BOILER EXPLOSION Explosion At Philadelphia’s Veolia Energy Station (crews responded to the scene of a boiler explosion shortly after 3 p.m. – employees were evacuated from the building after the explosion and were accounted – there were two injuries – one employee suffered minor injuries and refused treatment – one civilian was injured by broken glass, was treated at the scene, and has been taken to a nearby hospital)

ELECTRICAL Six injured after electrical accident at Christchurch fertilizer manufacturing plant (contractor in his 60s is in a serious condition in hospital after he received an electric shock – five others suffered minor injuries – contractor had been working on a circuit breaker at the time – he was in a critical but stable condition – five other people were taken to hospital for a “precautionary smoke inhalation check” but had since been discharged)

CRANE FATALITY Worker dies in crane accident at Mosinee plant (worker, 51, died in an industrial accident involving an overhead crane just before noon – he died at the scene – plant does aluminum extrusion and fabrication as well as coatings)

EXPLOSION 1 worker injured in small explosion at Sterling explosives plant (one person was injured in a small explosion at a business which manufactures explosives and detonators – the injured man was an employee and suffered non-life threatening injuries – the explosion was small, similar to a gunshot or a firework going off – the apparent cause of the explosion was a machine malfunction that caused a detonator to go off)

LINEMAN ELECTROCUTION Utility Worker Electrocuted In Tulsa (utility worker was electrocuted at about 10:20 a.m. – he was a contractor for Public Service of Oklahoma and was finishing up a job – witness heard a loud pop, and the next thing he knew, a man was dangling from a line – police said the man was wearing full protective equipment and it’s unclear what went wrong)

LOTO/GUARDING FATALITY Concrete worker killed in Jessup construction accident Id’d (worker, 32, died after becoming trapped in a piece of heavy machinery around 5:20 a.m. – crews found the man unresponsive and trapped in the machinery and he was declared deceased at the scene – investigators have not released any further details about the type of machinery involved, or how the accident happened)

LOTO/GUARDING FATALITY Worker killed in accident at Lehigh Valley steel company (worker, 53, was fatally injured while operating a press when he was struck by an object – he was taken to a Hospital where he was pronounced dead at 8:45 a.m. – the cause was sharp-force injuries to the abdomen, and the death was ruled an accident)

FATALITY Accident claims life of worker at central NY waste hauler (accident occurred around 7:45 a.m. – emergency crews arrived at the business they found a worker who had suffered serious head injuries – he was taken to a Hospital where he died a short time later)

LOTO/GUARDING FATALITY  Worker crushed to death by hydraulic press in west Columbus (worker, 60, is dead after he was working in a press area with a co-worker when he was crushed to death by a hydraulic press just after 5 a.m. – he suffered “catastrophic” injuries to his head and was pronounced dead at the scene – he had worked at the business for about one year)

FORKLIFT FATALITY Man killed by forklift in Rocky Point workplace (a pool company worker, 21, died on the job when he became trapped under a forklift about 4:30 p.m. when the machine he was operating fell on its side, pinning him under the roof – he was taken to a Hospital where he was pronounced dead)

CONVEYOR BELT GUARDING/LOTO Worker at Ocean Spray factory in Middleboro suffers crushed hand in conveyor belt accident (worker’s hand was crushed when he accidentally got it caught in a conveyor belt at the company’s warehouse at about 9 p.m. – when firefighters arrived, they found a man in his 40s, whose left hand was caught in a conveyor belt up to his wrist – he was conscious and was medically sedated while firefighters used hand tools to free the man from the conveyor belt – injuries were non-life threatening but his hand was crushed and it was unclear if it would have to be amputated)

LOTO/GUARDING FATALITY  Worker killed in machinery accident in Sparrows Point identified (worker, 20, died in an accident working with a machine that rolls steel, the coil polish machine, when he became entangled in a stainless steel coil, pulled into the machine and crushed – he had been employed for about a month)

CHEMICAL REACTION Worker hospitalized in chemical accident in Palo Alto (worker was hospitalized and more than a dozen co-workers were evaluated for chemical exposure after an “unexpected” chemical reaction – two employees were mixing epoxy at about 9 a.m. – they used the wrong quantities of chemicals and the solution began emitting gas and melted the plastic bucket in which it was being held – the  pair ran outside with the bucket so the fumes could be released outdoors – fourteen employees complained of shortness of breath and abdominal pain – one man was transported to a hospital and the rest were treated outside the building)

