operating an aerial work platform when he became trapped between the railing of the aerial platform and a steel catwalk approximately 11 feet above the ground – the cause of death was asphyxia due to mechanical compression) FRONT END LOADER FATALITY Man Dead In Peabody Industrial Accident (a worker, 26, was killed in an industrial accident – he was bending down to clear a drain while a front-end loader was backing up and did not see him) MINE FATALITY One dead, another injured in quarry accident (worker, 32, died on the spot and another sustained injuries in an accident at a stone quarry – they were working at the quarry while stones fell on them) SANITATION FATALITY W-S Sanitation Worker Hit By Car Dies (a sanitation worker, 60, that was hit by a car while on the job died – he had worked for the city for 21 years – he was hit by an SUV on November 19 when he stepped into the path of the SUV) AMPUTATION Man loses legs in Exxon Baton Rouge refinery incident (a contract worker lost his legs in an accident at a refinery – company did not confirm the extent of his injuries – he was injured during the course of regular operations at the refinery – he was treated at the scene and transported by emergency medical service to a local hospital for further treatment) HI PRESSURE PIPE Sanitary worker injured as hose pipe bursts, may not regain speech (sanitary worker, 49, was injured after an old pipe burst – this is the fifth incident this year of a worker being injured by a burst pipe – he suffered a hole in his jaw and tongue – powered by a 140 bar pressure compressor, the high pressure pipe was pumping water to clean underground drain) CONFINED SPACE FATALITY – ENGULFMENT Grain accident claims life (a long-time employee, 52, of a Dairy Cooperative died as the result of a grain engulfment incident – he was removed from the grain silo and transported to a Medical Center where he was later pronounced dead – he had entered a grain storage bin of which he had been fully trained, was wearing appropriate safety equipment and following procedures for entering this confined space) CONFINED SPACE ENGULFMENT & RESCUE Successful rescue Farm worker saved by dedicated crews (a longtime employee went inside a grain bin trying to get the beans moving again after the auger that offloads the beans stopped feeding them to the truck – a cavern had developed above the auger and the dome above the cavern collapsed, causing the worker to drop into the hole – beans quickly filled in the space around his body – another worker called for rescue crews to help while he entered the bin to help stabilize his co-worker – a ladder truck was used to access the top of the bin and oxygen was supplied to the worker while other workers stayed with him – worker’s head and arms were about all that was visible when work began – saws were used to cut holes in the bin and crews on the ground began draining soybeans out of the bin in order to lower the beans from below, which would ultimately cause beans to shed away from his body – it took three to four hours to drain the bin, which was about three-fourths full at first and down to one-fourth full at rescue) MINE FATALITY North Korean worker among four killed in Malaysia mine accident (death toll from an explosion in a coal mine has risen to four, while around two dozen others were injured, some with serious burns – underground gases are believed to have ignited triggering the blast and a subsequent fire – the mine was operating legally and employed 119 workers) AMMONIA Emergency services contain ammonia gas leak in Welshpool (three people were treated for respiratory problems – operations had returned to normal but two staff members were taken to hospital as a precautionary measure – seven crews attended the leak from a 1000-litre ammonia cylinder – nearby houses were also evacuated and roads closed) MINE FATALITY Worker killed in Anglo mine accident (a scraper winch operator was killed in an accident at a mine – he was fatally injured in a fall of ground incident) UPDATE on CONFINED SPACE FATALITY Man dies after being burned alive in industrial oven (worker, 54, suffered horrific burns and later died from shock after becoming trapped in an industrial moulding oven when co-workers failed to notice he was cleaning inside – the oven can reach temperatures of 280 degrees Celsius – he went inside the oven to clean the equipment, but a co-worker switched the machine on without realizing he was inside – the ovens at the plastics manufacturer are fitted with an energy saving lock, which automatically engages when the machine is in use – the court heard a safety door has now been installed, but at the time of the accident he had no means of escape – firm is on trial for corporate manslaughter – another man on trial is the one who designed and installed the oven’s control systems) FORKLIFT FATALITY Worker killed in forklift accident east of Winnipeg (worker, 34, has been killed in an industrial accident when a forklift fell on him – he was pronounced dead at hospital) FATAL EXPLOSION Oil platform explosion in Gulf of Mexico : Worker killed, 3 others injured (an explosion at an oil rig has killed one person and injured three others – explosion occurred just before 3 p.m. – platform was not in production