INDUSTRIAL

PULP MILL FATALITY 1 killed, 1 injured in pulp mill accident (one man was killed and another was seriously injured in a pulp mill accident – a hydraulic line behind a worker became pressurized and burst – officials believe the pipe then struck the employee, killing him – in addition to the man killed, another man was seriously injured – both victims were employed by a major maintenance outage contractor – the pulp mill is undergoing its annual maintenance outage, meaning the entire plant is shut down for the week)

LINEMAN ELECTROCUTED KSEB Worker Electrocuted at Alakkode (a contract worker, 50, was electrocuted while he was engaged in reconnecting electric lines that were disengaged in the heavy rain and wind – he was repairing a snag in a line at 11 a.m. when the accident occurred)

GUARDING/LOTO FATALITY Man dies at fortune cookie factory: Worker died after falling into dough mixer (worker, 26, died at a fortune cookie factory after he reportedly fell into a giant dough mixer – he was pronounced dead at the cookie making factory – medical examiner’s office said he died of “multiple blunt trauma injuries” after tumbling over and being pulled into the dough-mixing machine – he was found mangled inside of the industrial sized mixer by his fellow employees)

SANITATION FATALITY Garbage truck hits, kills worker in Orlando church parking lot (a man, 26, is dead after he was hit by a city garbage truck on which he was working – he and another sanitation worker entered the parking lot just before 5 a.m. as part of their normal routine, when things when tragically wrong – it appeared he was run over by the truck)

LINEMAN ELECTRICAL Journeyman injured on Eklutna line (a journeyman lineman was injured while working on a project – the employee sustained electrical shock injuries at approximately 2:45 p.m. – the worker’s condition continues to improve)

FORKLIFT FATALITY Idaho man dies after being crushed by steel beam (a steel worker, 53, has died after a 50,000-pound steel beam fell off a forklift and pinned him shortly before 11 a.m. – he was trying to move the beam with a forklift when the load shifted and the beam fell – fellow workers were above to lift the beam off him, but he died shortly after medical personnel arrived)

FALL from BARGE Barge worker OK after fall into Mississippi River (a barge worker is recovering after falling into the Mississippi River about 1:30 a.m. at a terminal where barges are loaded – a tugboat was able to rescue the man soon after he fell into the river – he was taken to a hospital after complaining of pain in his arm and back, but is expected to recover)

OIL RIG FATALITY Man dies in oilfield accident (worker, 20, was found unresponsive on the well pad and was pronounced dead on the scene – he was employed in the oilfield as a flow tester on oil rigs)

AUGER GUARDING (PERFECT EXAMPLE of VERY POOR PROXIMITY GUARDING!) Worker’s hand trapped (a worker at the centre of a dramatic rescue is waiting to see if there if there is any tendon damage to the hand that got caught in an auger – the man, in his 30s, was washing a hopper when the 9.40 a.m. accident happened during the final clean of the year at the end of harvest – the hopper was not working at the time but the man could not free himself)

POWER STATION FIRE 3 workers hurt in fire at Bryan Texas Utilities plant, cause of blaze sought (three utility workers have been hurt during a fire at an electric power plant – one worker remains hospitalized following the fire – investigators are trying to determine what sparked the fire at the city-run plant, which was offline at the time of the accident – firefighters extinguished the blaze within minutes – two workers have minor injuries and were treated and released – neither man was burned – no further details on their wounds – a third worker was transported to a hospital – declined to release details on that injured man)

LOADING DOCK FATALITY Worker dies at food warehouse (a worker, 65, died in an apparent early morning accident at a warehouse – first responders found a man dead at about 1:15 a.m. at the warehouse – he was between pinned between a wall and a metal ramp in a loading area)

CRANE FATALITY Dock worker killed while unloading ship (worker, 53, was killed at a shipping dock after being struck by a cargo tray that weighed more than 4,000 pounds – incident occurred in an area where fruit from a ship was being unloaded onto the tray by a crane)

FIRE Fire at International Wax Inc. plant in northwestern Pennsylvania injures 1 employee (a fire has injured a worker – fire was reported about 11:50 a.m. and was accompanied by witness accounts of an explosion – initial reports indicate the fire burned wax in a filter room, but state police fire marshals have yet to pinpoint a cause – the worker suffered minor injuries and isn’t hospitalized)

DUST EXPLOSION Industrial blast injures 11 (an explosion and fire possibly caused by a dust buildup at a metal-polishing shop injured 11 people, including two men who were critically burned – the blast occurred in an overhead duct system shortly before 10 a.m. – took more than 50 firefighters about 40 minutes to douse the resulting fire)

