SANITATION AMPUTATION Artist who works for NYC Sanitation Department has drawing hand amputated after on-the-job injury (worker, 43, fell from the cab of a collection truck and had his left hand crushed under one of the vehicle’s front tires – doctors amputated his left hand at the wrist)

Man dies in Stemilt Growers accident (worker, 34, died in an accident at a facility – no details)

FATAL EXPLOSION China factory explosions kill 17 workers (at least 17 workers were killed by explosions in an automotive components factory – another 20 people were injured after three blasts ripped apart a machinery factory)

India BHEL Accident Kills Worker (an accident caused one apprentice worker to be killed and another seriously injured – two workers were struck by heavy machinery falling from above – the man who was killed was in training, though the report did not indicate how long he had been on the job – no details have been released about how the accident happened or what the machinery involved was)

STRUCK BY FATALITY Worker killed in accident at Ford Claycomo Assembly Plant (a contract worker was critically injured about 8 a.m. and died at a hospital a short time later – the man was working in a pit when the beam came loose and call on top of him – no additional information is available at this time)

FATAL EXPLOSION @ OIL RIG Explosion, Fire At Oklahoma Oil Drilling Rig Kills Two Workers, Leaves Three Others Critically Injured (two workers, 26 and 27, have died of injuries suffered during an explosion and fire at an oil rig – they died after suffering injuries during an explosion at the rig – out of the three injured workers, two were transported to trauma centers in critical condition, while another sustained burns to his hands)

LANDFILL TRACTOR FATALITY Worker struck, killed by tractor at Delaware landfill (landfill worker, 42, was struck and killed by a tractor at about 12:35 p.m. – he was standing behind a crawler tractor being operated by a co-worker – the tractor backed up and struck him – he was pronounced dead at the scene)

FALL or CONFINED SPACE PD: Man in coma after falling into well in Daytona Beach (worker, 41, is in critical condition after falling into a well while on the job – a co-worker found him lying unresponsive at the bottom of a 3-foot well at the gas station under construction just after 6 p.m. – co-worker flagged down a passer-by, who then called 911 – Police said he may have hit a pump in the well and collapsed – there were unsafe amounts of gases being emitted from the well – worker was taken to a Medical Center, where he is in a coma – he works for a company that cleans underground tanks – it is unclear whether he was injured during the fall or overcome by gas fumes within the well)

PLATFORM Worker airlifted to hospital after offshore accident (an offshore worker, 57, has been airlifted to hospital after being hit on the head by a falling object – he was reportedly hit on the head by a falling pipe or hose – he suffered head and rib injuries, but it is not known how serious his condition is)

ELECTROCUTION Engineer electrocuted in mini-power plant at Gudalur (an electrical engineer, 27, working in a mini-power house on contract basis was electrocuted while he was engaged in a trial run of the plant – he was electrocuted while he was making readings – this was the second fatal accident in the power plant within a year)

FATAL EXPLOSION 1 killed, 13 injured in blast at Visakhapatnam ferro unit (an explosion rocked a ferroalloys unit killing a worker and injuring 13 others – the explosion occurred due to excess gas eruption in a furnace – one employee died on the spot while the condition of two employees among the injured is said to be critical – preliminary investigation shows that various gases formed in the submerged arc furnace erupted all of a sudden resulting in the blast)

NITROGEN ASPHYXIATION Gas leak kills three at South Korean n-reactor’s construction site (three workers have been confirmed dead, while another one was wounded, in a gas leakage accident where a new nuclear reactor is under construction – the workers were suffocated at about 5.18 p.m. at the construction site – nitrogen gas was believed to be leaked underground at an auxiliary building of the reactor, but the exact cause was under investigation)

FATAL FALL from LADDER A shiprepairer has fallen to this death on a research ship in Galveston, Texas (worker was on a ladder about 30 feet above the deck at the time – he lost his balance and fell, striking his head on a beam)

LINEMAN ELECTROCUTED SaskPower worker killed (worker, 45, electrocuted – workers had been repairing a high voltage transmission line when a worker was electrocuted – he was injured but did not die at the scene – he was rushed to a hospital where he was pronounced dead shortly after)

FLASH FIRE 2 hurt in explosion, fire at Standard Aero plant in Winnipeg (the spray from an aerosol can ignited, creating a ‘flash’ that burned two employees – it’s not known why it happened – the injuries were not life threatening – one worker had burns to the arms, the other to the hand and face – employees were taken to the burn unit – caused almost no damage to the plant)

MOLTEN ALUMINUM Employee burned in explosion at Mentor die casting company (one worker suffered burns during an explosion at a business where they specialize in aluminum die casting – worker was working with a molten aluminum die machine when moisture got into the aluminum, causing it to explode – the aluminum got on the operator, which was taken to an area hospital with burns – employee is listed in stable condition)

