INDUSTRIAL

Worker injured in accident at Aviagen plant in Lawrenceburg, TN (contractor was injured at a chicken house at a poultry breeding company – worker was moving chickens or eggs in a cart, and the cart turned over on the worker – cart weighed about 500 pounds – worker was flown to a Hospital for treatment)

BOILER EXPLOSION FATALITY Worker killed, 12 others hurt in Bataan refinery accident (Department of Labor and Employment (DOLE) has stopped a Power Plant and its subcontractor from operating a boiler inside a Refinery after an accident there killed a worker and injured 12 others – fatality was a pipe fitter – ten other contractors and two employees suffered burn injuries – DOLE said the accident may have been triggered by “work methodology lapses particularly in the operation of the equipment” and “insufficient safety provision” for workers)

FALL thru SKYLIGHT CLEARING SNOW U.S. Foods worker injured in fall through skylight (an effort to clear a snow-covered roof led to serious injury when an employee, 47, stepped on a skylight, apparently hidden by the snow, and fell through – he hurtled 27 feet to the warehouse floor below – employee worked in the maintenance department – he was Medflighted to a hospital with two broken legs, abdominal issues and difficulty breathing)

LOTO/GUARDING FATALITY Industrial accident claims life at Tyson Foods (a contractor, 38, of a cleaning company was killed while cleaning and sanitizing equipment – he was dead after apparently falling into a piece of machinery – the plant was not in operation but was being cleaned and sanitized – it happened while the worker was cleaning some equipment – co-worker heard a noise about 10 to 15 minutes later, heard the machine bog down and saw him caught in it – the co-worker pushed an emergency shut-off button, then went to get the supervisor, who went to get the cleaning firm’s top on-duty supervisor while a third supervisor called 911)

CRUSHING FATALITY Worker crushed to death by truck in Joliet (worker, 22, was crushed to death while working – he was involved in a work-related accident and was pronounced dead at 4:54 p.m. – he died of multiple injuries due to being crushed between a trailer and a semi – the victim was operating a piece of machinery or truck moving containers about 4 p.m. when it got stuck on a patch of ice – he exited his truck to try and figure out how to get out of the ice, but “unbeknownst to him,” another driver began backing into the victim’s truck to try and free it when the victim was pinned)

COMPRESSED GAS CYLINDER on FIRE PROTECTION SYSTEM Offshore worker injured by “projectile” gas cylinder (an offshore worker was seriously injured when a gas cylinder shot through the air and hit him on the leg – he had been working in a diesel turbine generator on a platform when the cylinder suddenly released the gas inside – HSE investigators said “On November 10, 2014 a serious injury occurred to a worker whilst attempting to change a compressed gas cylinder on the fire water spray system. There was instantaneous release of the contents from the compressed gas cylinder resulting in the cylinder acting as a projectile and striking the worker, resulting in serious injury.”)

CRANE Man hurt in Christchurch glass factory accident (a glass factory worker, 35, was freed in a “meticulous” rescue operation after he was pinned under 1.9 tonnes of glass – employee was fortunate to escape with bruising to his pelvis and a pinched nerve after the workplace accident – he was straddling a knee-high glass rack as a crane lifted a 1.9 tonne load of glass panes – load unexpectedly dropped 50 centimetres on to the worker’s pelvis, trapping him against other panes of glass)

CONFINED SPACE Rescuers free worker trapped in container at warehouse in SE Houston (crews rescued a man who was trapped inside of a container at an industrial warehouse – the worker may have been cleaning a tank when he became trapped inside – he was freed around 11 a.m. and loaded onto a stretcher and transported to the hospital in stable condition)

CRANE into POWER LINES Worker shocked while installing power pole in Flagler (an electrical worker was airlifted to a burn center – he was part of a crew installing power poles – the poles support high power lines that run 13 kilovolts of electricity – he was standing on the ground guiding a new pole into position and as a crane lowered the pole, the pole accidentally touched a live wire – voltage ran through the pole, into and through the worker, and into the ground – he was not wearing protective gloves at the time)

MINE FATALITY – TRUCK TIRE EXPLODES Man dies as tyres explodes at Qld mine (an exploding tyre has killed one mine worker and seriously injured another man at a coal mine – the tyre on a large mining truck exploded – the injured man suffered a possible fractured shoulder)

