INDUSTRIAL

ELECTROCUTION Worker Electrocuted at Dairy Plant (worker, 28, died after he was electrocuted while doing electrical work – he was transported to the hospital where he was pronounced dead – the industrial accident at the yogurt plant occurred at around 7:50 a.m.)

LOTO Man cut free after arm caught in potato grader machine (worker, 31, had to be cut free from farm machinery after his arm was pulled into a potato grader while he was working on it – he was airlifted to hospital after the accident, which is thought to have happened when an item of clothing got caught in the mechanism – he suffered a serious arm injury – he got his arm trapped between the roller system – crews used cutting equipment and air bags to release him)

PIPELINE FATALITY Contractor dies in Chevron pipeline accident off Louisiana coast (a contractor was fatally injured and two people suffered minor injuries while performing maintenance on a natural gas pipeline shortly after 11 a.m. – the incident caused a small amount of natural gas condensate to be released into the water but that it dissipated and containment crews were en route to the scene)

OIL RIG FATALITY Worker killed in Oklahoma oil rig accident is identified (a worker, 42, was killed in an oil rig accident – he was an operator and was performing workover operations at the company’s rig when the accident happened around 3:00 p.m. – he was working on the tower when a high pressure release forced a pipe out of the well hole and fatally struck him)

UPDATE on LOTO FATALITY American Apparel Settles Worker Death Suit for $1M (company will pay $1 million to settle a lawsuit over the death of an employee in a knitting machine accident – worker’s daughter will receive $150,000 of the money – the rest covers civil penalties and the cost of the accident’s investigation – employee, 49, was mangled by the machine in 2011 – the machine had been turned off, but wasn’t locked and disconnected from its power source for maintenance)

BARGE UNLOADING FATALITY Worker dies after ‘incident’ at job site (worker, 40, was pronounced dead at a Medical Center at 11:51 a.m. – he was off-loading heavy pipe from a barge at the Port when one of the loads struck him)

HOTWORK on BARREL EXPLOSION Barrel explodes, injures two industrial workers (two workers were transported to the hospital after sustaining burns from industrial accident at 2:40 p.m. – at the time of the accident a worker was attempting to cut a 55-gallon barrel with a cutting torch – while the worker was cutting the barrel it exploded – he received burns to his arms, face and part of his chest – the barrel was empty but had residue of the chemical Xylene inside, which is used as paint thinner – worker thought he was cutting into a barrel that previous contained coolant – the second worker who was assisting the injured worker sustained burns and was transported to a Medical Center)

EXCAVATOR FATALITY @ QUARRY Man dies after excavator accident at quarry (emergency crews recovered a worker, 25 – he was killed when his excavator rolled down an embankment – he was creating a wind break when the accident occurred – it would appear he has for unknown reasons picked up a boulder and has turned it downhill and the weight of that boulder in the bucket has toppled it over)

GARBAGE TRUCK FATALITY City employee dies in trash truck accident (worker, 52, was fatally struck – he had been employed a short period of time as a temporary worker with the City as a Solid Waste Collector – he attempted to exit the vehicle while it was still moving and fell beneath the back wheels – he was pronounced dead at the scene)

DUST EXPLOSION Worker burned in blast at former mill (a worker was burned in an explosion while he was tearing down a storage tank at a former pulp mill – he was using a cutting torch on a hopper when dust from the cornstarch it once held ignited – he was hospitalized, but authorities have not said how badly he was injured)

EXPLOSION Explosion at chemical waste plant (an explosion has rocked a chemical waste facility injuring several people – one person was severely burned in the blast and that the explosion damaged a nearby residential area – about 30 to 40 houses were damaged – some of them only 65ft (20m) from the facility – as the blast shattered windows, ripped doors from their frames and tore off roofs – he cause of the blast was not clear, as the site was still not yet accessible to investigators)

BOILER EXPLOSION Faulty valve behind paper mill explosion (explosion at a mill was the result of a malfunctioning valve in the coal feeder system – the explosion slightly injured a work and caused some structural damage – the automatic isolation valve malfunction allowed hot gas to connect with coal dust which caused the blast)

HOTWORK Fire destroys Sterling fertilizer plant (the fire that destroyed a fertilizer plant is being blamed on a welding accident – 95 percent of the factory building was destroyed – several workers who were in the building escaped)

FATALITY Fatal accident at Hussmann plant (worker, 58, is dead after a fatal accident – County Medical Examiner investigators are preforming an autopsy on the man, no other details are available until after the investigation)

HOTWORK w/ GRAIN DUST EXPLOSION Flagstaff plant explosion injures 4, authorities say (four people were burned in a grain-dust explosion – four contractors were doing welding work on the fifth floor of the plant when dust produced during the food-making process ignited – the explosion blew out several metal doors and burned all four contractors, who were sent to the hospital – one has severe burns)

