fire that happened while the two workers were reportedly installing a sand separator at a well pad) MINE FATALITY One killed in Lonmin mine accident in Marikina, South Africa (one mine worker was killed and three others were injured in an accident at a platinum mine – no details) CHEMICAL BURNS Chemicals burn contractor worker at Alcoa plant (an employee for a contractor was burned by chemicals – the incident follows a Sept. 9 accident that burned five employees in what is called the digestion area of the same plant – the worker who was injured Thursday suffered burns while in the precipitation area of the plant – he was flown by helicopter to a hospital – the precipitation area recovers and classifies the dissolved alumina from the plant process liquid – how the contract worker was burned is under investigation – no one else was injured) UPDATE on LOTO Wis. plant fined for cement mixer accident (OSHA has fined a Milwaukee plant $4,900 after a worker was trapped in a cement mixing machine – plant received a serious violation citation because the worker was exposed to the accidental restart of the machine – the worker, 43, was injured while cleaning the mixer in May – authorities describe the cement mixer as a portable machine with an elevated platform, about as high as the second floor of a building, and a 5-foot-deep mixing box) FORKLIFT FATALITY Worker pinned under forklift killed in accident (a worker pinned under a forklift was killed in the accident just after 2 p.m. – the forklift tipped over and trapped the worker under the “extended lift”) MINE Mine worker seriously injured at Fortescue’s Christmas Creek (a mine worker, 42, has been seriously injured when two haul trucks collided at a mine – a truck driver was injured when his vehicle ran into the back of another at about 1am – the man was trapped in the rear of the truck’s cabin for more than five hours, suffering serious leg and pelvis injuries – he was airlifted to a Hospital, where he remains in a serious condition) FATAL FALL during SIMULATED RESCUE CPS Energy worker dies from fall (worker, 54, died after falling from a 30-foot utility pole on October 1, 2013 – he had worked at the utility for 31 years and was performing a simulated rescue when he fell from the pole) Welder injury at factory site (worker, 23, had to be taken to hospital after he received burns while welding at a factory at around 10 a.m. – the man was not wearing safety clothing, despite asking supervisors for a safety jacket to be provided – a spokesman for the company said that safety clothing is optional but available for all workers – he added that the company was now considering making the wearing of safety jackets compulsory) LOTO Workers exposed to cement powder in Tucson plant accident (workers at a cement plant were exposed to cement powder when a silo valve malfunctioned and dumped the material onto them – two employees of the cement plant were working under a silo containing cement powder when the automatic control valve malfunctioned and caused the silo to remain in the open position shortly before 6:30 a.m. – both men had respiratory complaints and one man had eye irritation – both were taken to the hospital for further treatment – two other employees worked for about an hour to plug the silo to keep the cement powder from continuing to leak out – they wore basic protective clothing, but also had to be decontaminated and were also taken to the hospital for further evaluation – all of the men were released from the hospital before noon) UPDATE on CONFINED SPACE FATALITIES Grain company fined following death of two workers (a grain company has been fined €125,000 and costs following an accident that claimed the lives of two men in August 2009 – workers, 34 and 19, both died while working in an elevator pit over a grain silo – the company manager had found both men lying on the ground in the elevator pit and efforts by him to pull them out failed because he became faint and at one point collapsed – pit could only be accessed by firemen using breathing apparatus – the men had been working in a confined space and the carbon dioxide levels were elevated while the oxygen levels were severely depleted – Oxygen levels were 11.5% – carbon dioxide was in the region of 20-30%) FATAL EXPLOSION Mexico candy factory blast death toll rises to two (a second worker has died following an explosion in a candy factory and 15 others remain hospitalized – the man died from severe burns at a hospital one day after an explosion ripped through the factory – more than 40 people were injured in the blast – of the 15 who are still hospitalized, five are in critical condition – rescue workers were still combing the debris at the US-owned factory, where 