Story of Interest Worker has leg amputated after repeat jetting lance accident (a specialist contractor had two employees suffer very similar accidents with a high-pressure water jetter after it modified the equipment against manufacturer’s recommendations) INDUSTRIAL FALL from SCAFFOLDING into and onto a worker in a PRCS 2 injured in accident at New Cumberland treatment plant (two people were taken to a Hospital after an accident at a water and sewage treatment plant – a worker involved in a construction project at the plant fell 15 feet from a scaffolding into an empty sewage tank, where a second person was working – the worker in the tank was able to climb out by ladder, while the person who fell from the scaffolding was lifted out on a basket – the man who fell sustained injuries that were described as “moderate”) ELECTRICAL VAULT FATALITY One dead after electrical vault catches fire (one worker was confirmed dead after an underground vault became energized and caught fire – contractors were in the vault when the accident happened at 12:17 p.m. – several workers, who were called to a scheduled outage, escaped the vault – after the fire was extinguished, rescue workers had to wait for the power company to de-energize the vault before entering) FORKLIFT/PICKER FATALITY Kings Mtn. man dies in Gaffney plant incident (a man, 24, was killed in an apparent industrial accident shortly before 9 p.m. – he was operating a picker lift machine and was about 30 feet off the ground, pulling orders of shoes from the shelves – his machine was struck by another machine – the force of the impact caused the machine “to topple, sending him to the concrete floor below” – he suffered severe head trauma and died at the scene – driver of the other machine was not hurt – it is the second death in six years at the plant – a worker was killed in a forklift accident in 2007) GUARDING/LOTO FATALITY with CONVEYOR BELT Worker crushed to death in Outaouais (a worker, 27, was crushed to death in a workplace accident at 8:48 a.m. -worker was caught between a loader and a conveyor belt and he was rushed to hospital where his death was confirmed) MINE FATALITIES 3 men killed in German mine accident (three workers, 24, 50 and 56, were trapped by a controlled detonation and gas leak at a potassium mine died – rescue workers found the bodies of the three men while searching the depths of the mine – initially, seven miners were trapped about 700 meters (about 2,300 feet) below ground, but four of them were quickly rescued, two of them requiring medical treatment) EXPLOSION Four workers injured during explosion at missile factory in Ankara (an explosion has rocked a missile factory injuring four people – two of the injured are in serious condition and were immediately transported to a hospital – all of those injured were workers at the factory – blast reportedly occurred while workers were performing a controlled explosion during the disposal of waste materials – a small fire broke out but was quickly extinguished by personnel) UPDATE HPCL blast: 3 injured workers discharged from hospital (three workers injured in blast at a Refinery were discharged from hospital following improvement in their condition – blast that occurred at the seawater cooling tower claimed 28 lives, including that of a senior executive) CRUSHED under PALLET 5,000-pound pallet crushes Idaho worker (a man was critically injured after a large pallet of mirrors fell on him at a warehouse – he was described as a worker at the building who is in his mid-50s – the man was crushed under a pallet that weighed about 5,000 pounds – he wasn’t breathing when they arrived, but emergency workers performed CPR and the man was hospitalized with life-threatening injuries) GUARDING on CONVEYOR Worker trapped in machinery at Bridgeview metal works co. (a worker crushed one of his legs when he became trapped under a machine – the man’s foot was pinned in a conveyer belt – fire crews worked for about an hour to free him – he is now in serious condition) GUARDING FATALITY Worker dies after falling into machine at flour mill (a worker, 52, trying to clear a block in a large grinder in a flour mill died when he accidentally fell in the machine and suffocated – he was loading the machine to process skinned urad dal when he lost balance and slipped into one of the ducts leading to the maw of the machine – he was found asphyxiated – he had been working at the Mill for more than seven years – he regularly loaded dal in the tunnel-like structure connected to a 10-foot-high machine alone, was following his daily routine when the accident occurred – “he noticed that dal was getting stuck in a corner of the machine and leaned over to free the block with a long stick when he fell in – nobody saw him falling in as the machine is very big – “They couldn’t even hear him cry for help” – “as soon as he plunged in, he was buried in a huge quantity of dal. He probably tried to escape but did not succeed” – when workers realized that he was missing, they went looking for him and found him unconscious in the machine – they tried to pull him out but failed – police personnel got him out of the machine and took him to a Hospital where doctors declared him dead on arrival) OIL WELL EXPLOSION WV DEP Investigating Line Rupture at Antero Gas Site in Salem (officials are investigating an incident at an gas well site that hospitalized at least one person – gas well workers were preparing to put a well online when they performed a pressure test on the well, a standard procedure before moving into the production phase, when the test failed and caused a burst of gas to knock a worker back 20 feet – the worker hit his head and was flown to a Hospital – he was