INDUSTRIAL HOTWORK FATALITY Worker Dies After D.C. Metro Tunnel Welding Accident (a workman died and two transit employees were injured in a welding accident in a Red Line tunnel – the contractor was providing welding services in the tunnel when the incident occurred shortly after midnight – the welding may have ignited hydraulic fluid nearby – the incident was still being investigated – the worker was taken to a local hospital where he died of his injuries – two transit authority employees also were transported to local hospitals and are being treated for injuries that are serious but considered non-life-threatening) MINING FATALITY Worker dies of head injury at W.Va. coal mine (a worker, 62, died of a head injury at a coal mine – he was a senior longwall maintenance coordinator – accident happened just before 1 p.m., but it was not immediately clear how the worker was injured – he died on the way to the hospital – the fatality is the sixth mining fatality in West Virginia so far this year) CRUSHED by GRAPES Worker crushed by 5,000 kilos of falling grapes in Spain (accident happened in a warehouse – rescue workers tried to save the man, 43, without success – firefighters had to remove a large quantity of the grapes to extract the man and despite cardiopulmonary resuscitation could do nothing to save his life) FORKLIFT FATALITY Worker, 61, crushed to death while repairing forklift in Queens (employee was fatally injured at a warehouse about 11 a.m. when the hydraulics used to elevate a rig broke and the forklift he was repairing fell on top of him – the forklift — which can weigh about 3,000 pounds — tipped over, trapping the worker underneath it) FALL from COOLING TOWER Worker Injured In Fall (a construction worker, 31, was seriously injured at 9:12 a.m. – he was working on top of the cooling tower building when he lost his footing and fell approximately 37 feet to the concrete – he was transported to the Hospital for initial treatment and was later transferred to another hospital by helicopter for further treatment of his injuries) MINE FATALITY – CO2 Three workers die in potash mine accident in eastern Germany (three workers died of inhaling carbon dioxide – the above-ground area was evacuated when explosions used to mine potash released a large amount of dust – seven miners were trapped at the site, but four were rescued – the victims were ages 24, 50 and 56 – after the explosions, workers carry out checks to see how much carbon dioxide is in the mine – this was the actual job of these employees, and they were then surprised by the amount of CO2 on the site) ELECTRICAL EXPLOSION Worker burned in Curtis Bay power plant explosion (an explosion injured a worker, 40, at a power plant at about 7 a.m. – explosion involved an electrical transformer located at the plant – male patient suffering from second- and third-degree burns over 60 percent of his body – victim was taken to a Burn Center with serious, life-threatening injuries) ELECTRICAL @ MINE THREE WORKERS HURT AT PALADIN MINE (three workers have been injured at a uranium Mine – two of them have significant burns after the serious electrical incident and all three had been hospitalized – one employee and two contractors were hurt in the incident which occurred at 3.30pm – two of the workers received significant burns while the third worker received smoke inhalation and has been discharged – the more seriously injured worker was flown for treatment and a full investigation is underway) AMPUTATION Worker’s fingertip crushed during refueling shutdown of western Pa. nuclear power plant (a worker’s fingertip has been crushed during refueling work – initial reports described the incident as a “partial amputation” – the worker’s fingertip was crushed while a shield wall was being removed during the refueling process) HOTWORK Welding sparks fire at Greenstar Recycling in Northampton (workers who were welding at a Recycling business ignited a trash fire that took area firefighters more than two hours to extinguish – a stray spark apparently ignited recycled material at the processing plant – workers could not extinguish the fire themselves and called firefighters around 3:32 p.m. – fire was eventually isolated to a compactor bin in the facility) HOTWORK Fire breaks out at warehouse in Mattydale (a fire broke out in a business at about 1:45 pm. – two employees of a cabinet and counter top business were working in the warehouse at the time of the fire – employees told investigators that they were using a torch when a nearby rag caught fire – fire quickly spread out of control, causing extensive damage to the warehouse – took firefighters one hour and 20 minutes to knock the fire down) LOTO/GUARDING w/ AUGER Industrial Accident At Purina Mill; Man’s Leg Gets Stuck In Auger (a worker got stuck in some machinery – an adult male patient that was pinned by one of the plant augers on the top of the roof, six stories high – patient had a serious leg injury so the crew placed a tourniquet to stop further hemorrhaging and continued with their life saving stabilization of the patient – a contractor’s crane was used to lift a basket to the level where the patient was and lowering the patient to the ground – patient was flown to a Hospital – cause of this incident is being investigated by OSHA) EXPLOSION Explosion at fertilizer plant near Saskatoon (sulpher-based product blows in transfer equipment at a fertilizer plant – no injuries – employees said they heard an explosion coming from equipment outside the building meant to transfer product from bins to be mixed and loaded for delivery – the substance that exploded was a sulphur-based product – fire crews used thermal imaging to make sure there was no flame before the company opened the hatch and dumped the remaining hot chemical into a truck – there were no injuries and no damage estimate was available) UPDATE on FATAL EXPLOSION Report: Geismar plant explosion caused by ruptured reboiler (company has released a preliminary report on the cause of a deadly explosion at its plant in June – according to reports, the fire was result of a rupture in an off-line reboiler that was in standby mode, next to a boiler