IMPALEMENT
Wisconsin Worker Impaled On Metal Rod Identified (a machine
operator impaled on a metal rod in an industrial accident is
expected to make a full recovery – rescuers were called to plant –
they found the man impaled on a metal rod that pierced his torso,
entering the body from one side and exiting from the other – the rod
was about 10 feet long and a little more than half an inch in
diameter – accident is under investigation)
UPDATE on LOTO FATALITY
Tortilleria Chinantla, Brooklyn Factory Where Worker Died In
Accident, Shut Down (a tortilla factory where a man was crushed
when he fell into a dough mixer has been temporarily shuttered –
facility was not closed because of the gruesome death, but his loss
of life did lead investigators to the facility – the state Workers
Compensation Board issued a stop-work order at the facility after
learning the factory{apos}s owner was not offering workers{apos} compensation
insurance to his employee)

Machine Traps Worker Underground In Monaca (an unidentified man
was rescued after he was trapped in a piece of machinery at about
4:40 a.m. – he was freed at about 7 a.m. – the extent of the
worker{apos}s injuries was not determined)
CONVEYOR BELT ENTANGLEMENT
Worker freed from Colona Transfer conveyer belt (a worker, 30,
had to be rescued from his underground work station after he became
entangled in a conveyor belt – he was loading limestone on the
conveyor belt to transfer it from an incoming barge to ground
transportation when his right arm got caught in the belt – emergency
workers couldn’t free the man’s arm, so a technical rescue team was
called for – that group disassembled the conveyor belt so the man
could be freed – that process took about two and half hours)
LOTO FATALITY
DeWitt Excavating accident: Man was killed in {apos}crusher{apos} machine that
became clogged (federal safety investigators are trying to
determine how a 20-year-old man was killed and another worker
seriously injured while trying to unclog a cement crusher – the
worker who was killed was 20 – the other employee, 44, was airlifted
to a Regional Medical Center with injuries to his legs and back –
the men were outside trying to remove a clog in a cement {amp}quot;crusher{amp}quot;
machine, which is about the size of a van and crushes recyclable
materials to use in road building, when something went wrong)
Kennecott worker wounded by shotgun-like device (officers
investigated an industrial accident involving a shotgun-like device
that left a hot-metals worker at a smelter wounded – it appears a
crew was cleaning ore-processing residue from the Smelter when the
incident took place about 7:40 a.m. – part of the process is that
they have to use an 8-gauge shotgun to loosen the sediment enough to
remove – it appears a fragment of the slug may have struck an
employee – the device fired is called an “industrial pellet gun”
within the mining and smelting industry, and its use is part of
routine, accepted procedures used to clean smelter furnaces – one of
the pellets ricocheted off the burner and through a steel inspection
cover – a fragment of the pellet struck a worker a few feet away in
the right leg, just below the knee)
Abattoir worker found days after death (it is believed an
18-year-old man, whose body was found at a workplace on Sunday, may
have been dead since Saturday – it is believed the man{apos}s head was
crushed in machinery which was located in an isolated part of a roof
space – officials are investigating the likelihood that the man{apos}s
clothing may have become entangled in a piece of machinery in a cool
room)
CONFINED SPACE
Man Injured in Construction Site Accident (a construction worker
was injured when he fell into a manhole at the site of the new store
– the victim is a supervisor with an excavating and grading company
– he was climbing down an access ladder in a retention pond tower
when he fell – it is believed the victim was attempting to adjust
some piping at the time of the accident – the victim was extricated
from the manhole shortly after noon and was transported to a Medical
Center – he was in stable condition at the time of transport, but
was {amp}quot;in a great deal of pain{amp}quot; due to injuries to his lower
extremities – due to instability at the top of the retention pond
tower, crews chose to extricate the man by transporting him through
an approximately 80-foot long vertical pipe)
LOTO FATALITY w/ CONVEYOR
One dead after industrial accident in Hampton (one person was
killed in an industrial accident at an asphalt facility – the
incident happened about 2:30 p.m. while workers were performing
winter maintenance at the asphalt plant – the victim was working on
a conveyor belt when somehow he got caught in a piece of machinery –
he was pronounced dead at the scene – the victim had been with the
company for about 10 years)

2 reported burned in accident at Gallatin plant that makes metal
powders (two workers are reported injured in what is being
described as a flash fire at an industrial plant – both sustained
burns and were listed in critical condition in a burn unit – plant
makes iron and steel powders for automotive and industrial uses)
Roosevelt man killed in oil field accident (a man is dead
following what investigators believe was an industrial accident at
an oil field – a water truck driver, 32, had been removing water
from a pit at a location when he climbed onto the pump jack at the
well site for some reason – he fell into the pump jack and suffered
fatal crush injuries – his body was discovered by other oil-field
workers when they arrived at the site)

