MACHINE SHOP FATALITY Coroner: Man killed in Akron machine shop was crushed (worker, 60, was killed in a machine shop accident – he died of a crushing injury to his torso – co-workers found him beneath a heavy metal frame – co-worker told a 911 dispatcher that the frame weighs 16,000 pounds – he was a foreman and had been employed there for 38 years)

FATAL FALL @ PAPER MILL Worker killed at Natchitoches Parish paper mill (worker, 42, dies from fall at paper mill around 11:30 a.m. – he was on the scaffolding crew when his harness became tangled in the machine and he fell in – this is the second death at the paper mill this year – in January a worker, 57, fell 15 feet to his death)

Worker injured in St. George Steel accident (worker was seriously injured after being struck and pinned by a steel beam – worker suffered a broken femur and large laceration on his left leg, along with injuries to his other leg – workers used a crane to lift the beam off the man’s body, splint the leg and keep him stable after calling 911 – injuries were “serious, but not life-threatening”)

FATALITY One man dead, another airlifted after Vaughan industrial accident (one man is dead and another is in critical condition after heavy equipment fell on two workers around 4:45 p.m. – a door fell on the two workers – paramedics and firefighters tried to resuscitate one of the men at the scene, but he succumbed to his injuries a short time later – the second worker, in his 50s, was airlifted to a trauma centre with critical injuries to his head and back)

FATALITY Worker killed at Ace Pole recalled as ‘family’ (worker, 35, died at about 7:19 a.m. – they were loading logs into the kiln to be kiln-dried and the victim was somehow caught and crushed between some logs)

LOTO FATALITY Man Dies Horrible Death After Falling into Meat Grinder (a sanitation worker, 41, was killed after falling into a meat blender – he was providing cleaning services when he reportedly fell into an industrial meat grinder while it was in service – another worker rushed to turn the machine off, but it was too late)

OPP identify man killed in tractor accident (worker, 39, was killed in a workplace accident involving a tractor and several stationary railcars at an oil recycling facility, where workers convert cooking oil into fuel – he was pinned between a rail car and a tractor while trying to move the rail car on a spur line)

FLASH FIRE Employee taken to burn unit after fire at metal factory (an employee has been taken to a burn unit after an accident at a metal factory – the fire started when a worker was mixing paint and a spark ignited the vapors – the employee that was mixing paint was taken to a hospital burn unit but did not clarify his condition – all the other employees in the building evacuated without injury)

CONFINED SPACE – ENGULFMENT Worker trapped in silo has ‘near miss’ in Great Village (worker, 20, was working in the silo trying to clear a clogged auger, when he was trapped – he called for help on his cellphone and two other employees responded and got a rope tied around the man sinking in the silo – after more than an hour, the man was free, unhurt, but cold and shaken as he walked under his own power to the waiting ambulance)

FIRE Worker injured in fire at business near Black River Falls expected to be okay (employee burned in a large fire at a business is expected to be okay – fire happened in a mechanical shop, and crews managed to contain the flames to that area – employee was taken to a hospital to be treated for burns on his leg and hands – those burns are not life threatening – cause is still under investigation)

FIRE Workers Injured in Suspected Explosion and Fire at Dyno Nobel (a fire at a chemical plant has burned three people around 9:30 a.m. – he says the company fire brigade at the plant had the fire under control by the time county firefighters arrived – two people were taken by ambulance to a hospital)

AMMONIA w/ FORKLIFT Pepsi plant in Mesquite evacuated, worker hurt after ammonia leak (an ammonia leak injured one person and triggered an evacuation at a bottling factory – the leak was caused when a worker drove a forklift into an ammonia line and ruptured it – the factory was evacuated – one person was taken to a Medical Center for respiratory problems and skin burns – no one else has been injured – fire department said it would take close to an hour to seal the leak – this same plant was also evacuated in December after an ammonia leak)