that was in operation – company says three things contributed to the explosion: the unexpected presence of liquid hydrocarbons in the reboiler in standby mode, the introduction of heat into the standby reboiler and the pressure relief system was isolated from the reboiler which was in standby mode – when the standby reboiler ruptured, a vapor cloud was released, then ignited by an unknown source – two people were killed and 77 others were injured as a result of the explosion) FIRE Fire damage put in millions at Kan. packing plant (a fire at a packing plant is estimated in the millions of dollars – fire broke out the afternoon of September 25th in the plant’s fabrication area – the cause is still being determined, but investigators have traced it to a conveyor belt – hundreds of contract workers are tackling the cleanup – officials hope to bring production up to 80 percent within a week – plant processes Black Angus beef) CHEMICAL REACTION Workers evacuated at Chandler Honeywell plant (26 workers were evacuated from a plant after an acid cloud formed from the mixing of unspecified chemicals – no injuries were reported – the workers were evacuated from the electronic chemical facility about 1:30 p.m. – the acid cloud formed in one room but spread to another – firefighters were able to contain it in the two rooms – the mixing of unknown chemicals caused the acid cloud to form) CHLORINE Crews contain Cedar Rapids chlorine leak (firefighters were able to contain a chlorine leak at a water treatment plant – employees from the water department reported an alarm activated around 6 p.m. – firefighters dressed in hazmat suits to check levels in the “chlorine container room” – around 7:45 p.m. workers stopped the leak) CONSTRUCTION CRANE Worker injured in crane accident at UND (a construction worker was injured at a work site when part of a crane boom fell several feet while it was being disassembled from a truck bed – the worker suffered a possible broken leg – crane crew had finished driving in pilings for the new bridge – they were taking apart the boom, laying sections on low-boy semitrailers to be hauled out, when the accident happened about 10:30 a.m.) WORKZONE Work Zone Accident On Highway 18 In Sheldon Sends Waterloo Worker To Hospital (a car versus pedestrian accident in the construction zone on Highway 18 sent a construction worker to the hospital at about 10:55 AM – worker, 51, was walking eastbound with her back to the vehicle – when the vehicle passed the worker, she struck her with the passenger side mirror) WORKZONE Construction worker hurt in Cass County car accident (a man, 64, was hospitalized after a driver accidentally struck him in a construction zone – driver entered a construction zone and struck the worker just before 11:30 a.m. – he is a flagman who was directing traffic at the time – after striking the worker, the vehicle then crashed into a Department of Roads pneumatic street roller) SCAFFOLDING COLLAPSE Scaffolding collapses at Braddell Road worksite, seven workers injured (seven workers were injured at a construction site – one worker was seriously hurt but conscious after the suspended scaffolding on the second level of the building collapsed – a worker is believed to have been clipping his harness to the scaffolding when the structure became unbalanced leading to its collapse – another six workers were injured but they were conscious) CRANE ELECTROCUTION Worker electrocuted on Vancouver construction site (a construction worker was electrocuted when the crane a crew were operating came into contact with an overhead power line at about 10:20 a.m. – the Hiab crane was being used to hoist building equipment to a construction site in the area and came into contact with a power line and the worker was electrocuted) Miscellaneous TRACTOR FATALITY Dairy Worker’s Death Shocks Family (worker, 45, was repairing a tractor in a silage pit when the tractor ran over him – he was lying between the front and rear tires when something went wrong – the tractor had broken down in the silage pit and was left in forward gear at full throttle – no one was in the cab at the time, but several workers witnessed the accident) HORSE FATALITY Worker at Canterbury dies following horse accident (a seasonal worker, 56, has died from injuries suffered in an accident involving a horse in early September – he was handling a racehorse in the saddling area on September 12 when that horse reared up and crushed him against a wall – he was rushed to a Medical Center where he died Monday – he was employed as a groom, a seasonal worker who feeds, bathes and maintains the overall health of racehorses) EXPLOSION Minor injuries in blast at Ohio munitions plant (an explosion at an ammunition facility has caused minor injuries to three employees – a small container holding ammunition parts exploded underneath a work table, which got the brunt of the blast – the three employees injured had minor cuts and burns and likely would be released from local hospitals) FARM TRUCK FATALITY Pumpkin patch worker killed Thursday in Farmington (worker, 42, died after he was hit by a truck while working in a field – incident happened at about 3 p.m. and the man died at around 5 p.m. – truck was delivering water to other field workers) CONFINED SPACE FATALITY – DIESEL FUMES Worker dies in petrol tank incident (one man has died and another is in a critical condition after they fell into a petrol tank – it is believed the two men were doing a maintenance check at the petrol station – they were carrying out work inside an underground fuel tank at a petrol station – it’s understood the two were cleaning the diesel tank at the service station – they were overcome by diesel fumes) HOTWORK Fire causes $2m damage at trampoline leisure centre (three people have been taken to hospital after a major fire at a trampoline leisure centre – one firefighter and two men were taken to a Hospital with smoke inhalation and minor injuries – it is estimated the fire caused about $2 million damage – the fire appears to be have been started by accident in a foam pit – a maintenance worker told fire crews that a spark from a welding torch started the fire) |