FORKLIFT Worker injured at Stamford construction site (construction worker, 31, was injured when a fork lift rolled backwards and struck a liquid concrete pump, knocking it off a makeshift outrigger – when the pump fell to the ground, it struck the worker in the hip, knocking him down – the man was conscious and alert after the accident, and was taken to the hospital where he was treated and released)

FORKLIFT FATALITY Pallets of mushrooms fatally crushed worker, officials say (worker, 45, was crushed to death by pallets of mushrooms – he was moving pallets of mushrooms with a forklift about 10:25 p.m. when the pallets began swaying – he then jumped off the forklift, but the pallets fell on him before he could get away – he was taken to an area hospital, where he was pronounced dead)

MINE FATALITY Worker killed in underground gas ‘blow-out’ at Boulby potash mine (worker died following a gas blow-out at a  potash mine – just months after seven workers were taken to hospital because of an underground blaze at the site – incident is believed to have involved a gas blow-out—a sudden and powerful release of gas)

FATALITY Worker dies at Chambers County automotive plant (worker, 20, at an automotive parts facility died as a result of injuries sustained while working – state agencies have not released the nature of her death and circumstances surrounding the accident are unclear – she was transported to UAB Hospital before being pronounced dead)

ELECTROCUTION Worker fatally electrocuted by power line in Anson County (employee working near a communications tower was fatally electrocuted when a boom truck hit a power line at about 1:10 p.m. – boom truck was used to remove wooden mats contacted a power line – the victim had been holding the hook used to remove the mats)

BURNS Worker scalded at Regina’s Co-op refinery (worker has been injured in an industrial accident just before 9 a.m. – worker was scalded by some hot liquid that escaped from a gauge as he was checking it and suffered burns to his leg – employee was taken to hospital)

ELECTRICAL PSE&G Worker Suffers 3rd Degree Burns In Accident At Newark Liberty Airport

(electrician, 38, was working on a transformer at Terminal A at an Airport when he was struck by electricity causing 3rd degree burns to his face, legs and chest – he was alert and conscious and was being airlifted to a Medical Center)

CONFINED SPACE Three workers injured in accident at Kia plant in Mexico (three workers were injured at a new car factory in northern Mexico after one fell into a sewer ditch and two others attempted a rescue but were also hurt – no explosion had taken place and the workers were being treated at a local hospital)

MINE Worker’s fingers amputated in truck maintenance accident (tyre fitter’s fingers have been partially amputated after an accident while operating a hydraulic bead breaker – it happened whilst a tyre fitter was positioning a hydraulic bead breaker in preparation for breaking an inner tyre bead on a water cart at a mine – he was holding the bead breaker in his right hand and the remote control pendant for the power pack in his left – worker then tried to gain extra length on the pendant control by pulling it over the power pack)

ELECTRICAL Two National Grid workers burned in Andover (two workers were burned, one of them seriously, while working on electrical boxes at a plant – one worker suffered what appeared to be third degree burns to his back, chest and arms, and was taken by helicopter to a hospital – second worker was taken by ambulance to a Hospital – incident occurred at about 5:45 p.m., while they worked on electrical boxes on the front lawn of the plant)

FATAL DRILL RIG FLASH FIRE Oil worker killed in fire, 2 others seriously burned near Watford City (crews were working on this workover rig when the well ignited killing one worker, 52, and three, 27, 28, and 40, were injured – owner of the well described the incident as a “flash fire” – two of the injured workers suffered third-degree burns on more than 70 percent of their bodies and one suffered burns to his face)

ELECTRICAL Two Gastonia linemen burned on the job (two city linemen received electrical burns at 8:47 a.m. – electrical service worker received second- and third-degree burns and was taken to a Medical Center Burn Center for treatment – the other worker was taken to a Regional Medical Center)

FATAL EXPLOSION @ BATTERY WAREHOUSE China: 2 dead, 18 injured after blast at battery plant (plant worker and a firefighter have been killed and 18 others injured after an explosion rocked a battery storehouse around 6 p.m. at a lithium battery storehouse – a total of 20 people, including 12 workers and eight firefighters, were transported to nearby hospitals, where two later succumbed to their injuries)

