INDUSTRIAL GUARDING/LOTO FATALITY Worker killed by robotic machinery in Ionia (worker was reported trapped by robotic machinery at 2:20 p.m. – responders and workers freed the trapped employee from the machine, but the employee was pronounced dead at the scene) PARKING LOT ACCIDENT Worker Hit By Tractor-Trailer At FedEx Facility, Driver Keeps Going (a security guard was hit by a tractor trailer at a Freight facility – the driver of the truck did not stop driving after the accident – the victim was taken to Regional One in critical condition) LOTO – AMPUTATION Idaho Sawmill Worker’s Arm Severed in Accident (worker, 24, was seriously injured when the man’s right arm was severed by a piece of equipment – he had been cleaning a piece of machinery when his hand or arm got caught – he was taken to a Regional Medical Center) LOTO – AMPUTATION PACKING HOUSE WORKER LOSES BOTH ARMS IN INDUSTRIAL ACCIDENT IN ESCALON (a veteran employee, in his early 60s, of more than 20 years lost both arms in a bilateral amputation while reportedly working on peeling machinery inside the plant during a cleanup operation – he was reportedly stuck above the equipment some 25 feet in the air where ambulance medics and firefighters used a sack-like retrieval assembly to carry him down the metal stairway) ELECTRICAL Power restored after City Light accident injures 3 workers (worker, in his 50’s, was taken to a Medical Center with serious injuries after an accident installing a ground rod at an electrical substation – crew was using a small excavator to suspend a jackhammer used for pushing down the ground rod – jackhammer came into contact with an underground power line carrying 26,000 volts of electricity – worker was thrown 20 feet and he suffered a concussion – he didn’t suffer any burns – fire medics returned to the same site around 2:30 p.m. for a report of a second person, 63, who suffered second-degree thermal burns in the incident, and a third with cuts and contusions – the third man, described as being in his late 20s, was taken to a Medical Center) UPDATE on PRCS FATALITY Illinois court upholds ruling in fatal 2010 grain bin accident (Illinois appellate court has upheld a $16 million judgment awarded to the families of two people who died in a 2010 grain bin accident – jury awarded $8 million each to the families of a 19-year-old Alejandro and 14-year-old – the two co-workers were killed after they became trapped in a grain elevator – a third co-worker started sinking in the bin when a full-time employee opened up the intermediate sump pump and turned on the conveyor – the other co-worker survived after a six-hour rescue process. But the two teens died of asphyxiation as a result of being engulfed in corn) FATAL HOTWORK EXPLOSION Worker killed in ‘explosion’ while welding a pontoon (worker, 37, was killed in an explosion at a welding company – he was welding a pontoon boat when the blast occurred, killing him – it is not yet clear what caused the explosion) Shocking moment flying piece of wood fractures worker’s skull in freak accident caught on CCTV (a flying lump of wood strikes a worker on the back of the head fracturing his skull in a freak accident – after workers examined CCTV, they spotted that a nearby dumper truck had run over the log, its back tyre catapulting it through the air, directly into the path of the unsuspecting worker – the injured worker instantly collapses to the floor at the lumber yard with a fractured skull) MOBILE EQUIPMENT FATALITY PLYMOUTH COUNTY TECHNICAL RESCUE TEAM: Worker dies in avalanche of sand (worker, 65, was apparently trying to get away when the avalanche of sand came down on him – the front-end loader was running and in reverse when rescue personnel finally uncovered it from beneath a mountain of sand – an autopsy will determine the cause of death – he likely died of suffocation and the force of tons of sand pressing down on him) FATAL EXPLOSION A worker killed, 7 injured in Montenegro arms factory blast (worker was killed and seven were injured in an en explosion in the weapons factory – the female employee, who was working in the final assembly section of the plant, died in the accident – the seven wounded workers are not in a life threatening condition) AMMONIA FATALITY Contractor Dies, 2 Hospitalized After Factory Accident Just Over Canadian Border (a contractor died and two other people, a facility employee and a roofing contractors, were hospitalized after a deadly ammonia leak at a food factory – leak