INDUSTRIAL PRESS Tesla Motors accident: Hot metal burns three workers at Fremont auto plant (three employees were injured when a low-pressure aluminum casting press failed – one employee was seriously hurt and two others suffered minor injuries when the machine failed, spilling hot metal on the workers – the three employees were taken to a Medical Center with second-degree burns – the most seriously injured worker received chest and upper-body burns) GUARDING/LOTO Worker’s hand injured after getting caught in machine (a worker suffered a hand injury – the employee got his hand caught in the roller of a tape-making machine – the man suffered non-life-threatening injuries) STEEL MILL FATALITY Man dies in accident at ArcelorMittal (a man, 39, died and two other people were injured in an accident at a Steel mill – he died of blunt force trauma near the mill’s basic oxygen furnace and was pronounced dead at 7:16 a.m. – he was a contract iron worker – the two injured people also were contract workers – the injured workers were transported to a Hospital – their injuries were not thought to be life-threatening) FORKLIFT FATALITY Man killed in forklift accident at Near-Eastside industrial complex (a worker, 30, suffered fatal injuries when he was pinned between a forklift and a guardrail – he was using the forklift while trying to remove a screen used for filtering recycled materials from an electronics shredding machine – at about 6:37 a.m., he parked the forklift while it was still running and got out of the vehicle to move the screen by hand – while outside of the forklift, it suddenly moved and pinned him against a guard rail – nobody was sitting in the forklift at the time – he was taken to a nearby hospital with critical injuries where he later died) FATAL FALL Highland oil worker killed in Norway fall (an oil worker, 37, has died in an accident in a Norwegian port – the pipe-fitter died in a fall) AMPUTATION Worker’s Arm Severed at Seattle Fortune Cookie Factory (a 55-year-old man’s left arm was severed in an industrial accident at a cookie company – the arm was severed after the man became stuck in a machine up to his chest – medics found the man conscious and alert but in a lot of pain – firefighters freed the man and brought him to a Medical Center, along with his severed arm – don’t know if doctors will be able to reattach the limb) LOADING DOCK Serious Accident involving UPS Worker (worker, 19, was taken to a hospital WITH LIFE-THREATENING INJURIES after he was crushed by a truck around 6:30 – it appears that he was probably assisting or engaging a truck coming to the loading bay – he was caught in the middle between the truck and the loading bay walls or doors which caused him to be crushed) AMPUTATION Elevator Accident Nearly Severs Man’s Hand (medics rushed an elevator repair worker to the hospital when an accident partially amputated his hand around 9:45 a.m. – the elevator worker somehow got caught in an elevator pulley system – by the time emergency workers arrived at the scene, the man’s co-workers had already untangled him from the cables and pulley – emergency workers reported the worker’s hand was partially severed at the palm – the man, who is in his 60s, was transported to the trauma center) FORKLIFT FATALITY Worker Killed in Forklift Accident in Amazon Warehouse (a worker was killed after a forklift fell on him around noon – the man who was killed worked for a company that the warehouse uses to rent the forklifts – he was loading the forklift onto a flatbed truck when it began to roll forward so the worker started running alongside it to try and stop it – the forklift then fell on him and was crushed to death) UTILITY FATALITY Worker dies from head injury (worker, 24, was pronounced dead by paramedics at the scene of a worksite accident – he was employed with a utility contracting company that was removing old power poles from the ground with a crane truck – one pole pulled fell from the truck due to apparent combining factors of strong winds and being too heavy – once striking the ground, the near end of the pole popped back up and struck the worker in the head) FORKLIFT FATALITY Worker killed in forklift accident (a worker, 68, died in an accident at the auto-parts business – he was found mortally injured near a forklift with its engine running shortly after 11:30 a.m. – he died at the scene – there were no witnesses to the fatality, but authorities concluded that the victim evidently had been run over by the vehicle) MINE FATALITY Worker, 36, dies in Franklin County mine (a 36-year-old man died while working in a mine after an accident – no details) CONFINED SPACE FATALITY – Nitrogen Two workers die in accident at Russian space launch center (two workers at space center have died while carrying out routine work cleaning out a propellant tank – another three servicemen were hospitalized after being exposed to poisonous nitrogen vapors while working in the cosmodrome – they were expected to recover – a failure to follow safety regulations was cited as the cause of the incident) CHEMICAL REACTION Chrysler Plant in Belvidere Evacuated After Report of Industrial Accident (the plant was evacuated at 12:06 pm as a precaution following a chemical mixing incident that occurred in an area adjacent to the old body shop – the plant will reopen with the 2nd shift at 6pm – the Fire Dept. has cleared some workers to return to specific areas of the plant where the air quality has been deemed safe) FORKLIFT FIRE inside BOX TRAILER Worker severely burned when forklift catches fire in Port Elizabeth (a man was severely burned after a forklift he was operating burst into flames in a box trailer at a facility – worker was removing