EXPLOSION @ ALUMINUM PLANT Ajman factory blast causes injuries to 2 workers (an explosion at the cooling unit of an aluminum factory left two workers seriously injured – doctors had to amputate a leg of one of the injured workers while the second worker suffered burns to various parts of his body – police are carrying out investigations to determine the cause of the blast)

UPDATE on FATAL COM DUST EXPLOSION China Dust Blast Explosion Death Toll Nearly Doubles (Chinese authorities on Tuesday said an August explosion at a factory that supplied auto parts for car makers killed at least 146 people, nearly double the initial death toll – government also said that it would prosecute three senior executives of the company that owned the factory where the explosion occurred, as well as 15 government officials – the statement added that the city’s top Communist party leader and the mayor were ousted from their jobs over the matter – the explosion was caused by an excess level of dust that exploded after exposure to certain heat sources)

ELECTROCUTION @ BILLBOARD 3 construction workers electrocuted (three construction workers have been electrocuted working on a billboard – they came in contact with a live electric wire from a power pole nearby – four of the injured were rushed to a Hospital where the on-duty doctor declared the three dead – another worker who was also critically injured was undergoing treatment at the hospital)

CONFINED SPACE FATALITY – SEWER GAS Toxic fumes claim worker’s life, civic body blames contractor (worker, 24, died due to suffocation while clearing the clogged pipeline – two other sanitation workers also fell unconscious but were saved after a compressor machine installed on a tractor forced air into the drainage pipe – around 7 pm, the deceased and two others opened one of the chambers and entered the sewer line – the deceased reached around 50 metres into the chamber but began suffocating and could not even shout for the help – the other two workers fainted at the spot – another worker standing near the chamber shouted and called for the compressor machine installed on a tractor – once the machine forced air into the drainage pipe, the two workers regained consciousness and were rescued – civic officials said the workers were not supposed to start the work and they were neither provided with adequate safety equipment to undertake such work)

WELDING EXPLOSION – CUTTING DRUM Officials: Man welding drum badly burned when it explodes (explosion happened just before 11 a.m. at a recycling business – an employee was welding on a 55-gallon drum outside the business when the drum exploded – the man was burned over his entire body)

HOTWORK EXPLOSION in CONFINED SPACE Torch ignited gas, causing blast and fire at La Crosse plant (the cause of a tank explosion and fire at a plant has been determined – Fire Department said a torch ignited flammable vapors from a tank at an Asphalt plant and that caused the explosion just before 7 a.m. on November 19 – the tank that exploded and caught fire was destroyed)

REFINERY FIRE Blaze at Oregon refinery under control (the fire was reported at 10:20 a.m. – at 11:20 a.m., the fire appeared to be under control – no injuries had been reported – it was not clear what started the fire)

SALT BUILDING COLLAPSE Morton Salt Was Piled Too High, Causing Wall to Collapse, Cars to be Buried (the salt kept inside the huge storage facility was piled too high, city officials said, causing a side wall to blow out and an avalanche of salt to pour out – the salt buried at least 11 Acuras owned or being serviced by the McGrath Acura dealership next door – nobody was hurt — although McGrath workers said the had to run when they saw and heard bricks falling from the neighboring wall – Department of Buildings Director of Public Affairs said “a preliminary inspection determined that salt inside the building structure was piled too high causing the exterior wall to fail.”)

CHLORINE Three workers hospitalized after chlorine release chemical plant (three people were taken to the hospital for exposure to chlorine at a chemical plant – officials with the plant confirmed there was a chlorine release from a product storage tank – a yellow-colored cloud was seen over the plant at the time of the incident – there was no off-site impact – workers were transported to a hospital for evaluation)

EXPLOSION Fire caused by pipeline explosion at Washington County gas plant under control (officials not yet sure what caused fire at a ethane pipeline meter station forcing 13 people to be evacuated – fire was under control and that there were no injuries – an explosion occurred at the facility at around 10:15 p.m., resulting from the over-pressurization of a gas line into the facility -fire was under control by 11:40 p.m.)

