CRANE FATALITY Crane topples at JaxPort, killing worker (a portable industrial crane brought in to dismantle a shipping crane collapsed around noon, killing one worker – the man killed was the crane operator – pieces of the retired crane he was removing for scrap toppled and crashed onto the cab of the crane he was operating – he died at the scene)
AMMONIA Ammonia leak causes evacuation, 3000 affected (at least 3,000 people either fled or were evacuated following leakage of ammonia gas from a cold storage – the gas started emanating from the cold storage late afternoon and it took fire brigade personnel nearly six hours to plug the sources of leakage – all three sources of leakage were plugged – the gas leaked from two valves and a cracked cylinder – it has spread over a square kilometre area – residents fled the area after they experienced breathing difficulties and burning sensation in the eyes)
CONFINED SPACE FATALITY – WOULD BE RESCUER DIES 1 Dead, 2 Critically Injured in Atwater Village Industrial Accident (a man died after being overcome by fumes while trying to help two co-workers who lost consciousness inside a tank they were cleaning at a pharmaceutical plant – when firefighters arrived at the facility about 4 a.m., one of the men had no heart rate and was not breathing, but paramedics were able to restore his pulse – all three were taken to hospitals, where one of the men died – the men had been cleaning the inside of a 4-foot-tall cylindrical tank with a 5-foot diameter – the tank has a 24-inch diameter opening at the top, through which workers enter to clean it – when firefighters arrived, two men were inside and one was partially inside – firefighters pulled all three men from the “confined space” and brought them outside – the man who died had called 911 and then went in to help his unconscious colleagues, but was himself overcome by the fumes – the workers were using detergent to clean the container of blood plasma – they were overcome by ethanol, which was used as a separating agent for blood plasma)
MINE PFAS gets tangled in machinery Konkola Copper Mines worker trapped (an employee has sustained serious injuries after he was trapped as he was working at the precipitator – the accident happened when his safety belts got caught in the electrical hammer, which resulted in him getting trapped – his right leg was cut off below the knee and that he also suffered multiple fractures to his right arm)
FORKLIFT FATALITY Worker crushed to death while operating forklift (a warehouse worker has been crushed to death while attempting to move a piece of equipment with a forklift – the man and another worker were using separate forklifts to lift the equipment when the incident occurred – one of the forklifts shifted and spun around, toppling over and throwing the middle-aged man out of his cab)
FORKLIFT Worker in amazing escape (a warehouse worker, 48, narrowly “cheated death” when a piece of equipment fell on him from a forklift truck – he was struck by a falling metal cage as he worked – he suffered “minor injuries” with severe abdominal trauma – it is understood the victim had been moving some metal sheeting out of the way that was suspended from a two pronged forklift truck – there was also a cage like piece of equipment on the trucks forks which is used to lift workers up – the cage toppled off the forks and glanced the worker, who is thought to act as a runner for the forklift truck driver)
FALL FATALITY – SAME LEVEL FALL! Alabama DOT Worker Killed After Slip and Fall on Icy Highway (a 22-year veteran highway maintenance worker slipped and fell on U.S. Highway 43 has died – the tragic construction accident took place when northern Alabama was in the grip of unseasonable cold, snow and ice – the 56-year-old man was a senior highway maintenance technician was working along U.S. Highway 43 during the snowstorm when he slipped on the ice and broke his neck – the man apparently exited a highway grading truck, stepped onto a snow bank and slipped)
FALL Natural Gas Accident (a natural gas company worker was airlifted to the hospital after falling during a shift at a gas well site – don’t know what caused the fall or how far the worker fell – the man has possible back and leg injuries)
FORKLIFT Worker hurt in forklift accident (a man, 54, was in fair condition after being trapped between two forklifts – he suffered a broken leg in the accident, which occurred in the parking lot – he was freed by co-workers and the Fire Department – it was unclear yesterday how he became pinned between the vehicles)
MINE FATALITY Worker dies at Harmony mine (a winch operator died at the mine after a ground fall 2,913m below the surface in a stope panel at about 10pm – his body was recovered at 2am – operations had been stopped until such time as all investigations into the accident had been completed)
BOILER 6 workers suffer burns (six contract workers sustained burn injuries after being caught in flames while working near a boiler of the plant – condition of two workers is said to be critical – the workers were pressed into duty of cleaning coal near the boiler – the coal usually fell from the conveyer belt during production)
FATAL EXPLOSION 7 workers killed in fireworks unit blast (seven workers were killed and 15 others injured following an accidental explosion in a fireworks unit – the condition of three of them is said to be critical – friction while making crackers for aerial display caused the accident at the unit at around 11 a.m – two working sheds were completely razed in the impact of the explosion – human flesh was strewn as far as 100 feet from the place of explosion – the blast triggered a fire in five of the adjacent working sheds, damaging them partially)

AMMONIA EXPLOSION Ammonia gas leak causes blast, two injured (two residents were injured after ammonia gas exploded in an ice-factory – the incident took place at an ice factory – leaking ammonium gas led to an explosion and the cylinder blasted into pieces – the floor of the factory and around six surrounding houses were damaged)
FATAL EXPLOSION One utility worker killed, 3 critically burned in Philly explosion (a gas main explosion caught on video sent a 50-foot fireball into the sky, killing a utility worker, injuring five other people and forcing dozens of residents from their homes – three Gas Works employees remained in critical condition in a hospital burn unit, a day after dispatchers received a report of a gas and water main break – also injured were another utility employee and a firefighter – the cause of the explosion, which destroyed nearby buildings and torched cars, is under investigation)

CHLORINE Chlorine spill hospitalizes Civic pool worker (a staff member is in hospital after a chlorine spill – she suffered minor injuries and burns to her face at around 9am while cleaning up a chlorine spill in a rear building at the pool – she was taken to hospital and is expected to be released – the spill was isolated to the work room)
EXPLOSION Explosion at CNPC refinery in Liaoning injures 30 workers (an explosion at a petrochemical plant has injured more than 30 workers – the explosion occurred after a catalytic cracker caught fire – the fire has been contained and no deaths were reported – the petrochemical complex is undergoing expansion work to boost its annual refining capacity to 11.50 million tonnes by August)

Barge worker rescued from harbor after accident (a man, 48, was injured after being pinned between a barge and a Wharf, and then falling into the harbor – his coworkers were able to pull him out of the water – he was placed on a stretcher and taken to a Hospital and was later flown in a Coast Guard HU-25 Falcon jet to a Medical Center – he was listed in serious condition – the accident happened as he and other workers on the barge were attempting to untie the vessel after unloading cargo at the wharf – with heavy winds and high seas in the harbor, the barge shifted as it was being untied and pinned him against one of the wharf’s bumpers)
Rescued man remains in fair condition at Downers Grove hospital (the man rescued from underneath two 500-pound air conditioner coils at a Hospital remains in fair condition – as a hospital employee, he and another male employee were doing their work at the hospital when heavy cooling coils fell on top of them – hospital is still investigating what caused the accident – the men were rescued and saved by a number of people from the hospital and Fire Department)
ELECTRICAL FIRE Worker burned in JFK Medical Center fireFriday morning (a hospital worker was being treated for flash burns to his face and eyes after rushing to extinguish a fire that erupted at a Medical Center – the 47-year-old electrician noticed flames from an electrical panel in the hospital’s basement at about 7:39 a.m. – the cause of the fire was unknown – the worker grabbed an extinguisher and snuffed the flames but suffered minor flash burns to his face and eyes – the fire was extinguished before firefighters arrived)
