Worker killed on the job (there were only two people working and
their job was to dismantle a heavy-duty shrink-wrapping machine –
the machine had been used to wrap plastic around wooden pallets
before they could be loaded and shipped -when this piece of
equipment was first installed, it had to be installed by a crane –
during the decommissioning of the machine, it appears the two
forklift drivers were trying to lift the machine using just the
forklifts and some chains – the piece of equipment shifted, and it
caused one of the forklifts to spin around and topple, and the
worker was thrown out of the cab and crushed – the second worker was
not injured – the worker killed was 39, a father who was saving up
to take his wife and three children to Disneyland in April)

Worker Loses Two Fingers in Work Accident (a bagel factory
employee severed two fingers, while operating a faulty bagel
dividing machine – he severed two of his fingers while attempting to
clear a blockage from the machine – an investigation revealed that
the machine had a fault, which meant that a rotating blade inside
continued to spin even when the machinery’s rear doors were opened –
the company was aware of the fault on the bagel machine but took no
action to prevent the worker from operating it)
Museum worker dies after machinery accident (a miner working
underground at the National Coal Mining Museum has died after being
crushed by machinery he was operating. – the accident happened at
around 8am – he was taken to hospital but died later from his
injuries – the 58-year-old man was doing contract work at the
museum, where an underground roadway for visitors is being extended)

Utility worker electrocuted in Double Oak (a utility worker, 32,
replacing power poles died from an apparent electrocution – he died
at a hospital from injuries sustained in an apparent accident while
on the job – he was working in the backyard at a home just after
noon when the incident occurred – initial investigation indicated he
was operating a metal bucket lift from the ground when a coworker
inside the bucket accidentally energized the lift, resulting in his
electrocution – the bucket itself was made of fiberglass, and the
coworker was uninjured)

Tortilla Worker{apos}s Death Caught On Video (police have reviewed
surveillance video of the gruesome death of a worker who was killed
after falling into a mixing machine at a factory – the video shows
him frequently reaching into the deep machine to push the dough
down, but around 2:30 a.m. his arm got caught on one of the blades
and was pulled in – the mechanical arms broke his neck almost
instantly – co-workers say there was no reason for him to have stuck
his arm so far into the mixer – they suspect he was listening to
music at the time and dropped his headphones into the mixer, falling
after he reached in to retrieve them)

Fumes hospitalise man in Gladstone (exhaust fumes from a truck
caused a chemical scare – it was believed the worker had suffered
from a serious chemical spill after a worker had been sent to
hospital – however it was realized after a number of hours that the
toxic fumes came from a truck parked near by – one person is in a
stable condition and was transferred to a hospital for observation)

Ammonia gas leak checked after 15 hours (fire fighters struggle
to control a fire that broke out in a building that served as a cold
storage – lack of maintenance caused welding of receiver to crack
causing the leak – more than 100 people were reported to have taken
ill after inhaling the gas that leaked from a receiver at the
cooling plant due to faulty welding of its joints – some of the
personnel fell ill as they lacked proper breathing apparatus and
there was a delay in locating the source of the leakage)

Fire threatens BASF facility (a dock door shows signs of fire
damage after a trailer docked at the plant caught fire – materials
contained in a trailer docked at the plant reportedly caught fire
and spread to the building – fire entered the building, which set
off a sprinkler located directly over the bay, suppressing the fire
until firefighters arrived – blaze was under control within
approximately 30 minutes, as firefighters were able to contain the
structure’s damage to the area around the bay door – one employee
was checked out by EMS personnel with possible smoke inhalation but

and found flames shooting from the building – hazmat crew advised

several days – their main concern is the risk that smoke drifting

leak – evacuation took place without incident and there were no

393 people, including children, who were admitted to hospital after

when the bucket of a Terex loader pinned him against the heavy metal

Forklift truck topples over and smashes Cart Road beach huts (a
construction worker, 36, had a lucky escape when the fork lift truck
he was driving toppled over and crashed onto beach huts – he was
unloading materials just before lunch when the accident happened –
the vehicle, known as a telehandler, toppled over with its extended
boom dropping over the side of the parade and smashing three private
beach huts – he was able to scramble out of the damaged cab
uninjured but was visibly shaken by his ordeal)

Worker Killed in Accident on UI Campus (a contract worker, 52,
was killed while working as an employee of a private contractor on a

on top of the construction worker and had him pinned to the ground –
crews took nearly four hours to remove the man from under the beam –
the beam had fallen in a way that it was wedged to the point it
