INDUSTRIAL

UPDATE Death Toll in Factory Blast Rises to 8 (death toll in a chemical plant explosion has risen to eight after authorities pulled out the last remaining body – the explosion also injured nine workers – cause of the explosion has not been established – plant, built in 1997, produces palm oil)

PROPANE FLASH FIRE Idaho plant worker injured in flash fire (a worker was injured when a flash fire ignited at a white pumice extraction and refining facility – worker received second-degree burns and was flown to a hospital – a vaporizer that converts liquid propane into vapor had a small leak and caused the brief but intense fire – the worker was in a three-man crew about to leave the plant – the other two men were uninjured – a safety valve popped off, causing the propane leak)

FATAL FALL from a FEED MILL Worker falls to his death in Huron Co. (a work accident killed a man, 58, who was doing contract work at the grain elevator – he was dismantling a feed mill when he fell 60 feet through an opening and landed on the ground – he died at the scene)

MULCH ELEVATOR 2 workers hurt in central Pa. elevator accident (an industrial accident left two workers hurt shortly after 1 p.m. – the two employees were operating a mulch elevator that had a mechanical failure – the accident left one man trapped – co-workers used a front-end loader to pry open the elevator and get the man out – the man who was trapped was taken to the hospital for serious injuries but no additional details were available – the other employee suffered a blow to the arm but is said to be doing fine)

LOTO/CONFINED SPACE 16 workers hospitalized after Batangas plant accident (sixteen workers were hospitalized after being exposed to poisonous chemicals in a pipe at a plant – an initial investigation showed the workers were inside the pipe when the chemical flowed through it – many of the workers suffered burns in the face and different parts of the body)

MINE FATALITIES Two Miners Killed in Australia (two coal miners, 35 and 49, have been killed in an underground mine collapse – they were working 500 metres below the surface and 10km along the coal mine when a wall collapsed and trapped them at about 9.15pm – the accident had occurred while the miners were operating a continuous miner)

PIPELINE FATALITY Workman dies in pipe-laying accident (worker was killed and seven others injured when a pipeline they were repairing exploded – incident occurred after 10:00 am. – the workers were carrying out repairs to the pipeline when water rushed suddenly from the line causing an explosion that flung the deceased and other workers several metres from where they were standing – worker died on the spot, while the others were rushed to hospital)

FATAL EXPLOSION McEwen, TN ammunition plant explosion ruled accidental (the deadly explosion at an ammunition plant was accidental, according to a preliminary investigation by the ATF – the explosion killed one worker and injured three others – one was listed in stable condition at a Medical Center – the other two victims were discharged earlier in the week)

UPDATE Rubber plant fire ruled accidental (investigators have determined the most recent fire at the plant was caused by accidental ignition caused by mechanical friction – the blaze was the second in four years at the plant – it was the alertness of the plant’s employees and their quick actions that limited the damage this time around – only one of the numerous storage units was damaged and the plant was operational the day after the accident – some damage to the vacuum unit in the storage bin – it was likely mechanical friction that ignited fibers inside two hoses that connected to a large storage unit)

AMMONIA General Motors Technical Center Closes Briefly After Ammonia Leak (Technical Center closed briefly after emergency personnel responded to an ammonia vapor leak in the wind tunnel at about 4 a.m. – the building’s alert system detected it – the leak was substantial – no one was injured)

AMMONIA Authorities respond to ammonia leak in Clanton (when authorities initially heard the dispatch call, they thought it was a 2,000-gallon tank that was leaking – they shut down roads to prevent people from driving near the area – once determined it was less than four gallons, we went ahead and opened traffic back up)

FORKLIFT Man airlifted to hospital after being ‘run over’ by forklift truck (a worker at a Wharf was taken to an Infirmary after reportedly being run over by a fork-lift truck just after 3pm – the incident involved a five-and-a-half ton forklift truck)

CONSTRUCTION

FALL from LADDER Worker Hospitalized After Fall at New Jersey Construction Site (a worker was injured when he fell off a ladder at a residential development under construction – the man was working in a loft area when he fell around 8:15 a.m. – he fell about 15 feet and reported head, back and leg pain but did not lose consciousness after the fall – his injuries were not considered life-threatening)

