LOTO FATALITYHawkins worker killed at MIS Inc. plant in Phipps Bend (worker, 34, was killed after getting stuck in a large piece of equipment while reportedly trying to dislodge a part that had gotten stuck – as soon as the part was dislodged, the door closed on the worker – death would have been instantaneous – “I observed XXXXXXXX slumped by a machine identified as a ‘hangar blast,’ wherein it appeared that his head and neck had been caught in a door panel which operated mechanically,” the report said. “Witnesses indicated that XXXXX had been attempting to retrieve an object from inside the hangar when the door slammed shut, trapping XXXXXXXX. Efforts to extract XXXXX from the machine were unsuccessful, leading to his demise”)

Worker treated at hospital after industrial accident in Oldcastle (it was reported that a die allegedly slipped from a forklift and injured a worker’s hand – the worker was transported to hospital, treated and released)

FLASH FIRE Man suffers severe burns after gas explosion near Suffolk airport (worker was flown to the hospital after suffering severe burns in a fire – he had been a temp worker and had been with the company for about nine months – he apparently tried using gasoline to light a heating boiler – it appears the fire came back into the gas can and exploded the burning gas onto him)

Alden worker escapes serious injury from falling glass (worker, 27, was injured in an accident and suffered bruising but no broken bones when pieces of glass weighing between 2,000 and 2,700 pounds fell off a rack – employee was taken to a hospital emergency room and released after about 2 1/2 hours)

Worker killed as drilling rig moved at northern Alberta oilsands site (worker has been killed in an energy sector accident involving a hauling truck and occurred as a drilling rig was being moved)

Worker, 63, dies after horror accident at West Lothian sewage plant (worker was rushed to hospital after the alarm was raised at around 10.30am but paramedics pronounced him dead as he was being taken for treatment – no other details)

ELECTRICAL Worker burnt in incident at Norseman mine (worker who suffered serious burns in a suspected arc flash incident at a gold mine is in a stable condition – the electrical worker suffered serious burns to his face and hands following the incident)

SCALPING & GUARDING Worker was seriously injured after hair caught in motor (company has been fined more than $14,000 after a worker at its chick hatchery sustained serious injuries last fall after his or her hair became caught in an unguarded motor shaft – worker had been “cleaning a conveyor on a chick-counting machine” when the incident occurred)

FATALITY Worker killed at Ambuja Cement’s Maratha plant (contract worker, 32, has been killed in an accident at a Cement plant – an apparent electrical problem in a wagon loading machine caused the incident that crushed the worker)

FIRE Worker mixing resin preliminary cause of Santa Maria industrial fire (a preliminary investigation into an industrial fire that forced temporary evacuations points to spilled resins in a plastics manufacturing facility – investigators are still trying to determine what ignited the fire that spread quickly and caused at least one explosion – a final determination of the cause of the fire, as well as the total damage estimate, is pending the outcome of the ongoing investigation)

FATAL HOTWORK EXPLOSIONWelder killed in barge blaze at Kolsar Bandar; helper missing (worker, 55, died after a fire broke out in a barge he was repairing – a helper is said to have sustained injuries, who, the police said, is untraceable after the fire – the blaze broke out in a 150-foot-long barge at 10.30 a.m. – the fire was doused within 30 minutes – worker was declared brought dead with 100% burns – he started welding parts of the boat at around 9 a.m..  and sparks from the welding machine came in contact with diesel that had leaked from the boat’s engine, leading to the fire)

CRANE FATALITIESManitowoc crane accident: 2 killed, but details on what happened are scarce (two men were killed and another was critically injured in a crane accident at 8 a.m. outside the plant – two other men were taken to a Hospital for minor injuries – one had an injured arm, and the other an injured hand)

FATAL EXPLOSIONFour dead in chemical plant explosion in eastern China (explosion killed four people and injured six – of those injured, two were in a serious condition – plant’s owner was in police custody and the cause of the blast was under investigation – no word on cause)

COM DUST??? Silo explosion downtown causes damage to businesses (an explosion occurred at a silo downtown – explosion could be felt across downtown and caused damage to some businesses – no injuries from explosion – no word on what was in the silo)