FUELING FORKLIFT FIRE Worker Burned in Blast at N.C. Cardboard Plant (a man was injured in a propane tank explosion with burns to his hands, arm and face – he was filling a (liquid petroleum) tank for a tow motor and something went wrong – plant has its own fuel center at its facility – the employee was filling up a tank when it caught fire – officials are still uncertain what caused the fire, and will have to interview the employee when he is feeling better to determine what happened)

FIRE One injured in fire at Cenovus site (one worker is in hospital after a fire ripped through two oil tanks at a facility at 4:20 p.m. – emergency crews contained the fire and extinguished it within an hour – the injured worker was involved in routine maintenance near the tanks when the fire started – worker is being treated for burns – a second worker was assessed by medical staff and treated on site – cause of the fire and the extent of the damage is still being investigated)

TREATMENT PLANT CHEMICAL REACTION in TANK Firefighter released by doctors after Kennebunk explosion (a mix of chemicals may have caused the explosion inside the sewage treatment plant – a firefighter and member of the Hazardous Materials Response Team was shaken up after being thrown off a ladder and onto a concrete floor by an explosion inside a building at a sewage treatment plant – employees at the treatment plant reported that a worker might have inadvertently mixed two chemicals in a holding tank at the plant – the explosion, which bent some interior piping, took place around 2:30 and set off the building’s sprinkler system – explosion created a gas cloud)

FATAL EXPLOSION @ OIL RIG Two killed, nine hurt in Texas blast (two workers were killed and nine injured in an apparent blowout as workers changed a wellhead at a producing site – no cause has yet been determined for the explosion reported at about 8:15 am – force of the explosion shattered windshields and deployed airbags on vehicles at the site and nearby residents reported feeling the blast at home)

CONFINED SPACE Branford Fire: Barge Worker Overcome by Carbon Monoxide Fumes (worker was apparently welding inside a metal barge when he was overcome by the noxious gas – fire officials state that it appears the worker was working within the confined interior space of the barge without proper ventilation – worker had been removed from the barge and was disoriented and confused upon fire department arrival – his partner recognized his peril and was key to removing him from the noxious environment – fire Department personnel had readings of 30ppm of carbon monoxide within the hull of the barge)

EXPLOSION Explosion injures 4 at gas plant in Ibadan (an explosion at a gas refilling plant injured four persons – explosion occurred at about 3p.m. – it was triggered by a mobile tanker that was transloading gas into a stationary tank)

AMMONIA 10 Intel Workers Exposed In Ammonia Spill (ten construction workers were exposed to ammonia – two of those people were sent to the hospital with shortness of breath – workers were doing upgrades when they ruptured an ammonia line – most of the workers were treated on site)

AMMONIA Workers evacuated after gas leak (a faulty valve led to an ammonia leak and the evacuation of workers – no one was hurt in the incident, which saw a road cordoned off after 5pm and reopened about 6.30pm – the safety systems had kicked in after a valve on a piece of machinery worked loose and let out “a squirt” of ammonia – the leak was immediately registered by detection equipment and a staff member alerted emergency services, as was standard procedure)

CHLORINE Some KCK residents urged to stay indoors after chlorine leak (a chlorine leak at a WWTP plant means some residents are urged to stay indoors – fire department was contacted at 4:30 p.m. to go to the plant – workers said they had a leak in a tank in the water processing area, and had shut off the tank – workers were unclear how much chlorine had escaped)

CONSTRUCTION

DEMO COLLPASE At Least 9 Injured In Building Collapse At Old Bethlehem Steel Plant (just after 10 a.m. a roof collapsed at an old steel plant – nine construction workers were rushed to the hospital – four of those nine workers were taken directly from the plant to Shock Trauma – three of them were transported as critical cases – the plant was being torn down for scrap)

CRANE TIPPED Crane tips over in Miami Beach, trapping worker (a crane toppled over on top of a building, trapping the crane operator inside – the truck and crane tilted to one side, lifting the truck from the road – they were putting gravel on the green roof and the boom overextended itself and cantilevered the truck up into the air)

FALL from CHERRY PICKER Two men plunge 12 feet in Beach construction accident (two men plunged 12 feet after a construction accident – crews are working on a parking garage – workers were adjusting a concrete beam when it became dislodged and fell hitting a cherry picker and a man who was inside of the basket was knocked off of it – another worker who was working on the beam also fell – both men fell 12 feet down – their injuries are considered minor)

WORKZONE Authorities identify worker run over by pickup truck at Garden State Parkway construction site (a construction worker, 60, was seriously injured when a pickup truck accidentally backed over him at a work site – he was hospitalized in serious condition with a head injury – he was among a crew working on the parkway’s southbound side when he was struck by the truck)

WORKZONE FATALITY Driver strikes, kills construction worker on I-459 (a driver struck and killed a construction worker, 59, on Interstate 459 – he was working on a repaving project)

