INDUSTRIAL CRANE FATALITY Worker killed in industrial accident at Shelby Township facility (a worker, 42, was killed around 1 p.m. while working on an overhead crane when a five-ton die fell on him, trapping him underneath – police and fire units attended to the employee, but he was pronounced dead at the scene – there were no other injuries reported and no signs of foul play) FLOOR COLLAPSE Worker injured when floor collapses at Woodridge egg plant (a worker at an egg-processing plant was injured when a second-story floor collapsed under him as he moved crates loaded with chickens – he was flown to a Medical Center with leg injuries after the accident – one worker at the scene said that crates weighing hundreds of pounds landed on him after he fell to the first floor) Worker killed in grain mill incident (a man, 49, died in an industrial accident at a grain grinding mill – he is thought to have been a contractor who was visiting the plant and not one of its employees – the man was pronounced dead at the scene – no other details) LOTO/GUARDING AMPUTATION KC worker loses legs in industrial shredder (a worker at a tire recycling business lost both legs in an industrial accident shortly after 10 a.m. – the cause of the accident was still under investigation, but the victim was either clearing or fell into the shredder – the victim was trapped by the machine and had to be freed – both of his legs were amputated) UPDATE Charges considered in Atchison grain elevator blast (a year and a half after a grain elevator explosion killed six workers, federal prosecutors now are considering criminal charges against the owners) UPDATE – now a FATAL FALL into DUCT WORK UPDATE: Harrison Power Station Worker Died After Falling into Duct (the worker who died at the Power Plant fell into duct work when he was removing insulation to prepare for a maintenance outage – officials stated that safety measures were in place to prevent direct contact with the duct and it is not clear at this time how the employee came to fall) PAPER MILL FATALITY White Pigeon man killed in accident at paper plant (a veteran employee, 61-year-old welder, at a paper plant has been killed, but it’s not clear if the death was due to an accident or a medical issue – other workers became concerned because he was missing and a search was organized – he was found dead in the paper pulper just before noon – police are interviewing co-workers to determine what he may have been doing near the big vat – an autopsy is planned – he reportedly had a history of heart problems) ACETYLENE CYLINDER FIRE Employee burned during tank explosion in Bakersfield (a tank explosion left one worker suffering from minor injuries after a tank exploded at about 11:30 a.m. – workers were in the back of the acetylene plant when a fire erupted from one of the acetylene tanks – workers were evacuated and water sprinklers were turned on in the building – fire officials isolated the tank from others to prevent it from spreading – one employee did receive minor injuries, but declined medical aid) DRILL RIG FIRE 2 burned in South Texas drilling rig fire (one worker was flown to a medical center in critical condition – both were hospitalized following a drilling rig fire – the workers were performing maintenance on the rig when oil began to leak, and the fire began when they tried to stop it – one of the workers was critically injured with burns on 70 percent of his body) FATAL EXPLOSION @ Detonator Plant Worker killed, 3 injured in blast at explosive factory (one worker was killed and three other laborers were injured in a blast at an explosives factory – there were 24 laborers working on detonators inside the plant, when the explosion took place – the three injured workers have been admitted to a hospital – police are trying to ascertain the cause of blast) ELECTROCUTION in BUCKET TRUCK Man killed instantly in explosion in Charlestown (a worker, 41, was killed when the bucket portion of his truck apparently came into contact with high-tension wires as he was working, caught fire and an explosion occurred – when police and rescue personnel arrived on the scene ait was determined immediately that he was deceased – it appeared the man was killed instantly when the bucket touched the wires, caught fire and an explosion occurred) EXPLOSION Explosion injures South Texas oilfield worker (an oilfield worker was hospitalized following an explosion – he sustained third-degree burns on more than 70% of his body when a gas-operated heater he was trying to light exploded – it was not immediately clear where the explosion occurred) ELECTROCUTION Worker electrocuted at Ligioner factory (a worker, 56, at a plant Industrial died after being electrocuted – an autopsy will be performed, but “it is presumed that the cause of death will be by electrocution”) EXPLOSION Four workers injured in Pekin plant explosion (four people were injured in an explosion involving chemical refrigerants at a refrigerants bottling plant – an explosion blew off a corner of the one-story brick building’s roof and wall, sending bricks and debris onto railroad tracks – the explosion reportedly took place near an assembly line and may have stemmed from a gas line malfunction – all suffered burns “on various parts of their bodies” – initial investigation at the scene included the possibility of a malfunctioning gas line that, fed by an unknown ignition source, produced the explosion) PHENOL SPILL Two plant workers hurt following toxic spill near Philadelphia (two people are hurt after a piece of equipment malfunctioned at a plant, spilling about 50 gallons of hazardous material into the Delaware River – the spill happened around 11 p.m. – 50 gallons of phenol, which is also known as carbolic acid, spilled into the river – two workers were taken to the hospital after they were exposed – they’re in stable condition) ELECTROCUTION Electrical Accident Kills One Near Lansing Smith Steam