FORKLIFT and CARBON MONOXIDE Forklift in poorly ventilated space lands Hudson worker on flight to Boston Hospital (high levels of carbon monoxide caused by a gas-powered forklift in a poorly ventilated space meant a flight in a medical helicopter to a Hospital for one man – the unidentified victim was flown to the hospital for treatment in a hyperbaric chamber – firefighters checked the building with air monitoring equipment and found very high levels of carbon monoxide and ventilation fans were used to force fresh air into the building to clear the gas – the source of the hazardous fumes was a gas-powered forklift being used in the building without proper ventilation)

GUARDING Man loses arm in factory accident (worker is battling for his life after he slipped into the roller squeezers that crushed his left arm – “There is a pipe which takes water where I was holding my right hand but as the rollers were turning, they kept dragging my left hand through the rollers until my hand was crushed to [the] elbow. I managed to call out for Peter who was at a distance but he managed to rush to prevent me from going through the rollers as you can see my left shoulder was almost in but through the grip of my workmate, the hand was cut off and my working suit, my T-shirt and my vest went into the rollers.”)

LOTO/GUARDING Man’s arm caught in meat grinder at JTM plant (officials are investigating an accident that ended with a man getting his arm caught in a meat grinder at a food plant – AirCare was called to the scene and the victim was transported to a Medical Center – severity of his injuries are not known at this time)

FLASH FIRE 3 injured in Elk Grove factory explosion (three people were hospitalized after an explosion at a packaging factory – when crews arrived, they confirmed there had been an explosion in a piece of machinery that does lamination inside the factory – three employees were transported to a Medical Center with minor injuries – resulting fire from the explosion was soon under control, but the building’s sprinkler system caused extensive water damage – cause of the explosion is still under investigation – officials believe it was accidental and that a collection of solvent vapors within the machinery found an ignition source and caused the combustion)

AMMONIA Meat factory near Prestwick Airport evacuated after suspected ammonia leak (a meat factory has been evacuated after a suspected ammonia leak at around 4.40am – crews entered the premises to isolate the leak after the night shift staff were accounted for – no injuries have been reported – firefighters in specialist suits closed off a valve before returning to the building to identify a second leak – after ventilation of the factory employees were able to return shortly before noon)

AMMONIA Minor ammonia leak in Benicia on Monday (a liquid ammonia leak at a facility created a Hazmat situation for a Fire Department but produced no injuries – at about 9 a.m., the Fire Department responded for reports of an ammonia leak – liquid ammonia was being released into the atmosphere subsequent to a pump seal failure within the plant – trained facility representatives isolated the leak, while on scene fire department resources provided oversight and backup rescue capabilities – leak was stopped and there were no injuries to staff or fire department personnel)

AMMONIA Northboro ammonia leak at Saint-Gobain (an ammonia leak led to the evacuation of a building at a Research and Development building – the release was contained, and employees were back in the building in about three hours – there were no injuries – ammonia is used in the process of manufacturing ceramic materials – leak was noticed by an employee about 9:15 a.m. – employees returned to the building about 12:30 p.m.)

CHLORINE Chlorine leak (a small chlorine leak in a 1-ton cylinder that had just been delivered to the plant – workers detected the leak after noticing discoloration on one of the cylinder’s safety plugs – fire department personnel covered the plug with a special kit and the cylinder was returned to the supplier)

FORKLIFT Man taken to Addenbrooke’s in Cambridge after becoming trapped by forklift truck in Ely (worker was injured when he became trapped by a forklift truck – worker was caught in the forks of the vehicle in the industrial accident – patient suffered bruising to his abdomen, but his injuries are not believed to be life-threatening or serious)

CONSTRUCTION

WORKZONE FATALITY Zoo Interchange construction worker killed (a construction worker, 33, was killed in an accident at a construction site – incident happened before 8 a.m. on the westbound side of Interstate 94)

AERIAL LIFT FATALITY Worker killed by lift at Sarasota mall (worker, 24, was crushed to death by a scissor lift at a shopping mall – bystanders performed CPR, but the man died on the way to the hospital – not clear what went wrong)

