struck by heavy piece of equipment, part of an operation of the oil and gas industry) FATAL FLASH FIRE Man dies after flash fire at Sherwin-Williams plant in Greensboro (worker, 48, died from injuries from a flash fire involving chemicals – the fire happened in a workroom, according to a fire incident report – the report lists the cause of the fire as undetermined) CRUSHED New York Worker Injured by 900 lbs. of Glass (worker, 25, is hospitalized after nearly a half ton of glass fell on him while he was working at a home improvement business – a dozen sections of tempered glass weighing about 900 pounds each fell on him, causing serious injuries – a co-worker found him unconscious with glass on top of him – he was airlifted to a Hospital where he’s in stable condition) CRANE FATALITY Worker Ejected From Crane, Killed On Job Site (a man was killed around 5:45 a.m. at the facility – he was operating a crane that tipped over – he was ejected from the crane and declared dead on the scene – no other injuries were reported – other details surrounding the incident weren’t immediately clear) FATAL EXPLOSION Mersen USA blames human error, incorrect placement of materials, for fatal Pennsylvania blast (human error caused an oven explosion that killed a visiting employee, 52, and injured another worker, 58 – “incorrect placement of materials in an otherwise safely operating curing oven” caused the explosion) Woman Flown to Hospital in Accident at Pepsi Cola Factory (a dispensing machine fell on top of a woman – she’s in critical condition – at this time, it’s not clear if the woman was a worker at the company) UPDATE on FATAL DUST EXPLOSION Babine Forest Product fined $1M for deadly Burns Lake sawmill blast (a fiery explosion at a sawmill that killed two workers and hurt 19 others has resulted in what’s believed to be the largest penalty imposed on an employer in provincial history – a report on the cause said the blast was preventable, but no charges were laid after the Criminal Justice Branch declared that flaws in the original investigation greatly diminished the likelihood of a criminal conviction – WorkSafeBC charged a penalty of $97,000, and a claims cost levy of almost $915,000 – the actual claim cost associated with the incident is nearly $5.2 million) Industrial accident leaves factory worker hospitalized (a worker suffered injuries to his head and hands and was taken to a Hospital – he has worked with the company for seven years was doing his normal tasks when the accident occurred – no one saw exactly what happened – victim is still hospitalized and has had at least one surgery) Worker dies in accident at St-Jean plastics plant (worker, 23, was hit in the head with a security gate at 8:45 p.m. – according to preliminary reports, for some reason the man chose to crawl under the gate and it came down, striking him – when ambulance technicians arrived, the worker was unconscious – he was rushed to hospital where he died) LINE BREAK FATALITY (Phenol) An employee at a chemical plant was killed on the job (an employee was fatally injured while working in the bisphenol-a (BPA) plant about 10:30 p.m. – company is not releasing any information about what caused the death, but does say that its on-site emergency response team responded to the accident and started providing treatment following their strict chemical exposure procedure – no other employees or contractors were injured in the accident – Family members say he was not working his usual site and had gone to help fix a pipe that was stuck which ended up bursting – the family says he was not in his safety suit because when working at his actual site it is not needed) HOTWORK FIRE Fire breaks out on navy tugboat INS Matanga (a fire broke out at the naval dockyard – no casualties have been reported yet in the fire – this is the 14th mishap involving a naval vessel in the last eight months – the fire broke out when naval dockyard workers were carrying out hot work (welding) on the tug – the tug was parked in dry dock area and fire tenders rushed to the spot to douse the fire) CO2 CYLINDER FATALITIES Four More Shipbreaking Workers Die in Bangladesh (four shipbreaking workers were killed and three others were critically injured when a gas cylinder exploded in a shipbreaking yard – the four workers died after inhaling carbon dioxide – the three injured workers were rushed to hospital) MINE Mine safety officials investigate accident at Greenbrier County mine that injured 8 workers (state and federal mine safety officials are investigating an accident that injured eight workers at a mine – a mantrip, a vehicle that transports miners underground, lost its brakes on an incline – three workers were treated at an area hospital and released – two workers remain hospitalized with non-life threatening injuries – three others declined medical treatment) FLASH FIRE & STATIC Fire breaks out at CCF Industries in Washington Township (at least 30 workers were inside when the fire was reported around 7 a.m. – witnesses also reported hearing explosions – business makes dovetail drawers, and had about 200 gallons of chemicals including acetone and wood-finisher in the building – no injuries have been reported – exact cause is still being investigated, but firefighters say it may have been caused by static electricity) LNG EXPLOSION Five hurt in natural gas explosion (an explosion rocked a natural gas processing plant injuring five workers, causing about 400 people to evacuate from nearby farms and homes, and emitting a mushroom cloud of black smoke that was visible for more than a mile – the 8:20 a.m. blast sparked a fire and punctured one of the facility’s two giant storage tanks for liquefied natural gas – fire at the facility was extinguished