Metrics are measures that are used to evaluate and track the performance of a facility’s process safety management program. For facilities that handle highly hazardous chemicals, metrics can be used to quantify how a process has performed historically, how it might perform in the future, and where improvements can be made to keep workers safe.
This document provides employers with examples of...

WARNING! Viewer Discretion is STRONGLY ADVISED. This video is a CCTV clip showing a worker dying in a hydraulic press, no blood or guts; but you see a man die before your eyes. He died from compression asphyxiation. We can see the press has serious safety issues (no two-hand controls or light curtain). I wonder how many times this worker has run this press? He...

This week’s photo is a perfect example of “breaking the plane” of a Confined Space, but it also shows a hazard that even OSHA does not cover. Hint: guillotine

OSHA has issued a Notice of Proposed Rulemaking to add two (2) quantitative fit-testing protocols to the agency’s Respiratory Protection Standard. The protocols would apply to employers in the general, shipyard and construction industries. This proposed rulemaking would allow employers greater flexibility in choosing fit-testing methods for employees. The proposed rule would NOT require...

OSHA has long stated that “entry permits” which contain exposure data MUST be maintained per 1910.1020. However, this requirement seems to not be well understood. It seems we find companies that prohibit entry into any hazardous atmosphere are keeping warehouses full of closed entry permits and those companies that are actually entering hazardous atmospheres are throwing...

Earlier this year, the Department of Homeland Security (DHS) temporarily suspended the requirement to submit Chemical Facility Anti-Terrorism Standards Top-Screens and Security Vulnerability Assessments (SVA) in order to allow for a phased rollout of the new Chemical Security Assessment Tool (CSAT) 2.0 surveys and enhanced risk tiering methodology. On October 1, 2016, the requirement to submit Top-Screens...

As part of an ongoing effort to revise provisions in its standards that may be confusing, outdated or unnecessary, the Occupational Safety and Health Administration is proposing 18 changes to the agency’s recordkeeping, general industry, maritime and construction standards. The proposed revisions would save employers an estimated $3.2 million per year. They are based on responses to a public...

Recently OSHA issued citations to a business that has its own LPG storage tank and fills its own forklift LPG cylinders. We have seen far too many of these types of set-ups that are FAR OUTSIDE any resemblance of OSHA/NFPA/Fire Code compliance. These two (2) citations drive home the point about PROTECTING our storage tanks from mobile equipment hazards (e.g. contact with the tank)....

Another great OSHA State Plan resource; this time from Hawaii. This is an excellent self-evaluation worksheet that covers pretty much all facets of a traditional occupational safety and health program/process. If a business uses this tool to its full advantage, this assessment can paint a pretty accurate picture of the state of their S&H program efforts. But like all self-assessments...
