STEAM Worker burned in triumph accident (worker was seriously burned in an accident about 10:30 a.m. – the woman was in stable condition after suffering steam burns on the front part of her body)

FORKLIFT FATALITY Worker Dies in Forklift Accident at Marine Terminal (a longshore worker was killed in a forklift accident at a Marine Terminal just before 3:30 p.m.)

FATALITY Worker killed in accident at Richmond plywood facility (worker has died on the job at a Plywood facility – the accident involved machinery, but could not comment further as the file had been turned over to Mounties and work safety investigators)

LOTO/GUARDING FATALITYworker entangled to machine, dies (worker, 22, met a tragic death after he got entangled with the belt of the machine of a mustard oil mill – accident occurred around 7:30pm)

Two killed, 6 hurt in blast in factory (two workers, 31 and 36, were killed and six others suffered burns when molten iron fell on them in an alloy factory – blast allegedly occurred in the induction furnace of the factory)

FATALITY Investigation launched at Holland plant following worker’s death (worker, 56, died seven days later at a medical Center after he was struck by a steel coil)

AERIAL LIFT FATALITY Family mourns worker killed in cherry picker accident (worker, 22, killed in cherry picker accident that saw him crushed to death between the machine and a steel frame when it overbalanced and wedged him between the machinery and a steel frame)

FATAL FALL from Communications Tower Police: Man’s falling death in Kansas ruled accident (investigators have ruled the death of a man, 39, who fell from a communications tower an accident – he died after falling 320 feet (97.5 meters) from a tower – he was working on the communications tower when he fell – a co-worker on the tower heard him click his safety harness before he leaned back and fell – another co-worker on the ground alleges he hit three guide lines as he fell)

HYDROGEN PEROXIDE Worker burned in Lawrence offloading chemical (worker suffered chemical burns to his face after a chemical, hydrogen peroxide, was being offloaded from a truck – as the worker was unloading the chemical from a truck, the fill pipe broke and hydrogen peroxide splashed into the worker’s face and spilled onto the floor)

BOILER EXPLOSION Worker hurt in boiler explosion at Wigle Whiskey in Strip District (worker was injured when a boiler exploded at a Whiskey Distillery around 11 a.m. – after firefighters arrived, they found no fire, but did determine that a boiler used in the distillation process exploded – injured worker was transported to the hospital in good condition)

HOT WORK FIRE Welding blamed for massive Murray Bridge abattoir fire (authorities have revealed a massive abattoir blaze was sparked by a maintenance worker who was welding a bin – worker had been working on an offal bin – worked tried to put the fire out but unfortunately it got into a highly combustible wall and spread quite rapidly)

PRCS FATALITIES – ENGULFMENT Workers identified from fatal grain bin accident (two workers, 28 and 32, were killed when they were buried by grain at a grain elevator – the bodies were recovered about three hours after the men were buried under 20 to 25 feet of grain – it isn’t clear how they got into the bin and what caused them to become trapped)

EXPLOSION Tanker truck explosion hospitalizes two in Fairfield County (one person was hospitalized after a reported tanker truck containing hydrocarbons exploded – the truck contained a mix of water and old gasoline – it was in a “workshop-like” building at an environmental salvage business at the time of the explosion – explosion reportedly happened while the truck was thawing in the building because of the cold)

LOTO FATALITYFactory worker sucked into machinery and crushed to death as he rushed to get order out during overtime shift (factory worker has been killed after he was sucked into a machine and crushed to death inside the gift bag factory – he was working an overtime shift late on New Year’s Eve, rushing to get an order complete and ready for New Year’s Day – he can be seen putting pieces of paper into the machine by hand, but then suddenly it grinds to a halt, with something jammed inside – he was working alone when the accident took place – he then leans in to see if he can fix the issue, but the machine suddenly kicks back into action, dragging him headfirst into the workings – he received a massive head injury and could not be saved)

WWTP METHANE EXPLOSION Gas build-up leads to explosion at Blue River Wastewater Treatment Plant (gas build-up led to an explosion at a Wastewater Treatment Plant – a methane build-up in the plant caused a partial explosion of the rear door around 10:30 p.m. – part of the building collapsed in the explosion)

GRAIN EXPLOSION/DUST COLLECTOR Explosion and fire damages East Grand Forks grain elevator (call came in just before 7 p.m. – there was heavy fire showing from the top of the plant and appeared to be part of the filtration system – estimates damage at $100,000 to $200,000 – cause has not been determined – two employees were inside the complex at the time but were able to get out safely)

AMMONIA Hazmat Situation In Baltimore County Investigated After Ammonia Leak (ammonia escaped from an exterior valve – the building where the incident occurred has been evacuated – a worker was exposed to the chemical and is being evaluated by medics)

CHLORINE Chlorine gas leak, water main break near Lake Ashburton in Northwest Baltimore (gas leak, caused by a broken vacuum regulator at the pumping station, was discovered about 4:45 a.m. and contained in about an hour and 15 minutes – it’s unclear how much chlorine leaked or whether it settled into the lake)

CRANE FATALITY Two die in Si Sa Ket crane accident (crane‘s sling breaks while the crane lifts construction materials to the upper floors of a hospital building resulting in the deaths of two workers – before the accident, three workers had loaded rubber floor tiles to be lifted to the top floor of the 71 building – when the load reached the fifth floor, a sling broke – one of the workers managed to jump into the building shortly before the sling broke and is being treated for his injuries – the other two workers, 21 and 29, fell from the crane with the rubber floor tiles falling on them)

CRANE FATALTIES Three dead, two injured as crane collapses in Powai (Three labourers, 32, 45, and 48) died and two others were injured when a crane collapsed as they were digging a pit for a sewer line – accident occurred before 5.30 pm when a private contractor was working on a sewer line – the rope holding the machine snapped and labourers fell into a pit at least 30 feet deep where work was underway – a portion of the crane collapsed and fell on the five on-site workers, killing three instantly)