CONFINED SPACE FATALITY – ENGULFMENT Worker who died in Cobb concrete plant accident identified (worker, 49, died in a concrete plant accident – his body was found around 9 p.m. – a 911 call around 3 p.m. from the business said a man was trapped inside a concrete silo – two workers had been doing repairs inside the silo when one slipped and fell – he was buried under the gravel – fire and emergency services personnel worked through the night to get to him)

CRANE FATALITY Ogallala Man Killed in Industrial Accident (worker, 59, died asworkers were doing maintenance to a elevator and were working with a crane when a large part of the leg collapsed – during the investigation deputies say the fertilizer leg appeared to be rusted at the base, which caused the base to give out and collapse on the roof of the building – other crew members were around, but no other injuries were reported)

GAS WELL EXPLOSION Worker burned in gas well explosion (contractor suffered burns during an explosion and subsequent fire at a gas well – the explosion happened in a 10-by-12-foot metal shed that housed a pump generator for the gas well – contractor went to check on the site at about 5 p.m. and suffered burns to his face, hands and arms – the site was a shallow oil and gas well – worker was transported to a burn unit)

UNLOADING Chemical cloud over Spanish town after factory accident (a chemical accident at a factory injured three people and cloaked large swathes of the region in an orange chemical cloud, forcing over 60,000 people to stay indoors – the “explosion or chemical reaction” happened when products being “loaded or unloaded” at a factory became mixed – the general warning was lifted about an hour later but maintained for another three hours for pregnant women, children, the elderly and those with respiratory problems – cloud may have been caused by a reaction between nitric acid, which is corrosive and toxic, and ferric chloride and another chemical which still has not been identified – two employees who were transporting chemicals were lightly burned and a worker at the plant was also injured)

Chemical explosion at St. Charles industrial plant injures worker (worker injured in chemical processing-related explosion – injured worker flown to a Hospital – Firemen found a small fire in a mixing vat in the mezzanine of the plant – the fire was extinguished in 30 minutes – the products used in a mixing operation at the time were part of a powder coating process – there were no other injuries – incident caused $25,000 damage to machinery and contents)

HOTWORK Sparks from spot-weld operation likely cause of Holland industrial fire (sparks from a spot-welding operation likely caused a roof fire that caused an estimated $10,000 in damage – firefighters responded about 6 a.m. on a report of a fire on the roof – workers had evacuated the building and firefighters discovered the fire was in the wood framing to a skylight assembly)

CONSTRUCTION

WALL COLLAPSE 4 workers injured in accident at Highway 99 tunnel project (five workers were installing rebar for a concrete wall at the tunnel’s north portal work zone – the wall of rebar gave way, injuring four of the five workers – the injured workers are awake and alert, in satisfactory condition and undergoing further evaluation – some have arm, back and neck injuries – the workers fell about 25 feet)

PRESSURE TESTING GONE BAD Construction worker hurt when pipe bursts at site of future hospital in Carrollton (construction worker was seriously injured when a pipe burst at the site of a hospital being built – two construction workers were pressure testing a cold water line when it blew – one of the workers was critically injured and taken to a Medical Center – another worker was blown back by the accident but wasn’t hospitalized)

UPDATE Family of worker killed in I-75 accident sues Ohio (parents of a worker, 35, killed when a highway overpass collapsed have sued the Department of Transportation – worker died Jan. 19 during demolition work to remove an Interstate 75 ramp bridge – lawsuit charges that the department was negligent and breached its duty of care by allowing improper and unsafe conditions at the site for work it had contracted to have done – seeks unspecified damages and payment of $11,295 in funeral expenses)

FALLING HAMMER Worker injured in industrial accident at Union Station (construction worker was injured after a hammer hit the back of his neck from about 30 feet around 2 a.m. and found the man at the bottom of an elevator shaft – he couldn’t move his arms or legs)

WORKZONE FATALITY Female flagger struck, killed by car in Perry County (a road worker, 52, who was struck by a car and killed – she was working as a traffic flagger and was helping to direct a truck that was moving a mobile home at the time of the incident – an oncoming car hit her parked car, which then struck her)

WORKZONE FATALITY Construction worker killed in Alabama work zone crash (construction worker, 25, was killed when a pickup truck crashed into the trailer he was working on)

