INDUSTRIAL FATAL FALL from CRANE Man killed in paper mill accident fell 28 feet (contractor, 35, died at a paper mill when he fell 28 feet from a crane – he was doing repair work on the crane) CRUSHED by STEEL Police ID steel plant worker killed in accident (a steel fabricating company employee, 60, was killed when a steel plate struck him – “a piece of steel fell on him and caught him right in the chest”) PRESSURE WASHER High-pressure waterblasting accident in Bayonne sends worker to hospital (worker, 52, was hospitalized for a thigh injury after letting slip a high-pressure water hose that he was using – at around 12:45 p.m. the worker was using the hose when it slipped from his hands and struck his left thigh – injury was non-life-threatening, but the worker was transported to a Medical Center, where he underwent surgery) FATAL EXPLOSION @ RECYCLING PLANT Mortar explosion at IL recycling plant kills 2, injures 1 (two workers of a recycling plant were killed and another worker was injured when a mortar exploded at the facility at 6:30 a.m.) ELECTRICAL Electric shock gives life-threatening injuries to Hydro One worker (worker was rushed to the hospital with life-threatening injuries after receiving an electric shock – accident happened around 10:30 a.m. – he was found hanging in a harness 15 feet off the ground and had to be rescued by firefighters – he sustained severe burns but that “he will pull through”) CRUSHED Man dies after becoming trapped between trucks in workplace accident in Kwinana (worker, 70, died after becoming trapped between two trucks in a workplace incident – he received serious head injuries – he was crushed between a prime mover and another truck and died at the scene) FATAL EXPLOSION Explosion kills employee at NW Houston industrial complex (an explosion killed a worker, 31, and it’s the second time an employee has died at the same facility in less than a year – the blast caused a large piece of equipment to fly through the air and land on an employee, killing him – exactly what went wrong to cause that calamity is now the subject of a federal investigation – in September 2013 a worker was ejected and crushed in a forklift accident) CONTRACTOR CRUSHED Worker injured at Heinz factory (worker was airlifted to a hospital at 3:27 p.m. – worker was stuck under a piece of industrial equipment – he had been freed by the time emergency services arrived – the third-party contractor was injured while installing a new piece of equipment) GUARDING/LOTO FATALITY Man dies in northeast Ohio industrial accident (worker, 45, was killed after he was pulled into a machine – his clothing became stuck in a machine and he was pulled into it – when rescuers arrived, he had been freed from the machine by co-workers, but he had died from crushing injuries) ARC FLASH Worker recovering from burns in electrical accident at Frisco hotel (an electrician is recovering from burns he received while working on an electrical box at 9:18 a.m. at the loading docks of the hotel – he was changing out a breaker in a breaker panel when it arced – he suffered first- and second-degree burns to his face) LINE BREAK Pipe burst wounds employee at Brooks plant (worker, 43, was working in the rendering plant when a pipe burst – he was fixing a blockage in the pipe where the hot water and steam is – as he took the cap off, the water and steam burned him and the electricity, it blew him right off the top of the staircase and he fell to the concrete floor – he was rushed to a hospital where he was treated for severe burns “all over his body”) CRUSHED Man killed in industrial incident at Campbellfield work site (worker, 43, was killed when a stack of boxes crushed him at a factory – a vertical cardboard stacker collapsed on him about 9am – he was found by a co-worker when he returned to the factory some time after 9am) ELECTROCUTION @ OIL FIELD Texas oilfield electrocution kills 2, injures 1 (accident at an oilfield killed two workers and injured one – the crew was moving a pumpjack to a new location when a cable from the winch truck hit a power line – a third worker was critically injured and taken to a nearby hospital before being flown to a hospital) IMPALEMENT Impaled Affco worker removed hook (worker, 42, impaled through his head by a hook managed to pull the piece of equipment out of himself – emergency staff worked to free him at the plant for 90 minutes as he stood attached to the device – the man, who was part of a cleaning crew, was conscious for the procedure and able to move away to free himself afterwards – he had been impaled by a spreader, which