Houston in electrical accident (power line worker was killed doing routine electrical work – coworker also was also injured in the incident, which happened just before 11 a.m. – the pair were contract workers) PALLET Iowa worker injured in accident (worker at a distribution facility has been critically injured after a pallet fell on her head – the woman was hospitalized in stable condition) MINE FATALITY Worker killed in mine collapse in southern Turkey (work in the chrome mine only began 17 days ago with six workers – one miner, 41, was killed and another, 25, survived the collapse – cause of the collapsed mine has yet to be determined) AIRBAG FATALITY Contract worker injured by 777 airbag at Boeing dies (contract worker who was seriously injured in a Nov. 13 accident has died – the mechanic and an employee of a different contractor were working on a first-class seat in a 777 when a “partially disassembled” airbag system accidentally inflated – worker was taken by helicopter to a Medical Center – the other worker was treated on site, taken to a local hospital and released) Worker dies in accident at Henderson plastics factory (accident occurred before 8:40 am. – initial reports indicate a large object fell on the victim – no other details) PFAS SAVES ANOTHER! shoulder area and injured him) EXPLOSION Plant Explosion Seriously Injures Worker (an explosion sent a worker to a Hospital with serious injuries shortly before 9 a.m. – employee was working on a high pressure molding machine and suffered a serious injury to his leg) AMPUTATION Worker’s three toes amputated after foot crushed by metal machine (worker, 25, was organizing a lift of the press die when it slipped and landed on his foot – worker had to have three toes amputated after his foot was crushed beneath a 300kg tool – he had pushed the die off the press, used a hoist to support the overhanging end, then planned to rest the die on his thighs while he pushed the lifting attachments into the middle to lift it out fully – before he took the weight the die slipped and fell, landing on the toes of his left foot) EXPLOSION Orlando worker burned in explosion (worker was seriously burned in an explosion at a warehouse -the worker was testing a piece of metal when it exploded – suffered burns on his face, hands and arms, was taken to a hospital for treatment – No other details) ELECTROCUTION Solar energy worker dies of electrocution while installing power lines (crew was installing new power lines, connecting a 5-megawatt solar farm to the utility grid when the man was electrocuted – he died while replacing a 40-foot power pole – the worker touched the guy wire after it came in contact with a transformer pole) UPDATE on CS FATALITY Family files lawsuit over worker’s death (worker was found unconscious inside a tanker trailer – he died and two others were hurt when they were overcome by fumes) HOTWORK EXPLOSION Four injured as truck’s diesel tank explodes (four persons suffered burns injuries when the diesel tank of a truck caught fire during welding work – welder was working close to the tank that exploded due to excess heat) UPDATE on REFINERY EXPLOSION Christmas Eve refinery explosion cause by frozen, ruptured pipe (investigators have determined that a ruptured bypass pipe is to blame for a powerful explosion rocked a Refinery last Christmas Eve – report revealed that a bypass line (part of Polymerization Unit 27) that had recently been serviced froze, and that the freezing process caused a rupture in a piece of pipe – once temperatures increased, petroleum product was released through the pipe, causing an explosion and fire – a similar rupture caused by freezing occurred in December 2008 – no one was injured as a result of the blast, which caused an estimated $77 million in damage to buildings and equipment) PIPELINE 5 million liters of oil leaked from Israeli pipeline (pipeline spilled 5 million liters of oil – cleanup is expected to take months and costs billions of dollars – latest reports from the pipeline company show air quality and bleeding off residual ammonia from piping at the plant – evacuation order was lifted and the downtown area was reopened by 5 p.m. barring unusual circumstances) CONSTRUCTION Industrial accident crushes 417 construction worker’s foot (a construction worker, 44, has a crushed foot after an industrial accident on the 417 – he was moving a large construction trailer at about 12:30 p.m. when the hitch collapsed and landed on his foot – co-workers freed him using a large jack – he was taken to hospital in serious but stable condition) FALL Worker falls into deep construction pit in Old Montreal (firefighters were called to a construction site after a worker fell into a four-metre-deep pit and had to be rescued – the man was trapped in a tight spot and the firefighters had to come with ladders to get him out – he suffered minor injuries) STEAM ROLLER Worker taken to hospital after pinned under machine in Beechville (worker was taken to hospital with leg injuries after the steamroller he was operating tumbled off an