ELECTROCUTIONWorker dies after electrocution at Chandler business (worker, 27, died after he was electrocuted on the job – he was working on an electrical box – he was pronounced dead at the scene)

FATAL DUST EXPLOSION Explosion at factory in China kills 68 (explosion killed 68 people at a factory – the blast also injured 187 – the plant produced car parts for automotive companies – explosion began in a wheel hub polishing workshop – preliminary investigations show that the blast was caused by dust produced as workers polished metal meeting with high temperatures or an open flame)

ELECTROCUTION on FORKLIFT Worker electrocuted in forklift (worker, 59, died after becoming entangled in power lines at about 8:40 a.m. – emergency personnel arrived to find the worker unresponsive inside a forklift – he was transported to a Hospital where he was later pronounced dead)

FORKLIFT FATALITY Worker killed at NC lumber mill (a worker has been killed in an accident at a lumber mill – a forklift overturned killing the worker)

LOTO/GUARDING Men trapped in equipment for hours, severely injured at facility in Empire (an industrial accident at a fish processing facility left two men trapped in equipment for hours and severely injured – accident happened just before 6 p.m. when two employees who were working on a fish counter/sorter became entrapped in the equipment – one one of the workers was freed at 8:50 p.m. and taken to a Hospital a severe injury to his right leg – other worker was freed at 8:45 p.m. and was airlifted to a hospital with severe injuries to both legs)

LOTO/GUARDING Worker in machine hand-trap ordeal (worker, 41, suffered serious injuries to his hand after an industrial accident at an engineering firm – incident happened when a worker’s hand was pulled into a milling machine after his clothing became caught in the mechanism – he suffered severe cuts to the back of his hand and suspected tendon damage and he is currently receiving treatment in hospital)

ELECTROCUTION ETX industrial worker electrocuted (worker, 50, was electrocuted while working with machinery at around 3:22 a.m. – investigators learned the employee was working on machinery when he was exposed to an electric shock – he was taken to a local hospital where he was pronounced dead)

LOTO FATALITY Alberta worker dies after being pulled into crusher (construction worker, 51, has died after he was pulled into a gravel crusher – he was performing maintenance on a gravel crusher when he was pulled into a conveyor)

HAZMAT At least 13 transported from Johnston HazMat (OSHA is investigating a hazardous materials accident at a recycling facility – at least 13 people were transported to the hospital after experiencing skin irritation and breathing problems – a container holding cleaning products went through a shredder at the agency’s recycling plant – exposure was not life threatening, and the workers were taken to the hospital as a precaution)

STEEL PLANT FATALITY Steel worker dies after accident near Seguin (one of the two steel workers injured at a Steel plant has died – worker, 21, suffered burns to as much as 80 percent of his body after an accident inside a “melting room” – the accident, which was the second major incident at the plant in less than a month, happened around 5:30 p.m. – his co-worker described as being in his early 40s, was released from the hospital – he suffered burns to his hands)

LOTO/GUARDING FATALITY Worker killed while operating aluminum press at BRT Extrusions (a worker, 49, was killed when he was crushed in an aluminum press he was operating at 8:41 p.m.)

CONFINED SPACE FATALITY Worker dies after toxic vapors detected (a man is dead following a workplace accident around 1 a.m.at a railroad track – responders found a tanker car with the top hatch open – witnesses told them their co-worker had lost consciousness inside the tanker and could not climb out – firefighters then found atmosphere inside the tanker contained highly volatile ethanol vapors – crews were able to clear the tanker of the hazardous vapors and remove the man’s body)

FATAL FALL Texas Rail System Worker at Airport Dies From Fall (a contract worker at an Airport has died after falling about 45 feet from the rail system’s elevated tracks – the accident happened at night as the worker checked the tracks – worker was walking along the line when he lost his footing or something led to his fall – the worker was doing preventative maintenance on the airport’s above-ground train system)

SULFURIC ACID Workers burned at local chemical plant (two workers at a local chemical plant are being treated for burn injuries – two employees were sprayed with sulfuric acid – one worker suffered significant burns – the other only had minor injuries)

HOTWORK on TANK EXPLOSION Blast injures worker at Bradford recycling plant (an explosion injured a worker at a recycling plant – explosion occurred when an employee at the recycling company was cutting into a crude oil tank with a circular cut-off saw – the vapors in the tank were somehow ignited by the gas-powered saw – force of the blast split the tank in half – one other worker in the area was treated on the scene for minor injuries)

FATAL PIPELINE HOTWORK EXPLOSION Pipeline blast kills worker near Artesia (a worker is dead after a pipeline exploded on an oil field – worker, 21, and three others were in a pit cutting a pipeline and disassembling PVC pipe when the pipeline exploded – three workers were able to escape, but one was trapped inside the pit by the pipe – he succumbed to the hydrogen gas fumes released in the explosion)