FATAL EXPLOSION One dead, one injured in work-related blast near Drummondville (worker, 29, has died and another was injured when they were doing work on a fuel tanker around 9 a.m. when the blast occurred)

PROPANE EXPLOSION Explosion at Stittco Energy propane facility in Flin Flon injures one, levels building (worker, 33, was injured in an explosion in a mechanical shop – the blast completely destroyed the building – worker was doing repairs to a propane truck when it happened – don’t know the extent of his injuries, but he is alert and conscious)

OVER PRESSURE EVENT Gas explosion at Middleton business, 1 injured, police say (a gas explosion involving laboratory equipment caused minor injuries to one employee at about 1 p.m. – facility briefly evacuated – the incident involved release of pressurized helium – the injured worker was treated at the scene)

MARITIME CONFINED SPACE FATALITIES A sailor and a civilian worker were killed in an accident on board the country’s largest warship, the 45,400-tonne INS Vikramaditya, which is currently undergoing a refit at the Karwar naval base – accident took place at about 5 pm when a team of workers were replacing a pipe in the sewage treatment plant located among the lowermost compartments of the 20-storey tall aircraft carrier – workers were trying to repair the leakage of sewage water and Hydrogen sulphide fumes – due to inhalation of toxic fumes, four personnel fell unconscious – two of them succumbed at the naval hospital in Karwar, while the other two are in a stable condition)

UPDATE on FATAL EXPLOSION Trial Opens in 2012 Offshore Fire, Explosion That Killed 3 (an offshore oil platform explosion and fire that killed three workers in November 2012 – platform was shut down for maintenance and was not producing oil at the time of the explosion – in a 2013 report on the blast, the Bureau of Safety and Environmental Enforcement said storage tanks weren’t properly purged of flammable liquid before a crew began welding nearby – in addition to the lawsuit, federal criminal charges were filed in the case. A January trial is currently scheduled in the criminal case, which includes involuntary manslaughter charges – both companies have pleaded not guilty)

HOTWORK Fire at I-Pull destroys building (total damages to the building and everything inside are estimated at $250,000 – Sheriff’s office has confirmed that the fire was started by sparks from a cutting torch – the torch was being used in the prep area when sparks ignited the area)

CRANE COLLAPSE A crane collapsed at the Exxon Mobil Refinery in Torrance (crane collapsed at around 9:30 a.m. – three contractors were taken to a local clinics to be evaluated for minor injuries – there is flaring at the refinery to safely stabilize operations)

BOILER EXPLOSION Boiler explosion injures two at Krabi palm oil factory (two workers were injured, one of them seriously, when a boiler exploded at a palm oil refinery around midday – worker, 21, was released from the hospital – worker, 29, was transferred to another Hospital, where she is undergoing further treatment – palm oil is traditionally refined by boiling palm pulp in order to remove its impurities)

FALL from SCAFFOLDING Accident reported at Syracuse sewage treatment plant (reports initially indicated a person was injured after falling about 25 feet off of scaffolding)

FATAL BOILER EXPLOSION At least 3 killed, dozens injured in chemical plant blast in India (at least three people have been killed and about 150 injured following a huge blast at a chemical plant – explosion was so strong it reportedly shattered glass in a 3km radius – the explosion was reportedly in a boiler inside the plant)

EXPLOSION No injuries reported at Wisconsin recycling plant explosion (there are no reports of injuries following an explosion in a scrap steel pile – the scrap steel pile stands about three stories tall and contains between 20 to 30 semitrailer loads of material)

TRANSFORMER EXPLOSION Richmond power plant explosion causes train delays (firefighters were forced to wait on standby as  electrical experts worked to isolate power to the transformer – about 1000 litres of oil were contained within the transformer)

CHEMICAL REACTION Chemical spill at Gehl Foods in Germantown sends five to hospital (five employees were taken to a hospital with respiratory distress following what fire officials described as a chemical explosion/spill – incident was reported shortly after 10:30 a.m. at the facility  – sulfuric acid was accidentally mixed with sodium chlorite 25% by a contractor, resulting in the creation of chlorine and sulfur dioxide – the gasses built up in a container causing a pressure breach that released product and gas inside the facility – employees were evacuated, however five who were in the area of the breach complained of respiratory issues and were taken to a hospital as a precaution)

