FATALITY Coroner identifies plant worker killed in early morning accident (worker, 49, was killed when a coil fell on him – he was working with steel coils, when a 6,000-pound coil fell on him – County Coroner pronounced the man dead)

PAPER MILL FATALITY Worker dies in Ladysmith paper plant accident (worker, 46, died from injuries apparently suffered in an accident at the plant – he was injured while working on a paper machine at 5:07 a.m.)

GARBAGE TRUCK WORKER FATALITY Monroe worker killed in Marksville accident (refuse worker, 42, was struck by a pickup as he crossed a street just before 6:00 a.m.)

FATALITY 1 worker dead after being hit by steel casing at Markham work site (worker died of his injuries after he was hit by a steel casing – the industrial accident happened just after 8 a.m. – worker succumbed to his injuries at the scene)

UPDATE on CRANE ACCIDENT TimkenSteel worker injured in crane accident, factory fined for 2nd time in a year (according to an OSHA report finalized Oct. 30, a crane’s safety latch failed on May 4 and 1,000 pounds of equipment fell on a worker below on the factory floor)

TRUCK UNLOADING FATALITY Worker killed in Hopkinsville factory accident (worker, 46, was killed while working at a factory – the victim was handling equipment on the back of a semi-trailer when the equipment shifted and fell on him – he was pronounced dead at the scene by County Deputy Coroner)

G.R. crews free worker pinned by steel beams (an accident trapped a man under several beams around 7:30 p.m. – worker called 911 saying he’d been trapped by several steel beams – fire department brought in special equipment to lift the beams off the trapped man – he was trapped for about an hour, getting freed around 8:40 p.m – extent of his injuries are this point are unknown – he was hospitalized)

FATAL HOTWORK EXPLOSION Volga expresses loss for worker killed in explosion (a tank explosion at a soybean processing plant kills a welder, 39, just after 2:30 pm Thursday – workers were installing catwalks high above storage tanks, then one of them exploded, killing the worker)

LOTO/GUARDING Worker taken to hospital after industrial accident in Brampton (one man has been transported to hospital after an industrial accident – he was working at around 3 a.m. when his hand became caught in a piece of machinery – he was taken to hospital but his condition is not known at this time)

ARC FLASH One injured in Georgia Power plant accident (one contract worker sustained electrical burns on both arms, but there was no explosion – contract worker was working on off-line maintenance when there was an electrical arc between pieces of equipment – worker was burned on both arms and immediately transported to a nearby hospital for treatment)

Bridgeton industrial accident sends man to hospital (worker was sent to the hospital after an industrial accident around 5 p.m. – worker was flown to a Hospital for injuries – no other details)

CONFINED SPACE FATALITY Myanmar worker killed in mixer incident (worker died in a workplace accident after being caught in a mixer at industrial park – it is believed that the victim, 38, was working alone at the water tank factory during the incident which occurred around 2.40pm. – victim was caught in the mixer – medical team had pronounced him dead at the scene)

FORKLIFT FATALITY FACTORY WORKER KILLED IN ACCIDENT (worker at an aluminum factory has been killed after the forklift truck she was driving overturned – the first fatality at the plant in over 20 years – it has been claimed that the woman was working unsupervised when the accident happened, causing a delay in her rescue)

MINE FATALITY Brookwood mine worker dies after being found unconscious on site (bulldozer operator, 58, died after he was found lying unconscious on site – unknown why he was unconscious and the death is under investigation – workers immediately performed first aid until he was airlifted to a hospital – preliminary report, the accident is classified as a “Slip or fall of person” – accident occurred at 5:45 a.m. – he had 10 years of mining experience)

MINE FATALITY Man killed at Waikaia goldmine named (worker, 30, was fatally injured while operating earth-moving machinery at a goldmine – it was the first death of an employee at the two-year-old goldmine)

CONFINED SPACE and HIGH ANGLE RESCUE Man rescued after hours in Zionsville water tower (an inspector, 33, was trapped in a water tank – rescue crews pulled the injured worker from the water tower and lowered him 150 feet in a stretcher buffeted by high winds and rain – it took rescue specialists from the Indianapolis Fire Department three hours and 44 minutes to get the contractor back on the ground – he was part of a crew inspecting the tower – he fell from an interior ladder and was injured about 3:30 p.m.)

