INDUSTRIAL GARBAGE WORKER FATALITY Public Works employee killed in garbage truck accident (police are investigating a fatal garbage truck accident that killed a Public Works employee and sent his co-worker to the hospital in critical condition – the garbage truck driver may have lost control as he tried to exit I-75 – as a result, police said the truck overturned and the passenger was ejected – the passenger died at the scene from his injuries – the driver was taken to a Hospital in critical condition) OIL PLATFORM DEMO FATALITY US Coast Guard recovers body of missing Filipino oil worker in Gulf of Mexico (an oil worker, 38, accidentally fell off an offshore platform in the Gulf of Mexico – at the time of the accident, he was working with a derrick barge crew in dismantling a platform – he was knocked off the platform by an empty storage tank, pushing him overboard into waters roughly 30.5 meters deep around 7:30 p.m. – his body was found pinned just below the platform – it was not clear how the storage tank ended up collapsing onto his as US authorities were still investigating the accident) STEAM Conditions unchanged of workers seriously injured in Phillips 66 refinery accident (the conditions of two workers who were seriously injured following an industrial accident at an oil refinery remain unchanged – the pair were airlifted to a Hospital after a steam leak at the plant – one of the workers is still in a critical condition whilst the other remains serious) FLASH FIRE Furniture factory worker scorched in bonfire (a worker, 58, at a furniture factory suffered serious burns to his face and upper torso while burning discarded pieces of wood around 8am – he was said to have thrown paint thinner at a burning pile of wood, causing the fire to flare up and scorch his face, chest and shoulders) ELECTRICAL FATALITY Paladin employee dies from burns (an employee has died, almost a month after receiving significant burns in an electrical incident at its Mine – three people, an employee and two contractors, were hurt in the incident on the afternoon of October 3 – one of the injured contract workers had recovered completely while the other remained in hospital, but was expected to make a full recovery) LINE BREAK, FLASH FIRE and MANLIFT 2 workers critically burned in Henrico explosion (two construction workers were listed in critical condition with life-threatening burns after an explosion at a warehouse – the men, one in his late 30s and the other in his 20s, were taken to a Medical Center after the explosion – the older man had burns to the majority of his body – the men were on a scissor lift removing pipes and ventilation ducting from the vacant building when some flammable vapors ignited – county fire marshal’s office is investigating the cause of the blast) FATAL AUTOCLAVE EXPLOSION Funeral held for worker killed in Rockmart plant blast (worker, 56, was a quality assurance inspector at the facility – contents in a pressurized vessel called an autoclave exploded inside a room at the plant causing critical injuries to the worker – she died an hour later at a hospital) CRANE FATALITY Man dies after being struck by a crane (a worker, 30, has died after he was struck in the head by a crane at a steel distribution facility at 6.30am – he was struck by a large magnet that was attached to a crane – colleagues of the man, who was bleeding heavily, performed CPR until emergency services arrived – he was taken to a Hospital, but died a short time later) UPDATE on FATAL EXPLOSION Fourth Person Dies in Mexico Factory Explosion (a hospital official says a fourth person, 35, has died from injuries inflicted by an explosion last week at a candy factory – the blast on the second floor of the factory caused the floor to collapse, injuring people working downstairs – the cause is still unknown) Evacuation called off as chemical plant fire in Quebec deemed non-threat (residents will be allowed to spend the night in their homes after an explosion and fire at a chemical plant prompted air quality concerns – tests concluded the air was non-toxic and an evacuation plan was scrapped after an explosion at the plant left a dark cloud of smoke lingering – there were people in the plant at the time of the explosion, but no one was injured – cause of the explosion is unknown) EXPLOSION @ BOILER Lausche explosion repairs could cost $300,000 (the explosion — caused by a chilled water valve that failed to reopen, causing a pump to overheat — resulted in an initial estimated damage cost of $300,000 – a contractor was working on air systems, which control the chilled water valve and other pumps in the building – when the air systems were turned back on, the chilled water valve did not – the valve did not have a sensor to tell Facilities that it had not reopened) CONSTRUCTION WORKZONE FATALITYDUI investigation after worker