at the time of the incident – facility damage was limited to the explosion area and there was no pollution reported – three people were injured and were being treated at an onshore medical facility – one employee of a contractor was killed and a second contractor employee was seriously injured) AMMONIA 20 workers fall unconscious following gas leak at meat plant in Meerut (at least 20 workers, 16 of them women, fainted following an ammonia gas leak at a meat packaging plant – around 50 workers were working at the meat packaging plant when the incident took place – workers, who fell unconscious, have been admitted to a hospital and the condition of four is stated to be critical – the ammonia gas leaked from the freezing chamber of the plant following which the 20 workers fainted – Police have registered a case against the factory’s owner under Section 284 (negligent conduct with respect to poisonous substance) and Section 287 (negligent conduct with respect to machinery) – cause of the leak is yet to be ascertained) UPDATE Colortech Inc. Fined $80,000 After Worker Burned, Permanently Injured (a manufacturer of color concentrated plastic additives, has pleaded guilty and has been fined $80,000 after a worker was severely burned and suffered permanent injuries – the worker was engaged on a production line of processing equipment used to mix, heat, form and cut plastic into small pellets – at approximately 1 a.m., some of the heaters near the die on the line stopped operating – a plant electrician attended and repaired the wiring at 3:20 a.m. – elsewhere in the line, the heaters remained operational and the hot molten plastic caused a buildup of pressure and gases at the blocked die and screen changer – as part of the procedure to re-start the line, the worker was required to change the screen in the screen changer – during this change, there was an unexpected release of pressure which resulted in the release of hot liquefied plastic, causing second degree burns to multiple part of the body – the worker also inhaled the hot gases, resulting in internal injuries – worker is not expected to recover or be able to return to work – screen changer was not equipped with a shield or guard to prevent workers from being injured by hot plastic or gasses) EXPLOSION Five MLGW workers hurt in gas line explosion (three people are in critical condition and were taken to the burn unit at the Regional Medical Center – the other two have since been released from the hospital – the men were trying to repair a gas line leak – the crew shut the gas line off before they started working – investigators are still trying to figure out how the fire started and what caused the explosion) EXPLOSION Gas plant explosion in Panhandle critically burns 2 (a building used for gas drying operations burned itself out – two men were severely burned as they ran from the building engulfed by flames and had to be extinguished by another worker – both men were transported to a burn unit – cause of the explosion and fire is unknown at the time – fire was allowed to burn itself out – an Elementary School was quickly evacuated for precautionary measures, and no children or school officials were injured) AMPUTATION Worker’s thumb crushed by falling steel in Cambridge (worker was taken to hospital after his thumb was crushed by falling steel around 11:30 a.m. – the man had been working on a machine when multiple sheets of steel fell onto his left hand) CRUSHING FATALITY West Texas worker crushed to death under semitrailer (part-time employee, 40, has died after being crushed under a semitrailer that he was working on – the semitrailer fell off of a jack onto the man, who died at the scene) AMMONIA & MALFUNCTIONING RESPIRATOR Wyoming man hospitalized after breathing ammonia (worker, 49, remained hospitalized more than a week after he inhaled ammonia fumes through a malfunctioning respirator at a chemical plant that agreed to improve worker safety three years ago in a settlement – the worker was exposed because of a malfunctioning respirator in the first of two accidental ammonia releases Nov. 11, – the first, at 6:30 a.m., released 54 pounds of ammonia over 16 minutes – a second release less than two hours later let out 374 pounds of the gas) CARBON MONOXIDE 13 sickened by carbon monoxide fumes at Suffolk warehouse (unseasonably cold weather was partly to blame for a hazmat situation that sickened 13 – employees were operating fork lifts inside a warehouse with the doors closed, which caused an unsafe level of carbon monoxide inside the building – eight patients were transported to the hospital and five drove themselves to be evaluated – at least one worker was unconscious due to the fumes) UPDATE on HOTWORK FIRE Storm Lake, IA plant damaged by fire will reopen (a turkey plant will returning led to 4 deaths (plant has reported that about 23,000 pounds of methyl mercaptan escaped in the building where four of its workers died two weeks ago – plant disclosed in a news release the quantity of the methyl mercaptan that led to the deaths Nov. 15, but company declined to release a more detailed report of its findings that were delivered to the Texas Commission on Environmental Quality – medical examiners say the chemical caused the asphyxiation