INDUSTRIAL STRUCK BY FATALITY Norfolk worker killed in shipyard accident (worker was killed after being struck by a piece of equipment – a preliminary investigation found that the man was struck while unloading a barge at a contracted site) MINE FATALITY Another worker killed at Fortescue’s Christmas Creek mine (one worker, 23, has been killed and another seriously injured at a mine – another incident occurred at the operation’s surface mining work shop late last year – the two workers were injured in a crush incident – one was killed whilst the other was airlifted from the site) AMPUTATION Worker’s Arm Cut Off in Meat Cutting Accident (an employee at thebeef processors lost his right arm below then elbow when it came in contact with a saw – paramedics retrieved the severed arm, put it on ice, and took it to the hospital – no word available on the victim’s condition at this point or whether doctors tried to re-attach the arm) LOTO/GUARDING FATALITY in CONVEYOR Worker dies in accident at local Amazon facility (accident at the packaging facility left a temporary employee, 57, dead – officials initially determined he had been dragged by a conveyor belt while working – the victim was still alive when he was transported to the hospital by emergency workers and later died) Worker killed at Lyttelton Port (a watersider, who was aboard a fertiliser vessel when an accident occurred – the man suffered a head injury – the death is the second in the port within a month – a contractor was killed in a logging truck incident on November 26 – the 58-year-old died after being pinned between a logging truck trailer and a forklift on the port’s No 2 Wharf) Wind farm worker lost leg after it became trapped by faulty tractor (a worker, 20, lost a leg in an accident – he was trying to pour water from a bowser onto an overheating rock-crusher when he was crushed between two heavy items of machinery) LOTO Worker seriously injured when his hand becomes caught in conveyor belt (a 48-year-old self-employed contractor suffered severe injuries when his hand became caught in a conveyor belt – he was repairing a conveyor system inside a silo when the accident occurred at 1:24 p.m. – he suffered serious injuries to his right hand and was transported to a Hospital) PRESSURE TESTING Two Astoria city contract workers injured in accident (two workers were transported to a Hospital after an accident occurred while testing the water pressure in pipes – one worker will have surgery for his facial injuries following the accident – a piece of aluminum equipment struck him in the face – he suffered two broken cheek bones and a fractured skull – accident occurred just after 4 p.m. when staff were performing the final pressure tests on the manhole as part of the combined sewer overflow project – an inflatable plug, made of thick aluminum, was in place where a manhole cover is supposed to be – it holds back the water while the tests are performed – the plug deflated enough to blow out, sending the plug 30 feet up in the air – it struck the worker in the face on its way upward – it also knocked down a second worker, who was transported to a Hospital and treated for minor injuries) MINE TRAM Industrial accident under investigation in Windsor’s west end (a worker, 56, at a salt mine was injured while working underground – the incident involved a tram underground around 9 p.m. – worker suffered minor injuries) LOTO 18-year-old injures arm in machinery accident (a temporary worker at a glass recycling firm had his arm trapped between a rotating metal drum and a conveyor belt – the 18-year-old employee was asked to clean a machine used to separate glass from waste material – the machine was still running at the time, causing the arm to become trapped – he suffered a fractured arm that had to be rebuilt using bone from his hip and metal rods – HSE found that the machine was unguarded, and the firm regularly allowed workers to clean it while running as long as they were careful) CONFINED SPACE FATALITY Contract worker killed in accident at Belton asphalt plant (a 19-year-old man was killed when asphalt fell on top of him at the plant – he was a contract worker from outside of the area hired by another company to clean out the tank – he was inside the tank when he died – he was pronounced dead at the scene) CRUSHED BY Coca-Cola Worker Dies in Freak Accident (employee, 34, was killed in an accident after a colleague reversed a company truck towards a wall and crushed the deceased against the wall – the incident took place at about noon while workers were offloading a consignment at the plant) GUARDING/LOTO Telford worker’s hand crushed in factory accident (a worker suffered serious crush injuries after his hand became trapped in “mangle-like” machinery – he suffered multiple fractures when his hand became stuck between two rollers – colleagues working the night shift heard his shouts shortly before 2am and found the man, thought to be in his 40s, with his hand trapped in a machine which makes plastic sheeting – fire fighters managed to free the worker in about 15 minutes using hydraulic cutters and he was taken to hospital by ambulance) FIRE 4 killed in fire accident at AP rubber factory (four workers were charred to death in a fire that broke out in a rubber factory – according to preliminary reports, leakage from a pipeline supplying hot liquid chemical resulted in