FLOOR COLLAPSE FATALITY Construction worker dies after condominium floor collapses (construction worker, 38, died on the spot when one of the floors of a high-rise condominium tower collapsed – he and a co-worker were inspecting the concrete floor from underneath when it suddenly collapsed – initial investigations showed that there were no criminal elements)

FALL from SCAFFOLDING Worker Hospitalized After Fall Into Swimming Pool (h reportedly fell from scaffolding while constructing the pool at a residence – he had non life-threatening injuries)

SLIP/TRIP/FALL from BRIDGE Harrison County Worker Dangles Off Bridge (it was a close call for a County road worker when he was left dangling off a bridge – he was with a co-worker around 2 p.m. checking out a bridge that was believed to be in need of repairs – he began to fall when his leg got trapped in a hole, causing him to hang off the side of the bridge – he was taken to a Hospital to be treated for his injuries – with no cell service in the area, he dangled for about 15 minutes while his co-worker held his head above water and the two hollered for help)

DEMO 2 Workers Hurt During Demolition (two demolition workers are recovering after bricks fell on top of them while they were working on an abandoned building – they were hit by falling debris from one of the adjoining buildings around 10:30 a.m. while the crew was starting the clean-up phase of demolition on a row home)

CRUSH FATALITY Worker Killed at Construction Site (worker, 32, was accidentally killed while working at a construction site when a large concrete form the crew was working on became dislodged pinning him around 2:10 p.m. – co-workers freed him from beneath the concrete form and heroically attempted to save his life but they were unable to do so and he was pronounced dead at 3:40 p.m.)

HOTWORK Fire Crews Quell Small Blaze on 5th Floor of High-Rise at Ninth and Broadway (a welding accident sparked a small fire in a high-rise building under construction – fire erupted about 9:15 a.m., when embers fell from a 12-floor work area and ignited waterproofing material on the fifth floor – workers were able to get the flames largely under control without help)

HOTWORK I-15 Closed In Hesperia As Crews Battle Bridge Fire (all lanes of Interstate 15 were shut down as crews battled a bridge fire ignited by a welder’s torch around 1:30 p.m. at the overpass construction site – a civilian has been treated for smoke inhalation – no further injuries had been reported – overpass was halfway through its construction when the flames erupted – contractors were still working on top of the bridge after it caught fire and were unaware of the blaze until being evacuated by firefighters because the flames were being pushed away by the wind – a third of the bridge has burned)

HOTWORK Fire out at vacant Charles Camsell hospital (firefighters were called to the scene around 10:30 a.m. after a torch used by a maintenance worker ignited insulation on the top floor – fire was contained to the insulation but still took awhile to put out – all the sprinklers and the standpipes were decommissioned)

TRENCH Worker stuck in trench in Brentwood (a worker has been trapped after a trench collapsed – the person was working in the area on sewer maintenance when the trench collapsed – he has been buried up to his waist and is expected to be fine – crews are working to remove him from the trench currently)

TRENCH TFD rescues fallen man in trench (Fire Department Technical Rescue Units rescued a worker who had fallen approximately 15 feet into a trench, hoisting him out approximately an hour and a half after the initial fall – first units on scene confirmed the man was in the trench and lying in the prone position – he was able to communicate with them to advise them how he was feeling but they did not attempt to go into the trench)

Miscellaneous

FALL from SCAFFOLDING Worker Falls 6 Feet at Carnival in Voorhees (a maintenance worker, 33, fell 6 feet from scaffolding while working on a carnival ride at a mall – he was treated for minor injuries and released from a hospital)

TREE TRIMMING FATALITY Odd-job worker killed in tree-cutting incident (an odd-job worker, 37, was killed when the tree he was cutting fell on another tree, which, in turn, struck him on the head – he and his friend, 28, were cutting tree trunks to be made into a fence post for a cattle farm project in the settlement – tree trunk pinned him to the ground and caused injuries to his head and face – his death was due to severe bleeding and internal injuries)

ELECTRICAL Utility worker electrocuted, taken to hospital (a utility worker was taken to the hospital after being electrocuted – the cable worker was injured around 3:45 p.m. – he was taken to a Medical Center, though his condition isn’t known – cause of the accident is still under investigation)

CIRCUS FALL Investigators looking at carabiner in circus accident that injured 9 (investigators are focusing on a failed carabiner designed to connect a team of acrobats to the ceiling to explain why a popular circus act abruptly ended with nine people injured – it seems as though a carabiner failed and appears to have caused the platform to fail and the performers to fall down – officials are still looking at why the carabiner failed)

CRUSHED Carwash employee survives being crushed by two vehicles (an employee became pressed between two vehicles after an accident – incident occurred when a co-worker of the victim was driving a van designed for a disabled person, and the co-worker accidentally made the van accelerate – victim was standing in front of the van, and when it accelerated, it ended up crashing into a car in front of it – accident led to the employee suffering a broken arm)