CONFINED SPACE FATALITY Man dies in construction accident (worker was crushed to death on a construction site after his clothing apparently got caught in a large construction drill in a manhole – he died on the scene)

WORKZONE Two construction workers hit by car in SW Austin crash (two construction workers were struck by a car accident around 4:05 p.m. – two vehicles were involved in what has been termed a minor collision – two construction workers were hit in the crash – both workers were taken to a Medical Center with non life-threatening injuries)

EXPLOSION Worker suffers burns to face and arm in explosion at Toronto construction site (a construction worker was sent to hospital with burns on his arm and face after an explosion while work was being done on GO train tracks in the area – damage was limited to the construction site)

EXPLOSION Two Riverhead construction workers suffer ‘extensive burns’ in gas explosion (two construction workers were injured when they accidentally cut a gas line in the basement of a home, causing an explosion that leveled the two-story wood-frame house – worker, 18, is in critical condition and worker, 40, is listed in stable condition – both were airlifted with “extensive burns” – they were working in the basement of the unoccupied house, whose owner had a demolition permit application pending)

CRANE Man crushed by four tonne machine during Sleaford Renewable Energy Plant (worker suffered major injuries when a four and a half tonne piece of machinery fell on him – he was helping to install a conveyor belt into the incinerator, which is made of several pieces each weighing four and a half tonnes – lifting equipment failed and one of the parts topped over and fell on the worker)

Explosion, fire shake Cleveland Clinic in Weston (an explosion and two-alarm fire at a Clinic facility led to a partial evacuation of the area – the blaze broke out on a low roof adjacent to the clinic’s $90 million neurological center, due to open later this month – there were no injuries – two construction workers on the scene speculated that the blast was caused by tar falling onto a propane tank – a 100-pound propane tank was being used in building at the time, and its exact role was being investigated)

FATAL EXPLOSION Man refinishing floor dies after solvent explodes (worker, 36, suffered severe burns to most of his body and was taken in critical condition to a hospital – worker was refinishing a living room floor using a solvent, which somehow caught fire at around 8 p.m. in the sixth-floor apartment – there was an explosion and flash fire – worker suffered severe burns to most of his body and was taken in critical condition to a hospital where he succumbed to his injuries)

HOTWORK Fire caused by cutting torch at Rogers building (fire broke out when sparks from a cutting torch got into the vent system and ignited metal dust – a construction crew was in the process of removing line equipment – workers had been using a cutting torch to dismantle equipment inside the building – sparks got into the duct system, igniting residual magnesium and aluminum dust – the ventilation ducts are large)

FATAL SCAFFOLDING COLLAPSE 7 killed in SW China scaffolding collapse (scaffolding collapse at a construction site has killed seven people and injured another eight – construction work was being done on a four-floor student center – out of the 8 injured, 3 people are in serious condition)

FATAL SCAFFOLDING COLLAPSE Morocco: Four Workers Killed At Mohammedia Thermal Power Plant (four workers were killed when a scaffolding structure collapsed at the thermal power plant – accident occurred when five workers of a company specialized in metal construction and boiler making were fixing the metal canister of a chimney of the Unit 2 of the plant – scaffolding structure collapsed, killing three people on the spot – the fourth victim succumbed to his injuries in hospital)

WORKPLACE VIOLENCE

2 dead, 2 injured following shooting at SD manufacturing plant (an armed man shot and killed an employee, 45, at a manufacturing plant, then shot a second person and assaulted a third before fleeing the facility – the gunman, 52, apparently took his own life later in the day – authorities found what they suspect is the gunman’s body near a burned-out semi-tractor a few miles from the plant – another worker, 46, was shot and is in critical condition)

MISCELLANEOUS

FATAL FALL from BUCKET TRUCK Tree trimmer dies in bucket truck accident in North Versailles (a worker from a tree service was trimming branches in a bucket truck – he either fell out or was thrown out of the bucket and to the ground and was killed)

UPDATE on HOTWORK FIRE AvalonBay workers who sparked massive Edgewater fire never called 911 (workers who sparked a massive blaze that displaced 1,000 people didn’t delay their call to 911 for 15 minutes, in fact, at no point did they call the emergency number – a review of dispatch logs, 911 tapes as well as interviews with officials has revealed no record that any employee alerted authorities to the Jan. 21 fire – one of the two complex buildings was destroyed in the fire, permanently displacing about 500 residents)
