INDUSTRIAL ELECTRICAL Millville glass factory accident severely shocks worker (paramedics and emergency personnel were baffled by what they saw after arriving on the scene of an electric shock accident at a glass factory – an unidentified male glass factory worker in his late 30s to early 40s was left with severe injuries after being severely shocked by an industrial machine – his hand suffered third-degree burns, skin nearly melted off with gray skin tacked over – he suffered second-degree burns on the left side of his chest, with redness going up to the left side of his face and around his stomach – the supervisor at the Rescue Squad who responded to the scene was taken back by how casual the man appeared – “as we entered into the building, the man was walking towards us to meet us. He was completely conscious. By looking at him, you wouldn’t have known anything was wrong” – according to officials, the man was shocked by about 220 volts or more – he was transported via helicopter to the burn center) LOTO/GUARDING FATALITY Worker killed in Dubuque industrial accident (worker, 19, died after becoming trapped in industrial equipment around 5:15 p.m. – firefighters and the county medical investigator’s office were called to the scene of the accident, now being investigated by officials) FORKLIFT Work accident near Trudeau Airport leaves man in critical condition (a worker, 42, has been seriously injured after being struck by a forklift in the parking lot of a commercial building – the worker was first struck by the fork lift, and then was caught between the lift and a flat-bed trailer – he was transported to hospital for treatment and remains is in critical condition) ARC FLASH 2 workers burned in electrical accident (two workers, 38 and 48, suffered electrical burns after an electrical short and fire in a support building to a downtown office building about 3 p.m. – both men were taken to a hospital, where the man with the most serious burns was airlifted to a burn center – he is a 7-year employee and is in critical but stable condition – the condition for the other worker is not known – there was an arc flash while the men were working in a small building that houses electrical equipment, burning both men) DRILL DROPPED from CHERRY PICKER Worker impaled in neck by falling drill (a construction worker, in his 20s, was rushed to a hospital after an accident left him impaled by an industrial drill – he was replacing telegraph poles when the drill fell from a cherry picker – it punctured his neck narrowly missing his head and spine – it was embedded in by several inches but fortunately it just missed both his head and spine) FATAL FALL from CRANE Two contract workers of HSL fall to death (two contract workers, 40 and 41, died when they fell from a ladder at a Shipyard – incident occurred at 9.45 a.m. when they were painting a bulk carrier to get it ready for floating at a height of 60 feet – according to an eyewitness, there were four workers on the gangway ladder that was hooked to a crane to facilitate the painters to reach the work area – as per norms, all the four ends of the gangway should have been fastened to the hook with the help of wire ropes, But in this case, only the opposite ends were fastened – this resulted in the gangway to dip sharply to one side as the load of the workers shifted – the deceased slipped and fell to death, the other two held on to the railing of the ladder till the operator of the crane quickly brought down the gangway) ELECTRICAL Suncor worker killed on Easter Sunday identified (worker, 27, killed on the job had just earned his electrician’s ticket – an official cause of death has not been released and an investigation into the incident is ongoing – at 11:30 a.m. emergency crews were contacted after he was discovered severely injured – RCMP called the death “sudden” – a stop-use order on electrical panels at the site has been issued as Occupational Health and Safety continue their investigation) FATAL FALL Contract worker, 26, dies in fall at Formosa Plastics plant in South Texas (a contract worker, 26, has died after falling about 25 feet while doing routine cleaning at a plant – he was dead at the scene) OILFIELD FATALITY Oilfield Worker Killed in W. TX Drilling Rig Accident (an equipment-related drilling rig accident has left an oilfield worker dead – nobody else was hurt – victim was dead at the scene – further details weren’t immediately available) COLLAPSE during CONCRETE POUR Three workers injured in Boca Raton construction collapse (three construction workers were injured when the second floor of a home they were working on collapsed – while pouring concrete on the first floor, the