the last forklift from a shipment of the vehicles that had recently been delivered to a warehouse, but when he started the machine it caught fire – he suffered third-degree burns on his hands, second-degree burns on his head, and damage to his throat – the fire started in the box trailer and caused a propane tank aboard the forklift to explode) NITRIC ACID EXPLOSION Two Falls Township, Pa. workers injured after nitric acid explosion (two workers suffered chemical burns after an acid explosion at a denture manufacturing facility – the two workers were filling a pail with nitric acid when they were burned when the chemical they were pouring suddenly exploded – the explosion left the two workers at the facility with burns to their airways and upper bodies – both were expected to survive their injuries – one of the injured workers was grabbed by his colleagues, who took him over to the emergency sink to wash away the chemical – the second employee staggered outside and was found there by arriving firefighters, who decontaminated him – the explosion ruptured the side of the pail but did not damage the facility – the two workers were pouring nitric acid from a container into a five-gallon pail – they were not wearing protective equipment other than gloves) UPDATE now a FATAL EXPLOSION One of two workers burned in Henrico warehouse explosion dies (one of two construction workers critically injured after an explosion inside a vacant warehouse last week has died – the man suffered life-threatening burns in the blast at the building – the men were on a scissor lift salvaging pipes and ventilation ducting from the vacant building when some flammable vapors ignited – both men, one in his late 30s and the other in his 20s, were able to make it down and out of the building on their own) EXPLOSION 2 workers injured in explosion at Woburn, Mass. industrial park (two people were rushed to the hospital following an explosion at an industrial park shortly after 12:30 p.m. – a second victim, a man, who police say tried to help the first victim, was taken to a hospital for precautionary reasons – the worker was in a garage at the time and may have sucked a chemical into a vacuum when it blew up – not 100-percent sure; it’s under investigation right now) ELECTRICAL Workers Suffer Electrical Burns (two men are lucky to be alive after the 40-foot communications tower they were attempting to install touched a high voltage line – worker suffered electrical burns to his hand and lower leg – they were lucky that the antenna that they were moving was still touching the ground so it was grounded out with the ground so it did not go through his body – he suffered non-life-threatening injuries and is recovering – the other man refused medical treatment) Ingredion worker crushed (a worker is in stable condition after being crushed between a railcar and a steel beam at a plant – the worker was injured when he came into contact with a stationary post while standing on the fixed ladder of a moving railcar – the victim was brought to a hospital with “serious, but at the time, not life-threatening injuries”) DUST EXPLOSION and STATIC Static charge caused explosion in Woburn Tuesday (an explosion that critically injured a worker was caused by an electrical charge that triggered a dust explosion – employee was using an improperly grounded vacuum to clean machinery when a build-up of static charge inside the vacuum ignited the dust, causing an explosion and subsequent small fire – one man was critically injured and airlifted to a hospital by MedFlight – a worker from a neighboring company attempting to help the man suffered minor injuries, and a police officer was also treated as a precaution) POWER PLANT EXPLOSION 4 Injured in blast at Mexican power plant (four people were injured in an explosion at a power plant – it is reported that they are stable and out of danger – authorities say they do not yet know what caused the blast – the injured were contracting firm workers who were in the area at the time of the explosion) AMMONIA SolBrew explains ammonia leak incident (leak occurred during routine maintenance inside the ammonia storage building – leak was a small leak contained in a matter of minutes and managed within established safety procedures which included a temporary evacuation of staff from the immediate vicinity of the leak) CHLORINE Small Gas Leak Forces Evacuation At ADM Milling (work was disrupted shortly before 8 a.m. when an alarm activated, indicating there was a gas leak – workers at the plant got everyone out of the building and called 911 – Fire Department responded with several fire companies, including the haz-mat team – workers were changing lines to chlorine tanks when a sensor designed to detect chlorine activated and set off an audible alarm) FORKLIFT and SCISSOR LIFT Man pinned between forklift and a scissor lift (a man in his 20s has been airlifted to hospital by a rescue chopper after a workplace accident – the man had been pinned between a forklift and a scissor-lift – he was airlifted with a broken pelvis and he is listed in a stable condition) FORKLIFT FATALITY Forklift driver killed at St Leonards (a forklift operator, 24, died in a workplace incident – he was using his forklift to unload goods from the rear of a flat tray truck when the forklift fell on its side about 3.50pm – he was pinned underneath the forklift – he was pronounced dead a short time later) CONSTRUCTION Redwood City Worker Suffers Head Injury When Hit By Truck (a man is in serious condition after having part of his skull crushed – he was working at a home when a Ford F-450 backed into him, pinning him against a utility pole) FALL from COFFERDAM More details released