HYDROGEN CYLINDER @ SEMI-CONDUCTOR PLANT Explosion at Hemlock Semiconductor plant injures one in Thomas Township (a cylinder at the facility exploded – a truck carrying hydrogen “experienced an explosion and subsequent fire while unloading” – the driver of the truck experienced some burns and was transported to a local hospital – there was no impact or danger to the community, however, the explosion was loud enough for neighbors of the site to hear)

AMMONIA 3 treated after ammonia leak at poultry plant in Gainesville (three people were treated for exposure to ammonia after a leak at about 1:30 p.m. – the leak began when workers accidentally cut an ammonia line that mistakenly believed to be inoperable – renovations at the plant are ongoing)

AMMONIA Firefighters shut down Nelson ammonia leak (firefighters shut off an ammonia leak from a meat works – a passerby who smelt the gas called emergency services – ammonia was leaking from a refrigeration area of the plant – two firefighters and a plant engineer wearing fully enclosed protective gear were able to stop the leak with a relief safety valve – plant was shut down for the Christmas break)

AMMONIA @ WWTP Ammonia leak at Saskatoon’s water treatment plant (half a dozen homes were evacuated after a high pressure gas line was ruptured -there was no danger to the public after an ammonia leak at a water treatment plant around 9 p.m. – there had been a spill or release in the ammonia room located in the basement of the building, setting off ammonia detectors – firefighters wearing chemical suits entered the room and used water on the liquid – built-in ventilation fans were activated to remove the vapours – a feeder valve, suspected to be the source of the leak, was then turned off – no injuries were reported)

AMMONIA Ammonia Leak in Auburndale Sends 2 to Hospital (warehouse is expected to resume operations after an ammonia leak forced the evacuation of seven employees, exposed three to the vapor and sent two to a local hospital – personnel were performing routine maintenance on warehouse equipment when the “small ammonia vapor release” occurred – one hundred pounds of ammonia were released in the building – Fire rescue used hydraulic fans and a water curtain to dissipate the ammonia from the building – the two employees who were transported to the hospital were examined for their levels of exposure – they were cleared and released by noon)

CHLORINE @ WWTP Chlorine leaks from Detroit wastewater treatment plant (neither members of the public nor employees were exposed to any harmful chemicals – the 13-minute chlorine leak occurred in a process area – scrubber inlets in that area pulled the gas in and treated it with chemicals to reduce the toxicity before releasing it into the air – spill happened between 3:06 p.m. and 3:19 p.m. – the cause of the leak is under investigation – it was initially discovered during a preventive maintenance check and the faulty equipment has been isolated)

FORKLIFT Man seriously hurt by falling steel plate at Wisbech firm (a man has been taken to hospital with serious injuries following an industrial accident – a steel plate had fallen on to a man from a forklift – a man in his 20s was treated for a suspected major pelvic fracture and was taken to a Hospital by the air ambulance for further treatment)

CONSTRUCTION

WORKZONE Deer Park Construction Worker Injured in Hicksville Accident (worker, 55, walked northbound on the sidewalk to assist other employees placing a portable traffic sign onto the trailer hitch of the truck, when he stepped off the sidewalk and was struck by the truck that was slowly backing up – he suffered multiple fractures and internal bleeding – listed in critical condition)

Two Workers Injured In Fort Worth Industrial Accident (two workers are recovering after an accident – a large metal beam fell on one patient and at the same time another patient suffered burns to his face)

BEAM COLLAPSE OSHA: Iron workers were on steel beam when it collapsed (at least four people were injured in a construction accident when a steel beam collapsed sending three iron workers spilling to the ground and injuring a fourth worker standing nearby – the three workers had just connected the beam about 14 to 20 feet above ground when it gave way – a fourth person was injured by falling debris)

SCAFFOLDING COLLAPSE At least 10 construction workers killed in scaffolding collapse at Beijing school (a scaffolding collapse at a high school killed 10 construction workers and injured four others – the accident occurred at the start of the school day but did not affect any classrooms or other buildings used for instruction – the injured were in stable condition – an initial investigation found that steel bars used to reinforce concrete had collapsed and caused the accident)

WORKZONE FATALITY Worker killed while putting reflectors on road (a road worker has died following a car accident in a construction zone – a 2014 Chevrolet pickup (construction vehicle), equipped with a large arrow board was stopped in the north bound outside lane – the truck was stopped while three construction workers were putting out reflectors on the road – the accident occurred when the tar machine broke down which forced the truck to stop – when the construction truck stopped, another truck stopped behind them with flashing arrows – a Cadillac ATS struck one worker head-on and injured two others – worker, 30, was the worker who was struck head-on – he was airlifted to a nearby hospital where he later died from his injuries – the two other construction workers are currently recovering from moderate injuries)

SCAFFOLDING COLLAPSE Two killed in LRT accident (two workers were killed and six others were injured when the formwork and scaffolding collapsed – all of the injured suffered minor injuries)

FATAL COLLAPSE Chinese among 4 dead in accident at Jhelum dam site (four people were killed when a wall collapsed at a construction site for a Hydropower Project – the workers were installing a steel reinforcement of 38 mm between the descanter and main dam when suddenly the reinforcement left its place and fell down hitting five workers – four workers died on the spot and one worker was found breathing in final evacuation who was immediately shifted to a hospital – according to initial investigations, the accident happened due to mistake of a worker who was cutting pipes)