900 people were working when the explosion tore down walls – cause of the blast is under investigation) LOTO/GUARDING FATALITY Worker killed when he fell into cement mixer (a worker, 23, was crushed to death when he fell into a cement mixer in a factory – he was fixing a problem with the running machine for the cement mixer around 8am when he slipped and fell into the mixer – there were four other workers at the site when the mishap occurred around 8.54am – the upper part of the his body was crushed and he died at the scene) TRUCK BACKING FATALITY Md. Scrap Yard Worker Dies After Being Hit By Truck (a man working at a scrap metal yard has died after a dump truck backed into him – the man who was driving the truck didn’t see his co-worker as he backed up – the man who was hit was taken to a hospital, where he later died) EXPLOSION Workers Hurt in Chattanooga Business Explosion (six workers were injured in an explosion around 7:30 a.m. – the fire was located around a large furnace – workers at the plant advised firefighters not to put water on the furnace, because it could make the situation worse – firefighters used several dry powder extinguishers on the furnace and hand-held hose lines to put water on the fires around the furnace – three suffered from smoke inhalation, and were treated on the scene – the other three were taken to a hospital – two of the victims had relatively minor burns, but one was more seriously injured – that victim had 2nd and 3rd degree burns to approximately 15 percent of his body – he is expected to be transported to a burn unit in the region – his injuries are not thought to be life-threatening – cause of the explosion is under investigation) REFINERY FIRE Gasoline futures prices spike on Citgo refinery fire (a fire has shut down the 175,000-barrel-per-day refinery indefinitely – air monitoring showed no hazardous materials were discharged – repairing the unit and investigating the cause has begun – some downstream units were up and running – no one was injured in the fire) REFINERY RELEASE St. Bernard Highway reopens after Valero crude oil accident (a tank at the refinery experienced an as-yet-unknown problem resulting in the release of an unknown amount of crude oil into the surrounding area – no injuries were reported, but the accident left oil on at least two school buses, some Sheriff’s Office vehicles and other vehicles passing the plant at the time, as well as cars parked nearby – all employees at the plant were accounted for shortly after the incident – the accident remains under investigation – refinery has offered car washes to anyone reporting oil on their cars – the oil was released from the Crude Unit, which is where crude oil begins the process of refining – a one-mile section of a Highway was closed between 2 and 7 p.m., as sand was deployed to absorb the crude oil on the road) FIRE Update: Petersburg plant fire ruled an accident (crews responded around 8:13 p.m. for reports of fire – it took until 11:40 p.m. to bring the blaze under control due to chemical run-off – there is major damage to the front of the building – preliminary damage estimates are about $500,000 – no employees were inside of the building at the time of the fire) UPDATE on FATAL AMMONIA LEAK 13 factories punished in ammonia checks (13 factories have been penalized over liquid ammonia safety, in checks that followed a leak that killed 15 people in August – 145 plants using liquid ammonia had undergone checks with 997 safety risks uncovered – five factories were each fined 70,000 yuan (US $11,501) and a further eight ordered to stop using equipment linked to safety risks – hazards included unqualified machinery, untrained staff and managers with poor safety knowledge – an investigation showed that a detached pipe cap caused the accident – it also emerged that the plant had illegally built a workshop, blocking emergency exits – last month, five managers were handed over to prosecutors for a possible trial) CHLORINE Chlorine leak at Seven Springs water plant contained (crews have contained a leak from a 100-gallon chlorine tank at a water treatment plant – a plant operator called 911 to report he was dizzy about 8:20 a.m. – crews contained the leak about 80 minutes later – plant operator was treated at a hospital) CHLORINE Power plant workers treated for chlorine inhalation (ambulance transported a worker, 50, to the hospital after he reportedly inhaled some chlorine at a power plant – there was no leak and the chlorine was some leftover in a line – victim walked out to the ambulance, before he was placed on a stretcher and taken to the hospital – second time in two days emergency crews responded to the power plant – crews responded at 6:38 