the blaze that spread to one of the rest rooms in the factory premises where the workers were sleeping – melted rubber fell on the workers and they were burnt alive – the pipeline passed atop the roof of the rest rooms and when the fire spread some workers escaped while four were trapped in the room which was engulfed in flames) FLASH FIRE w/ STATIC One person burned by flash fire at Lynn company (worker was mixing chemicals to make adhesives when there was a flash fire – static electricity was to blame for the accident, according to the District Fire Chief – he suffered burns to his hands and face and was taken to a Hospital and is expected to be okay – the fire was powerful enough to set off the building’s fire suppression system, which fire officials credit with saving the injured man’s life – business was evacuated – no one else was injured in the fire) UPDATE now a FATAL EXPLOSION Worker Injured in Bulgaria’s Lukoil Neftochim Dies (one of the four workers who were injured at an oil refinery, sustaining heavy burns, has died – the man, who suffered 45% 2-3 degree burns, has succumbed to sepsis – the 51-year-old man developed sepsis four days after hospitalization, after which the infection was suppressed, only to undergo a relapse) FLASH FIRE PSE&G worker suffers second-degree burns to face while repairing gas line in Union City (a flash fire during routine work on an underground gas line caused second-degree facial burns to a worker at around 9 a.m. – no injuries were reported – fire was quickly put out without the assistance of firefighters – exact cause of the flash fire has not yet been determined – the man who was burned was taken to a Medical Center) FLASH FIRE Injured worker flees with clothes burning from factory near Shoemakersville (a worker was injured in a small explosion and ran from the plant with his clothing burning – the man ran across Route 61 where a passing motorist tackled him into a snow-covered embankment – victim was flown by helicopter to the burn unit – he appeared to have burns to his head and upper body – victim told firefighters that he was working in the pump house and trying to use a lighter to ignite something when a flash fire occurred shortly after 1 p.m. – the fire was the third in four months at the business, a fabric-bleaching plant – the plant sustained significant damage and that a state police fire marshal was called to assist with the investigation) GASOLINE FIRE Worker suffers burns in Salisbury fire (a man was taken to a Hospital with burns to his arms from an industrial fire before 7:00am. – two people were in the premises when the fire started – crews had the blaze under control in about 20 minutes and out within an hour – building was extensively damaged and authorities now are worried part of it might be structurally unsafe – workers were draining a fuel tank when the fire started, but fire investigators are still formally determining the cause) HOT LIQUID Worker suffers burns at candy plant in Franklin (worker, 20, suffered burns to his face and chest while working on a machine in the kitchen at 7:10 p.m. – he stated that he was sprayed with a mixture of gelatin, sugar, corn syrup and water – the mixture was heated and dispersed at a temperature of 250 degrees Fahrenheit – he had first- and second-degree burns on his face and torso) FORKLIFT Falling container injures port worker (a forklift operator was injured after a shipping container fell on him at about 7.30am – driver suffered head injuries and was taken to a Hospital – he may have to undergo reconstructive surgery to repair shattered bones in his skull – the injured driver had pulled a stack of seven shipping containers down, with one crushing the cab of his forklift – the driver, aged in his 20s, managed to get out the cab by himself and was coherent when emergency services arrived – he had been with the company for three weeks when the incident happened) FATAL BOILER EXPLOSION Boiler explosion death toll rises to 9 (another worker injured in the boiler explosion on December 26 died at a hospital, raising the death toll to nine – eight other workers were being treated) UPDATE on FATAL EXPLOSION Explosion at Mexican candy factory ruled accidental (Mexican authorities have ruled that an explosion at a candy factory in two months ago was accidental – just before lunchtime on October 24 a powerful explosion rocked the candy factory, killing one worker immediately and seven others over the following few days – twenty-one people were injured, some with severe burns – explosion was the result of a chemical reaction involving corn starch and oxygen, according to a report issued this weekend by the Chihuahua State Attorney General’s office (FGE) – corn starch is a primary ingredient in the candy-making process) EXPLOSION Explosion in Hanover, Va. injures construction company worker (a man was injured when there was an explosion at a bay at a construction company facility – explosion happened at approximately 4 a.m. – officials continue to investigate the incident) PROPANE FLASH FIRE Maintenance Worker Injured When Fumes Inside Tanker Ignite (a maintenance worker, 54, was injured while fixing a propane delivery truck when fumes from the tank ignited, causing a minor explosion – he was working near the back of a tanker truck when the fumes inside ignited – co-workers were able to put the fire out but not before he