INDUSTRIAL FATAL FALL from CRANE Port worker falls to his death at Hunterston coal terminal (port worker, 38, has fallen to his death at a coal terminal – no further details are available at this time) MINE FATALITY Female killed after Land Cruiser collides with coal truck (a female mine worker, 38, was killed at an open pit coal mine when the truck she was driving was crushed by a coal truck – the deceased driver failed to heed warning signs while approaching a T intersection – she was instantly killed after her truck got wedged in the coal hauler’s back axle – recovery of the body took four hours – driver of the coal truck was treated for shock – accident happened just before midnight) CONFINED SPACE FALL Rescuers Save Man In Industrial Accident (a contract worker was being lowered down about 90 feet into a grain silo when there was an equipment failure causing a ladder to fall, breaking the worker’s arm – fellow employees managed to pull the injured worker up near the top of the silo, but could not get him all the way out – Fire Department was called along with some personnel and equipment with the Urban Search and Rescue (USAR) team – firefighters set up a rope and pulley system and lowered a Captain down about 15 feet to get to the victim – the victim was conscious and alert, but was in a lot of pain – firefighters lifted him up and out of the silo and he was rushed to the hospital) Worker trapped by 3-tonne machine (a man, 47, was taken to a hospital after being impaled by the handle from a 3 tonne machine – the man was pinned face-down on the floor by a lathe machine after it fell over while being moved – he was trapped for about an hour as paramedics treated him, while rescue teams worked to free him from the machinery – rescue crews first tried to dismantle the lathe while the man was still trapped, however it proved too difficult and dangerous – they were forced to cut the handle off the machine, leaving the five-centimetre handle lodged in the man’s shoulder) FATAL SILO COLLAPSE Cause of silo collapse unknown; search for worker to resume Wednesday (efforts to stabilize a collapsed corn silo will resume at daylight Wednesday as crews try to find the body of a worker believed crushed in the avalanche of grain – crane and welding crews worked to stabilize three silos and a building – welders attached brackets to the top of the damaged silo so it could be held in place by a crane, enabling workers to remove the feed and search for the missing worker, who is presumed dead – firefighters at the station across the street heard a loud crash as a silo containing 100,000 pounds of corn collapsed in on itself, spilling grain out of the bottom – the silo hit two nearby silos as it fell, damaging one and knocking another, containing peas, sideways into a metal office building – the partially collapsed silo pushed about 15 feet into the office, bending structural beams and breaking water lines – the missing worker and a manager were working outside the silo when its support system gave way – the manager ran west and escaped safely -the worker ran east and could not be found after the incident) LOTO FATALITY Worker at tobacco firm crushed under robotic arm (a maintenance engineer, 63, was crushed by machinery at the cigarette factory – he was carrying out repair work on the equipment when a heavy part of it fell on top of him – he ended up lying partly on the ground and partly draped over a piece of scaffolding which was used to allow maintenance access to the machinery – death was due to the accident in which he was trapped between the scaffolding bar and the arm – front of his neck was compressed, presumably against the metal scaffolding, fracturing his lower jaw and a bone in the front of the neck) SANITATION TRUCK FATALITY Greenville sanitation worker hit, killed by trash truck (the wreck happened just after 9 a.m. and the victim was 22 – during a routine garbage pickup, the driver was backing up towards resident’s trash can when worker got off the truck – they believe the man may have tripped and he was trapped underneath the wheels of the truck) HOTWORK Maple Ridge mill damaged in weekend fire (welding is being blamed for sparking a fire that destroyed a section of a sawmill at 12:30 a.m. – electrical arcing from the mill’s high voltage power lines illuminated the sky for miles and the building that housed the “chipper unit” was engulfed in flames by the time crews arrived – the mill’s fire pump, designed to supply water from the river, had been removed for service a week earlier so water had to be ferried to the site via tanker trucks – investigators believe the fire started as a result of welding that had taken place earlier in the day) HOTWORK Pennsylvania woodworking company destroyed in 2-alarm blaze; no injuries reported (a woodworking business is a total loss after a devastating two-alarm fire shortly after 9 a.m. – the fire was accidentally caused by a spark from welding or grinding maintenance work in the diesel room of a wholesale furniture manufacturer – there were only three or four workers in the building when the fire broke and they escaped without injury) LOTO/GUARDING FATALITY w/ CONVEYOR Covington man dies in industrial accident at Pratt Conyers plant (an employee, 31, died after getting caught in a conveyer belt at the company’s bio-energy plant – he reportedly appeared to have been clearing debris below a feeder belt that carried wood chips into the gassifier when his left hand and arm got caught and he was pulled into the belt structure – when coworkers found him around 6:30 p.m. and called first responders, he was reportedly already unresponsive – he was pronounced deceased at the scene – he had just started working at the plant in November) FATAL FALL from CHIMNEY SCAFFOLDING Four killed in power plant accident (four workmen died as a result of a fall at the Power Plant – the men fell from a height of about 200 meters – they had been working on a specially raised platform by one of the plant’s chimneys – at about 1 pm local time, the platform gave way) FATAL EXPLOSION Explosion at incineration plant leaves 1 dead, 5 hurt (an explosion killed one person and injured five more at a garbage incineration plant – the plant suffered a methane