FATAL HOTWORK on TANK EXPLOSION Explosion at Omega Protein Plant Kills Worker in Mississippi (worker, 25, was killed and three others were injured in an explosion at a fish processing plant at about 9:30 a.m. while employees of a subcontractor were working on two storage tanks – one of the tanks blew up – welding was taking place at the time of the explosion)

FIRE Fire hits at CNPC’s Lanzhou refinery (refinery has extinguished a fire at its 250,000-b/d refinery and petrochemical plant – fire broke out as a result of a leak in a 300,000-ton/year gas fractionation unit, was completely isolated and extinguished by early afternoon – no casualties have been reported as a result of the incident)

FIRE Investigation into cause of Diamond Green Diesel fire underway in Norco (no one was injured in the explosion and subsequent fire, which occurred at 9:30 p.m. – fire was contained and extinguished by midnight – investigation into the cause of the fire is underway)

FIRE Four hurt as fire hits US refinery (refinery has been shut down after a blaze broke out in its isomerisation unit, injuring four people – employees had been working on a pump when it caught fire with flames engulfing the area – fire began just after midnight and burned for just over an hour before being put out – no explosion was reported and that all of the injuries suffered were burns)

CHEMICAL REACTIONS No injuries in chemical accident at Oscar Mayer (no one was injured following chemical spill at a plant that happened at 3 p.m. – HAZMAT team was called in to neutralize the fumes from the accident after lactic acid and sodium nitrate were mixed together – the team neutralized the reaction using soda ash)

EXPLOSION Manure digester explosion sparks fire (officials are investigating an explosion that destroyed the roof of a 1.25-million gallon manure digester – the blast sparked a fire that destroyed a $250,000 nylon inflatable cover over one of its three digesters – no injuries were reported – company believes the explosion was caused by the ignition of methane gas as an employee started an electric blower)

AMMONIA Authorities investigate ammonia leak at frozen food plant (authorities are investigating an anhydrous ammonia leak that led to the shutdown of some local roads near a frozen food plant – officials say they were able to stop the leak in all but one room inside the building – because they are unable to reach the last tank, officials say they will allow the ammonia to dissipate – each tank holds 300 to 500 gallons of ammonia – officials say the leak was caused by a failure in the plant mechanical system – no evacuation and no injuries have been reported)

AMMONIA Ammonia leak cleared in south Edmonton (plant was evacuated around 4:30 a.m. – the main supply line to the compressor ruptured – there was an emergency shutdown – crews were able to ventilate the plant which allowed employees to return later in the morning – traffic was briefly disrupted in the area – no injuries were reported)

FORKLIFT FATALITY Man killed in North Dakota industrial accident (a man, 51, was killed in an industrial accident – he had been working on a payloader hose when the forklift portion of the machine collapsed, pinning him – he was taken to a County Medical Center where he died of his injuries)

 

FORKLIFT Man injured in industrial accident at Dartmouth Crossing store (a man in his 20s was injured in an industrial accident at a big box store – the victim received leg injuries – extent of the injuries, reportedly caused by a forklift, was not immediately available)

 

CONSTRUCTION

TRUCK LOADING FATALITY Giant log crushes, kills 30-year-old madison county man at Arlington construction site (a man, 30, was crushed to death at a construction site when a tree trunk fell on him shortly before 6 p.m. – subcontractors were loading large sections of trees onto a flatbed truck and the workers had trouble securing one giant log – the driver exited the vehicle and was out there attempting to secure it with a strap when one of the sections rolled off, ultimately crushing him)

WORKZONE FATALITY CHP ID’s Mercedes Driver Who Veered Off Road, Job Train Worker Killed on Highway 101 (one construction worker is dead after a car struck him – officers are looking into why the driver veered off the highway and fatally struck a worker cleaning up litter on the side of the road)

Construction worker burned at Fair Lawn excavation site (a construction worker suffered electrical burns on his hands while working an excavation project just after 8:30 a.m. – no details)

TRUCK UNLOADING FATALITY Bronx construction worker fatally crushed by crate (a construction worker, 62, was killed when a crate fell on him while he was unloading a truck just after 8 a.m. – a battery weighing about 2,700 pounds crushed his head when it fell from a truck as the worker was unloading it – he was pronounced dead at the scene)

JACK HAMMERING & ELECTRICITY Construction worker injured in River North accident (a construction worker using a jackhammer hit a power line and was shocked – he was taken to a Hospital in serious condition)

Falling glass from midtown building injures 3 (three people suffered minor injuries after a window fell from the 18th floor of a building -window, which weighs several hundred pounds, came crashing down after workers inside accidentally hit the window with their equipment – a worker on a scaffold lost balance, then pushed the scaffold into a 5-by-7-foot plate glass window)

TRENCHING 2 workers pulled from Rye trench collapse (emergency responders managed to free two workers trapped in a collapsed trench – both men were removed on stretchers, their injuries were described as non life-threatening – the collapse occurred outside a residence)