suffered burns to his face, hands, and legs) One person injured in Delek Refinery accident (one person was injured after an industrial accident around 10:30 a.m. at the refinery – an employee was working with a machine when an issue occurred causing the employee to receive damage to his head – he is currently at a local hospital being treated for his injuries) SULFUR DIOXIDE 11 treated after gas leak at LA refinery (a leak at an oil refinery has exposed nearly a dozen workers to sulfur dioxide gas – the leak was reported at about 10 a.m. – 11 people were treated for possible breathing problems – they were taken to the hospital in fair condition – after about an hour, authorities reported there was no active leak and no danger to the community) REFINERY EXPLOSION No injuries reported after explosion at Regina’s Co-op Refinery (explosion and resulting fire took place in a large tank in the propane-butane processing unit – no one was injured in the explosion at around 3:30 p.m. – there were people working in the area at the time, all escaped unharmed – those workers, it’s not yet clear how many were there at the time, were cleared out of the area while plant firefighters suppressed the flames – no estimate on damage or how production might be affected) WWTP Man killed at FW water treatment plant is identified (a worker, 54, was killed in an industrial accident at a water reclamation site – the accident happened while the worker was delivering a load of liquid food waste – other workers at the unit discovered the victim – treatment plant wasn’t damaged in the incident – further details have not been released) GUARDING/LOTO w/ CONVEYOR Woman injured at Terra Bella pistachio plant (a woman, 45, was injured when her arm became entangled in a conveyor belt at a pistachio plant at about 3 p.m. – her left arm had to be extricated from the machine) OIL TANK EXPLOSION Explosion reported at Sweetwater plant (fire officials think the explosion was caused by a frozen valve on an oil tank – no injuries were reported – explosion occurred about 2 a.m. – fire officials are allowing a second tank to burn out, and they are monitoring the fire) CONSTRUCTION STRUCK BY FATALITY Construction worker dies in accident at Chicago-area shopping mall (a worker, 25, is dead after he was struck in the head with falling concrete at a mall on the day after Christmas – he was one of three members of a demolition crew hurt while working at a restaurant – crew members were removing brick from a wall when concrete pieces fell from the top of the wall) FATAL FALL Construction worker falls to his death at site of new Sharyland district high school (a construction worker, 47, fell at the construction site of the new district high school) STRUCK BY Construction Accident Leaves Worker With Life Threatening Injuries (a construction accident has left a worker with life threatening injuries – accident happened at a home construction site at about 7:50 a.m. – the construction worker was struck by a vehicle – a construction vehicle may have rolled over on the worker) FALL Construction worker hospitalized after equipment falls from Fort Worth bridge (a construction worker was rushed to the hospital after a piece of equipment he was working on fell 50 feet from the under-construction bridge off – worker was helping pour bridge decking at the time of the accident just after 1 p.m. – worker was taken to a local hospital by MedStar with serious, but non-life-threatening, injuries) WORKZONE INDOT Worker Seriously Injured in Accident (DOT worker, 48, was injured while working along State Road 62 – INDOT was dumping a load of rock in the driving lane which would later be moved into a nearby ditch – that lane was closed to traffic but a one-foot diameter rock rolled into the passing lane and was struck by a westbound grain truck – the rock bounced back toward the driving lane, hitting the worker, who was taken to a Hospital with a serious leg injury) ANGLE GRINDER Investigation launched after university construction worker sliced arm with angle grinder (worker suffered horrific injuries when he sliced into his arm with an angle grinder – the construction worker had been working at the University site when it’s thought he somehow lost control of the power tool – shocked passers-by saw the man in the street with the tool still stuck in his arm as a colleague flagged down a passing police car on Saturday at about 12.15pm) ELECTROCUTION Construction worker electrocuted (a construction worker was electrocuted and another sustained injuries after the two came in contact with a live wire at a work site – they were working as barbenders at the third floor of a building – the long iron bar which they picked up came in contact with a high-tension wire next to the building – both suffered severe electric shock and were rushed to a Hospital – one succumbed to his injuries – the condition of the other is stated to be critical) HOTWORK FIRE Hydro Arena blaze was caused by a welder’s torch, an investigation has revealed (fire was caused by a welder’s torch setting fire to insulation, an official investigation has confirmed – fire took hold as the 12,000-seat stadium was still under construction – investigation found a welder working on a gutter running round the building noticed “ignited material” dropping onto insulation below, which started the blaze – a five-hour operation to battle