INDUSTRIAL

MINE FATALITY Suncor oilsands worker dies following workplace accident (worker, 37, dies after being struck on the head by the falling reservoir or sump pan of a large bulldozer – he was doing maintenance work on the machine at a the mine site – a reservoir or sump pan fell on the worker’s head – he was taken to a hospital where he died of his injuries)

Martin County Sheriff’s Office Investigating Deadly Industrial Accident (a man, 47, is dead after a deadly industrial accident – the exact cause of the worker’s death is still under investigation – no other details)

Worker Dies in Accident At ZPC (employee died in an accident at a stone crushing plant at a Portland Cement company – he was pronounced deceased at the scene of the accident – no word on cause or details)

LOTO/GUARDING FATALITY Conveyer belt crushes worker to death at Zambezi Portland (worker, 27, was crushed to death by a conveyor belt – have not established what he was doing with the conveyer belt – company spokesperson later explained that he was operating the conveyor belt but he got trapped in it – the conveyor dragged him through the crushing process, killing him instantly)

TRUCK LOADING FATALITY One person dead after industrial accident in Vaughan (a man has been declared dead after an industrial accident shortly before 7 a.m. – the worker, a transport truck driver, was crushed by a shipping container while a truck was being loaded)

RAILCAR SPOTTING CSX employee injured in Social Circle (railroad employee was significantly injured at a distribution center around 1:30 p.m. – he was spotting a train as it backed up and got tangled or caught between or under the cars)

LOTO/GUARDING Worker suffers severe injury at manufacturing plant in Guelph’s south end (a female worker is suffering from a severe injury following a workplace accident at a business – at around 6:20 p.m. Police, Fire and EMS responded to a business that custom designs and manufactures rubber products and components – employee operating a heated press had one of her arms caught inside the machine – she was airlifted to a hospital)

MOLTEN ALUMINUM Mason worker burned at Mitsubishi Electric (worker was burned by liquid aluminum just after 6 a.m. – crews also contained a spill – worker was taken to a Hospital)

HOTWORK Blast in Ooty Cordite factory; 8 injured (a blast at a factory injured eight persons – two persons were grievously injured in the blast, which reportedly occurred shortly before noon – the blast occurred at a Nitrocellulose (gun cotton) workshop of the factory – it was not in operation – maintenance works were in progress – when a corrugated pipe was being welded there had been an explosion – the presence of some residual explosive material is suspected to have triggered the explosion, he said that the building had been partially damaged)

HOTWORK Mill blaze extinguished quickly; damages of roughly $8,000 (a small fire caused by a welding spark at a mill did about $8,000 in damages – fire was reported at 7:53 p.m. – it was confined to that area and involved sawdust and a section of the conveyor belt)

CONFINED SPACE FATALITY Man killed in residue tank at Vietnam bauxite refinery (worker, 34, died in a tank of red mud residue – a kind of waste produced while refining bauxite into alumina – he was in the process of clearing the residue tank with a high-pressure hose when a chunk of mud fell apart and hit him – he was standing at the bottom of the tank to hose down the tank’s inner wall when the accident happened – he died ten minutes after rescuers got him out of the tank, while the co-worker survived with injuries)

AMMONIUM NITRATE FIRE 70 tons of ammonium nitrate delivered to warehouse before fire (officials say 70 tons of ammonium nitrate were delivered to the fertilizer plant warehouse just before a fire broke out – it’s not clear how much of it had been sold and what actually remained in the bins when the fire started at the facility – 47 residents spent the night in shelters after being evacuated when the fire broke out at the plant – officials have labeled the plant as a crime scene because they think it’s suspicious that there was a 30-minute window between the last employee leaving the plant and the first reported call to authorities – no injuries have been reported – residents say they heard two loud explosions at the fertilizer plant, and claim that emergency crews told them initially to hide behind their apartment complex)

AMMONIA Ammonia leak at ethanol plant sends man to hospital (an ammonia leak at the ethanol plant sent a man to the hospital around 3:30 p.m. – plant was evacuated as emergency personnel responded to what was originally reported as an explosion at the plant – what actually happened is that a pressure relief valve on an ammonia tank opened up as it was supposed to in the case of too much pressure – as a result, one of the workers in the area seemed to have inhaled some of the ammonia vapors and was transported to a hospital – EMS crews also had to be decontaminated following the incident – scene was cleared two hours later)