Plant (a subcontractor, 30, working on overhead power lines was killed in an accident – he was working on the overhead lines at the plant to do some inspections and work on the power lines – he was in charge of “grounding” the lines so that crews could inspect them and provide service, but somehow touched a “live” portion of the line carrying 250,000 volts of electricity – he was killed instantly) AMPUTATION Man’s fingers severed in accident at Readington Farms food plant (a worker, 33, cleaning equipment lost two fingers in an accident at a distributor of dairy and other products – the man’s two severed fingers were packed in ice and taken with him to the trauma center in the hope that they could be re-attached – the man was cleaning an overhead track that moves milk cases through the plant when the accident happened – the track wasn’t operational at the time) STEAM PIPE RUPTURE Explosion Rocks Long Beach after Steam Pipe Rupture at Gas Plant (an explosion caused by a steam pipe rupture at the natural gas-fired Generating Station rocked a neighborhood – no injuries were reported – Unit 3 at the 2,000-MW plant experienced a rupture in a five-inch pipe that carries high-pressure steam and water to the boiler at 7:44 a.m. – when the rupture occurred, an immediate shutdown of the Unit was initiated and the site’s emergency response procedures were followed – plant officials verified that all personnel onsite were safe and accounted for – the release of steam reportedly caused a large visible plume and “excessive noise” for about 45 minutes until the unit was fully depressurized and taken out of service) AMMONIA One person sent to hospital after plant ammonia leak (an ammonia leak at a plant sent one person to the hospital – a relief valve at the ice plant vented ammonia into the atmosphere around 2 p.m. – the fumes circulated in the air for approximately 20 minutes – no evacuations were ordered because the direction of the winds kept the fumes under control – one person from another plant next door was taken to the hospital for inhalation problems – only two workers were inside the crystal ice making plant at the time of the leak, they are said to be unharmed) FACILITY SITING Accident causes small ammonia leak (a tractor-trailer struck an anhydrous ammonia storage tank causing a small ammonia leak – the incident occurred about 6:20 a.m. when the driver of a tractor-trailer attempted to turn around in the Heritage Cooperative facility’s parking lot – the trailer struck a large-capacity storage tank at low speed, moving the tank about six inches and creating a small hazardous materials leak when a supply line was breached – the truck also struck a small out-building, moving it off its foundation and knocking over a light pole) CONSTRUCTION FALL Construction Worker Injured after Accident at UVA Medical Center (a construction worker is recovering after an accident – the worker lost his footing while moving a steel beam and fell off the platform he was standing on, and the beam hit him in the head – proper safety gear and fall protection prevented the worker from getting more seriously hurt) FALL from SCAFFOLDING Workers injured in construction accident at St. F.X. (two construction workers were injured when scaffolding collapsed at the site of two new residences – the men sustained injuries after both fell about six metres from scaffolding – both were taken by ambulance to a Regional Hospital) CRANE into POWERLINE Crane worker gets electric shock in Trowbridge accident (a crane worker has had an electric shock after his crane hit an overhead power cable at 9:05am on a construction site – the worker was the only person injured) CRUSHED by ELEVATOR MOTOR Worker ‘severely’ injured at old Citadel Hotel site (a construction worker, 44, had both legs severely injured in an industrial accident at a construction site just after 7 p.m. – the crews were hooking up a construction elevator and the motor for the elevator fell and pinned one of the workers – both legs were severely injured – the motor weighed about 1,100 kilograms) FALL Worker Falls To Death At Grand Mosque (a worker fell to his death from the Grand Mosque – he reportedly lost his balance and fell to his death while carrying out renovation work on the Mosque) COLLAPSE 4 workers hurt in Sulphur Springs construction site collapse (four workers have been hurt when the wooden frame of a one-story structure they were building collapsed – the workers suffered apparent minor injuries but all were transported to a hospital to be examined – the framing of the building was nearly complete with the walls and trusses in place – all of the injured workers had managed to free themselves by the time firefighters arrived – authorities are trying to determine what caused the collapse) Worker rescued from beneath building site in NYC (a worker was rescued from an MTA subway construction project after being trapped up to his chest in debris for several hours – rescuers freed the construction worker trapped in mud and debris 75 feet below ground at the Second Avenue subway line construction site shortly after 12:30 a.m., about four hours after he became stuck up to his chest – he was awake and conscious and was being evaluated at a local hospital) Construction workers hurt in fall at Gowanus Whole Foods site (three male construction workers were hurt after falling at a construction site at 1:02 p.m. – condition of the workers was not reported) MISCELLANEOUS FATAL HOTWORK Hudson golf course worker dies after explosion (a golf course worker, 35, may have been lying on the floor in a pool of his own blood for several hours before he was found – he was hurt when the sealed 55-gallon drum he was working on exploded at the Golf Course – he was using an acetylene torch at the time – the explosion caused the lid on that drum to shoot up and hit him in the head, causing severe injuries – a UPS driver found the worker semi-conscious) |