EXPLOSION Praxair’s Buffalo, N.Y., plant damaged by explosion (an explosion rocked a research and development complex, causing extensive damage to at least two buildings on the site – officials have given a preliminary estimate of at least $250,000 in damage as a result of the blaze, which took several hours and fire crews from throughout the region to get under control – no employees were injured as a result of the explosion)

AMMONIA Warehouse ammonia leak stopped in seven hours (it took the Fire Department crews, and two ammonia specialists, about seven hours to stop a leak at a cold storage warehouse just before 9 p.m. – fire crews also encountered a fire at the scene – employees from several surrounding businesses were evacuated and power was shut off to the adjacent businesses and neighborhoods to protect firefighters at the scene of the leak – no injuries were reported and the incident is under investigation)

AMMONIA Ammonia leak in Pasco sends five to hospital (five people were taken to the hospital, treated and later released after an anhydrous ammonia leak – plant was evacuated – leak occurred about 2:30 a.m and about 200 workers were forced to leave)

AMMONIA Ammonia Leak Poisons Over 30 in Northwest China Chemical Plant (more than 30 people have been poisoned after a pipe containing liquid ammonia went loose at a chemical plant – at least 33 people became sick after inhaling the ammonia gas from a broken pipe – most of the victims shown minor signs of poisoning though four have been reported to have suffered severed poisoning – the four victims were declared to be out of danger)

CHLORINE Chlorine gas leak forces evacuation of water treatment plant in Oregon City (a chlorine gas leak forced evacuation of employees – a fire engine and a hazmat crew responded to the leak, which was reported around 7 a.m. – number of employees involved in the evacuation is unavailable – nature and cause of the leak are still under evaluation)

CHEMICAL REACTION Chemical plume released at Haviland Enterprises in G.R. (pool chemicals, known as oxiders, got wet, creating a chlorine plume at the chemical plant – firefighters evacuated businesses where the plume was strongest – residents living north of the area were advised to stay inside because the gas release could create breathing discomfort)

CONSTRUCTION

TRENCHING Construction accident: OSHA investigating after two workers buried in berm collapse (two men were buried when an earthen berm collapsed around 10:41 a.m. – the berm was underneath a bridge that was being constructed – the men were trapped for approximately 15-20 minutes before fire and construction crews could free them – one worker was taken to a Hospital – the other worker, who was buried deeper, was taken by helicopter to a Hospital – both men were conscious and alert when they were transported)

WORKZONE MoDOT worker involved in accident on I-435 (Department of Transportation worker was transported to a hospital on after being involved in an accident in the northbound lanes of I-435 – worker was patching potholes when his vehicle was struck from behind – the vehicle was heavily damaged and had to be towed – extent of the worker’s injuries is not known)

Construction worker suffers foot injury on Staten Island Expressway (a construction worker was taken to the hospital with a foot injury when a piece of equipment rolled onto his toes – worker was assessed at the scene and transported to a University Medical Center after the equipment, a steel device used to shape and form concrete, rolled over his toes at approximately 12:16 p.m. – he was wearing protective footwear)

SAW 34-Year-Old Worker Injured on Palm Coast Parkway 6-Laning Project as Circular Saw Jumps (worker, 34, was hospitalized after he was struck in the face by a circular saw while working on cutting a large metal pipe – he was in shock after the incident but the injury was non-life-threatening – he had cut through a large metal pipe with a circular saw, the two halves of the pipe pinched the saw, causing the saw to buckle – he let go of the saw right after the blade became stuck, but the saw broke loose, shot up in the air, and struck him in the face)

Worker Injured While Rebuilding Stonewall Post Office (a piece of building material fell on a man, knocking him unconscious for a short time – he was taken to a hospital to be treated for a concussion)

STRUCK BY FATALITY Worker decapitated in accident (a banksman, 24, who was run over and killed by a tipper truck could have been giving directions to its driver at the time – he had been guiding lorries in and out of a soil stockpile site when he was knocked down – drivers at the site said he had been on the phone just before he was hit by the reversing vehicle – he was decapitated and his helmet was left half buried in the sand about a metre away from his “twisted” body)

WORKZONE Accident sends tree-trimming worker to hospital (a crew member for a tree-trimming company was transported to a local hospital at about 4 p.m. – he was a sign holder and was struck by a white sport utility vehicle – he has a broken leg and head injury)

CRANE into POWER LINE 4 injured in North Port power line accident (three construction workers were hospitalized after a crane boom that one of them was operating made contact with a live power line around 1 p.m. – workers were preparing to pour concrete for a signal pole foundation – the line was carrying 23,000 volts of electricity – four people were injured)