AMMONIA Two sent to hospital after ammonia leak at Pinnacle Foods (employees returned to work after an ammonia leak – the leak outside of the plant sent two people to the hospital and shut down nearby roads around 9:30 a.m. about a malfunctioning valve on a tank – it took 20 minutes to shut off the valve, which was spilling ammonia into the air – one plant worker and one bystander were sent to the hospital for precautionary reasons)

FORKLIFT FATALITY Man Dies in Accident at Tides International (worker, 62, was killed around 1:00 p.m. in a work related accident – he was not driving but rather riding on a forklift when he fell off and was run over by the machine – he was transported by EMS to a Hospital where he was pronounced dead on arrival – an autopsy confirmed he died of severe chest and abdominal injuries as a result of the accident)

CONSTRUCTION

Fall Construction worker falls into pit at The Rim (construction worker suffered what were described as “significant injuries” in a fall at a construction project – he toppled from 10 to 15 feet into a pit where workers were forming concrete walls on a building under construction)

WORKZONE FATALITY Police probe crash that killed Pa. turnpike worker in Chesco (a tractor-trailer hauling food struck two Turnpike maintenance workers doing work around 8 a.m. in the westbound lanes, killing one, 61, and injuring a second – a preliminary investigation indicated the tractor-trailer entered a lane that was closed for maintenance and struck the turnpike employees  -the second worker, a maintenance foreman, suffered minor injuries and was treated and released from a Hospital)

Construction worker dies in accident at Mapusa (a construction worker, 26, died – he was on the ground floor when a boulder from the eighth floor fell on him)

FATAL FALL Union worker falls to his death (a worker fell off the Anthony J. Celebrezze federal building – the victim, 51, was putting new glass around the perimeter when he fell seven stories to his death – he had just finished putting the last piece of glass up for the evening, when he unsnapped himself to get off the building – after that something went wrong causing him to fall to his death around 10:30 p.m. – he was pronounced dead at the ER)

FATAL FALL from CRANE BASKET 2 workers die in Yolo County bridge construction accident after falling from crane (two construction workers died after falling from a crane during work on a bridge project – the two workers died after falling into a deep hole dug for the new bridge’s footings – the workers fell from a basket on the crane into the hole)

FATAL FALL from ELEVATOR Elevator accident kills worker at Wichita building under renovation (worker, 40, was working in a building being renovated has been killed in an elevator accident around 10:30 a.m. – the worker was riding in an open-door, pulley-style service elevator when he fell, catching his head between the elevator door and a brick wall of the shaft – he was pronounced dead at the scene)

PAVING Paving worker pinned between equipment taken to the hospital (a paving company worker was injured while paving a parking lot around 10:15 a.m. – police investigators were on the scene trying to figure out exactly what happened – “He was pinned between two pieces of their equipment”)

FATAL FALL from ROOF Worker dies after 120-foot fall from roof (a man, 26, working on a roof at a hangar at an air field died when he fell more than 120 feet – the deceased worker was attempting to move from one section of the roof to another when he untethered from his safety harness and lost his footing – his brother was working with him and witnessed the fall – winds may have played a role in the fall – gusts in the area reached up to 45 miles per hour)

1 construction worker killed, another injured in floor collapse at New Jersey restaurant (a collapse in the basement of a restaurant has killed one construction worker and injured another – it’s not clear what the workers were doing when the floor above the basement apparently collapsed – the injured worker is in serious condition – a third worker escaped injury)

EXCAVATOR 2 workers hurt in Chatham excavation accident (an excavator rolled over and down a steep embankment while being operated, injuring two workers at 1:32 p.m. – the excavator, operated by a worker, 51, rolled over and down the embankment as he was digging out a new home site – the second worker, 63, was operating another excavator downslope and jumped from his machine when he saw the other excavator approach – both men suffered non-life-threatening injuries)

WORKZONE Highway worker dragged 50 feet by car (a Highway Department worker, 57, was injured when he was dragged by a car for 50 feet in a roadside ditch – worker was treated at the scene by paramedics and taken to a Hospital where he was treated for leg and facial injuries – the injuries were not life-threatening – he was briefly pinned under the car that hit him – about 1:10 p.m., a 2000 Hyundai was southbound when he applied his brakes on wet pavement, lost control of his vehicle, crossed the road and wound up in a ditch on the east side road – speed was a factor)