a.m. Sunday to a chlorine gas leak at the plant – the leak was small in nature and contained to the gas tank storage room – two employees who complained of possible chlorine gas inhalation were treated at the scene and released – cause of the leak is under investigation) CONSTRUCTION Construction worker stable after Friday accident (a man, 29, was working as a contractor at a construction site was injured in a workplace accident – he was in stable condition after he was transported by a Flight for Life helicopter – he was working at a residential construction site when the accident occurred – his injuries appear to be accidental, based on the preliminary investigation – his condition was critical at the time of the accident) FATAL FALL Worker killed in Labrador City (a worker, 45, fell from the roof of an apartment building – he died after falling six-stories – some heavy equipment also fell to the ground – there were high winds and blowing snow in the city at the time of the accident) WORKZONE UPDATE – Industrial Accident at 406 Roundabout (a construction worker, 26, was airlifted to hospital after he was hit by a paving machine – the worker was seriously hurt after he was hit by the roller but his injuries are not life threatening) FATAL FALL Contractor dies in accident at Port of West St. Mary (a contract worker is dead following a job site accident around 4:15 P.M. – the victim was working on the roof of a building that the port rents when he fell) FALL unloading STRAW Construction worker seriously injured (a worker was airlifted to a hospital with a severe head injury – worker was unloading “straw” out of the back of a trailer – piles were about 8 feet high – he was reportedly on top and the whole thing fell out of the trailer with him on it) BOAT River worker struck by barge on Missouri River near St. Charles (a river worker, 51, is hospitalized after his boat was struck by a barge – his condition was not immediately known – he was doing work on a train trestle that crosses the river and was driving a boat struck by a barge, even though the tug boat pushing the barge sounded its horn several times – the boat capsized and went under the barge – workers on a rescue boat took the victim to shore – Coast Guard is investigating the crash) PAVING ACCIDENT Construction Worker Injured At Indianapolis Motor Speedway (one construction worker was injured in an accident of some kind, but officers do not expect it to be a fatality – incident happened between turns No.2 and 3 of the speedway course, involving a member of a paving crew – the man sustained non-life-threatening injuries in the accident, and was taken to a Hospital) FATAL FALL from ROOF Kennebunk construction worker dies after fall from roof (a construction worker, 56, died after falling off a porch roof – he was standing on the roof of a farmers-style porch when he stepped backward and fell to the ground – he had been working on the roof of the house at the time) ELECTROCUTION Man electrocuted at work site on West Lane Avenue (an electrical worker, 55, was killed while trying to pull a cable through a conduit inside a 480-volt switch carriage – he was transported to a Medical Center where he died – workers at the site reported hearing a loud noise around 10 a.m. – they found him in contact with the large electrical panel) UPDATE on FATAL CRANE Crane accident caused by human error, officials say (the crane accident last week that killed three workers and injured four was caused by the crane’s metal counterweight, which weighed 2.5 tonnes, crashing down, according to a preliminary report – was caused by human error and not due to equipment malfunction – crane’s counterweight was tilted at a dangerously steep slope and because of its heavy weight, fell down at a height of 45 metres onto the concrete slab where the construction employees were working) FORKLIFT Man injured when forklift skewers car (a man in his 50s is in hospital after hitting his head when the forklift he was operating struck a car – occupants of the car were not injured) Miscellaneous Worker Hurt Disassembling Ride At NC State Fair (a worker, 35, dismantling a ride at a State Fair has been hurt – officials are trying to determine how the man was injured while working on a ride called The Vortex around 3:30 a.m. – the ride is not the same version of The Vortex where five people were hurt last week – the man hurt his leg – no word on his condition – he was taken to a Hospital) FATAL FALL from BILLBOARD Contractor dies in fall from Atlanta billboard (a worker, 53, on a billboard fell to his death at about 3:50 p.m.) FORKLIFT FATALITY Man killed in forklift accident at Dumberning hay business (a man, 48, has died after when he was struck by the
