tank leaking ammonia into a containment area, as well as vapor released into the environment – approximately 20 employees were safely evacuated and nearby residents that were downwind of the leak were asked to shelter in place and turn off air conditioning units – roads surrounding the facility, including residential roads, were closed during most of the operation – amount of ammonia leaked is unknown at this time – no reports of fire crew, hazmat or civilian injuries) FIRE Fire contained at Domino, TX IP plant (a fire at a paper company kept area fire departments busy for most of the day – the blaze was located in the “long wood” yard of the plant – the fire alarm went off around 9:30 a.m. and mill emergency crews answered the alarm, but it wasn’t long before they realized more help would be needed – at least 10 different fire departments came in to help put the fire out – the fire was contained to one area – cause of the blaze has not yet been determined) FORKLIFT FATALITY Man, 52, killed in South County forklift accident (the man, 52, was working on a forklift when he was killed when the vehicle ran off the end of the parking lot and down an embankment – forklift rolled onto the victim, trapping the man underneath the machine – another forklift had to be used to lift the overturned vehicle and free the victim – the man died of his injuries before being transported to the hospital) CONSTRUCTION VEHICLE Construction worker hit by trailer, dies (a vehicle accident killed a construction worker, in his 50s – the driver of the vehicle accidentally ran a trailer over a co-worker who had picked up metal forms used to build curbs – metal forms used to make sidewalk curbs had fallen off a trailer pulled by the truck at the intersection – he had picked up the forms, was between the truck and the trailer when the driver inadvertently moved the vehicle – he was run over by the trailer) FALL from LADDER Sault worker injured in accident (a worker suffered non-life-threatening injuries after falling from a ladder to the concrete floor of an empty, six-metre deep basin at a sewage plant – he was involved in a painting job on part of the secondary clarifier, a larger settling tank, when he fell around 10:45 a.m. – he was expected to be released from hospital later that day after undergoing a CAT scan – it appears he fell after taking a water break during the severe heat – “He was getting bottles of water and when he came back into the tank, that’s when he slipped on the ladder”) RETAINING WALL COLLAPSED Construction Accident In Meriden Injures Worker, Lifestar Called (a retaining wall at a construction site collapsed injuring a worker who was pinned against a truck by the collapse – the site was a new car wash and it happened at about 3:11 p.m. – the crew had been digging along the retaining wall to install new curbing – the wall became unstable and collapsed – he was extricated and taken to a Medical Center – he was later taken to Hospital by a Lifestar helicopter) FALL from SCAFFOLDING Middle School worker breaks leg after fall from scaffolding (a worker suffered a broken leg after falling from scaffolding at a Middle School construction site and dropping about 6 feet – crews treated an approximately 50-year-old male construction worker, who had sustained a lower leg fracture – he was transported in stable condition to a Hospital – the accident marks the second time this year OSHA has been summoned to investigate a workplace injury at the construction site – in May, two workers suffered injuries to their lower extremities when they were struck by pieces of drywall – the men were moving drywall with hydraulic lift at the time – the lift apparently developed a mechanical failure) CRANE into POWER LINE One injured, another trapped after construction crane clips power line (worker, 46, said when the crane cable hit the power line all hell broke loose – the crane clipped a power line, trapping the crane operator in his cab – he was finally brought down about three hours later at around noon – a worker was injured when a crane operator at the construction site was pulling off concrete forms and somehow got caught up in the power lines – the damage to the power line caused a brief fire and explosion and forced a full evacuation of the work site) TOOLBOX EXPLOSION One Injured After Toolbox Exploded In Midtown Hotel (one person was injured after a toolbox literally exploded in a midtown hotel – the blast, which occurred just before 7 a.m., was caused after a construction worker opted for a torch instead of a flashlight when attempting to illuminate the combination lock on the toolbox, presumably unaware that flammable gas was inside – the resulting blast was reportedly felt 27 stories high – the only injury reported was an unidentified man who was knocked down following the explosion, though the FDNY spokesperson said the injuries were minor – the building did not suffer any damage, though a partial stop-work order has been issued on the basis that workers were “using compressed gases without proper tank storage”) HOTWORK Fire quickly extinguished at Geisinger (a small fire caused by a welding torch in a room that is under construction at a Medical Center was quickly extinguished – no one had to be evacuated when the fire broke out on the fifth floor – there were no patients or staff on the floor, and no one was in any danger – a welding torch ignited insulation inside a wall – firefighters tore the wall apart to locate the insulation, used a water extinguisher and then vented smoke from the area – the fire alarm was sounded at 11:30 a.m., and it was extinguished by noon) CRANE FATALITY Man Killed in Crane Accident at Graphic Packaging in Macon (one man, 21, is dead and three others are injured after a crane accident – worker was crushed between the crane and the door of a trailer around 10:30 a.m. – three others were injured and transferred to a Medical Center – “they are expected to survive” – the deceased was a subcontractor and the group was taking the crane apart when the strap broke) MISCELLANEOUS CO POSIONING Worker dies from CO poisoning (a construction worker died from carbon monoxide poisoning at an apartment – he was found dead in a residential compound – rescuers had to break into the apartment – a neighbor had knocked on the door after smelling gas, but no one answered from inside – the deceased was working for the family who owned the apartment) 20,000 POUND AMMONIA RELEASE Ammonia Leak at Southern Illinois Power Cooperative (20,000 pounds of anhydrous ammonia leaked at a Power plant – it happened in the early morning hours so few people were affected and no one was hurt – the leak itself was caused from vibration – a quarter inch pipe nipple apparently had failed and broke into threads and fell off – no one was hurt and the course was only impacted for a short time period) |