happened in the engine room of a facility around 12 p.m. – the two people hospitalized are expected to recover) PLEASE NOTE: I have two social media groups, Facebook and Linked in Groups, and you can follow me on TWITTER and GOOGLE Groups. MINE Worker injured at Boggabri coal mine (worker has been injured at the coal mine – a truck driver was trapped in his vehicle after an interaction with a dozer on the site – driver was freed from his vehicle soon after the incident – truck driver suffered no injuries and was taken to hospital as a precaution and was discharged from hospital that morning) BURNS Worker burnt at Phoenix Park Gas Processors (process technician suffered burns to his arms and legs when a leak developed in a hot oil flange – employee was conducting routine maintenance when the flange in hot oil service developed a leak at around 10.30 a.m.) FATAL HOTWORK EXPLOSION Chemical Plant Explosion In South Korea Kills 6 Workers, Investigation Underway (an explosion at a chemical plant has killed six workers – explosion occurred when workers were welding the interior of a waste water storage facility – fire authorities suspect that the gas trapped in storage area might have triggered the blast – “Gas from the waste water, which had piled up inside the tank, was apparently ignited by the welding torch”) FATAL HOTWORK EXPLOSION OSHA Investigating Nebraska Explosion (crews were cleaning out propane tanks when the explosion happened – two workers were critically burned and were airlifted to a hospital – in 2006 the company was fined $6,375 for two serious safety violations in Cheyenne, Wyoming when a worker was killed using a cutting torch) ELECTRICAL WORKER ELECTROCUTED IN LEESBURG (worker, 22, was electrocuted as a result of an accident involving a welding machine – workers were moving a larger electric welding machine when the machine tipped over and trapped the worker – he was caught under the machine for several seconds before others could get it off him, during that time, he came in contact with about 240 volts of electricity – he was taken to a hospital and his status was last reported as critical) OIL FIELD FATALITY OSHA investigates death of Kansas oil field worker (an oil field worker, 30, was found unresponsive at a well site – no other details) CHEMICAL Six people treated for exposure to ‘highly corrosive’ chemical at Melbourne Airport freight storage facility (six people have been treated by paramedics after being exposed to fumes from a sodium butoxide spill at a freight storage facility – it was released from a container in the storage facility – police had to track down one worker who went home with vomiting and the man was taken to a Hospital where he was treated and released) CONFINED SPACE or MEDICAL Contractor dead at Dow Oyster Creek site (contractor, 46, was found unconscious after entering a confined space – don’t really know if it was natural or if it was something else – plant emergency crews were called immediately after he was found unconscious and CPR was performed until they arrived – he was taken by ambulance to a Regional Health System, were he was pronounced dead) MARITIME CRANE 4 Port of Galveston dock workers hurt when crane breaks (four dock workers have been hurt when a ship’s crane broke while unloading a 60-ton section of a wind tower – workers suffered injuries not believed to be life-threatening – one worker was transported to a hospital – a boat had a malfunction with one of its cranes located on the ship) CONSTRUCTION FATAL FALL from LADDER JACK SCAFFOLDING Worker dies after fall from scaffolding in Ballantyne (a siding contractor fell 14 feet to his death while working on a house – he fell from scaffolding that had been set up near the house – victim had been standing on a ladder jack scaffold – paramedics arrived about five minutes later and pronounced the victim dead at the scene) FATAL FALL through FLOOR HOLE Construction worker killed on Camp Johnson building site identified (worker, 58, and three other employees were cutting a hole in the floor of a building to install an air duct – the deceased fell through the newly cut hole to the second floor below, a distance of approximately 25 feet – he was flown to a Regional Medical Center where he died) AERIAL LIFT FATALITY Worker killed at Innovation Quarter in downtown Winston-Salem identified (construction worker, 65, was operating cherry picker – his body was on the elevated train tracks – CPR was attempted to no avail) FATAL FALL