in I-90 bridge construction accident (a construction worker, 19, fell into a cofferdam while removing framework set up to pour concrete – a witness working with him told a Deputy that he stepped backward causing him to fall 42 feet – according to the County Sheriff’s Department accident report, a protective handrail on the cofferdam had been removed in order to remove the concrete framework – the report also states that he was wearing a safety harness, but it was not being used properly – he landed on the cement bottom of the cofferdam and was trapped – he had to be extricated by police and fire crews) WORKZONE FATALITY Two workers die in construction zone accident in northeast Nevada (two construction workers died in an accident on Interstate 80 and two others were injured – worker, 53, died at the scene of the accident – worker, 47, was injured and taken to a Regional Medical Center, where he died – worker, 50, was taken to a Regional Medical Center and was in stable condition – a fourth worker, 50, was taken to a Hospital with minor injuries – investigation so far shows the driver, 69, was driving a Honda east on Interstate 80 in a construction zone in the fast lane at about 1 p.m. – the slow lane was blocked with orange construction barrels – driver failed to slow for the stopped traffic and swerved to the right and hit four construction workers – the vehicle continued east and hit a parked construction truck – neither she nor passenger, 75, were injured) HEAVY EQUIPMENT FATALITY Man dies after Beach construction site accident (a worker, 49, has died as a result of an accident at a construction site at 8:15 a.m. – he was in cardiac arrest due to a significant trauma – CPR was administered by co-workers on the scene -he was hit by a piece of equipment – when crews arrived, it was determined that “the patient’s injuries were not conducive to life” – he was pronounced dead at 8:28 a.m.) Worker dies after equipment crushes him (a man, 46, died after heavy equipment crushed him at 11 a.m. at a construction site property – he was the operator of the “roller/compacter” was “crushed under the heavy equipment as it tipped over on its side”) WORKZONE FATALITY Construction worker dies after being struck by vehicle on I-26 (a construction worker, 35, was killed after being hit by an SUV on Interstate 26 – the SUV driver went into a lane closed for construction work around 11 p.m.) Worker killed at King’s Cross building site as concrete stairwell fell on top of him (a man working on a building site died after a concrete stairwell being moved fell on top of him at 4.50pm – the victim, believed to be in his early 30s, was not breathing and despite efforts to resuscitate him he was pronounced dead – a concrete stairwell was being lifted and it dropped in from floor height onto him) CRANE Crane tips over while moving tree in Fort Collins (a crane tipped over – a tree company was doing work when it appears a tree that was being moved proved to be unexpectedly challenging for the crews – more equipment was brought to the scene to stabilize the tree that caused the problem, as well as bring the tipped-over crane back to an upright position – it wasn’t known if the accident caused any damage to a nearby home) Miscellaneous CRUSHING FATALITY Gravestone kills cemetery worker in Edinburg (authorities continue to investigate the death of a cemetery worker, 64, who was crushed under a 2,000-pound, 5-foot-tall tombstone at 9:05 a.m. – it appears he may have been trying to level a 2,000-pound headstone that was more than 5 feet tall) CRUSHING FATALITY Mechanic dies after accident at Placentia auto shop (a worker, 44, died after an accident at an auto-repair business resulted in a vehicle pinning him against a wall shortly after 2:45 p.m. – an employee was working on a vehicle that came out of gear and rolled back, pinning him against the wall and causing internal trauma – paramedics performed CPR on the man, who was taken to a Medical Center – he was pronounced dead a short time later) COUGAR FATALITY Officials: Cougar killed Oregon sanctuary worker (worker, 36, died from an attack at a Sanctuary – it is suspected she was attacked by a cougar while alone with the animal in its enclosure – she had “devastating injuries in the critical areas of head and neck” – she died inside a cougar enclosure where she worked as head keeper for the past eight years) FATAL FALL from LIFT GATE Worker dies at Woodfield Mall in Schaumburg (a man, 44, died after he fell from a truck outside the Mall shortly before noon – he and a coworker were unloading a shipment of shelving units from a truck outside the mall – while standing on the truck’s rear lift gate, he fell about four feet to the ground – he was taken to a Medical Center where he was pronounced dead at noon – autopsy found he died of multiple injuries from the fall and from being crushed by falling boxes) FATAL FALL from TREE Mirage worker dies in fall while trimming trees (a worker, 31, died after he fell while trimming trees – no further details about the incident were made available) HOTWORK Man welding gas tank airlifted to hospital after truck catches on fire (a man was flown to the hospital after a plow truck he was working on caught fire – it was the second day in a row that a Rochester truck fire resulted in a victim with serious injuries being airlifted to a medical facility – the man in Tuesday’s incident was making repairs under the truck when it caught fire – he sustained burns to his face – it is not known if those injuries are life-threatening or why the man was working on the truck in the motel parking lot – the truck caught fire as the man was welding its gas tank without first emptying the tank) |
