Record number of grain bin accidents in 2010 (last year a record
number of grain bin accidents were recorded as U.S. farmers
continued to increase production and deal with storage of a wet crop
from 2009 – a Purdue University report showed 51 grain bin accidents
last year, up from 38 in 2009 and the most since tracking began in
1978 – twenty-five people died, and five of them were children under
age 16 – the previous record for grain bin accidents was 42 in 1993
– the accidents prompted OSHA to send letters to 10,000 grain bin
operators across the U.S. telling them they are responsible for
preventing the deaths of their workers in grain bins – the letters
follow a batch OSHA sent last summer to 3,000 grain storage
operators after three people died in two accidents at storage
facilities in Illinois – one of the accidents involved two teenagers
who were sent into a grain bin to break up grain that had clumped
together – the bin, which held up to 500,000 bushels, was about half
full, and rescue workers had to cut holes in its sides and drain
thousands of bushels of grain to reach their bodies)

Contract worker killed in steel plant accident (investigators
are looking into the accidental death of a man, 57 – he died after
he was crushed by a hydraulic truck door – he was performing
cleaning services at the plant and was working with a vacuuming crew
– a coworker apparently saw him walk away from the rear of the truck
and closed the door without a view of the back of the truck – he
returned to the rear for the truck for unknown reasons and was
caught in the mechanism – he was trapped in the closing door and his
torso was crushed when the door came down on top of him at about
8:30 p.m. and declared dead at a Hospital at 10 p.m.)

Worker killed at Simmers{apos} mine (a mine employee has died and
another has been injured in an accident at a gold mine – the
accident, which occurred earlier in the day at the mine{apos}s number
seven shaft, had involved “explosive material”)

Offshore oil rig worker dies in fall from platform south of Lake
Charles (incident occurred at approximately 7:45 a.m. when the
worker fell into the water, apparently while transferring from a
vessel to the platform – other crew members were able to get the
worker back on the platform, but shortly thereafter he became
unresponsive and was subsequently pronounced dead)

Arizona mine worker killed in crash identified (an employee, 55,
at the Mine was killed when his service truck collided with a
tractor and burned – he had 30 years of mining experience – diesel
fuel in the service truck ignited in the accident, and miners used
two fire extinguishers and then 35,000 gallons of water to put out
the flames, but the truck{apos}s cab was incinerated)
GUARDING FATALITY
Woman laundry worker brain dead after scarf gets trapped in
dry-cleaning machine (a woman, 32, loading towels into an
industrial steamer at a dry cleaners was crushed when her scarf
became caught in the machine – she was pronounced brain dead after
the accident at 4:45pm – she was looking into the machine and she
forgot she had something on her neck – the machine pulled her and
then the machine stopped – she had worked there for three years)
FORKLIFT FATALITY
Probe follows forklift death at Steel Warehouse (a man, 47, was
killed after he was crushed by a forklift – autopsy showed he died
from blunt force trauma to the chest – paramedics were called to the
plant at 11:21 a.m. and they pronounced him dead four minutes later
– initial investigation showed that he was behind a forklift that
was parked on a inclined ramp, when the forklift gave way, rolled
backward and crushed him between the forklift and a truck)
LOTO FATALITY Lumber company worker dies in auger accident (a
worker, 21, at a lumber company was killed after his clothing got
caught in an auger – he had been grinding up scrap wood to feed into
an incinerator – he was working alone, trying to free up something
stuck in the machinery when his clothing got caught)

Man trapped in forklift truck accident (a worker, 52, was taken
to a hospital with suspected internal injuries after getting stuck
in a forklift truck at a warehouse – he was wedged between the
driver{apos}s cab and the machine{apos}s lifting mechanism – firefighters and
specialist search and rescue officers had to rip the truck apart to
free the worker, who was then taken to a Hospital)
FORKLIFT FATALITY – FREE RIGGING FROM TINES
Merchantville man killed in industrial accident in Moorestown (a
man, 52, was killed after a piece of heavy equipment being hoisted
fell and crushed him – he was pronounced dead by paramedics around
10:55 a.m. – responders found him seriously injured with trauma to
his head and chest – his co-workers lifted a piece of trailer
undercarriage off of him – they had it chained to a hoist and were
using a forklift to move it – somehow it dislodged)

Chemical accident sparks major rescue operation (a chemical
accident at a transport company spurred a large-scale rescue
operation – 15 people were hospitalized with respiratory complaints
and surrounding buildings were evacuated – a worker accidentally
punctured a 200-litre tank of the toxic chemical thiophenol with a
forklift around 1 am, and some 30 litres of the sulphur compound
escaped)

Erie man injured in Walnut grain auger accident (a man, 44,
appeared to suffer “minor but disabling injuries” to his leg, ankle
and foot after his leg was caught in a grain auger – he was working
when he lost his footing and fell into the operating auger of a
grain storage building that holds an estimated 300,000 bushels of
corn – he called the Sheriff’s Department at about 2:26 p.m. and
said his leg was caught in the auger and that a co-worker had turned
off the equipment – crews had difficulty getting him out of the
auger because of the corn in the bin continued to slide toward the
auger – a farm provided a grain vacuum to remove some of the corn)
Worker crushed to death at Wolverhampton factory (a worker, 46,
died after being crushed under a ton of glass at a factory – he
suffered a heart attack during the accident at a glass manufacturer
– a machine inside the factory is behind police tape as an
investigation was launched into the death)

Worker killed in equipment accident (an equipment operator, 40,
working at a plant died after a road grader backed over him –
incident happened about 7 a.m. at the start of his shift – he was
working on equipment at the site when someone called for him – as he
walked toward the person, he didn{apos}t notice the grader backing up –
his son was a witness to the accident – the driver, whose name
wasn{apos}t immediately available, was taken to an area hospital because
he was traumatized and distraught after the accident)

Five injured in accident on Interstate 57 (several people were
injured after a man driving a semi truck crashed into two Department
of Transportation vehicles – DOT workers were parked in the right
lane doing road construction when a semi truck drove into a DOT dump
truck – the dump truck consequently struck another DOT dump truck –
one man, 65, not in a vehicle was struck and injured during the
accident – he was filling pot holes along the side of the road when
the accident occurred – semi driver was cited for failing to yield
to a stationary authorized emergency vehicle and failure to reduce
speed to avoid an accident)
COLLAPSE
Work on Collapsed Texas Parking Garage Won’t Resume Until Root Cause
Determined (a parking-deck structure under construction
partially collapsed injuring two construction workers – footage from
a nearby security camera caught the entire collapse as it occurred –
it appeared to happen as two events, one immediately after the

men were finjured in a propane fire – the fire actually started when
a worker accidentally bumped a propane shut-off valve while working
to remove flooring from a second-story room – the men were working
on a water damage project when the incident occurred – one man was
wearing a respirator while operating an electric roto-hammer,
apparently to remove the flooring – a propane stove had been moved
out of the room and the valve to the appliance was initially turned
off – the worker apparently accidentally bumped the valve to the
{amp}quot;on{amp}quot; position – since he was wearing a respirator, he didn{apos}t smell
the propane – the electric motor ignited the gas and the two men
were burned)

his 50s, was injured when he fell into a trench – worker was
accidentally knocked off a ladder, sending him into a massive hole –
