AMMONIA Ammonia leak at Munn Arena contained, MSU says (Arena was evacuated after an ammonia release inside the building – officials said they’ve stopped the leak – six employees were in the arena when a monitoring system detected an ammonia leak about 1:10 p.m. – system triggered an alarm and automatically turned on exhaust fans – employees were safely evacuated – leak was traced to a valve on an indicator gauge)

HOTWORK Welding crew sets elevator shaft on fire at high-rise in downtown Dallas (fire crews made quick work of a blaze sparked by welding sparks at the building – fire broke out about 10:20 a.m. in an elevator shaft at the high-rise building which is under construction – sparks from a welding torch fell down the shaft and set trash at the bottom on fire – a second alarm came in soon after the first, bringing several more fire trucks to the scene – fire was quickly extinguished, and the building was cleared – no injuries reported)

HOTWORK Official: Five-alarm fire at apartment complex in Phoenix suburb was accidental (a five-alarm fire at an under-construction apartment complex was an accident – Fire Department, along with investigators from the ATF, found the only heat source in the area was a roofing torch)

FATAL STORAGE RACK COLLAPSE Worker dies in industrial accident on the Southside (worker, 50, died after falling and hitting his head when a wall of heavy-duty industrial shelving collapsed – he and a co-worker were demolishing the large wooden structure inside a vacant warehouse – structure gave way and the victim who was on top of it, fell about eight feet and hit his head on the concrete floor – other worker wasn’t injured)

LUGGAGE CART FATALITY Man dies in luggage cart rollover at Toronto’s Pearson Airport (worker, 24, pronounced dead at the scene after he was thrown from a luggage cart he was driving- he was driving the luggage cart when it rolled over and he was thrown to the tarmac around 11 p.m.)

CRUSHED BETWEEN FATALITY Texas worker crushed between two charter buses (worker at a charter bus company died after he was crushed between two of the big vehicles – the man parked a bus and was standing outside with another worker when he noticed the vehicle started to roll – victim tried to reach the bus door, to stop the moving vehicle, but was crushed between the front of the bus and a parked bus – victim died at the scene)

ELECTROCUTION in AERIAL LIFT Man electrocuted in sign fix called beloved dad, inventor (worker, 48, was working for his family business when he was electrocuted while he was atop a bucket lift fixing a sign for an air-compressor supply store – he was shocked for about a minute, causing him to fall 30 feet – the assisting subcontractor electrician saw him stop breathing and performed cardiopulmonary resuscitation until paramedics arrived)

FATAL EXPLOSION One killed in gas explosion on tanker (worker was killed and five other crew members injured in an explosion on a tanker – the vessel was on its way when the blast, which is believed to have been triggered by a gas tank, occurred at about 10pm)

HOTWORK Welding blamed on Fort Madison fire (someone welding on a car sparked a fire just before 9:00 pm – owner was welding on a car, which caught the car on fire – fire spread to the rest of the garage – garage and the car are a total loss)