treated and released Saturday – three other workers were injured, including a man who tried to leave the area following the burst of gas, and tripped and fell, causing him to knock out a few teeth – he was also treated and released – the two other men were treated on site for minor injuries) HOTWORK Sparks from welding machine triggers fire (a fire broke out in a firm supplying scientific items when company workers were engaged in vacating the premises – workers were using welding machines to cut on equipment when the sparks fell on chemicals stored in the premises causing huge smoke in the thickly populated area of town – none of the workers was injured as they managed to immediately come out of the premises – fire and rescue service squad used five tenders to douse the fire by 1.pm after fighting for nearly 30 minutes) UPDATE on FATAL EXPLOSION Lawsuit over fatal explosion at Okla. refinery (two widows whose husbands were killed in a boiler explosion at a crude oil refinery have filed a negligence lawsuit – the lawsuit seeks more than $1 million – the Sept. 28, 2012, explosion killed two workers, 34 and 45 – lawsuit alleges the company was using an outdated boiler that had to be restarted manually instead of a system that would allow employees to restart the boiler from a remote location) EXPLOSION @ REFINERY No injuries in Sinclair explosion (at 10 p.m. an explosion caused a fire in the hydrogen unit at a refinery – the Refinery Fire Brigade responded to the scene and had the fire under control by 3 a.m. – no injuries were reported – no homes were damaged from the explosion) EXPLOSION Crews douse vegetable oil fire at Louisville refinery (a fire at a refinery kept firefighters busy – fire broke out just before 1:30 a.m. – an overflow tank of vegetable oil exploded and sparked the fire – crews were able to put out the flames within an hour – no injuries were reported) Nucor plant opening delayed by accident (the collapse of a storage facility will delay the opening of a $750 million plant – there were no injuries or environmental impact from the accident – company is finishing construction of a direct-reduced-iron plant and had planned to begin production within a few weeks – based on a preliminary assessment, the company now expects the start of operations to be delayed until year end – the collapsed storage dome was one of three on the site) LIGHTNING STRIKE TANK EXPLOSION Explosion caused by lightning destroys Fayette County facility (lightning struck one of five combustible containers at the Salt Water Disposal Plant around 6:30 a.m., causing it to explode – the blast triggered fires in the remaining four containers – took firefighters an hour to get the fire contained and under control – fiberglass tanks are used as containers – the facility was a total loss) FIRE Lisbon furniture plant damaged by fire (sawdust on the roof ignited from a spark from the plant’s boiler – it took fire fighters an hour and half to control the fire – fire fighters say this is not the first time the boiler caused issues at the plant – they say the roof sustained extensive damage) AMMONIA Three Factories Evacuated From Ammonia Leak in Salinas (three factories were closed down and evacuated after Fire officials say there was an ammonia leak inside one of the factory’s compression building – factory had thousands of pounds of ammonia leaking into the building and several streets were shut down in the area because of the possible leak – hazardous materials team is calling it one of the biggest ammonia leaks ever in the city – there are no injuries associated with this ammonia leak – there was such an amount of ammonia already inside of the building, hazmat teams had trouble even walking through the building as ammonia was raining on them constantly – the visibility inside of the building was so bad that they couldn’t even see their bright blue gloves right in front of them – teams were eventually finally able to find the source of the leak and stop it later in the afternoon – the ammonia leak was first reported after an odor was detected in the area) PROPANE FIRE Fire At Wells Pet Food Plant In Monmouth, IL (firefighters are investigating the cause of a fire at a Pet Food Plant – fire broke out just before 1:30 a.m. – a liquid propane storage tank fire also spread to the surrounding area – firefighters were able to knock down the flames in a short time – no injuries were reported) AMMONIA Ammonia leak at chicken processing factory, workers evacuated (250 workers at a chicken processing factory were evacuated after an ammonia gas leakage incident – no injuries were reported in the 5.40pm incident – leakage was detected at a storage area where there are two cylinders of liquid ammonia used to freeze the processed chickens) CONSTRUCTION CRANE FATALITIES Thai, Bangladeshi workers killed in Singapore crane accident (a crane accident at a construction site has killed two workers, 50, and injured several others – accident occurred when a crane partially collapsed at the site of a new National Art Gallery – two of the injured remain in the hospital, one in critical condition – it appears some of the crane’s wires may have snapped, though the investigation is still at an early stage – according to witnesses, the crane’s counter balance mechanism appeared to fail when it was attempting to lift building materials) WORKZONE FATALITY Iowa road worker hit and killed; charges likely for driver (a road construction worker, 27, has been killed, and a semitrailer driver accused of leaving the scene of the fatal accident faces charges – incident happened Thursday afternoon – the semi plowed into a construction zone, killing the worker who was flagging traffic) EXCAVATOR FATALITY