AMMONIA TRUCK FILLING Kochi ammonia Leak: Police claim human error, book tanker driver for negligence (an accident at a fertilizer factory resulted in a panic, but the situation was soon brought under control – police have investigated the case and have blamed it on human error – ammonia gas leaked during a transfer of the chemical from the factory to a tanker for transport – a faulty nozzle in the tanker had earlier been identified as the source of the leak – however, police now claim that negligence on part of the tanker driver caused the leak – “The tanker capsule was being filled when the accident occurred. The driver overheard a direction given to another vehicle to take it out of the filling point. The driver, confused, took it to mean his own vehicle which was being filled and moved forward. The valve got disconnected, leading to the massive leakage,” a senior police officer said)

AMMONIA FATALITIES3 workers die due to gas leak in Natural Dairy factory in Patna’s Patliputra (three workers were killed in a Natural Dairy factory due to gas leakage – reason behind the gas leakage is yet to be known – the three workers died of suffocation)

FORKLIFT FATALITYDeath at Fort Myers farm due to forklift accident (landscaping worker, 57, died as result of a workplace accident while repairing a forklift and was pronounced dead at the scene)

FORKLIFT Man suffers serious injuries after falling off forklift (worker at an area auto repair shop was seriously hurt after falling off a forklift – he fell an estimated 10 to 15 feet – his injuries are non-fatal)

Construction

2 workers crushed by 1,000 pounds of rebar in construction accident on West Side (two construction workers were crushed by around 1,000 pounds of rebar in a construction site accident – two other workers suffered minor injuries from debris and were not hospitalized – one construction worker is in critical condition and one other suffered serious injuries – a row of rebar support beams about 20 yards long came crashing down on the workers around 2:35 p.m. at their Department of Transportation work site – workers had been tying the rebar together, forming a support structure which would be lifted atop a pillar on which a new access road)

SCAFFOLDING COLLAPSE 3 workers injured when scaffolding falls at construction site (3 construction workers suffered severe injuries when they fell from a scaffolding inside an apartment building under construction shortly after 10:30 a.m. – four workers were hanging drywall inside the building when the 16-foot-high scaffolding the were standing on collapsed – three of the workers suffered severe injuries – the fourth worker had only minor injuries, which were treated at the construction site – at least one of the workers had life-threatening injuries)

TRENCHING MACHINE AMPUTATION Family Of Man Who Had Leg Amputated In Construction Accident Shares His Recovery (a construction worker became tangled in a digging machine as he was putting cables in the ground – rescuers worked to free him after his leg got wrapped in the blades of a trenching machine – he is recovering in the ICU after he had his leg amputated to save his life in a construction accident – hospital surgeon was flown in to operate)

Worker dies from Santa Fe hospital construction site accident (Fire Department was dispatched to the site around 8 a.m. where an injured worker was laying on the ground surrounded by coworkers – the deceased was hit by a spool of wire – worker died after being transported from the site via ambulance)

FATAL FALL from ROOF Midlands man dies in construction accident (construction worker, 45, fell 30 feet from a rooftop to his death around 2 p.m. – he was laying tin on a rooftop when a gust of wind caught the tin, which flew up and knocked him off the building – he was wearing safety lines at the time, but he had loosened them to move to a new position on the roof, the coroner said – he was taken to to a local hospital, where he was pronounced dead)

FALL from SCAFFOLDING Worker is seriously injured after falling from scaffolding in industrial accident in Melbourne (worker, in his 20’s, fell from scaffolding and hit his head – he was winched from the site and he is being taken to a Hospital in a stable condition)

REBAR FATALITYWorker dies in accident at construction site in Jalan Sultan (construction worker, 41, died after his neck was pierced by a reinforcing bar in an accident at a work site – he was carrying out concreting works at a building under construction, was found with his neck pierced by a rebar (or reinforcing bar) – he was pronounced dead by paramedics at the scene)

WORKZONE FATALITYWorker killed while repairing road on I-95 (a contractor for the Virginia Department of Transportation was killed on I-95 South near mile marker 35 around 11:15 a.m. – worker was struck by a vehicle while working in a mobile pothole repair work zone and succumbed to his injuries at the scene)

WORKZONE FATALITYMan charged in I-294 hit-and-run crash that killed Illinois Tollway worker (charges have been filed against a truck driver, 25, accused of a hit-and-run crash that killed an Illinois Tollway worker, 48 – he has been charged with reckless homicide – worker was picking up debris along the right shoulder when he was struck)