keeps the legs of a cow open) BARGE Goderich Worker Injured (worker, 71, hurt while securing a barge to break-wall – he was taken to a hospital with injuries suffered in the accident – a large wave struck the barge at the same time an excavator on the platform started moving backwards – he got his legs caught under the tracks of the excavator – he was taken to a Hospital with non life-threatening injuries) Shipyard accident kills one (a burst launching airbag killed one worker and injured another – workers had just finished inflating the inflatable ship launching airbag – less than a minute later it burst) CRUSHED Man killed in accident at northern New York plant; worker got trapped under heavy equipment (worker, 28, has been killed in an accident involving heavy equipment being moved at a manufacturing plant – crews were moving equipment when the worker got trapped underneath the equipment – he was pronounced dead at the scene) HOTWORK EXPLOSION on DRUM Man injured after welding project atop oil drum causes explosion (a man was hospitalized after his attempts to build a lawn roller resulted in an explosion – incident took place at about 9 a.m. when the man was welding atop of a 55-gallon drum in his garage – sparks ignited remnants of fuel in the drum, which caused an explosion and sent the top of the barrel through the wooden garage door – the man sustained burns to his arms and face, he extinguished a small fire on the wooden garage door before authorities arrived – he was then taken to a hospital for non-life threatening injuries) EXPLOSION Indianapolis Plant Explosion Caused by Malfunction (a malfunction in an aluminum heating machine caused an explosion and small fire at a factory – no injuries were reported from the explosion about 2 a.m. – the heating machine malfunction sent a fireball through a vent to the factory’s roof – workers were evacuated from the factory and firefighters were soon able to put out the blaze) SEWER EXPLOSION Sauget sewer gas explosion sends manhole covers flying, damaging power line (a sewer gas explosion sent several manhole covers flying, at least one of which damaged a power line and cut off electricity to a factory – the village’s sewer system lost power at around 5:15 a.m. – during efforts to repair it, an electric line arced and that triggered a fire in the sewer system that eventually caused the explosion at around 7:50 a.m. – worker was slightly injured in the explosion but refused medical treatment and remained on the job) AMMONIA Mission ammonia leak: Lougheed Highway 7 reopens (an ammonia leak at a food processing plant closed the highway around 3 a.m. PT. – it has since been contained and hazmat crews have inspected the plant – no injuries have been reported as a result of the leak – highway was reopened around 7:30 a.m.) AMMONIA Gas leak lands 10 in hospital (ten people fell unconscious while eight buffaloes died when a cloud of ammonia gas spewed from an ice-making plant – rescuers said that they shifted 10 people to hospital after they fell unconscious because of gas leak – as the workers arrived to start the processing, all of a sudden, they witnessed a large plume of ammonia gas shooting from a refrigeration unit inside the building – they tried to plug the leak but failed to do so because of massive gas pressure – investigation is underway but the rescuers pointed to faulty pressure relief valve in the refrigeration unit) CHLORINE Buffalo Factory Temporarily Evacuated After Chlorine Leak (factory was temporarily evacuated after a leak caused chlorine levels in the air to soar to dangerous levels – six crew members in chemical suits entered the building to make sure everything was shut down, and quickly got the chlorine levels down – no one was injured in the leak, and officials say there are no residential areas nearby that could have been threatened – plant uses chlorine in the milling process) CONSTRUCTION ELECTROCUTION w/ CRANE Glen Allen construction worker electrocuted in ‘freak accident’ (worker, 24, has died after an accident at a construction site – he was lowering metal scraps onto a crane to set down onto a set of rafters – when he pulled on the straps, they came into contact with power lines overhead and he was electrocuted – he was taken to a Hospital where he was pronounced dead) WORKZONE FATALITY Road worker killed in New Jersey (one worker, 45, was killed and another was injured after an SUV ran into the back of their construction equipment at 1:19 a.m. – authorities pronounced the worker dead on the scene) UNLOADING STEEL Worker struck at future Delaware