deaths of the four workers – officials have said a faulty valve may be responsible for the fatal chemical release) EXPLOSION Explosion at were life threatening) EXPLOSION Lea county: Oilfield explosion under investigation (an explosion at a remote oil field site is under investigation – the explosion left two people injured around 3 p.m. – one with a broken leg and a second with burns to the head and torso – they were airlifted from the site and taken for medical treatment, but their conditions were unknown – it is unknown what company the two men worked for and who the operator of the site is) FIRE No injuries from industrial fire at Bynum plant (an industrial fire broke out at a particle board plant – it was contained within minutes – the fire started in a piece of equipment called a forming bin, where dust and wood fiber from the plant’s operations end up – fire resulted in no injuries – the plant ceased production while the bin was emptied) CHLORINE Suspected Chlorine Leak Affects 41 Workers in Raisen (as many as 41 employees working in various private factories in Industrial Area in the district were affected by suspected chlorine gas leak from one of the units – leak started during the lunch break – workers complained of vomiting and burning sensation in the eyes following which they were rushed to a hospital – 38 were admitted at the local Hospital, three were sent to Bhopal as their condition was serious) CHLORINE Seven mill workers fall unconscious in chlorine-leak (seven laborers fell unconscious because of the effect of chlorine gas that leaked from a pipe – workers were present in large numbers near the spot of leakage that occurred around 9.30 am. – the leakage was plugged traumatic to overcome – she was pronounced dead a few hours later) CONSTRUCTION COLLAPSE Construction worker rushed to hospital after structure collapsed (injured worker, 46, got caught under the falling wooden planks that tumbled down – crews at the scene were working on the structure when they removed a piece of lumber – seconds later, the whole thing came crashing down – they all ran out of the way, but one didn’t make it out on time – worker suffered a broken arm and two broken legs) FALL from LADDER Worker airlifted after Haywards Heath building accident (worker, in his 20’s, has been airlifted to hospital after falling from a ladder on a building site – he was suffering from shoulder and wrist injuries – was fully conscious before he left the scene) FATAL FALL 1 killed, 1 hurt in Jamaica Plain deck collapse (one worker was fatally injured and another critically hurt when wood decking they were installing on a residence collapsed – the men were working on a third-floor rear porch when they fell at 10:37 a.m. – one worker fell about 30 feet to the ground and later died – the other worker landed on a porch attached to the second floor and was being treated for critical injuries) TRENCHING Construction Worker Alive After Work Accident (worker, 24, was laying pipe and got pinched between the bucket and the trench box – a coworker pulled him out of the hole – his head was nearly crushed – he had seven skull fractures – the bone in his temple is broken and then underneath my eye socket is broken) FATAL COLLAPSE Construction Worker Dies After Ceiling Collapse at Staten Island Car Dealership (a construction worker, 43, died after part of a concrete ceiling in a car dealership collapsed – he became trapped under debris around 8:15 a.m. when a large section of the mezzanine level fell inside the dealership – the work permits posted at the car dealership were expired – at the time of the collapse, the building’s interior partitions were being demolished by a company that was involved in a previous fatal accident) HOTWORK FIRE Spark from welding gear caused Shanghai hospital blaze (a spark from electric welding equipment is believed to be what triggered the fire – the flames were ignited near the middle of the building, possibly by sparks falling from welding gear being used above – the blaze was likely fueled by bamboo boards used on scaffolding) EXCAVATOR FATALITY Worker killed while tearing down bridge near Carseland (worker was crushed to death while removing a temporary bridge at about 2:30 p.m. – he was removing a temporary bridge at the time and working along side another employee, who was using an excavator to pull cement blocks away from the river – the two workers were operating heavy equipment when the accident occurred – it appears the victim was crushed) SCAFFOLDING Two injured in scaffolding accident at Sumter Mall (two people were injured in a scaffolding accident – victims were flown to a Hospital after falling about Adelaide Hospital construction site (worker, 54, died from serious head, neck and back injuries when he was crushed between a scissor lift and a concrete slab above) WORKZONE FATALITY 49-year-old trimming palm trees and was working with a friend who watched the accident happened – he was trimming a palm tree when some of the fronds came down and hit him) ELECTRICAL Worker electrocuted, then resuscitated (a worker was electrocuted while attempting to fix an appliance – he was unconscious and was not breathing when emergency crews arrived – paramedics managed to revive him, however, and he