Construction worker dies in accident at Barclays Center in Brooklyn (a construction worker was crushed to death when several steel beams fell on him – it happened while the man was installing a “green roof” on the arena just after 1:30 p.m. – he was taken to a hospital where he was pronounced dead)

Update: Construction worker’s leg partially amputated in Millville accident (the sub-contractor, 39, was seriously injured at approximately 7:30 a.m. while working on the new high school – he was working with a steel beam when the beam tilted over, pinning the victim’s leg below the knee against a concrete wall – the man’s leg was partially amputated)

FORKLIFT FATALITY Construction worker killed by falling forklift attachment at Rochester hotel site (construction worker, in his 30s, died after a large metal attachment from a forklift fell on his head – he was working at the site of a hotel project when a heavy attachment fell from the arm of the forklift)

One worker killed, another hurt in MRT Semantan mishap (one worker was killed and another injured following an accident at a work site at approximately 1.20pm – workers were on the ground assembling a rebar (reinforcing bars) cage – during the assembly the tie gave way – the rebars, a circular cluster of long steel rods each weighing 40kg, fell and hit the workers)

TRENCH FATALITY Young man killed in workplace accident south-east of Melbourne (worker, 19, has been killed while working in a trench at 2.10pm – he was laying sewage pipes at the site of a new housing estate when a trench collapsed on top of him – his colleagues realized he was missing and searched the site for him, eventually finding him under a metre of dirt – emergency services and workers at the site worked to free the man but were unsuccessful)

SKID STEER Man in serious condition following Bobcat accident in Prahran (a construction worker is in a serious condition after his leg got trapped in a Bobcat in a workplace accident – Fire Brigade rescued the man, aged in his 30s, after his leg was pinned between the bucket and the body of the Bobcat he was operating at a construction site)

FATAL EXPLOSION Man killed in explosion while working on gas line (a man in his 40s was killed in an explosion during the construction of a natural gas pipeline – the explosion was not caused by natural gas, but could not confirm what caused the incident – he had been laying pipes when he died at around 4pm)

FIRE Fire damages Lunda Construction, injures employee (worker was injured and a shop was destroyed when a fire broke out – worker sustained burns to his leg and hands and eventually was transferred to a burn unit for the non-life threatening injuries – a fuel transfer operation caused the fire, which destroyed the shop and all its equipment and other contents)

FATAL FALL from CRANE Man who died after falling from Bacton Gas Terminal crane named as David Eastick (investigations are under way after a man died following a fall from a crane on a major gas site at around 9.15am – accident victim suffered serious head injuries after falling from a crane at the gas terminal – the incident took place before crane lifting operations began – it is believed the man fell 10ft)

MISCELLANEOUS

FATAL FALL from JETWAY American Airlines worker dies after fall at DFW Airport (worker suffered fatal injuries during a fall from a jetway, a movable bridge that provides a covered walkway for passengers between an aircraft and the terminal at about 10:30 a.m. – he fell between the walkway and an aircraft near Gate 30)

Ski Worker Killed In Snow-Clearing Machine (the body of a woman in her early 20s was discovered trapped in the vehicle, which is designed to suck in and spit out snow – the ski worker’s body was discovered in the vehicle, which is designed to vacuum up snow before expelling it out as water at the side of the road – the tragedy occurred in the early hours of the morning as several snow ploughs were sent out to clear the roads – the driver, who is experienced at operating snow-clearing machinery, only made his discovery when the vehicle’s engine started making unusual noises – he immediately contacted the emergency services but the victim, who was in her early 20s, had already died)

FARM FATALITY Mabton dairy worker dies in waste pond accident (worker, 27, drowned in a wastewater pond when his front-end loader fell into the pond and he could not escape – it appears he was driving along a fence next to the pond when the wheels went off the shoulder)

Workers hurt in Timaru port accident (two men were injured, one seriously, while unloading a container stack)