explosion around 3 pm – the explosion brought down a 400-square-meter section of the building – two of the victims were listed in critical condition after suffering head and chest injuries – the other three were in stable condition – one person was still missing) EXPLOSION Explosion at Canton oil refining plant caused millions of dollars in damages (an explosion that took place at a chemical processing plant caused about $12.5 million in damages – the plant is an oil refining and chemical processing company – two employees were working inside of the plant when flames broke out – the employees were able to hit the emergency shut offs, and then evacuated the building safely – firefighters said hitting the emergency shut offs stopped hazardous materials from spreading to the fire – no one was hurt – a portion of the plant is a total loss with damages estimated at $12.5 million dollars – cause of the fire is under investigation) FIRE Explosion starts fire in Browerville plant (an explosionat the plant caused a fire that firefighters battled for about seven hours – a piece of equipment exploded at about 4:35 p.m. and caused a fire that spread to the roof of the building – one firefighter received minor burns battling the fire and was taken to the hospital by a private vehicle) Explosion rocks Missouri natural gas plant (an explosion rocked a natural gas plant – something ruptured at a compressor station causing the blast – the fire has been put out and some living nearby were evacuated from their homes as a precaution – no one was hurt and the cause is under investigation) FIRE Five Workers Injured in Industrial Plant Fire (a fire at an industrial plant injured a handful of people and caused millions of dollars in damage – fire started just before 2:30 p.m. at a plant that makes and sells steel – several workers inside at the time of the fire were all evacuated and after a short time, were all accounted for – the five people injured had problems like smoke inhalation, broken bones and first degree burns) FIRE Fire damages nitrogen fertilizer plant east of East Dubuque (a fire damaged a nitrogen fertilizer facility and it will remain closed for weeks while repairs are made – it occurred in the facility’s ammonia plant at about 9 a.m., but it was confined to the plant and was put out by plant personnel and the Fire Department) HOTWORK Fire damages Moorhead milk jug plant (firefighters quickly brought a fire under control at a company that manufactures plastic milk jugs – fire broke out at around 9 am – plant operations manager said the fire may have been started by a cutting torch – the fire was mainly confined to the building’s maintenance shop – the cause is under investigation) CONSTRUCTION CRANE FATALITY Two die in Brazil FIFA World Cup stadium accident (two workers, aged 44 and 42, were killed when a crane collapsed at a stadium – the workers were crushed to death when a crane hoisting a 500-ton metallic piece to the top of the roof collapsed – a third worker was rushed to hospital with injuries after the accident) FALL from SCAFFOLDING Construction worker injured in fall (victim was working on a construction project at the location of a new pharmacy and received head injuries as a result of falling off scaffolding – victim was then transported by helicopter to a Medical Center – no word on the victim’s condition was released) FATAL FALL @ SAME LEVEL on ICE Construction worker dies in accident on Thanksgiving (worker, 56, died in an accident on Thanksgiving morning – he was working on a townhouse construction project when he slipped and fell on ice frozen over some concrete on the site) One dead, two injured after retaining wall collapse at upstate New York construction site (a retaining wall gave way as concrete was poured, trapping one worker and injuring two others – it took hours to lift and secure the wall to recover the body of the dead worker – one of the survivors was flown to a medical center) FATAL FALL from LADDER Worker dies in fall from ladder in Maine (a roofing contractor fell off a ladder and died at the scene – workers were installing a roof on an addition to the one-story building, a 13,380-square-foot building that houses classroom, shop, office, and storage space) Miscellaneous CONFINED SPACE AUGER DOUBLE AMPUTATIONFarm worker loses arms in accident, two others badly injured (one farm worker lost his arms and two others were badly hurt in separate accidents – the first happened around 11 a.m. involving a man in his 20s who had suffered crushing injuries when his legs were caught in some machinery – his legs did not need to be amputated – second incident involved two men who somehow became trapped in a silo – one of the men may have fallen into the silo and got caught in machinery, possibly an auger – the second man may have been caught in the machinery while attempting to free the first man – the first man had both arms removed in the accident – despite his arms being amputated, the man ultimately managed to free himself from the silo – a colleague wrapped the man’s arms in towels and sent them with him in hopes they could be re-attached – the second victim had to be extricated by firefighters and was taken by land ambulance to a local hospital with severe leg injuries) SNOWBLOWER Slemon Park worker injured in snowblower accident (employee was seriously injured after his hand got caught in a snowblower – a worker was using a snowblower to clear snow – there was a severe injury to his hand – the man has been employed for 20 years) CONFINED SPACE FATALITY Two workers suffocate to death in MG Road sewer (two sanitation workers employed by a private firm suffocated to death in a sewage drain under while trying to repair a blockage – workers, both 27, went in without masks and the mandatory safety kits around 10.30am – fellow worker, who went in when they didn’t return after several minutes, and a local gardener tried to rescue them but choked on toxic gases and fainted – it was almost an hour later that a fire brigade team arrived at the site after being alerted by local residents and managed to recover all four and rushed them to a hospital – the two entrants were declared brought dead while the two would be rescuers were admitted to ICU) |
