INDUSTRIAL TRUCK UNLOADING FATALITIES Two workers killed by granite slabs in San Francisco (two workers died after being trapped under giant slabs of stone that fell on them as they were unloading a truck – the wooden supports that were holding the slabs in place may have been knocked loose in transit – firefighters arrived to find a manager and another worker trapped under giant stone slabs that apparently tumbled out of a shipping container) CONVEYOR Amazon Worker Was Hospitalized After Warehouse Accident (a few days after a third-party temp worker died at a facility, a woman working as a packer at a new distribution center was hospitalized due to a workplace accident – the female worker was injured after getting her hand caught in a roller on a conveyor belt – she was taken to a hospital emergency room, and later admitted – she did not lose her hand, but did “need skin treatment”) ELECTROCUTION Oil field worker dies in accident near Artesia (an industrial accident has killed a man, 28, after he was electrocuted when a hoist came into contact with overhead electrical wires) MINING FATALITIES At least eight dead in South African gold mine incident (rescuers recovered eight bodies and continued to search for another missing worker today after a fire and rockfall at a Gold mine – initial reports were that the fire was triggered by a small earthquake – it is the most serious accident in South Africa’s mines since nine workers died in a rock fall at a platinum mine in July 2009) GUARDING/LOTO FATALITY w/ CONVEYOR Utah Mine Worker Trapped In Conveyer Belt Died (a contract worker, 56, died at a mine after speciously getting stuck in a conveyer belt – despite several efforts of saving his life, he died at the scene – he was working alone at the time) TRAIN MAINTENANCE Rail worker who was hit by a train at Newark Northgate Station dies from injuries (a rail worker, 49, who was struck by a train has died from his injuries – he suffered injuries to his face, chest and pelvis when he was hit at a Station at 11.39 a.m.) NUCLEAR PLANT Worker Dies on Refueling Floor of Cooper Nuclear Plant, Natural Causes Suspected (a worker at a nuclear plant died, although the circumstances suggest he died from natural causes – the control room received a report at 7:38 a.m. that a radiological technician was found unresponsive on the reactor’s refueling floor – EMTs and sheriff’s office personnel arrived on site but found that the man had died – although he was working in a contaminated area, he was wearing appropriate safety gear and contamination was not found on his body – a release from NPPD said the death was believed to be the result of natural causes) FATAL BOILER EXPLOSION Worker killed, three hurt in factory boiler explosion (worker was killed and three other people sustained multiple injuries when the boiler of a factory exploded – workers told reporters that the boiler of the cardboard making factory exploded due to excessive pressure) HOTWORK Fire at cement mixing plant in De Soto (Workers at a cement mixing plant accidentally started a small fire while attempting to thaw out frozen pipes using a blow torch – the fire occurred around 9:25 a.m. – insulation in the building caught fire, spreading to some of the wooden studs in the walls) CHEMICAL LEAK and TANK FIRE Ecolab plant accident near Martinsburg prompts changes for first responders (a fire and subsequent leak of glacial acetic acid from a 10,000-gallon, above-ground tank prompted authorities to halt trains on CSX Corp.’s nearby rail line and evacuate residents from a town-home development downwind from the plant as a precaution – railroad service was halted for about 25 minutes – no one was injured, and operations resumed at the plant hours after the county’s emergency dispatch center alerted responders of a reported fire on a tank – the fire started in the insulation of the tank as a result of a heating system malfunction and was blamed for causing the leak) Coal plant accident spews coal ash into North Carolina river (tens of thousands of tons of coal ash and tens of millions of gallons of polluted water have burst out of a shuttered power plant – the power plant operated from 1949 until 2012, and the coal ash being stored on site was residue left behind after coal was burned – a state agency and environmentalists have been suing the company in an effort to force it to clear out 14 such coal-ash dump sites across the state, including the one that ruptured) EXPLOSION Small Explosion, Fire At Mesquite Manufacturing Plant (two workers escaped a small explosion and fire without injuries around 7 a.m. – a loud popping sound was heard and smoke was seen at the facility’s research and development lab – a fire in one of the lab rooms was extinguished within 30 minutes – the lab was not open at the time, however, two members of a cleaning crew were inside the building – both made it safely out – the damage to the plant was confined to the lab area of the manufacturing facility – solvents were present in the lab, but exact cause of the fire is unknown at this time) EXPLOSION Nine injured in Chinese corn plant explosion (at least nine people were injured in an explosion