EPA issues RMP citations (NH3 Refrigeration)

EPA conducted a compliance inspection of Respondent’s facility located in Orem, Utah, and found that the facility had violated regulations implementing section 112(r)(7) of the Act by failing to comply with the specific requirements outlined in the attached RMP Program Level 3 Process Checklist-Alleged Violations. Fall from Scaffolding

The injured worker was an insulation contractor employee who was working on a 10 feet scaffold while blowing insulation into walls. The name of the contractor and builder have not been shown. The worker sustained head injuries and complained of pain to his hip and leg. We slide these platforms on trailers prior to setting containers upon them. They are secured, once aligned, via four legs, bolts and nuts.