within a couple of hours – another four people were taken to local medical facilities – none of the injuries was believed to be life-threatening – the tank that punctured was about a third full – the 14 employees working at the time were all accounted for – too early to determine the extent of the damage or the cause of the explosion – the pipeline was shut down in the area of the storage facility, but was still carrying gas on other stretches) FIRE Fire injures worker, destroys potato plant (a fire ripped through a potato-storage facility and injured one worker – damages could reach up to $3 million – fire crews were called to the property about 12:25 p.m. after reports of an explosion – a worker was applying chemicals used to prevent the potatoes from sprouting while in storage when he suddenly noticed his hose was on fire) ELECTROCUTION Worker found dead at Thai shrimp farm (a worker at a shrimp farm discovered a fellow worker, 25, dead – he did not did not return from his morning work routine – his body was lying face up in a shrimp-feeding boat that was floating next to a generator used to power the aerators in the pond – at this stage, we believe he was electrocuted) CONFINED SPACE FATALITY w/ H2S Hydrogen sulfide kills four, injures four in Phuket sewer (four workers were killed by hydrogen sulfide fumes in a sewer and four others remain hospitalized after attempting to rescue them with inadequate equipment – the four who died had been tasked with maintenance of a sewer access point and they succumbed to the fumes one by one, trying to rescue each other – the first rescue worker on the scene, 25, held an oxygen mask to his face as he descended into the sewer to recover the men – the mask failed to protect him from the fumes and he started to climb back out, but was overwhelmed and fell in, hitting his head on the side of the manhole – other workers used fans to dispel the gas and managed to pull him out in about five minutes, but were weakened and unable to retrieve the other four – using the masks, rescuers were able to secure rope to the four workers and use a crane to pull them out of the drain – “It only took about 10 minutes to get them out, but by then they had been in the drain for almost two hours. Tragically, they all died” – the four rescue workers remain in the hospital – the first one is unconscious and the three others are recovering – when a co-worker saw that he had fallen unconscious, he descended the ladder to offer assistance and also fell prey to the fumes – the two remaining men, one a relative, also entered the drain in an attempt to help and were overcome – two other workers realized the danger and called for help) UPDATE on Nitric Acid Explosion Rolls-Royce Working To Determine Cause Of Chemical Explosion (company says it’s working to determine what led to the chemical explosion, injuring nine workers – eight workers who were hospitalized after the incident were released that same evening – a ninth worker was checked out at the scene – the explosion occurred during the transfer of nitric acid – a chemical cloud filled the area and about 500 employees evacuated the facility as firefighters contained the spill and cleared the building of dangerous fumes) FATAL HOTWORK FIRE Two die as oil tanker catches fire (the driver and the conductor of an oil tanker died when it caught fire during a maintenance process at a gas station – the loaded oil tanker caught fire during the welding work) UPDATE on BIOFUEL PLANT FIRE Mississippi Biodiesel Plant Fire in January Ruled Accident (state fire marshal has ruled an accident the January fire and explosion at the Biofuels plant – the fire marshal found the likely cause of the incident was a problem with a thermal oil heater or pump used in the distillation process to produce biodiesel – investigators couldn’t determine an exact cause for the fire and subsequent explosions because of extensive damage to the area of the blast – estimated damage to property and contents was $3.5 million – an employee walked out to take a sample from one of the chicken fat container tanks, he heard a hissing sound around 5:45 a.m. and saw an orange glow from one of the chicken fat tanks – while he didn’t see a fire, he told investigators he knew the glow was a fire – he then ran to a trailer where another employee was sleeping and woke him up – as they ran, the plant exploded) AMMONIA Warehouse evacuated after ammonia leak (a freezer storage warehouse was evacuated after an ammonia leak was detected in the building’s cooling system – no injuries were reported, and everyone in the immediate area was evacuated) FORKLIFT FATALITY Warden killed in accident (worker was killed in an industrial accident – he was installing electrical service in a vacant warehouse when he was caught between a forklift and a pole) CONSTRUCTION WORKZONE FATALITY Storm clean-up worker dies after avoiding falling tree (a worker, 34, employed by a contractor to remove storm debris from Interstate 26 was cutting tress in the median – when the tree started falling, the worker ran into the roadway, crossed the left lane and into the right lane where he was struck by a 2006 Honda SUV – the worker was taken to a Medical Center where he later died – he driver and passenger in the SUV didn’t sustain any injuries) HOTWORK FIRE Fire chiefs say the high rise blaze in Southampton was accidentally caused by a heat gun (fire chiefs have confirmed a devastating fire that brought the city to a standstill was started by accident – the fire was a result of a construction worker using a heat gun, which reacted with bitumen and insulation material – roads surrounding the blaze were cordoned off by police for more than three hours as firefighters ascended the 15-story high building