FORKLIFT FATALITY OSHA investigating after worker fatally struck by forklift in Alsip (worker, 59, was fatally struck by a forklift at a manufacturing plant – he was struck by the forklift, which was backing up, as he was walking)

FATAL EXPLOSION Man dies in Qld factory explosion (worker, 38, has died after an explosion at an oil recycling plant after oil sludge exploded and caught fire – two other men, a truck driver and a plant worker, were in the vicinity of the blast when it occurred)

FATAL FALL into HOT OIL Worker, 37, dies after falling into hot oil (confectioner, 37, died of burn injuries after he fell into a cauldron of hot oil being used for sweet meat preparations – he was making famous sweet balu shahi when suddenly he fainted and fell into the cauldron of hot oil – as soon as fell in, two other workers tried to pull him out, but they suffered burns as well – it was learnt that he has been running high fever for the past couple of days but due to shortage of staff he had to come to make sweets)

FATAL HOTWORK EXPLOSION Welding explosion near Kinistino kills 19-year-old worker (worker, 19, had been performing welding work was pronounced dead at the scene – no other details)

FATAL FLASH FIRE from MALFUNCTIONING GAS PLANT FLARE Man Dies Following Explosion at Howard County Gas Plant (worker has died following an explosion at a gas plant – the flare at the plant was somehow emitting gas but did not properly ignite and vapors collected near a plant vehicle that contained a worker – that’s when the vapors ignited and engulfed the vehicle in flames injuring the worker who was inside the vehicle – he was airlifted to a hospital and died)

ELECTROCUTION Worker at Trinity Marine electrocuted (worker, 22, was working on a barge at the plant when the accident happened – he was near a welding machine that was plugged into an electrical outlet – plant manager told investigators that workers heard screams before they found the worker)

HOTWORK FIRE 2 injured in welding fire at LG&E power plant (workers are being treated for burns after a welding accident at a Generating Station just after 1:30 p.m. – fire started while two contractors were welding something at the power plant – crews rushed the workers to the hospital – it is not clear how badly the patients are injured)

HOTWORK FIRE Fire causes evacuation of General Cable mill building (dozens of workers had to be evacuated when a welding operation sparked a fire that damaged the outbuilding attached to the loading dock – it took firefighters about 15 minutes to extinguish the fire and another 15 minutes to blow smoke out of the basement level of the mill building)

HOTWORK FIRE Fire breaks out at mushroom plant in Fillmore (a welder was using a torch while doing welding work on the roof of one of the straw covers – due to strong winds, sparks from the torch were blown to the straw bales down below – there were no injuries)

AMMONIA Anhydrous ammonia leak closes I-90 (two 1,000-gallon tanks began leaking around 4 p.m. at a grain elevator – the leak was stopped around 6:25 p.m., but a cloud of vapor remains – some nearby homes were evacuated, and traffic was rerouted from I-90 in the area – road was back open around 9:30 p.m.)

FORKLIFT FATALITY TEEN DIES IN FORKLIFT ACCIDENT (worker, 17, passed away from injuries gained in a forklift accident – he was riding on the side of a forklift around 4:45 p.m. when he slipped off and was run over – he was flown to a Medical Center and later died of his injuries)

FORKLIFT Man run over by forklift at Pipetech in Burton (worker, in his 30s, was run over by a forklift – he suffered a serious leg injury which was not life threatening)

CONSTRUCTION

EXCAVATOR Construction worker seriously injured in Danbury (construction worker was seriously injured when he was pinned by an excavator – accident happened when two employees transferring materials around 1:30 p.m. in a parking lot – worker, 49, was operating a large excavator that struck and pinned another employee, 53, against another excavator while he was walking near the large machines – he suffered head and leg injuries and was transported to a Hospital)

FATAL FALL Construction worker killed at Linden Street site in Winnetka (worker, 68, fell 12 feet and was then hit by beam while working with a crew building a house – incident took place about 11:50 a.m., when the man fell roughly 12 feet and hit a beam while he was working on the base of the foundation of the home)

CRANE FATALITY Worker Killed by Falling Concrete in Madison County (worker, 53, was killed when a concrete wall fell on him -company was delivering concrete walls to a construction site – one of the walls fell while being transferred from one truck to another)

CRANE THE SOURCE CLOSES TEMPORARILY AFTER CRANE ACCIDENT (a crane collapsed over the parking structure being built and impacted the roof at the back of a store next door – construction worker injured on the scene of the crane accident was treated for bruises but no major injuries)

BULLDOZER North Naples construction worker dies in accident with bulldozer (worker, 21, died after he was crushed by a bulldozer – he was working when the bulldozer fell on him – company is working with authorities to determine how the accident happened)

FATAL FALL thru ROOF Worker fatally injured in southeast Nebraska fall (worker, 39, has died after falling to the ground while helping to put a new roof on a building – he fell about 21 feet through an unsupported part of the roof)