from SCAFFOLDING Worker Dies After Fall From Scaffold at Geisinger (worker, 47, fell 100 feet to his death from a scaffold – contractor has been working to restore the bricks on the building – he was working on the exterior of the eighth floor when he fell – the three-person crew was getting ready for their lunch break and they were preparing to bring down the scaffolding when he fell – coroner would not say if the man was wearing a harness) TRENCH FATALITY Trapped Trench Worker Dies, Experts Say Death Was Preventable (worker, 31, died at a Hospital – he died from multiple blunt force trauma and traumatic asphyxiation – he was working in a trench on the site of the retirement facility when the trench caved in, trapping him – accident occurred after heavy rain fell in the area) EXPLOSION Worker Suffers Burns to Face, Arms in Construction Fire in Bethesda (worker suffered serious burns to his face and arms after a home under construction caught fire – fire started in the basement of the home – injuries are not life-threatening – cause of the fire is under investigation) CRUSHING FATALITY Idaho man fatally crushed in construction accident at Cassia County dairy (worker, 35, died in a construction accident at a dairy and feedlot – he was crushed to death while working with some siding just before 6:30 a.m. – he went into a large, metal shipping container to get some siding – the siding came down on top of him and crushed him against the side of the container – co-workers performed CPR on him until emergency workers arrived, but he was pronounced dead at the scene) MISCELLANEOUS Officials won’t comment after maintenance worker killed at Hays State Prison (maintenance worker, 50, at a State Prison was killed after an accident on the job – no word on cause or details) ELECTRICAL Worker shocked when pole touches power line in Bettendorf (worker, 18, was injured when a pole he was using to hang a flag touched a power line – the man, an employee of a local food vendor, was on top of a trailer attempting to hang a flag from a metal pole – the pole fell forward and touched a power line, shocking the man – he fell about 15 feet to the ground – he was given CPR by a Police Officer until MEDIC EMS and Fire Department paramedics arrived on the scene – he was taken to a Medical Center for treatment) CHEMICAL REACTION Nursing Home Worker Inhales Chlorine-Acid Mixture, Taken To Hospital (maintenance worker at a nursing home accidentally mixed chlorine and acid and inhaled the fumes and was taken to a hospital – employee was outside the building working with pool chemicals when he accidentally mixed chlorine and acid, forming a cloud – he was listed in stable condition – fire investigator stated the accident happened because the facility hired a new vendor for its pool chemicals, and the new company uses a different color coding system than the previous one) FLASH FIRE Cheektowaga burn victim listed in fair condition (worker burned as he was trying to fix an oven was in fair condition – he suffered burns to his hands, chest and face – there was likely a buildup of gas as the employee was working and then a flash which caused the man to suffer the burns – there was no fire and no damage in the incident) FIRE Fiery explosion traps four maintenance men in unoccupied D.C. school building (four workers were in the school’s underground utility room when a fiery transformer explosion sent them running for protection – thick grey smoke blocked the main exit, leaving an exterior stairwell as the only feasible escape route – upon realizing the open air stairwell was padlocked shut, the men began to yell and shout – firefighters freed three of the men by cutting open the locked stairwell – crews had to enter the basement with thermal cameras in order to rescue the fourth worker) CONFINED SPACE FATALITIES – SEWER GAS , ONE ENTRANT and ONE “would-be” RESCUER Manure Pit Claims Two Lives (father and son died at the farm where they were working – rescue workers pulled both of the men from a manure tank, and they tried to revive them with no luck – they were pronounced dead at the scene – father climbed down a four-foot ladder into the manure pit to try to retrieve a wheel that had broken off a manure conveyor – he was quickly overcome by the manure gas – the son then climbed down to try to get his father out of the pit and he succumbed to the poison gasses as well – according to the County sheriff’s office, the two died within minutes of going down into the pit) |
