FALL into VAT Van Nuys Brewery Worker Suffers Serious Burns In Accident (a brewery worker, in his 40s, suffered serious burns after falling into a vat of hot mash at the bottling plant – he was attempting to funnel sediments from the mash into a tanker truck when fell into the vat containing the 180-degree mixture – he was able to get out of the vat himself, and paramedics took him to a Medical Center – he suffered burns to 30 percent of his body, including first- and second-degree burns to his thighs)

UNKNOWN OSHA to investigate Hillshire accident (worker was seriously injured during the overnight sanitation shift while cleaning equipment -police said the man suffered severe injuries to both arms)

FATAL FALL from TRASH TRUCK Accident involving trash truck kills Greenwood worker (a trash collector, 62, died after losing his footing and falling from a trash truck at 3:25 p.m. – the driver of the truck was turning left when the worker, standing at the rear of the truck, lost his footing and fell, hitting the roadway – he was taken to a Hospital, where he died from trauma to the head)

MINE FATALITY Worker killed in industrial accident at Olympic Dam (worker, 47, has been killed in a rock fall in a mine)

MINE FATALITY Doe Run Employee Killed in Mine Accident (miner, 54, was killed – he was performing a standard safety practice of scaling the mine roof (checking for and removing loose rock from a working mine area) when some loose material fell and struck the cab of his equipment, fatally injuring him – no other employees were injured)

CYLINDER FILLING EXPLOSION Feds investigating explosion that injured 3 at AGL Welding Supply in Clifton (worker, 45, was severely burned and another worker was injured at a supply store when oxygen they were using to fill cylinders inexplicably exploded – worker was taken to a Medical center and it appeared he would survive – the other worker, who suffered from minor smoke inhalation, and a truck driver working nearby who also was shaken up by the force of the explosion were treated and released)

EXPLOSION Man burned in explosion at Box Elder Autoliv plant (worker, 50, was severely burned in an explosion – he and his co-worker were preparing spare airbag parts for incineration when the explosion rocked the facility at about 4 p.m. – “As they were removing one completed batch and preparing to put in a second batch an explosion occurred” – other worker was not injured – the explosion caused a fire that destroyed the facility)

CRUSHING FATALITY Worker chopped by factory machine (factory worker, 35, was crushed to death as he stuck inside a machine during labour – he was working at a factory located in industrial area when all of a sudden he fell on a machine)

EXPLOSION Worker Injured in Explosion at St. Charles Powder Coating Company (at least one person was injured in an explosion at a powder coating company around 10:30 a.m. – a small fire broke out in a mixing vat on the mezzanine level of the building – the mixing operation was part of a powder coating process – one person was taken from the building by helicopter to a Medical Center – fire took roughly 30 minutes to put out and the building’s structure wasn’t damaged)

FATAL FIREWORKS PLANT EXPLOSION 2 killed in fireworks plant explosion (an explosion has claimed two lives and injured several other people – two victims are in critical condition, and two victims have non-life-threatening injuries – an injured female was airlifted with severe burns on her body, an injured man suffered a severe injury to his left knee and face, another man suffered minor cuts and scrapes and was treated and released, and one man suffered minor cuts and scrapes and is still at the Hospital)

CHEMICAL PLANT EXPLOSION Explosion reported at chemical plant in eastern Ukraine (an explosion was reported at a chemical plant – a number of buildings in the area were damaged, including residences, but no casualties have been reported)

8 injured from Warsaw Chemical fire, home evacuation lifted (residents within 1,000 feet north and east of the plant were asked to evacuate – area remains under evacuation until further notice and until air quality tests have been performed – eight people were injured – out of those eight people, five were with the City Waste Water Treatment and two were firemen – their injuries included burning eyes and lungs – one person was also treated for burns on his hands – about 50 homes and businesses within 1,000 feet of the plant were evacuated – evacuation was lifted around 3:30 p.m.)