Worker drowns during an accident with an excavator (a man, 29, died after a lake bank partly collapsed, causing the excavator he was operating to overturn and pin him under the water – he was pronounced dead at a Medical Center – cause of death was drowning – he was submerged for approximately 25 minutes) CRANE FATALITY Worker Killed in Crane Accident (a worker, 25, has been killed in the collapse of a crane mounted on a flatbed – the crane was being used to lift a 5,000 pound generator when it overturned, crushing the worker – fire chief quotes other workers who say the dead man may have underestimated the weight of the generator – he had not extended the “outriggers” which would have provided support the crane) FALL Man injured in fall at house construction site (a construction worker, 23, was taken by medical helicopter to a Medical Center after he fell at a work site – worker was part of a framing crew that was building a home at the site, and he appeared to have fallen about eight feet – worker was conscious and injuries did not appear to be life-threatening) ELECTROCUTION Worker killed in Honda Center accident ID’d (a worker, 30, died after coming into contact with live electrical wires – he died at a Medical Center after an incident while on the job at the arena as one of a group of contract workers – the shock left him with serious injuries, and he died about an hour later at the hospital) CRANE Crane nearly topples and injures worker (workers were installing a safety net around the new turf pitch, a construction crane gave way and leaned over to one side – a worker was injured and an ambulance – the worker, who suffered light injuries, was taken to a Hospital and he was released earlier that afternoon – at no point were students or school personnel in any danger as this area was out of bounds throughout the day) EXPLOSION Explosion in Paris building, one killed (one person was killed and three others were injured in an explosion at a construction site around 3.45 p.m. in the basement of a three-storey building – the ground floor of the building broke down in the explosion as four workers were trying to repair old fuel tanks – while the three injured were evacuated, two other workers were spotted alive in the rubble and another was missing late in the afternoon) FATAL FALL from TRUSSES Construction worker dies in fall (a construction worker fell to his death while framing a new home – he was working on top of the home – witnesses say that the man just slipped as he was trying to reach for something and fell – he died at the scene just after 11 a.m.) FATAL FALL from COLLAPSED SCAFFOLDING Worker killed, 1 hurt as scaffolding gives way (a labourer, 23, was killed and his coworker seriously injured when they fell from the top of a mosque, which was being renovated, after its internal wooden scaffolding collapsed -the incident occurred around 3.15 pm when the two were constructing an internal portion of a minaret – they fell from a height of nearly 60-feet after the scaffolding gave way – one died on the spot due to severe head injuries while the other was rushed to a hospital, where his condition was said to be serious) Miscellaneous LOTO Worker injured in beet harvesting accident (it took firefighters working with hydraulic rescue tools about half an hour to remove a worker who was trapped in a beet-digging machine in the middle of a beet field where the worker had tried to clear a clog when he slipped – his leg became stuck in conveyor belt) FRONT-END LOADER FATALITY Stearns County farm worker hit, killed by front-end loader (a farm worker, 69, was struck by a front-end loader on a farm and killed in what authorities are saying was an accident – he was hit about 8:15 a.m. on a farm – the operator of the front-end loader struck him as he was walking in an area near a silage pit not normally used by pedestrians, and the driver was unable to see him) HCL ACID UW lab worker suffers burns in acid spill (a lab worker, 25 is in the hospital with first-degree burns and a University building is evacuated while haz-mat responders clean up an acid spill – the lab worker was removing a flask containing a hydrochloric acid solution from a cabinet in a second-story room when the bottom fell out of the container, causing two liters of the solution to spill out – worker was able to jump into a decontamination shower to clean himself, but was taken to a hospital for treatment of first-degree burns – responders contained the lab and ventilated fumes through the roof – when the Fire Department arrived, first responders could not safely enter the building because the levels of acid fumes were so high) DELIVERY WORKER CRUSHED Baltimore delivery man crushed to death while unloading package (a delivery worker, 43, was killed in an accident when a package he was unloading fell and crushed him – the man was unloading a large piece of computer equipment to be delivered to an office building at about 11 a.m. – as the man placed the equipment onto a truck lift, he somehow fell to the ground, then the equipment fell on him, crushing him – he was pronounced dead on the scene) CRUSHED Public works employee dies on the job (a County Public Works employee, 49, died while unloading equipment – he suffered severe injuries while unloading equipment at a job site – he quickly succumbed to his injuries at the site) FATAL EXPLOSION 1 killed in Czech fuel tank explosion (an underground tank at a fuel station exploded, killing one person – the accident took place during maintenance work at the facility – the worker died after his head was seriously injured by the shock of the explosion – the 10,000-liter (2,642-gallon) fuel reservoir was nearly empty when the blast occurred) |