the fire and secure the arena – report concluded the fire, which caused up to £5million in damage, was started accidentally – contractor (welder) stopped work on smelling smoke and went to investigate the source) HOTWORK FIRE Welders sparked underground fire in Kakaako (a smoldering underground fire was caused by welding – a 4 by 6 foot piece of lumber, part of the shoring for underground work to replace the sewer lines, caught on fire when a piece of hot metal slag fell into the hole while workers welded metal plate shut – firefighters poured water through holes in the metal plates, before construction workers used heavy equipment to remove the plates so that firefighters could extinguish the blaze) FATAL FALL Construction worker at Texas school dies in fall (a construction worker has died after falling about 20 feet at a high school – scaffolding was up at the site at the time of the accident – investigators did not immediately release details on where the 47-year-old man fell or other circumstances of the accident) CRANE Crane collapse at hospital construction site (a construction crane and the truck it was attached to toppled onto its side at the site where an annex to a Hospital is being built – the operator of the crane was able to jump out of the cab before the crane tipped over – he suffered minor injuries to his legs and was taken to a hospital – it is not clear what caused the crane to topple) Miscellaneous ELECTROCUTION Florida tree trimmer dies from electrocution after accident with power lines (a landscape worker was killed instantly when he accidentally touched a tree saw to a power line – a second worker was also electrocuted but is recovering – around 10 a.m. a crew of four men from a Landscaping business was trimming trees – one of the workers, 22, accidentally touched a tree saw attached to the end of a metal pole to a nearby powerline – doing so grounded him and the voltage in the electrical wire killed him – he died instantly – another worker was on ladder touching the tree and was also shocked, but is recovering at the hospital and expected to survive – two other co-workers were not harmed but shaken by what they saw) Storage facility worker crushed to death by 100-foot-pile of rock salt (the man was moving rock salt with a front-end loader at a storage facility when a pile gave way and fell on him – a 100-foot tall pile of road salt collapsed and crushed him – accident occurred at a Salt storage facility at around 3:30 p.m. as crews prepped for the massive snowstorm has blanketed much of the Northeast – a giant pile of the substance gave way and fell on him, burying him in the machine – emergency crews scrambled to the scene and used shovels to dig a path before hooking the machine to a truck and hauling it out – he was still in the driver’s seat – he was pronounced dead at the scene – it appeared he suffocated, but an autopsy would be performed to determine a cause of death) Worker dies repairing heavy equipment near Fort McMurray (worker, 22, died in an accident during a service call – he was called out to repair a tire on a piece of heavy equipment – during the repair, the tire flew off the rim and a ring from the tire assembly struck the worker, causing fatal injuries) STRUCK BY FATALITY Logan Airport worker’s death likely an accident (it appears the death of a private company worker, 59, at an international airport was an accident and not the result of a criminal act – he and a co-worker were working on the tarmac at the north cargo area around 7:30 p.m. – the co-worker was behind the wheel of a Ford F-350, modified for work at an airport, and started driving the truck in reverse – he standing behind the truck, was struck by the vehicle and suffered fatal injuries at the scene – the driver was overcome with grief and shock and was rushed to a Hospital) CRUSHING FATALITY Worker Killed in Apparent Industrial Accident (a worker was killed when a box truck fell on him – accident apparently taking place in the body shop area of the business) FARM TRACTOR FATALITY Manure accident claims one life (an employee on a dairy farm died in a farm accident – he was reportedly scraping manure into a lagoon with a tractor when he got too close and tractor went in – the safety chain, which is supposed to stop anything from getting into the lagoon, gave way and the tractor ended up on top of him in the lagoon) FORKLIFT Man ‘critical’ after accident at farm (the man fell from a forklift truck or a tractor and that a heavy load the vehicle was carrying then landed on top of him around 3.55 p.m. – he was in a critical condition as a result of the injuries he suffered) ELECTRICAL FIRE INJURIES Vacuum Cleaner Sets Texas Mall Worker’s Clothes On Fire (worker, 39,who was using a backpack-type vacuum cleaner at a store was seriously burned when the cord shorted and set her clothing on fire – the contract-worker, who received second- and third-degree burns to her back, was taken to a hospital burn unit) FORKLIFT FATALITY Man Killed in Forklift Accident Near Idaho Capitol (a man, 51, has died after being pinned beneath a forklift behind one of the state Capitol buildings – it appears the man was operating a forklift as it was being unloaded from a trailer and that he fell out of it when it tipped over at about 9:50 a.m. – he and two co-workers were at the state building to move furniture when the accident happened) |
