was performing routine maintenance when killed (the worker, 56,
killed in an accident at an asphalt production plant got caught in a
piece of equipment on top of a set of towers used to store the
manufactured asphalt – he was doing routine maintenance, not mixing
asphalt, at the plant when he was killed)

Plant accident injures worker (a small fire inside a
manufacturing plant caused “serious” burns to one worker who was
rushed to a Medical Center via Mercy Flight at about 2 p.m. – the
reported explosion turned out to be a small fire that apparently
erupted when a forklift operator accidentally bumped into a
55-gallon drum containing a mixture of chemicals – the driver
attempted, unsuccessfully, to douse the blaze with a fire
extinguisher – the operator’s condition was not immediately known,
although crews characterized his injuries as “serious” – they

accident involving a natural gas pipeline has claimed the life of
one worker – an equipment operator, 50, was struck by a truck that
was backing up at a gas well pipeline job)

the company gave him no training on a peeler machine from which a
safety guard had been removed, and its {amp}quot;surgically sharp steel blade
sliced through his pelvis, cutting off his !@#$% and test!@#${apos}s
while virtually cutting his body in half – peeling machines have a
thin blade that spans the width of the mammoth machine, which slice
blocks or cylinders of foam as they pass through – the complaint
states: {amp}quot;At some point prior to any work-related activities with
Future Foam, Future Foam removed protective guarding that was
intended to prevent workers from coming in contact with the
machine{apos}s steel cutting blade{amp}quot; – worker was working through nonparty
staffing company – he did not ordinarily operate the machine and had
not received any training in the operation of the machine and
specifically to conduct the work that he was asked to do when this
incident occurred)

Worker killed in tow motor accident (a 64-year-old man was
pronounced dead at 1:30 p.m. at a Hospital after a tow motor
accident at a manufacturing plant – plant specializes in metal
stampings, tube bending, welded parts and subassemblies – he was
scheduled to retire in June and was looking forward to it, a family
member said)
Railroad worker rescued after being pinned under grain car in
Willmar BNSF yard accident (emergency responders used hydraulic
jacks to free a man, in his mid-30s, pinned under a 221,000-pound
grain car in the railway yard – he suffered severe injuries to his
legs – responders found the man was under the grain car, pinned by
his legs, and that it was unsafe to roll the car off of him)

Bulldozer driver dies at Consol mine in W.Va. (a worker has died
after an accident at a coal mine – a bulldozer operator died – the
operator was pinned between the bulldozer and a water truck – the
vehicles are owned by separate contractors – the man was connecting
a tow chain to the truck when the dozer rolled backward, pinning him
– he was alive when freed, but died before he could be flown to a
hospital)

Drill rig operator loses arm below elbow (a worker operating a
diamond drill rig at a Mine ended up having his arm amputated below
the elbow after an accident – the worker{apos}s pants were caught in the
rotating head of the machine, and he was pulled in – after the
accident, the injured man was brought to a Hospital for treatment)
FATAL MINE EXPLOSION
Romania mine blast kills five (an explosion killed five workers
in the mine – the four electricians and an engineer had been working
on an electrical installation when the blast happened at a depth of
around 400 metres (1,300 feet) – at 14:30 the workers said they had
finished their job but some ten minutes later we smelt smoke coming
out of the ventilation system – a rescue team found the bodies five
hours later)

fell into a resin-filled tank at a chemical factory died while the
other survived due to timely treatment – the workers had
accidentally fallen into the tank while trying to remove a barrel –
the strong fumes of the chemical emanating from the tank had made
the rescue difficult – the factory{apos}s rescue team and fire brigade
were struggling for rescuing the duo – the fire brigade had to cut
open the metal tank and drain out the sticky resin for taking the
workers out of the tank – a barrel had fallen into tank and the
victims were trying to remove it when they too fell)

built up inside the apartment, and around 2:30 p.m. the compressor
in the refrigerator kicked on, sparking an explosion so powerful
that it shattered a glass bay door and knocked the plasterboard off
the walls in three rooms – he was in the apartment at the time and
suffered minor burns – he was treated at the scene but otherwise did
not require hospitalization)

Ferndale grocery worker{apos}s arms crushed in machine (an employee,
in her 50s, suffered serious crushing injuries to both legs in a
cardboard box crushing machine at about 9 a.m. – it was unknown
whether emergency surgery performed would be able to save her arms
and hands – the store has had the machine for at least a decade and
the employee had used it many times – the injured woman was talking
and had been removed from the machine by coworkers when emergency
medical technicians arrived – Her coworkers “managed to turn off the

NFL Workers Recovering from Falling Ice at Cowboys Stadium (six
people, including contracted workers, were injured by falling ice at
a Stadium – the most seriously injured was hit in the head and

worker was injured Wednesday afternoon after an open bucket of
gasoline ignited at a car repair business -workers had drained
gasoline out of a vehicle into an open bucket on the floor – an
employee used a torch to cut some bolts off a vehicle and do exhaust
work and the gasoline caught fire – the burn victim said the fire
flashed in his face and ignited his shirt, which he and another
employee removed and threw on the floor – employees drove the man to
the hospital after putting out the fire with a couple of fire

electrical problem compounded by a person rushing to help was the
cause of an explosion and fire at a tire store that injured two
employees and severely damaged the building – an employee was
working on a motor vehicle fuel pump in a garage service bay at
about 3:35 p.m. when the worker removed a wire from the fuel pump
harness, resulting in an arc, or stream of current, which ignited –
a second employee attempted to douse the blaze with a fire
extinguisher, but instead the force of the discharge knocked over a
tray containing gasoline from the gas tank, fueling the fire and
triggering the explosion – the 31-year-old employee working on the
fuel pump suffered burns to his hands – he was treated and released
from a Medical Center – the 42-year-old employee operating the fire
extinguisher suffered burns to his hands and legs and remained in
serious but stable condition)