Worker survives electric shock at Water Pollution Plant (worker
was intitially said to have been knocked unconscious by electric
current, but a township official said the worker was conscious when
he was taken to an area hospital for treatment – extent of his
injuries are not known at this time – details of how the incident
occurred have not yet been released)
FATAL EXPLOSION
Two die in explosion in chemical unit in Thane district (two
people were killed and another person was seriously injured in an
explosion in a chemical plant – the deceased were identified as the
manager, 50, and lab-in-charge, 28 – another worker,35, who received
severe burn injuries, was rushed to a local hospital – cause of the
explosion is yet to be ascertained)
HOTWORK FIRE Warehouse Fire Is Biggest
in Pa. Town{apos}s History (about one million board feet of lumber was
stored in the 50-foot by 300-foot building – two people were working
inside the building at about 8:15 p.m., removing scrap metal with a
torch, when the fire accidentally started – no one was hurt – fire
took about a million gallons of water, which drained two nearby
ponds)

fire at an oil refinery occurred at the plant{apos}s coker unit –
experienced a small pump fire on a coker unit – a filing to the

workers were not endangered but two of them were admitted to the

were reported due to a gas leak at a refinery that forced the

were flown to the hospital as a result of the fire – although they

reported after fire forced an evacuation at a plant – the fire was
contained to one of the operation{apos}s fine crushers and was
extinguished within an hour – extent of the damage is unknown and

Beer Distributor Has Ammonia Leak (hazmat crews are called out
to a business after a leak is detected after an alarm was set off –
the company distributes bottled beer, and the ammonia is used as a
cooling agent for the large coolers they are stored in – there was
no danger to any homes around the building, which was evacuated –
there were no injuries)
CHLORINE
Chlorine leak: 17 fall ill in Haryana (at least 17 people
complained of nausea, giddiness, sore throat and irritation in the
eyes after chlorine gas leaked from a factory – they were taken to a
nearby hospital for first aid -all of them are fine now and
discharged from the hospital)

has been injured after being thrown from the machine – the forklift
operator was found lying on the ground near the forklift – he was

taken to a Hospital for treatment of a cut to the head)

Worker At EMU Falls Down Elevator Shaft (a subcontractor working
on a dormitory was seriously injured after he fell about two stories
down an elevators shaft – the man appeared to have a serious leg
injury when firefighters brought him out)

Workplace accident claims man’s life (a 23-year-old man was
killed when the iron worker was in the basket of an aerial working
platform, securing joists – a joist released and struck the worker –
he died of his injuries at the scene)

Mud wall caves in, worker killed (a construction worker, 24, was
killed on the spot and another labourer sustained injuries when a
mud wall caved in at an underpass construction site – the injured

five others on the one-km underpass – along the underpass, two
concrete walls have come up half way and there is a distance of
nearly six feet between the concrete wall and the mud wall – the
work to clear the mud to lay concrete on the ground is in progress –
the two workers were standing in a big pit removing mud in some
containers while others were standing at the pit’s edge)

Austin Worker Killed In Construction Accident (one worker is
dead and another is hospitalized after they fell 20 feet from a
wooden scaffold on which they were working at an apartment complex –
it{apos}s unclear whether the workers fell form the scaffold or the
scaffold collapsed under them – the surviving worker was taken to
University Medical Center with injuries not thought to be
life-threatening)

Construction Accident Kills 10 in Manila, Philippines (a
construction accident in the business district has killed ten
workers – eight workers were on the platform to install glass panels
on the 28th floor when it fell, and they plummeted all the way to
the seventh floor – the platform fell on three other workers,
killing two of them – one construction worker survived the fall,
suffering some fractures now being treated at the hospital)
Man injured in well site accident (a construction worker was
injured by a piece of construction equipment in an accident at a gas
well site around 2:45 p.m. – he was injured in his lower extremities
and was transported from the scene to the hospital for non-life
threatening injuries – no other details were available)

Worker injured at Evonik laboratories dies of injuries (a man,
36, has died a day after he was badly injured – he was operating a
mobile lift vehicle when he became lodged next to a building while
in the vehicle{apos}s basket around 1:30 p.m. – he was doing insulation
work at the facility – after being freed by other workers and
emergency responders – he had been placed on a ventilator, but after
doctors consulted with members of his family, he was taken off life
support systems – he died shortly thereafter)
3 killed in China construction site collapse (three workers were
killed and eight others slightly injured after a construction site
collapsed – the accident happened at a construction site of a hotel
– eleven people were at the construction site when the accident
happened – rescuers retrieved two bodies and sent eight injured
workers to local hospital)
CRANE FATALITY w/ POWERLINES
Man dies after crane hits powerlines (a 24-year-old man died
while he was standing near the crane at the time of the accident –
the man was helping to move large steel containers at the premises
when a crane hit powerlines – the mobile crane has actually touched
overhead powerlines, which has caused the death of a man that was
walking along beside the steel container)
MISCELLANEOUS
Whiteface worker hurt during lift malfunction (an employee is at
a hospital after he was injured on the job – he was trying to
restart the Kids Kampus lift at 10:15 a.m. when he was struck by a
chain and broke his arm – this was while a standby, auxiliary engine
was being used to unload two skiers and two instructors at the top
of the lift, which had malfunctioned and stopped)