Highway Worker Injured In Mishap Near Higgins, I-90 (a construction worker was injured when a panel from a retaining wall being built along the eastbound lanes of I-90 fell, pinning him on an easement to the tollway – construction workers removed the retaining wall panel from on top of the worker using cranes at the site before first responders arrived on the scene – worker was transported to a local hospital)

WORKZONE FATALITY Name released in fatal Batesville accident (a Highway Department employee, 52, died after construction equipment rolled over him – he was helping with road construction when a piece of equipment flipped and rolled on top of him – crews were laying asphalt when the accident occurred – he was pronounced dead at a Medical Center)

WORKZONE Worker hospitalized after being run over by supervisor (worker, 58, was hospitalized after his supervisor accidentally ran over him at a construction site – supervisor was driving the construction equipment at around 1:30 p.m. and believed his employee was standing on one side of the vehicle and began backing up – several witnesses yelled at the victim to move but he wasn’t able to get out of the way in time and the vehicle ran over his lower body – he was conscious and alert when he was flown by helicopter to a Medical Center – he remained hospitalized for a crushed pelvis and ankle fracture)

CARBON MONOXIDE Farmville Carbon Monoxide Leak Sends 17 Workers To Vidant (grocery store has reopened after a worker passed out and many others were affected by high levels of carbon monoxide – store was closed at the time for remodeling – eight people were sent to a Medical Center for carbon monoxide poisoning symptoms, then nine more after it was a confirmed CO leak – sixteen were construction workers, while one was a store employee – a piece of equipment used with the remodeling caused the leak)

WORKZONE Right-of-way worker airlifted following accident (worker, 21, was struck by a 2008 Ford Focus shortly before 1 p.m. – he was working for a private contractor which does right of way mowing – he allegedly stepped in front of traffic while weed trimming the guardrail area)

ELECTRICAL Electrician burned on the job in Greenville (an electrician is recovering after being burned at a construction site – the worker was making a connection to the main power line when he was shocked – he was airlifted to the hospital for burns on his arms, but is expected to be okay)

WORKZONE FATALITY Construction worker killed by driver on I-89 (a construction worker, 61, was killed along I-89 after he was struck while working in a construction zone – a 22-year-old driver hit him as he was picking up line stripe markers in the work zone around 11:15 pm. – preliminary investigation shows the woman was not speeding or impaired)

MISCELLANEOUS

MOWER FATALITY Worker killed when mower lands on top of him (county employee, 29, was killed when he was crushed by a lawn mower while mowing a steep ditch – co-workers found him in the ditch and called for help – he was dead when emergency crews arrived)

MOWING FATALITY Shelby County worker killed in tractor accident (a public works employee, 29, died in a tractor accident – he was maneuvering a tractor while cutting grass along a ditch when the tractor flipped over – he died at the scene)

ELECTRICAL BURNS GPD identifies injured electrician in Graham Walmart fire (worker, 34, was reported to have severe electrical burns on his body – he was a contract electrical worker who was working alone inside the “electrical room” – he was there to replace a breaker – according to witnesses at the scene, he came running out of the “electrical room” and it appeared that he was on fire – store employees who were in the stockroom came to his aid and extinguished the flames)

ELECTRICAL w/ FALL from LADDER Worker suffers serious electrical shock at Carleton University (an electrical contractor, 32, is in hospital after suffering an electrical shock just before 11 a.m. – he was on top of a ladder and removing wires from a ceiling fixture when he was shocked, causing him to fall off the ladder – he was treated for second-degree burns and a serious cut to one of his hands – he is in stable condition)

CONFINED SPACE FATALITY Texas Wildlife Employee Dies at Hatchery (a Parks and Wildlife Department employee has died after working in a confined space at a state fish hatchery – the man was in a chamber about 20 to 30 feet underground and apparently operating a gasoline-powered pump when he collapsed – investigation continues into his death)

CARBON MONOXIDE 6 treated for carbon monoxide exposure while cleaning up flooded UCLA Pauley Pavilion (6 people who were helping clean up UCLA’s flooded Pauley Pavilion are being treated for exposure to carbon monoxide – the arena’s court was covered with up to 10 inches of water after a 30-inch water main broke nearby and sent water cascading onto campus – the men were helping pump water out when they fell ill from generator exhaust – two were taken to the hospital in fair condition and four were treated at the scene – the equipment was turned off and the building was aired out)

CONFINED SPACE FATALITY – ENGULFMENT Teenager dies after falling into farm silo in Borders (worker, 18, has died after suffocating in a 50ft high silo tower at a farm – he died at the scene – it is understood he fell into the tower and become trapped shortly after 9am – fire crews cut a hole in the tower using a turning saw – they managed to retrieve the young man’s body and hand him over to waiting paramedics but he was pronounced dead at the scene)