and tackled the fire) FATAL FALL Man dies at scaffolding outside a hotel plummeting eight stories to his death – the man believed to be in his 40’s landed on top of a sidewalk shed covering the front entrance of the luxury hotel around 2:10 p.m. – the man who died fell through a spot where a missing plank should have been) FATAL FALL from ROOF Worker dies after falling through roof in the Heights (a man died after he fell through the roof of a building around 11 a.m. – worker was putting materials on the roof when he fell – officials did not say what he was doing on the roof at the time of the accident) SCAFFOLDING 2 men safe after scaffolding gives way on high-rise building (two workers were repairing the stucco on the building at the time of the accident – they were adjusting the level of the scaffolding when one of the motors failed, causing it to tilt and dangle about 30 stories above ground – one of the men jumped onto the balcony as the scaffolding started to fall – he threw a rope to his co-worker and pulled him to safety) FATAL FALL Three killed in Turkey construction site collapse (three workers were killed when they fell while working on a viaduct – the workers fell from a height of 50 metres – 11 were rescued by a crane when they were stuck in the viaduct – accident happened when part of the concrete on the viaduct, a bridge composed of several small spans for crossing a valley or a gorge, collapsed during construction) FATAL EXPLOSION Four Chinese workers killed in Ecuador hydro plant blast (an explosion at the construction site of a hydroelectric station killed four workers – the four were working in a vertical shaft when the explosion occurred, and there were no other casualties in the accident – the men were explosives experts and the cause of the explosion was under investigation) EXPLOSION Worker injured in explosion (a worker was injured during an explosion that occurred in a cube truck outside an apartment building – a company was undertaking trenching on the hospitalized (two workers, 26 and 36, were electrocuted and another hospitalized with severe burns, after a metal ladder which they were using came into contact with a high voltage wire – the two were attempting to drag a large fiberglass and metal ladder through a window when the incident occurred) FATAL HOTWORK FIRE Boston Officials: Sparks From Welding Work Caused Fatal Fire (sparks from a welding project for which no one had obtained a permit caused the wind-driven fire that tore through a brownstone last week, killing two firefighters and injuring 13 – investigators are confident the fire was unintentional, but they have not decided whether anyone will face charges – it did not appear anyone had obtained a permit for the work on an iron railing in back of an apartment building next to the one that burned – the sparks somehow got into the building and the fire, fanned by strong winds, spread quickly – officials declined to discuss specifics of the case, including the scope of the welding work, what firm the welders worked for, whether they called 911 to report the fire or whether they are cooperating with investigators – permits are required for all welding projects in Massachusetts and most also must have someone from the fire department watching to make sure they are being done safely – no such fire detail was assigned to the project – the company that owned the adjacent building where the welding took place, said in a statement that it hired an unnamed third party to install the railing and thought it was going to be built offsite – company would not comment killing a County worker – the worker, 58, was pinned against a parked trailer by the car – he was flown by medical helicopter to a Hospital where he died) SKI LIFT WORKER FATALITY Lift worker killed in skiing accident ID’d (a lift operator, 47, was skiing by himself at the time of the accident – he died after suffering multiple chest injuries – the skier who found him called Ski Patrol at 1:50 p.m. – he was unconscious and unresponsive when patrol arrived at the scene) CHAIN SAW Tree Trimmer Survives Chainsaw Lodged In Neck (a tree trimmer, 21, was seriously injured on the job – he was struck in the neck with the chainsaw – a co-worker then climbed the tree and brought him down – police arrived at the scene where they found the blade stuck in his neck and shoulder – he had surgery to remove the blade and is recovering – the chainsaw just cut into the muscle, missing his major arteries) DOCKING Two Dock Workers Dead In Boating Accident (an accident involving a cruise ship and a launch boat left two dock workers dead – both were in a launch boat that capsized in the berthing area of the cruise pier around 5pm – somehow the vessel reportedly got pulled under the currents of the huge liner as it pulled out from the pier and both men disappeared under the water) LANDSCAPE FATALITY w/ EQUIPMENT Greene County Worker Killed Monday Identified (worker, 33, was killed in an equipment accident – he was working on a landscaping project when he was struck with a piece of equipment on a Bobcat tractor – the worker reportedly was shifting positions in the vehicle when he was struck by a piece of the equipment) HOTWORK Welding accident causes garage fire in Groby (a car inside a garage was damaged after a fire was started by a welding torch at about 7.40 pm – no-one was injured in the incident) CHLORINE Chlorine leak sends 17 to hospitals (an apparent chlorine leak at a hotel led to 17 people being transported to area hospitals for evaluation – no one had life-threatening injuries – firefighters were called to the hotel because someone was having breathing problems – rescue workers learned as many as 20 people may have inhaled chlorine and transported