AUGER AMPUTATION Worker has leg amputated following freak accident (worker slipped and fell into an auger at a highway construction site was forced to have his leg amputated and remained in the hospital – the worker apparently fell and become trapped in a running auger – crews were able to free the man from the auger, and then he was immediately airlifted to a Medical Center – he was working to control street traffic in the area around the construction site when he apparently slipped and fell into the running auger)

FATAL FALL from PALLET Construction worker dies in fall, sheriff says (worker, 52, was working on the installation of an overhead door when he fell from a raised pallet and died)

WORKZONE Woman hit construction worker on I-215, UHP says (a driver suspected of driving under the influence drove into a work zone and hit a construction worker on I-215 – the contract laborer was fixing a barrier guard rail at the time of the accident – the driver ran onto the shoulder and struck the worker – the worker, who is approximately 40 years old, was in fair condition with injuries that are not considered to be life-threatening)

FATAL TUNNEL COLLAPSE Three Turkish workers killed at tunnel construction site in Iraqi Kurdistan (three workers have been killed due to a collapse at a tunnel construction site – workers were found under the debris of a structure that had collapsed in the tunnel – two of the dead workers were brothers – another worker broke his leg during the accident)

TRUSS COLLAPSE Three injured in construction accident at Timberwood Church (three employees were injured when about a dozen trusses collapsed – the injuries were not life-threatening – there were 10-12 employees and subcontractor employees at the scene at the time of the accident – the injured employees were partially trapped by collapsed truss work – the construction crew was working within the remodel/addition area at the church)

2 workers hurt in Erlanger construction accident (two workers were injured after a construction accident – they were transported by medical helicopter to a Medical Center – conditions and identities were not immediately available – the workers were subcontractors – no word on cause)

EXCAVATION EXPLOSION Fire and Gas Explosion (a construction worker suffered significant burns to his arms with possible facial burns during a flash fire involving a gas line shortly after 1:30 p.m. – contractor was apparently using a hand held electric digging device with a metal spade and hit a natural gas line – the worker was extricated out of the hole by a co-worker, and transported via private vehicle to a Hospital)

TRENCH FATALITY Man killed in trench collapse in Cherrybrook (a construction worker has been killed in a trench collapse – he died at the scene – it is understood no one else was injured)

SCAFFOLDING Man hospitalized after Ala Moana construction accident (a man was hospitalized after an accident at a construction site at 8:35 a.m. – an Emergency Medical Services report indicated a man in his 40s fell roughly 20 feet, apparently from scaffolding, but a source tells KHON2 scaffolding actually fell on the man – paramedics treated him on scene, then took him to the hospital in serious condition)

MISCELLANEOUS

Worker airlifted to hospital after industrial accident near Caruthers (worker was airlifted to a Medical Center after an industrial accident – victim was pinned between two vehicles about 7 a.m.)

2 parks workers hurt by falling tree limb (two county workers were injured at a Park when a large limb from an old oak tree fell on them during this week’s windstorm – workers were part of a crew of four or five parks employees clearing another fallen limb when the accident occurred – one of the workers was taken to a hospital for treatment of a head injury – he was wearing a hardhat but was knocked to the ground when the limb fell – he was conscious and talking after the accident – the other worker was treated and released from an area hospital)

GRAIN AUGER Man flown to hospital after farm accident (a farm worker, 55, suffered a severe leg injury in an accident at 3:40 p.m. – he was injured when his pants got caught in a grain auger – extrication of the man from machinery was not needed – injuries did not appear life-threatening, they were severe enough to contact Flight For Life to transport the man to a Hospital)

HOTWORK FIRE Killeen: Apartment Maintenance Worker Burned (a maintenance worker was burned at an apartment complex after some insulation caught on fire as he soldered pipes – worker received second-degree burns on his hands as he extinguished the flames)

HEATER EXPLOSION Propane explosion ‘lifts the roof off’ Whitehall apartment, injuring 2 men (a propane explosion “lifted the roof off” an apartment and injured two men after a worker tried to restart a gas-fueled heater there – a gas employee and an apartment tenant suffered burns in the 9:15 a.m. blast – gas employee has facial burns and burns to his hands – the propane worker was right next to the heater when it blew up)

Nursery worker crushed by 4,000 pound weight (accident critically injured an employee of a nursery – the victim was operating a front end loader when a large attachment used to tie and bail trees slipped due to the fact that it had not been properly stabilized with a hydraulic locking mechanism)

HOTWORK Welding sparks gasoline fire at LaGrange auto body shop (fire started just before 1 p.m. as employees were pulling a gas pump out of a truck and also welding when sparks flew onto leaked gas and caused the fire – the building is not a complete loss)