CONFINED SPACE FATALITY Two Calgary men killed in Saskatchewan workplace accident on sewer (two men are dead following a workplace accident -incident occurred while three males were working on a sewer line – two men fell nearly four metres into sewer lines and a partial collapse occurred – fire personnel were able to pull a 25-year-old male up to the surface – he was transported to a Hospital, but pronounced dead on arrival – because of noxious fumes, emergency workers weren’t able to immediately access the lines following the 6:20 p.m. accident – a local farmer attended with equipment able to pull the victim out and a 24-year-old worker was lifted to the surface and pronounced dead on scene)

HOUSE COLLAPSE One person dead after house collapse in North Toronto (one person is dead and another has serious injuries following a house collapse at a house reportedly under renovation – one construction worker is dead and another has been taken to hospital with serious injuries – two other construction workers, who were outside the home, suffered minor injuries)

WORKZONE FATALITY Road commission worker killed by motor grader (a road commission worker, 42, has been killed after being hit by a motor grader at road construction site – he was working at the site when he was struck by the grader which was being driven by another road commission employee – the driver of the grader didn’t see him – he had 16 years of experience working for the road commission)

WORKZONE Highway worker hurt in accident (a highway worker has non life-threatening injuries, after a crash in a construction zone – the semi hit a highway department truck – the work zone was well marked)

MANLIFT Marshall County Worker Pinned Between Piping, Hydraulic Lift (a worker was flown to a hospital after getting pinned between piping and a hydraulic lift – the accident occurred around 10:30 a.m. when the worker was pinned between some piping he was working on and a hydraulic lift – extent of the employee’s injuries remained unknown)

STEAM ROLLER Langley construction worker run over by a steam roller (a construction crew was laying asphalt when a worker was run over and crushed by a two-ton steam roller – when crews arrived, the victim was conscious and breathing – he was taken to hospital with what are believed to be lower body injuries)

HOTWORK Fire at welcome center caused by welding (with three weeks left to go on the project, fire broke out at the new welcome center, causing damage to at least two sections of the roof – no injuries and the Fire Department had the fire under control in less than an hour – a state fire marshal was on the scene getting ready to do an inspection, was able to see the fire and smoke coming off the roof – the fire started when a worker, who was welding the guttering, stepped away from the welding to get another tool -“ When he came back the fire had broken out in the roof system”)

TRENCH FATALITY UPDATE: Names of boy and man killed in trench collapse released (sheriff’s department says it received a call that two people were trapped when a trench collapsed at a home construction site – rescuers used hand shovels to dig through the dirt, and recovered the bodies of a two year old and a 56-year old – rescuers tried to revive them but were not successful – the trench was about six feet deep – another man was in the trench at the time of the collapse, but was able to get out and call for help)

TRENCH FATALITY Man dead after industrial accident in city’s east end (a man in his 50s is dead after an industrial accident – he was digging a trench behind an apartment building at around 9:40 a.m. when he became trapped – he was then extricated by emergency crews and taken to hospital without vital signs)

FALL from SCAFFOLDING Two construction workers taken to hospital after fall (two construction workers were taken to a hospital after they fell off a scaffolding 16 feet above the ground while working on a home at about 11:30 a.m. – both workers were taken to a Regional Medical Center)

MISCELLANEOUS

FedEx worker dies after being pinned by his delivery vehicle (worker died after being pinned by his delivery vehicle at about 4:30 p.m. – worker was outside of his delivery vehicle at a residence when it rolled and struck him – authorities arrived and found the worker pinned between the vehicle and a house)

TRACTOR Orchard Worker in Serious Condition After Tractor Accident (worker was in serious condition after the tractor he was operating rolled over on top of him on – he was transported via medical helicopter to a Medical Center for treatment)

American Airlines contract worker killed at Detroit Metro Airport (contract worker, 24, died from multiple injuries near a belt loader at an Airport – he was working on the ramp in the area of the belt loader adjacent to a jet – death has been ruled an accident by a medical examiner but a definitive cause has not been determined – he was getting ready to load items onto a plane bound for Dallas at approximately 5 AM)

FALL from LADDER Police: Worker Seriously Injured in Fall From Ladder (employee is in serious condition after he fell off a ladder – he fell two stories and hit his head on a retaining wall – he was transported to a Hospital)

AIRPORT PDX employee in serious condition (airport worker, 49, crushed between two shuttle buses remains in serious condition – he was seriously injured on the job after another shuttle bus driver left his vehicle without putting it in park –  when the driver realized his mistake, he hit the gas rather than the brakes, pinning the worker between the two vehicles)

HOTWORK Fire that damaged Colossus roller coaster sparked by welders (welders working on a roller coaster sparked fire that damaged the wooden coaster – no one was injured in blaze – the fire that damaged the wooden coaster was sparked by welders working on the ride – a portion of the structure collapsed, opening a hole in the track)