embankment – he had been driving the piece of heavy machinery at a work site at about 3:15 p.m. when the roller fell about three metres to the ground below – his leg was trapped under the machine until firefighters and paramedics arrived and freed him) WORKZONE FATALITY Construction worker killed in late night crash on Route 422 near North Coventry (a road construction worker, 60, was killed when he was struck by a vehicle – he was working in the active work zone when he was struck by a Jeep Liberty traveling west – he was employed as the project superintendent and was assisting with setting cones for a lane closure and line painting when he was struck – he was pronounced dead on the scene) CRANE Man injured in crane accident (worker was rushed to a Hospital after being injured by a crane boom – he suffered moderate back injuries in the accident – it was believed the boom of the crane fell off the back of a truck – no other details were available) WORKZONE FATALITY Worker killed after being struck by vehicle on M-142 (accident involved a road worker being struck by a motorist) FATAL WALL COLLAPSE Man dies after wall collapse at London building site (man was thought to be a construction worker and in his 30s – he was killed after a wall collapsed on a building site) FALL One Injured In Construction Accident On UCO Campus (a construction worker was injured after falling from a clock tower on the campus – the worker was breathing when they arrived – scanner traffic indicates the worker may have fallen from 30 feet – he was transported from the scene by ambulance, in Serious Condition) WORKZONE Tree worker in serious condition after struck by car (tree worker was seriously injured when he was hit by a car – person was taken to a Hospital in critical condition – the tree crew was apparently cleaning up from a job and had a person directing traffic when the incident happened about 10:15 a.m.) PAVING FATALITY Worker Dies in Construction Accident at BWI Airport (worker died after he was struck by a piece of paving equipment at about 2 p.m.) SKID STEER FATALITY Man dies after being crushed by Bobcat bucket at Edmonton workplace (worker, 45, died after being struck by the bucket of his Bobcat while he was tending to a leaking hydraulic line – the bucket fell on top of him – bucket hit him in the neck and pinned him between the arm of the bucket and the Bobcat – emergency crews pronounced him dead at the scene) HOTWORK Two men hurt in north Wichita house fire (two men, 52 and 56, were hospitalized after being burned in a fire shortly after 6 p.m. – the men were welding copper pipes in a crawl space when the torch ignited surrounding combustibles – they were taken to a Hospital for treatment of man were unsuccessful and he was declared dead at 9:21 am) FORKLIFT Toppled forklift causes excitement but little damage to Keene home (a forklift raising a load of roofing shingles had fallen forward into a home – a temporary worker operating the large telescopic forklift misjudged the weight of the pallet of shingles he was lifting with the more-than-30-foot arm, and did not anchor the machine’s front stabilizing pads – forklift pitched forward, hitting the side of a house) MISCELLANEOUS DROWNING @ GOLF COURSE NC golf course worker killed on course (maintenance worker, 46, at a golf course has died after the equipment he was driving went into some water on the course – circumstances of the accident are unclear – a preliminary autopsy indicated that he drowned – two other workers pulled him from the water near the golf course’s fifth hole and performed CPR but could not revive him. CHEMICAL Country club worker overcome by cleaning fumes (a Country Club employee was transported to a Medical Center after being overcome by hazardous fumes while trying to clean an ice machine – the employee, in response to a recent health inspection, was cleaning the ice machine using Nu-Calgon liquid cleaner that contains Phosphoric Acid and is known to cause burns to the eyes, skin and mucous membranes – the employee was overcome by the fumes when the cleaner was opened – another employee was monitored on scene but refused treatment) HOTWORK Equipment catches fire, damages Athens Street business (no one was injured when equipment being repaired caught fire and damaged a business -fire started about 1:37 p.m. on a piece of heavy equipment called a skidder – skidder was in an area where an independent contractor was making repairs and had just finished welding. – fire appeared to be accidental in nature and is being turned over to the insurance companies – damage to the building was $75,000 to $100,000, and the skidder was valued at $50,000) FORKLIFT Baby boy fighting for life after forklift accident (nine month-old boy is in critical condition after being struck by a forklift driven by his father at an automotive workshop – boy suffered critical head injuries when he was hit by a forklift – he was taken to a Children’s Hospital in a critical condition – the boy underwent surgery and his conditioned had remained critical throughout the day) |