PINNED Construction worker in San Marcos injured after getting pinned by beam (worker working on a TxDOT project was injured when a beam pinned him – Emergency crews worked quickly to free a construction worker who was pinned just before 11 a.m. – he was transported to a hospital with undisclosed injuries)

TRENCH COLLAPSE Worker injured in trench collapse at Virginia construction site (worker was injured in a trench collapse at a construction site – the man had multiple injuries in the accident and was taken to an area hospital for treatment – he was trapped “from the waist down” when a side the trench sidewall collapsed – he was removed from the space by his co-workers)

WORKZONE Construction worker in critical condition after hit on interstate (construction worker is in critical condition after being hit by a vehicle just after 5 a.m. on Interstate 85 – worker was taken to the hospital in critical condition)

FALL thru FLOOR HOLE Construction worker falls 20 feet at Hackensack mall (construction worker, 37, fell 20 feet through a hole in the floor at a mall – he was allegedly walking when he fell through the hole – he was taken to a Medical Center after he complained about pain in his ankles and the left side of his back – injuries are not considered to be life threatening)

TRENCH Injured trench worker rescued in Clark County (worker was stuck 20-25 feet in a trench underground for an hour before he was rescued – worker was taken to the hospital for minor injuries – he was one of many people working on a sewer project for a Housing Development – as they lowered some equipment in there there was an accident – a piece of equipment hit the man, knocking him into the trench)

WORKZONE CDOT worker in serious condition after being struck on Highway 160 (Department of Transportation worker, 64, is in serious condition after being struck by a vehicle while he was filling potholes – extent of his injuries was not immediately known)

TRUSSES Durham construction worker injured by falling roof (construction worker was injured when roof trusses on an 3-story apartment building he was working on fell on him about 9 a.m. – Rescuers used a ladder truck to get the worker from the roof – he was transported to a local hospital in stable condition)

FATAL FALL Worker dies after fall at new Irving Oil headquarters in Saint John (sub-contractor, 33, has died after falling – he later died in hospital from injuries)

Worker dragged down manhole on tube job: Video (Video footage has emerged of an accident which saw a worker dragged down a hole during tube maintenance work – a winch was being used to pull a cable through a platform invert and a worker had a rope tied around his waist with the other end being fed through the winch – the winch snagged and lifted from its mounting moving forward five metres pulling the worker with it – his safety helmet was knocked off before he fell headfirst into a manhole – victim suffered deep cuts to his head and shoulder and back injuries which were treated in hospital)

WORKZONE Two hospitalized after driver enters construction zone in Sherwood (a driver ignored a flagger and several road signs warning him of a construction zone before hitting two workers around 10 a.m. – driver was not injured and is cooperating with police – two construction workers were taken to a hospital via an ambulance with non-life threatening injuries)

AERIAL LIFT & ELECTRICALMan electrocuted in South LA after boom-lift basket touches power line (worker, 22, was electrocuted when a boom lift made contact with a power line at 6:51 p.m. – the entire boom was energized with electricity when it made contact with the power line – witnesses said they heard an explosion and screams – the 22 year old worker died in the boom basket – a second individual, believed to be a 17-year-old male, jumped out from the boom and fell about 15-20 feet onto a metal roof – he was transported to a local hospital in fair condition – firefighters had to wait for more than two hours until Los Angeles Department of Water and Power crews came out to turn off the power before they could get to the 22-year-old man)

FRONT-END LOADER FATATLITY Worker killed in farm machine accident (farm worker, 57, was killed in a machine accident – he had just arrived for work around 6 a.m. when he was run over by a bucket loader – the operator of the machine, 22, didn’t see him – he was pronounced dead at the scene after first responders couldn’t save him)

BILLBOARD Billboard worker burned, hospitalized after Fairfax Village accident (worker suffered serious electrical burns in an explosion and another worker also was hurt at about 6:15 a.m. – one is in serious condition with burns, and the other in fair condition with unspecified injuries – the men were hurt while working on a billboard platform)

HOTWORK Man injured in explosion at auto repair business (one man was injured after an explosion rocked a car repair business – the owner was welding in the back of the building when a nearby 50-gallon oil drum somehow ignited and exploded – he suffered burns on his hands and arms and was transported to a hospital – he was “conscious, alert and walking around” prior to being transported)