AMMONIA Crews respond to ammonia leak at midstate chicken plant (crews say employees got out safely after ammonia leaked from the facility’s refrigeration system – two security workers smelled the gas about 1:00 a.m. and reported it – fewer workers were in the building than normal because the plant wasn’t operating on Memorial Day – not clear what caused the leak)

AMMONIA Parker Hannifin evacuated due to ammonia leak (plant was evacuated due to an ammonia leak – No injuries were reported, and EMS services left the scene without taking anyone to the hospital)

DUMP TRUCK FATALITY Construction Worker Killed In Worksite Accident (construction worker has died during morning rush hour at a commercial construction site – it appears that a dump truck started to roll and pinned the male worker underneath – truck was delivering construction materials to that area at the time that this occurred – First-responders pronounced the man dead at the scene)

TRENCH FATALITY Construction worker killed in trench collapse in Blue Earth Co.(construction worker, 28, was killed when the sides of the trench where he was working caved in and buried him around 2:45 p.m. – emergency responders were unable to recover the body of the worker until 8:15 p.m. because the area near the accident was determined to be unstable, hampering rescue efforts)

GAS TANK EXPLOSION OSHA investigating gas tank explosion that injured worker (OSHA is investigating after an old gas tank exploded and seriously burned a construction worker – firefighters say the explosion happened around 2:30 p.m. at an old gas station as the worker was cutting up the tank with a saw when it exploded – tank still had some gas inside – worker was taken to the hospital – not clear what his current condition is)

HOT TAR Construction worker burned by hot asphalt in Four Corners (paving company worker doing resurfacing was airlifted to a hospital after falling into hot asphalt sometime after noon – crews did a patient assessment and decided to airlift the worker because of hand and facial burns)

FATAL SCAFFOLDING COLLAPSE Collapse of scaffold at construction site in Kaohsiung kills 2 (two workers died and five were injured in a scaffolding collapse – police are still investigating the cause of the incident – preliminary investigations indicate the scaffolding collapsed when its supports buckled, bringing down with the structure seven workers then working on the structure, 50 meters above the ground)

Three workers injured at The Shopping Channel’s office (three workers doing a studio renovation were taken to hospital, one in critical condition, after a set wall fell on them around 1:20 p.m. – two others went to local hospitals in serious but stable condition – the injured workers were external contractors)

CRANE FATALITY Worker killed in freak construction site accident in Makati City (construction worker was crushed to death after a steel beam in a building under construction collapsed on him – initial investigation showed that the incident happened while workers were transferring a glass panel of a building from the first floor to the second floor, using a motorized lifter – the worker was waiting at the second floor balcony when the iron beam of the lifter holding the panels broke off and fell on him)

TRENCHING FATALITY West Virginia worker killed in ditch cave-in identified (construction worker, 38, was killed after a ditch caved in on him while he was installing a water treatment system at a former site – the work was part of a bond reclamation project for the DEP’s Office of Special Reclamation)

ELECTROCUTION Contractor electrocuted in Wilton identified (utility contractor, 44, was electrocuted the previous day while working near power lines – he was a private contractor replacing utility poles  – preliminary investigation showed that he was in the process of placing a pole when he fell to the ground from a standing position)

FATALITY Worker killed at Casper construction site (worker, 29, has died in a construction accident while driving a four-wheel vehicle when he was struck by a metal gate about 3:30 p.m. – he was among a crew working on at large project at a High School)

FALL through ROOF Construction worker in stable condition after falling through roof (construction worker is in stable condition tonight after falling through the roof of a bakery – he has been with the roofing company for more than 31 years)

FALL through Skylight Worker injured falling through skylight (firefighters and paramedics transported a man from the scene around 1:45 p.m. after he fell from a roof through a skylight – the victim was conscious when he arrived at the hospital)

CRANE Two construction workers hurt in accident near Phoenix (two people were injured at a roadway construction site when a 150-pound supportive beam fell onto equipment – one of the injured workers fell from 40 feet and was flown to a trauma center – the other victim was inside a piece of heavy equipment when the beam fell and that worker’s injuries are only minor – the beam was part of a system of girders being used to build an elevated roadway)