BUZZ SAW Wild buzzsaw slices woman’s thigh on New York sidewalk (an unidentified woman was hit in the thigh by a 3-foot blade when it broke off a construction worker’s machine and uncontrollably cycled into the air – the buzzsaw struck the woman – the buzzsaw reportedly nicked a tree before slicing into the woman’s thigh and knocking her down onto the sidewalk – it traveled an estimated 100 feet before impact)

TENT COLLAPSE Workers injured after high winds cause construction tent collapse (construction planks, scaffolding parts and other materials were sent flying by high winds that collapsed a tent where workers were gathered, sending as many as a dozen to the hospital – EMS transported eleven patients in stable condition to Hospitals – one additional worker was taken to an Urgent Care – none of the injuries were reported to be life-threatening)

SCAFFOLDING COLLAPSE Scaffold collapses, three workers killed in Istanbul (three construction workers died due to the collapse of a scaffold)

TRENCH Worker’s leg crushed by metal plate in Freehold, police say (a metal plate fell on a man’s legs at a construction site – he was working at a construction site behind a private home at around 9:30 a.m. when the plate fell on him while he was in an approximately 6-foot deep trench – the plate fell on the worker’s leg above the knee – he was taken to the hospital with non-life-threatening injuries)

TRENCH FATALITY Report: Worker Dies In Construction Accident (a construction worker, 51, reportedly died on the job in an accident – he was killed in an excavator accident around 1:30 p.m. – he was crushed between a backhoe and trench box while inside the trench – they were apparently working with the state Department of Transportation to install a replacement water line in the area)

CARBON MONOXIDE FATALITY One dead, three sickened in carbon monoxide poisoning (a worker died after being overcome by gas fumes in the underground garage of a highrise around 2:30 p.m. – it appears they were working in the garage with gas-powered machinery – one man was found without vital signs and taken to local hospital, where he was later declared dead – three other males were taken to hospital with minor injuries – all four males suffered from carbon monoxide poisoning – it is believed the victims were operating a power washer)

HOTWORK Plastic dome of People’s Mall catches fire (a fire broke out at a Mall – no shop or other outlet was damaged – at time of the incident, there were not many customers around as welding work was being carried on – the fibre dome of the Mall was gutted in the fire that flared up when welding work was being carried on at the top of the mall building – as the dome was made of hard plastic, the fire spread quickly and clouds of thick, black smoke filled the sky)

UPDATE on FATAL HOTWORK FIRE UPDATE Welder at Back Bay fire cited before for no permit (company being sued for failing to obtain a city permit for work that spawned a fatal, nine-alarm Back Bay fire in March has apparently operated before without a permit, a practice not that unusual in the welding industry – fire officials investigating the Back Bay fire say a company was performing repair work when welding sparks jumped over to the neighboring four-story brownstone row house – after festering under the clapboards for a time, the sparks eventually ignited the fire – whipped by fierce winds blowing off the Charles River gusting up to 45 miles an hour, the fire blasted up from the basement and raged out of control through the entire eight-unit building, gutting it and taking the lives of two Boston Firefighters)

FATAL FALL from SCAFFOLDING Coroner: Man dies after falling off scaffolding (worker, 56, died after he fell off scaffolding at a construction site – the deceased was a subcontractor at a home site – he was a self-employed painter and owned his own painting business – the scene investigation found the man was painting at the time he fell from a ladder which was leaning against a wall on top of a scaffold – he fell down a stairwell to the main floor level of the two-story home – total distance of the fall is estimated at approximately 22-24 feet – he was pronounced dead at the scene)

MISCELLANEOUS

HOTWORK Eastside auto body shop owner in critical condition (owner, 59, of an auto-body shop was in critical condition – he sustained second-degree burns from a fire at his business – he was working with a torch when somehow his clothes caught fire about 7 p.m. – worker, 59, saw his boss run out of the shop and ran after him, tearing his burning clothing off – both were taken to a hospital, owner is in serious condition and worker is in good condition – third worker in the building was not hurt)