at a corn processing plant at about 9:40 a.m. – the injured, including three in critical condition, have been sent to hospitals – investigation into the cause of the explosion is under way) EXPLOSION Tonawanda Coke Says Cold Temperatures Caused Explosion (multiple gas line backups caused by “extremely cold temperatures” were responsible for the explosion at the coke plant – the additional pressure created a disk rupture on a manifold underneath the coke oven battery – the coke oven gas underneath the battery then combusted, causing an explosion – a small fire burned for 12 minutes before it was extinguished – no one was injured in the incident) AMMONIA Ammonia leak at Pryor Chemical (reports of an ammonia leak at a chemical plant caused a temporary evacuation of several plants in an Industrial Park – cause of the leak is still under investigation – authorities are able to confirm it was a small quantity of anhydrous ammonia that leaked from an outside release valve – the valve was located in the area used to fill trucks – officials on scene speculate the valve may have burst – roads were blocked off to prevent drivers from entering the park before it was declared safe and several plants and companies sent their employees home – reportedly transported a male, 60, to a landing zone so he could be lifeflighted to a hospital) AMMONIA Ammonia leak, fire shut down beef processing plant in eastern Nebraska; no employees injured (an ammonia leak and fire have shut down a beef processing plant but no employees were injured – workers detected the ammonia leak around 3:40 a.m. – smoke and flames were reported several minutes later above an area where beef carcasses are trimmed to smaller cuts – local authorities responded and the fire was quickly extinguished – damage was limited and very little product was lost) AMMONIA Ammonia leak causes street closures (an ammonia leak at a cold storage facility forced the shutdown of some city streets at about 6:30 p.m. – “They’ve been attempting repairs and, unfortunately, it didn’t get repaired initially as they had hoped”) CHLORINE Chlorine leak causes talk of revamp (a slight chlorine leak was detected around 10 a.m. – a simple equipment failure that could have been serious when a canister in the tower by an Elementary School was leaking chlorine – students were evacuated as a precautionary measure – this small incident, though harmless, has caused officials to take a good, hard look at the way water is treated so close to a school) FORKLIFT Great Yarmouth: Man trapped by pile of bricks in forklift accident taken to hospital (a man who became trapped when a pile of bricks fell on top of him in a forklift accident was taken to hospital with a broken leg just before 3.45pm – the man, believed to be in his 40, was “conscious and alert” when paramedics arrived but said his leg was “clearly broken”) FORKLIFT Man injured in forklift accident in Loxahatchee, flown to hospital (a worker, 47, was seriously injured in a forklift accident around 3:38 p.m. – forklift rolled over and the driver was injured – the man had suffered injuries that met trauma alert criteria and he was flown by Trauma Hawk to a Medical Center) CONSTRUCTION CRANE FATALITY CONSTRUCTION WORKER DIES OUTSIDE WORLD CUP STADIUM IN BRAZIL (a worker, 55, was killed in an accident outside the World Cup stadium – the man died while dismantling a crane used to install the roof of the stadium – it was the third death of a worker in accidents in or near the stadium – he sustained head and chest injuries when he was hit by a metal bar) CRANE w/ FALL Crane Malfunction Sends Construction Worker Plummeting From Transbay Transit Center Site (a worker was injured when he fell from a platform at a construction site at about 8:10 a.m. – the worker was struck by a crane and fell about 12 feet – the crane malfunctioned and part it struck the worker, knocking him from the platform – worker did not suffer any broken bones and is expected to be released from the hospital later that day) TRENCH FATALITY Long-time Natick DPW employee killed in ‘freak accident’; co-worker injured (employee, 48, was killed and a second town employee was injured while both were participating in the emergency repair of a water line – incident was witnessed by four co-workers and a police officer -the deceased employee worked for the town’s Department of Public Works for 26 years and held the top union job of general foreman at the time of his death – the second worker was a foreman who was rushed to a Medical Center, where his condition has been upgraded to stable – the incident took place around 7:15 p.m. when six members of the town DPW went to repair a water line break – a backhoe was accidentally pulled forward, and the stabilizer of the backhoe struck two employees who were working in a trench) FALL thru HOLE Worker injured at Maple Leaf Foods plant (a construction accident at a foods plant injured a worker when he fell through a floor, dropping nearly seven metres – the man was in stable condition following the accident) An investigation is underway after an accident at Tyson Food in Dakota City, Neb. (an investigation is underway after a