fatally struck on I-680 (the accident happened at about 2:40 a.m. when the driver of a silver Nissan went off the road and on to the freeway’s shoulder hitting two workers while they were out of their truck picking up a road construction sign – one worker died at the scene, the other was taken to the hospital with minor injuries – the driver of the Nissan had to be pulled from the wreckage by firefighters – he was taken to the hospital with life threatening injuries – a limited field sobriety test was done and officers determined alcohol was a factor) WORKZONE FATALITY ALDOT worker killed in Montgomery County accident identified (a DOT employee, 51, was struck by a van – a Dodge Caravan traveling on U.S. 231 struck and killed him – occupants in the minivan were injured in the crash) CONCRETE BOOM PUMP TRUCK Construction worker critical after arm severed (a construction worker, 49, is in a critical but stable condition after his left arm was severed and he sustained serious leg injuries in a building site accident – a concrete boom fell on the man just after 7.30am as concrete was being poured on a residential building site) WORKZONE FATALITY Construction worker killed in parkway accident (a worker at a construction site on the Parkway was killed when he was accidentally struck by a dump truck just before noon at mile marker 50.1) Construction accident at Ft. Lauderdale airport (workers at an airport construction site went running for cover after five concrete beams each 125 ft long came crashing down around them – the crews were working on a new runway at the airport – the concrete beams fell across railroad tracks near the site, briefly suspending freight traffic) FALL Discovery Park sends three to hospital (three construction workers fell from the train exhibit and sustained undisclosed injuries at approximately 5 p.m. – the three individuals were working on the exhibit ahead of the park’s grand opening on Nov. 1, when the accident occurred – they were not believed to have life-threatening injuries – two of the men were airlifted from the scene, while the third was transported to a Hospital for observation) FATAL COLLAPSE 6 die in tunnel accident at energy firm work site in S Africa (six construction workers were killed when a platform collapsed inside a tunnel at a work site – about 15 people were working in the tunnel when a working platform failed – several others were injured) CRANE COLLAPSE Second crane accident in Dubai injures three workers (three workers were injured when a window cleaning platform, which was suspended from a crane, fell on top of them from the 34th floor – incident comes just a matter of weeks after three workers died when a crane collapsed on the construction site of a luxury hotel – in Tuesday’s accident, the men were working under the platform, on the ground floor, when the collapse took place – one of the workers was seriously injured and two sustained moderate injuries – crane also caused damage to six vehicles in the area, one of which was still occupied by its driver, as well as the injury of two passers-by – windows on the 31st floor were also damaged) ELECTRICAL FP&L worker badly burned after coming into contact with live power line (a worker, 53, came in to contact with a live line while working in a trench – he was airlifted in serious condition – he was alert and conscious when emergency crews arrived on scene around 11 a.m. – the worker suffered electrical burns and was flown out as a trauma alert – he is expected to survive) HOTWORK Welding sparks likely set off South Pattaya fire that destroyed 8 buildings (welding sparks blown from the roof of a restaurant to a storeroom filled with flammable liquids are believed responsible for setting off an area fire that damaged eight buildings and caused up to 100 million baht damage – worker, 28, said it took less than 10 minutes from the time she spotted smoke for the fire to spread – fire investigator believes sparks from welding equipment being used on the restaurant’s roof set off either an explosion or fire in a storage room for paint thinner and other chemicals at the adjacent construction-supplies depot) CRANE FATALITY Man killed in accident involving cherry picker crane identified (a man, 66, was killed while trying to replace a sign after a cherry picker crane malfunctioned at about 11:14 a.m. – a man and his son had come to replace a sign using a flatbed mounted cherry picker crane – some kind of malfunction pushed the man through the sign and pinned him between the ceiling entrance and the basket of the crane – he was out of the bucket when responders reached the scene, but police said it wasn’t immediately clear if the man fell from the bucket or was pulled down by bystanders or the man’s son – bystanders were attempting CPR on the man when emergency personnel arrived – hospital officials later