scaffolding above collapsed and fell on top of them – a structural beam made out of wood and filled with concrete and iron fell on the men – beam was estimated to weigh “several thousand pounds” – one of the workers was transported as a trauma alert to a Medical Center for his injuries – a second worker was transported to a Regional Hospital, while a third was being treated on scene) EXPLOSION @ PLYWOOD MILL Explosion injures workers at Georgia-Pacific plant in Corrigan (a fire has been extinguished after an explosion at about 6:15pm – it took crews about two hours to put out the fire – four employees were airlifted to a Hospital, suffering mostly from serious but non-life threatening burns – three other victims were taken by ambulance to a hospital – one of those was soon treated and released – cause of the explosion is not yet known – plant manufactures plywood and flooring products) UPDATE on FATAL FLASH FIRE 2 weeks after naphtha leak at Manali plant, two injured staff die (two workers who suffered burns after naphtha spilled on them at an ammonia plant on April 11, died at a hospital on Saturday – five others sustained burns in the accident that occurred when workers were taking samples – the deceased had suffered 80% burns) FLASH FIRE and EXPLOSION Phoenix FD: Faulty equipment to blame in oil recycling fire (a fire at an oil recycling business that burned two men has been traced to faulty equipment – two people were badly burned when an oil tanker exploded in an industrial area – fire inspectors said that gas dripped from a faulty pump onto a hot motor and ignited – the fire occurred as used automotive oil was being transferred from a rail tanker to a truck – the blast and fire left both of them with severe third-degree burns – one worker had about a 50 percent total body surface area burn – the other is about a 15 percent total body surface area burn – the worker who is most critical onto a the back of a truck when it suddenly fell off the ramp sideways and onto the worker below – the man was pinned under the vehicle for about 20 minutes before firefighters freed him) PNEUMATIC PRESSURE TEST of IBC FATALITY Sierra Chemical had safety violations prior to today’s explosion (worker was performing a test operation on a 550-gallon bulk (tote) package using compressed air when the tote burst – there were no chemicals or fire involved in the explosion, and no other persons were injured) AMMONIA Ammonia leak sickens one at Stahlbush Farms (one employee was briefly sickened by an ammonia leak – the leak was minor and no one else had been affected – business uses ammonia as part of the packaging process for frozen foods – worker who fainted after being exposed to ammonia – workers isolated the source of the leak and fixed the problem – “It was a safety release valve that was stuck” – the employee who was exposed to the fumes declined to be taken to the hospital) EXPLOSION 300 Evacuated Following Blast of Oil Cars in Northwestern China (three railway cars with crude oil have exploded injuring three people – another 300 have been evacuated from their homes – firefighters had put out the fire by 6 am local time – all the injured have been rushed to hospitals and an investigation has been launched in what may have caused the incident) EXPLOSION Wyoming explosion raises gas pipeline safety concerns (an explosion and fireball at a natural gas plant injured no one, but all 98 residents of a nearby town were evacuated – cause of the explosion is still World Cup construction worker killed in Brazil (worker, 55, is third worker to be killed during construction – the man died while dismantling a crane that was used to install the roof of the arena – he sustained head and chest injuries when he was hit by a metal bar in the early morning accident – the cause of the accident was being investigated) GROUND FALL FATALITY Worker killed in accident identified as Scioto man (worker, 45, was killed in a construction accident – he had just started his third consecutive summer working for the same company when tragedy struck at a bridge project – accident occurred as he changed drill bits at ground level with the heavy equipment – the ground slipped beneath his feet causing him to fall into an estimated 10-foot deep hole – rock, dirt and other debris fell around and over him) WORKZONE Construction Worker In Knox County Hit By Car (a flagman that was hit by a car is out of the hospital – driver told the Sheriff she didn’t see the man at the time of the accident – the victim is an employee with a company that repairs and replaces guardrails – investigators said the man was wearing a neon green vest and holding a stop sign when he was hit – he was flown to