construction worker is injured in an accident – a construction worker was working on the new addition at the time of the accident – worker was flown to a Medical Center) EQUIPMENT FATALITY Construction worker dies in accident on the job in Kona (worker, 38, had been operating a single drum roller while constructing a private driveway when the vehicle rolled backwards down a steep slope and crashed into a carport at a neighboring residence – worker was thrown from the roller and sustained injuries – he was transported to a Hospital in critical condition and was pronounced dead at 4:15 p.m.) TRENCH FATALITY Utility Worker Killed in Trench Collapse (a work place accident has killed one construction worker – the man was working in a trench to repair a cable line that had accidentally been cut by a nearby construction crew – the man was doing repair work on the line in a 15-foot deep hole when the sides collapsed on top of him – he died at the scene – rescue teams responded to the accident and it took over eight hour to recover the man’s body) TUNNEL COLLAPSE FATALITY 1 construction worker killed in Yozgat tunnel collapse (one person died and three were injured while drilling a tunnel at the construction site of a high-speed rail – worker, 23, died trapped under the wreckage of the tunnel – the injured said the structure collapsed all of a sudden) FORKLIFT City woman’s car collides with forklift (no one was injured when a car collided with a forklift – driver was traveling between two parking lots about 9 a.m. when the accident occurred – vehicle collided with a forklift that was heading north – the driver of the forklift said workers had stopped northbound traffic on Sixth just before the collision – he was heading for a work site when the accident occurred – no one was cited)
Miscellaneous Lower Swatara Township worker injured in leaf pickup accident dies (a man, 27, has died of injuries suffered last fall while he was working on a leaf pickup crew – he had been injured Oct. 30, 2013 around 1:27 p.m. while working with a township leaf pickup crew – he was run over by the rear wheel of the leaf vacuum machine, which was being towed by a dump truck – the dump truck, which was driven by another worker, was in the process of moving to another location when the accident occurred – the deceased and another worker, who had both been operating the leaf picker, were walking to the other worker’s pickup truck to drive to go to the next location when the accident happened) CONFINED SPACE FATALITY – ENGULFMENT Man dies in grain bin accident (a man, 77, died after apparently being accidentally buried in a grain bin at 4:40 p.m. – he was missing and was believed to be in a grain bin that was having its contents removed – workers told deputies that he had been in and out of the grain bin and was last seen around 2 p.m. but was believed to have left with his wife – when the wife returned and workers realized e wasn’t with her, they began to search the property and then called 911 when they couldn’t find him – the grain bin was emptied, a process that took 6½ hours, and he was found inside – the county coroner pronounced him dead at the scene) FATAL FALL from EQUIPMENT Fairbanks DOT worker dies in fall at Dalton Highway maintenance camp (a worker, 64, died after falling from a grader onto concrete at the DOT camp at 6:55 a.m. – he succumbed to his injuries and was pronounced dead by medics at approximately 10:30 a.m.) FATAL EXPLOSION of TIRE Maryland worker dies after tire explodes while being filled with air (worker, 31, was killed after the tire of a truck exploded in his face – he had been working at a trailer sales company when the accident occurred – it was reported that he was kneeling between the two front tires of a farm tractor, filling one of the tires with air – the tire unexpectedly exploded, knocking him to the ground – he was pronounced dead at the hospital shortly after he arrived there) MOBILE EQUIPMENT FATALITY Worker Killed Setting Up For World Ag Expo (a worker died around 1:30 p.m. while setting up for the event – he was on a piece of equipment that was moving, when he fell off – he was then run over by another piece of equipment) ELECTROCUTION Tree service worker electrocuted in St. Pete (a tree service worker died when he was electrocuted as he came in contact with a power line at about 12:27 p.m. – crews found an unconscious employee of a tree service hanging in a tree by his safety harness, about 40 feet above the ground – the fire crew freed the man, who was in cardiac arrest and started CPR – he was taken to a Medical Center where he was pronounced deceased) CRANE Crane collapses onto two Lakewood garages (a tree-trimming service was working in a back yard when a crane collapsed – the crane fell on two garages, but police said the damage was minor and there were no injuries – incident was first reported around noon, and the crane had been removed by 2 p.m. – two garages behind houses had dents on their roofs, but the damage didn’t appear extensive – it was the second crane collapse in Northeast Ohio in as many weeks – on Jan. 30, a crane toppled over in a parking lot and crushed several cars) |