WALL COLLAPSE Wall collapses in Merrimac; workers injured (a second-floor wall collapsed on construction workers yesterday afternoon, resulting in three people taken to area hospitals via ambulance just before 1 p.m. at a new home under construction – the wall fell from the second floor and crushed three workers – a fourth worker sustained minor injuries and refused treatment at the scene – the three injured workers sustained serious but non-life-threatening injuries)

CRANE Beam falls on worker (construction worker, 32, suffered traumatic injury to his leg after being struck by a beam about 9:38 a.m. – they were transitioning a beam from a crane when this accident occurred – he suffered extreme trauma to his leg and might lose his leg)

WORKZONE Construction worker struck in hit-and-run in Tampa (construction worker, 33, was struck in an active construction zone by a driver who fled the scene – he was standing in a lane closure on US 41 southbound when the 1992 Honda Civic collided with the construction barricades and struck the worker – he was transported to a Hospital where he remains in critical condition – driver has been charged with DUI with serious bodily injury and leaving the scene with serious injury)

WORKZONE FATALITY WORKER KILLED ON HIGHWAY 23 (worker, 20, who was in his third year of engineering at the South Dakota School of Mines and Technology, was killed when he was struck by a semi truck – he was taking measurements to verify erosion control along a completed construction zone, both on and off the highway – he was taking the measurements for the engineering company that he was doing co-op work for when he was struck by a 2009 Kenworth semi truck at about 1:30 p.m. – he died at the scene)

TRENCH FATALITY Clark County coroner identifies crushed construction worker (construction worker, 30, was crushed to death by heavy equipment as he dug a trench – cause of his death is pending – workers were digging a trench when he fell in and was hit by the equipment)

FIRE Worker Injured in Equipment Fire at Apartment Complex Construction Site (worker, 57, was rushed to the hospital with burns to his body after a fire broke out at a construction site around noon – fire was contained to a piece of equipment that erupted in flames – worker who was using the device was rushed to the hospital with burns to his arms and head – he is expected to be okay)

TRENCH FATALITY Construction worker crushed to death by pipe (construction worker, in his late 50s, was killed on the side of a highway after a massive piece of steel pipe rolled into a ditch, crushing him around 8:20 a.m. – pipe came loose and rolled into the ditch – victim was dragged down and trapped under the pipe at the bottom of the muddy ditch)

FATAL EXPLOSION Worker dies after explosion on Melbourne construction site (worker has died following an underground explosion on a construction site about 11.40am – he was trapped in a trench or a tunnel following a “below ground explosion” – paramedics tried to reach the injured worker but he died a short time later – explosion appeared to be an industrial accident)

TRENCH Man hospitalized after trench collapse (worker, 25, is seriously injured after a trench collapsed on top of him – he was taken to a Hospital following a nearly two-hour rescue effort – a 3m-deep trench the man was in collapsed on top of him at about 4pm, pinning him by his legs)

FALL from SCAFFOLDING 23-Year-Old Man Injured In Industrial Accident (worker, 23, fell off scaffolding around 1:00pm – he fell approximately 8-10 feet – the victim sustained a laceration to his head – injuries are not believed to be life threatening)

FORKLIFT Forklift accident halts hospital construction (a forklift smashed a window in the day surgery department at the Hospital – the forklift operator was hoisting building forms when the machine’s forks punctured two windows in an area where patients are prepared for day surgery – the day surgery unit is at least 10 metres above the bottom of the pit where construction work is taking place)

MISCELLANEOUS

FATALITY Haileyville PWA employee dies after on-the-job accident (city worker, 60, died from injuries he suffered while working on waterlines – details of what happened are limited, but city officials said two city employees were searching for leak in one of the city’s waterlines)

FATALITY Worker killed as stage lights collapse ahead of concert in China by singer Jolin Tsai (worker was killed after stage lights collapsed as staff were setting up for a concert – firefighters, rescue workers and police pulled 14 people free caught under equipment – one person died at the scene and another was seriously injured)

ELECTROCUTION Electrical worker suffers fatal shock at Madison-Grant (a contracted electrical worker, 44, suffered a fatal electric shock at a High School – he was working in a secluded area of the building near the south end of the high school on an air-handling unit, which is for heating and cooling in the south end of the high school – he had removed the panel and climbed inside to work on the unit when the coils electrocuted him – he hadn’t been seen since he checked into the high school about 8 a.m. – a maintenance worker walked to the secluded part of the building to ask him a question, found him unresponsive about 9 a.m. and called 911 – he was pronounced dead at 9:36 a.m.)

LIGHTING STRIKE FATALITY Cattle station worker killed by lightning strike near Normanton in north-west Queensland (a 48-year-old cattle station worker was killed by lightning as he was tending to horses at a property shortly after 3:00pm (AEST) – he died at the scene)