a Hospital for treatment and later released) WORKZONE Major Traffic Back-up on I-74 in East Peoria (a morning accident involving a semi and a pedestrian sends an DOT contract worker, 57, to the hospital – he is listed in fair condition – the driver of the semi was headed westbound on I-74 at the time of the accident – worker was setting up traffic signs for upcoming construction when an item blew into traffic – he ran into the roadway to retrieve it when the semi, hauling organic fertilizer, hit him and then rolled onto its passenger side – worker was ticketed for improper walking on the road) WALL COLLAPSE Construction worker killed on job in downtown Ottawa (a worker, 45, was fatally injured at a downtown construction site – construction workers administered CPR to their colleague while they waited for paramedics to respond to the scene – he was pronounced dead at the hospital – a work crew was demolishing a cinder-block wall in part of the building when the accident occurred – it appears that a portion of the wall struck the man, noting that he’d suffered a “severe head injury”) AERIAL LIFT Construction worker on grandstands suffers head injury at Daytona International Speedway (a construction worker, 43, suffered a head injury while working on the grandstand renovation – he was operating a scissor lift about 94 feet above ground when his head was compressed between the lift and a steel beam – he was rescued by his co-workers before firefighters arrived on scene – he was taken to a Medical Center in serious but stable condition) ELECTROCUTION??? Worker dies in suspected electrocution near Mount Gambier (worker, 43, is believed to have been electrocuted when a metal section he was lifting onto a shed came into contact with overhead power lines) HOTWORK Fire engulfing Dalian building suspectedly caused by welding work (the inferno took place at midday after a welding operation accidentally ignited heat insulation wall on the building surface – strong wild caused the fire to climb up through the air conditioning machines quickly and the building was soon completely on fire – no casualties reported) Miscellaneous SKID STEER FATALITY Man, 21, dies in skid loader accident at Turner County Dairy (a farm worker, 21, died while operating a skid loader at a dairy – he was by himself with no witnesses – not exactly sure what happened – the man might have been trying to get out of the skid steer when the loader pinned him -“ All the safety features of the skid steer loader were working properly. We are trying to figure it out. It went down somehow, pinning him under the arms of the loader and inside the cab of the skid steer.”) FATAL FALL from TREE Luzerne Man Dies In Tree-Cutting Accident (a worker, 41, died when a tree he was trying to cut down at a private residence fell on him – he was the owner of the company – he and a co-worker were cutting down a birch tree located in the front yard of the residence – he ascended the tree that was approximately 50 feet in height before cutting a six foot section of the tree – as that section of tree was being lowered, the entire tree fell to the ground – he clung to the tree as it fell and landed on a blacktop area of a driveway – he was transported to a Hospital for treatment, and later transferred to a Medical Center, where he was pronounced dead at 3:48 p.m. – autopsy revealed the cause of death to be massive internal injuries due to blunt force trauma) Costco dock worker injured, authorities said (early reports indicated a dock employee suffered a leg injury – cause of the accident is not yet known – the man’s injury did not appear to be life-threatening) Grayslake dry cleaners chemical spill injures 4 (firefighters are cleaning up a chemical spill at a dry cleaners – firefighters were responding to a call of a collapsed person when they discovered the chemical spill – two workers and two firefighters were taken to area hospitals for treatment – their injuries are not life-threatening) oxy-acetylene torch EXPLOSION Mavis Tire employee hurt in torch explosion at Ulster store, police say (a worker, 28, suffered head and possible internal injuries in an explosion at a tire store – he as using an oxy-acetylene torch while working on a vehicle when the explosion occurred, throwing him to the floor of his work area – he was treated at the scene by emergency medical technicians – a medevac helicopter was requested and he was airlifted to a Medical Center – his injuries are not believed to be life threatening – a preliminary investigation determined that a disproportionate mixture from the torch caused the explosion – no one else was injured in the blast and there was no damage to the building) |