FALL from SCAFFOLDING Construction Worker Critically Injured In Hartford Scaffold Collapse (construction worker, 47, was in critical condition after falling than 60 feet from a scaffold sometime before 10:45 a.m. – he was on a “rolling scaffold” when it fell off the building – the scaffold either fell off the roof or off an opening inside the building)

WORKZONE Interstate worker injured in Augusta County (one worker is fighting for his life and another was injured after a woman ran her minivan off Interstate 81 and hit them – driver, 53, was charged with reckless driving after State Police say the 2006 Kia minivan she was driving struck the two contractors – the vehicle ran off the left side of I-81 at mile marker 213, struck a guardrail, then crossed southbound lands and ran off the right side of the road striking a cement bridge guardrail – two construction workers, contracted by Department of Transportation to paint the I-81 bridge that spans U.S. 11 where the accident occurred, were on the bridge at the time of the accident – Both men jumped onto the guardrail where one man was grazed by the Kia’s bumper and another man fell 15-feet over the guardrail to a grassy embankment below – the worker, 49, who fell, was flown by helicopter to a Medical Center with life-threatening injuries)

CRANE Worker injured in central Sioux Falls construction accident (worker, 33, was hospitalized following a construction accident where he was struck with a steel beam that broke loose from a crane while he and another worker were guiding the beam into place – co-workers helped the victim get back to the ground, and he was taken to a hospital – his injuries were not life-threatening)

CRANE FATALITY Worker crushed, killed by 26 ton fan at Rensselaer power plant (worker, in his late 40’s early 50’s, has died after an accident involving a heavy fan – initial reports were that there was a male trapped under a large fan, which was located on the roof of one of the buildings – he worked for a rigging company, which is a sub-contracted company working at that facility – crews found the man still trapped in the fan – he was dead. His name is not being released at this time)

Victim injured in accident on 22nd floor of Seattle construction site (worker was reportedly working on the 22nd floor of a building under construction when the accident occurred at about 10:35 a.m. – the high angle rescue team will use crane and ropes to assist in bringing the victim down – the man sustained neck and head injuries when a pipe fell on him – injuries are said to be non-life-threatening)

FALL Construction Worker Injured In Fall On Chestnut Street (worker, 56, was seriously injured in a construction accident around 11 a.m. – workers were demolishing the interior of the brick structure, which is being converted into an apartment building – the worker on was the roof using a jackhammer when the concrete below him gave way and he fell about 25 feet to the ground floor)

WORKZONE D.O.T. worker walks away from near-death accident with semi-truck (a Department of Transportation worker walked away from a nearly deadly accident – a semi-truck driver veered off the highway, slamming into the back of a D.O.T mowing tractor)

Two injured in construction accident at Doak Campbell Stadium (two workers were injured in a construction accident when the workers were cutting a piece of material when it fell and struck two workers below – one worker suffered an arm injury – second worker fell and hit his head but was alert and conscious when crews arrived)

WORKZONE Landscape worker struck by car on Rt. 189 in Granby (landscape worker, 23, was struck by a car just before 9 a.m. – he was working with a commercial landscaping firm at a home nearby when he was hit and sustained serious, though non life-threatening, injuries and was taken to a Hospital)

TRENCHING FATALITY Worker killed in accident at area construction site (worker, 29, was killed at a natural gas pipeline construction site accident that included an excavator – worker was killed when he was pinned between some heavy equipment and a pipe – two people, the victim and the excavator operator, were the only two involved – the incident happened while pipe was being installed – he was pronounced dead at the scene)

FATAL FALL from CRANE U.S. Coast Guard: Ship worker dies after falling 50 feet (worker died on a ship in the Houston Ship Channel when the employee was trying to connect a pipe to a crane around 4 p.m. when he lost his balance – the worker dropped 50 feet, landing onto the ship – he was dead when emergency crews arrived)

CRANE FATALITY Coroner identifies worker killed in Walker County crane accident (worker, 62, died in a crane accident on a Highway 269 bridge construction site – he died shortly after 8 a.m. when a crane collapsed on top of him – crane operator jumped out when the crane started shifting, and the crane landed on top of him)