AMPUTATION Purina plant fined for safety violations after worker loses leg (the plant was cited for more than a dozen safety issues, including the October 2013 incident that left a worker, 29, trapped in a horizontal auger – he was working on a catwalk when his leg got caught in a horizontal screw used to push feed through the plant – surgeons drove to the scene and freed him from the machinery by amputating his leg)

FALL thru OPENING Construction worker rescued by ladder truck after fall; brought to Morristown Medical (Fire Department rescued a worker who fell through an opening in the sixth floor of a building under construction, landing on the fifth floor – worker appears to have fallen through a hole where they were building a stairwell – extent of his injuries is unknown at this time)

WORKZONE 1 dead as car slams into truck filling potholes on I-71 (alcohol is a factor in a fatal crash involving a state crew filling potholes – one male driver died at the scene and a road worker was injured when a Range Rover crashed into the back of a DOT truck about 1 a.m. – the DOT worker, who was in the truck when the SUV struck it, was taken to a hospital, treated and went home to sleep)

WORKZONE Construction Zone Wreck Injures Three on Emmet Street (a medical issue may have led to the crash that sent three people to the hospital – the driver, 74, was driving erratically in the moments before the wreck – a utility worker, 45, remains in serious condition at a Medical Center – he and a second utility worker, 23, were standing behind their truck when the driver plowed into them, pinning one of the workers – a third worker, who was in the utility truck bucket, was not injured)

COLLAPSE FATALITIES Construction Accident Kills 4 in Northern Mexico (four laborers working on the remodeling of a restaurant were killed when the marquee over the entrance crashed down on them – workers were apparently remodeling the restaurant)

WORKZONE Road workers injured in Highway 80 accident (two subcontractors working on a lane restriping project were struck and injured by an alleged drunk driver – the contractors were closing the second lane of the ramp using a trailer with an arrow board when the collision occurred around 9:15 p.m. – the driver of a red Chevy Impala struck the two employees and the arrow board trailer – the two road workers were taken to a hospital with minor to moderate injuries)

Two killed after truck plunges into pit (accident took place around 3 a.m. when a speeding ready-mix concrete container rammed the barricades and fell into the pit – two construction workers were killed and other two sustained injuries when a truck fell into a pit dug up for laying metro rail piers – a total of eight workers were working in the pier pit minutes before the accident occurred and it was a lucky escape for six of the labourers, as they had just gone out to have water)

CRANE Crane falls on road due to soil subsidence (a construction crane situated fell across a Road at around 6 p.m. – the accident caused no casualties – crane was being using for constructing an annex – crane operator explained that he had not overloaded the crane but that a sudden soil slump was responsible for the accident – when the crane fell onto the road, no cars or civilians were passing by)

Miscellaneous

TIRE EXPLOSION Workers injured in tractor tyre explosion at Aberdeenshire garage (two men were injured when a tractor tyre exploded at a garage – the men are understood to have been inflating the tyre when the accident happened at around 11.40am – one of the men suffered a fractured arm while the other sustained a head injury)

FLASH FIRE Man escapes with minor injuries in Juneau incident (a worker at an International Airport was treated and released after a small explosion involving a de-icing machine – employee had a singed beard and superficial facial burns, described like a sunburn, in the incident – the man was refilling a small, heated de-icing machine with ethylene glycol when a large flame suddenly shot out of it – the cause remains under investigation, but authorities say it may be related to the vapors in the heated container)

ELECTROCUTION Man dies of electrocution on Keene State campus (a member of the school’s Physical Plant staff was killed while working outdoors on campus – out of safety concerns, crews couldn’t immediately approach the man because of the high voltage line – the downed line also sparked a brush fire that covered about a quarter-acre before firefighters were able to bring it under control)

CHLORINE Chlorine leak forces evacuation of Tempe fitness club (a chlorine leak caused an evacuation of an fitness club at about 8:15 a.m. – there was a green haze inside the pool area, several people were having difficulty breathing and and a 67-year-old woman was transferred to the hospital for respiratory problems – responders had stopped the leak and were in the process of venting the building just before 10 a.m.)