confirmed that the man had died) Miscellaneous ROLL UP DOOR COLLAPSE Supermarket worker’s leg mangled by door (a worker had his leg mangled by a metal rolling door – it is understood a delivery truck entered the storage area of the supermarket earlier in the day, hitting the door and damaging it – several hours later, it collapsed on the man – a group of four men had lifted the door off the worker’s leg, which was in a “gruesome state” and bent at an unnatural angle – he was taken to a Hospital, where he remained for several days, and was now recovering at home) LOTO/GUARDING FATALITY w/ CONVEYOR Man Dies In Accident On Lake Michigan (a worker has died in a conveyor belt accident aboard a barge) ELECTRICAL and TREE TRIMMING 2 tree service workers injured in NJ accident (two tree service workers, 20 and 39, were injured when one of them made contact with a power line while they worked at a home around noon – the 39-year-old was on a ladder and trimming a tree when he made contact with the power line – he suffered burns and received an electrical shock, which caused him to fall from the ladder – his 20-year-old co-worker was standing on the ground and suffered leg injuries when he tried to catch his colleague – the 39-year-old also received further trauma injuries when he hit the ground – both men were taken to a hospital, but further details on their conditions were not disclosed) LOTO FATALITY w/ ELEVATOR Sharjah man crushed between lift and wall (an accident has killed an elevator maintenance worker in a building – the man, in his 30s, was working in the elevator shaft when the elevator fell on him, trapping him in between the wall and the lift itself – the worker performed the functions of cleaning the facades and entrances to the elevator – incident took place on the third floor of the building) UPDATE on STUMP GRINDER FATALITY OSHA investigation of fatal grinder accident continues (OSHA continues to investigate four months after a tree service worker, 43, was killed in a stump grinder accident – he died in what authorities described as an industrial accident involving a stump grinder while working at a home on June 20) LEAF COLLECTION TRUCK Worker critical after leaf truck accident in Middletown (a man is in critical condition after an accident involving a leaf collection truck – the man, who reportedly works for the township public works department, somehow ended up underneath a truck with a vacuum – he was working with a crew raking leaves behind the truck at the time) AIR CONDITIONER FATALITY Worker killed in town centre food store (a maintenance man, 58, who was carrying out work at a town centre store died – the sub-contractor was carrying out routine air-conditioning work) LOTO FATALITY w/ ELEVATOR Hospital worker killed in elevator accident (a hospital employee, 41, was killed after an incident involving an elevator around 6:45 p.m. – it appeared he was killed when the elevator he was making repairs on came down on him – two other maintenance workers were in the same spot with him moments before the elevator came down on him – those two team members went to another area of the hospital, so when this happened he was by himself – the elevator was a freight elevator and was not used for patients and was not accessible to the public, only to employees) FATAL FALL down ELEVATOR SHAFT Maintenance Worker Plummets Down Elevator Shaft To His Death (an elevator technician died after he fell 20 feet down an elevator shaft in a parking garage – it happened at 8:12 p.m. – the technician was servicing equipment located in the mechanical room next to the open shaft – paramedics declared him dead at the scene – they don’t know what caused him to fall or whether there was foul play involved – at this time, it appears it was an accident) FATAL FALL from TREE Tree Service Worker Dies from Fall in Haddonfield (worker died after falling approximately 40 feet from a tree he was trimming at 8:22 a.m. – he was pronounced dead at a Hospital after he was taken there by ambulance) ELECTROCUTION Landscape worker electrocuted while cutting hedges (a landscape worker was electrocuted while cutting hedges – the worker was using a long pole with a saw connected to the end of it to cut 20-foot tall hedges when at one point, the pole made contact with a 7,000 volt power line – when the pole hit, the electricity in the line tried to take the most direct route to the ground, electrocuting the man – he died instantly) HOTWORK Renovation work sparks fire at Great Northern Casino (welding during kitchen renovations led to a rooftop fire at a Casino – the heat from welding operations caused combustible materials in the unit to catch fire around 12:20 pm – the work was being done as part of renovations to the building’s kitchen – all staff and customers were evacuated from the Casino as smoke filled the building) |