FATAL EXPLOSION @ CYLINDER FILLING PLANT At least 3 workers killed in gas plant explosion in southern Macedonia (an explosion in a gas plant has killed at least three workers and injured two others – the explosion occurred in a department where gas cylinders were being filled in the plant – one of the injured workers was transferred to a clinic to be treated for severe burns)

AMMONIA 24 PEOPLE TAKEN TO THE HOSPITAL AFTER MERCED COUNTY AMMONIA LEAK (fire officials found a one to two gallon ammonia leak at a strawberry production plant – about 475 employees had already evacuated – crews transported 24 people to hospitals for inhalation hazards, but they said no one actually displayed any symptoms – several others were treated and released on the scene)

AMMONIA Ammonia leak at New Bedford business (an ammonia leak forced the evacuation of a seafood distributor around 8:30 p.m. – workers evacuated the plant, while firefighters isolated the leak and ventilated the building – one worker was treated at the scene for ammonia exposure, then taken to a Hospital – the worker was expected to recover)

AMMONIA Ammonia leak reported at soft drink bottling plant (workers were evacuated from the plant – no nearby buildings were in danger – they don’t know how large the leak is or the cause – no one has been taken to the hospital at this time)

CHLORINE 50 pounds of chlorine leaks at water facility (one person is recovering because of a chlorine leak at a water treatment facility – a worker left a valve open on a chlorine gas tank – around 50 pounds of the gas leaked from the tank – the person who was injured is expected to be okay)

TRENCH Worker pulled from 30-foot-deep hole after construction accident (worker was pulled from a 30-foot hole after an accident at a construction site – the victim was working in the hole at the time of the accident, when he was injured by machine equipment – he suffered multiple injuries)

WORKZONE INDOT worker struck in highway accident (DOT worker, 29, was transported to the hospital with non life threatening injuries after the dump truck he was operating was struck from the rear by a semi around 1:30 p.m. – crew members were repairing holes in the right lane – driver drove into the rear of a DOT dump truck and arrow board that was stationary in the right lane protecting a road crew working just ahead)

TRENCH FATALITY City Worker Killed in Accident (worker, 31, was killed after a construction accident at approximately 11:40 a.m. – he was four to six feet below ground level working on some pipe repairs, when a cave-in occurred – co-workers immediately began digging him out and called 911 – he was transported by EMS to a hospital, where he was pronounced dead at 12:08 p.m.)

WORKZONE w/ BUCKET TRUCK Utility Worker Pinned Between Vehicles In Suspected DUI Accident (a drunk driver hit a utility worker, pinning him between her vehicle and a truck – the crew was operating a utility bucket truck at the intersection around 1:30 a.m. when the woman’s vehicle hit the worker – he was airlifted to a hospital – utility worker in the bucket was not hurt – Police say she will face a drunk driving charge)

WORKZONE SDDOT worker injured south of Dell Rapids (worker on a DOT crew was seriously injured – crew was doing work on a bridge – a boom from a drill rig was extended into the southbound lane when a semi-truck hit it, causing the boom to swing back and hit the victim)

CRANE Worker injured when crane topples at construction site (a crane toppled over at a construction site, injuring a worker who was briefly left dangling in mid-air – worker suffered head injuries and a broken leg in the accident – the crane hit a cell phone tower on its way down – worker was expelled from the crane and was left hanging about 20 to 25 feet above the ground)

SCAFFOLDING Workers injured as scaffolding collapses at Greens Farms Academy construction site (three construction workers were hospitalized for non-life-threatening injuries after scaffolding collapsed on them – workers were injured shortly after 10 a.m. when scaffolding, about 25 feet high and 100 feet across, gave way at the private school – four construction workers were checked at the scene by emergency medical personnel, and three of them were transported to a Hospital for treatment of non-life-threatening injuries)

ELECTROCUTION Worker electrocuted (construction worker, 22, was electrocuted and another sustained injuries – the two workers came in contact with a snapped electric wire while they were working at a house around 5:00 p.m. – the two were rushed to a Hospital, where the doctors declared one of them dead)

ELECTROCUTION Comcast worker killed in Glen Ridge accident recalled as accomplished fighter, dedicated father (worker, 31, was killed while working on a utility pole – he was found unresponsive while working – authorities have ruled that his death was an accidental electrocution)

HOTWORK Welding spark blamed in Utah mechanic shop fire that hurt 1 (a welding spark is believed to have caused a two-alarm blaze that injured a shop owner and gutted a mechanic shop – a man was treated at a Hospital after firefighters were called about 7:15 p.m. – the mechanic shop a total loss)

HOTWORK Firefighters extinguish oil fire on barge at South Roxana scrap yard (a cutting torch set fire to a partially-sunken barge being scrapped along the Mississippi River – leaking oil was captured by containment booms already in place – fire occurred about 11:43 a.m. after a spark apparently ignited pads put down to absorb some spilled oil – fire was brought under control within 35 minutes)