WORKZONE Worker on Loop 101 critically injured when hit by car (worker, 22, is in the hospital with life-threatening injuries after being hit by a car while working to reopen a highway closed for construction – a crew with a construction barricade company was removing the barricades to reopen the southbound lanes following a construction closure – shortly before 3 a.m., one of the workers was hit by a car near the HOV lane – he was taken to the hospital and is reported in critical condition)

ELECTRICAL Worker injured in River North electrical accident (construction worker was seriously burned in an electrical accident when he came in contact with an electrical panel – no other details)

FATAL FALL Construction worker killed in fall at oceanfront East Hampton site (construction worker, 55, died when he fell from the second floor of a construction site at 8:43 a.m. – he lost his balance while working on the second floor and fell 12 feet to the concrete floor below and was pronounced dead at a Hospital)

TRENCHING Worker trapped in ditch at S. Lebanon business (contractor working at a feed mill was injured when he was trapped in collapsed ditch at 9:40 a.m. – rescue crews worked for about 30 minutes before freeing the worker from the horseshoe-shaped trench that was 25-feet wide and 15-feet deep – buried him up to his waist at least – worker was conscious and alert during the rescue – his condition was not available but officials described his injuries as serious)

FALL Kingston: Worker Injured During Insulation Job, Flown to Boston Hospital (construction worker is lucky to be alive after an incident on a job site – they were doing insulation on a building under renovation construction – in the area he was working in, some of the supports let go and he fell approximately 20 feet – he never lost consciousness, had some internal injuries and was flown to a Hospital)

BRIDGE COLLAPSE German bridge collapse leaves one worker dead and six seriously injured (40-metre section of the bridge undergoing restoration tumbled together with its scaffolding onto about 20 workers below)

FATALITY Construction worker killed in Perth County (a construction truck with a water tank struck the driver, 54, after he got out of the vehicle – the driver-less truck then took out a hydro pole causing a powerline to land on the roadway – he was pronounced dead in a local hospital)

ELECTRICAL Worker seriously burned while stringing cable between utility poles (worker is seriously burned while stringing fiber optic cable along utility poles – he was working in a bucket truck when he suffered burns to his hands, chest and abdomen at 9:11 a.m. – co-worker helped the man to the ground and called for help – he was pulling a mule cable from one pole to another when the cable came in contact with one of the three 13,200-volts power lines atop the pole)

Send somebody out there, please!” – worker was operating a front-end loader when he had words with the site superintendent – the fight escalated from expletives to an equipment-engineered attack, saying the worker then dumped a load of dirt onto his boss, pinning the man to the ground – worker exited the loader, picked up a level, struck the victim with it while laughing, then used the loader again to dump more dirt on him – victim suffered severe trauma: a 4-inch gash to his head, and needed surgery – worker told deputies he struck the victim to defend himself and claimed the second dumping of dirt was an accident – he was arrested and booked into jail on an aggravated battery charge, but was able to make bond)

AMPUTATION Elevator repairman severs arm in Financial District accident (elevator repairman lost part of his right arm as he was working in the machinery room on the 36th floor when his arm got caught in the elevator’s cable wheel about 10:30 a.m. – his arm was severed below the elbow – the severed limb was put on ice inside an industrial size garbage pail, which was seen rushed into the Hospital’s emergency room)

BottleRock Production Worker Killed Is Identified (worker, 36, was killed in an accident while dismantling a tent at a festival site – he was killed when he was dismantling a large metal A-frame tent – the A-frame had fallen on him and he was pronounced dead about 40 minutes later)

FATAL FALL Adventureland worker who died just 6 days on the job (worker, 68, died after he fell while tending the Raging River ride – the retired postal carrier took a summer job and on his sixth day on the job he was tending the amusement park’s Raging River ride when he fell on the conveyor belt fracturing his skull and suffering a major brain injury – he was unresponsive and on life support until he died Saturday at a Medical Center)

Report: Eden city worker hospitalized after inhaling fumes (a city worker was hospitalized after inhaling possibly toxic fumes while collecting trash – worker was operating a crane about noon, when he picked up a container with a leaking liquid and inhaled the fumes – paramedics took him to a Hospital for treatment)