North site still in hospital (worker struck by a steel beam at a construction site remained hospitalized and is expected to make a full recovery – the worker was injured around 8:15 a.m. Twhile unloading steel beams at the construction site) WORKZONE Bridgeton city worker hit by pickup truck (employee, 48, was injured when he was struck by a vehicle – he was trying to remove a manhole cover when he was hit by a pickup truck – he sustained injuries to both legs and was taken to a Hospital – he was working in a marked work zone when the accident occurred, and was struck after the motorist drove his truck over a safety cone – he was also wearing a fluorescent safety vest) FALL from ROOF Worker who was hospitalized in fall from roof is improving (worker, 33, fell from the roof of a house last week and ended up in a hospital’s intensive care unit – he suffered a fractured skull, broken ribs and a collapsed lung during the fall – accident occurred about 8:30 a.m.) WORKZONE FATALITY Md. highway worker killed in Eastern Shore crash (worker, 40, was killed after an accident in which he was guiding traffic around a work zone) ELECTROCUTION Construction worker who died identified as South Prairie resident (worker, 38, died as a result of “high voltage electrocution” – he was in a ditch, working with a compressed air tool, when he made contact with a power line at 1:50 p.m. – he was pronounced dead at the scene) ROLL OVER FATALITY Worker Crushed to Death in St. Petersburg Roller Accident (worker, 53, was killed after he was pinned under a large piece of equipment at about 3:45 p.m. – he was driving a roller on top of a sand berm – he was pinned under the machine after it slipped and rolled on top of him) Parkway worker injured by falling pole (worker was injured unloading utility poles – worker was taken by ambulance to hospital – he was snipping metal ties on a trailer when he fell off and one of the light poles landed on him – the pole hit him in the chest and face shortly after 6 p.m.) CONCRETE SAW Worker injured in trench accident at Jennings Road and King’s Highway in Fairfield (worker was cutting concrete in a trench when the saw slipped and cut his chest – emergency crews had to used a stretcher to get him out of the three foot ditch) FATAL FALL from PIER Worker dies after falling from structure of Eastbourne pier while he was helping to repair damage from fire (worker, 44, was helping to repair the fire-ravaged Pier fell to his death – he fell through a boardwalk onto the beach) MISCELLANEOUS FATAL FALL from LADDER Employee dies after falling from ladder at Kalispell Walmart (employee fell from a ladder and died – worker was on a very tall ladder retrieving merchandise when he fell) NEAR DROWNING Wet ‘n Wild worker in critical condition after near-drowning (worker is in critical condition after becoming trapped underwater while cleaning one of the pools – another worker called for help as he pulled the trapped employee out of the water) CONFINED SPACE FATALITY – H2S 2 workers die inside shopping center sewer in Scottsdale (two men died after getting trapped inside a sewer at a shopping center – three employees were doing some maintenance or repair work on the sewage pumps, which were approximately 15 feet below the surface – firefighters responded to the scene after getting a call at about 4 p.m. – the initial call came from somebody that heard them [the victims] screaming – when rescuers arrived, one of the three workers was climbing out of the sewer – he was described as being very lethargic – inside the hole, the hydrogen sulfide, or sewer gas, measured at lethal levels – restaurant had to be evacuated – the two workers who died in the sewer were 36 and 41 – the third worker who managed to climb out of the sewer is 18 and he remains in the hospital and is expected to recover) ELECTRICAL State Fair Employee Shocked On Ride (worker was cleaning one of the rides when he says he tripped and fell onto a machine – he felt bolts of electricity surge through his body – he is recovering after badly burning his hand – a bystander who tried to help him was also shocked – he ended up suffering third-degree burns – doctors do believe he’ll eventually regain full use) HOTWORK Employees evacuated after fire in basement of high-rise (contract workers were welding in the basement of the building when the fire started around 2:15 – a few embers hit the plastic on a hot water tank, melting it and causing the basement to fill with smoke – workers evacuated the area while firefighters contained the fire and cleared out the smoke – no one was injured, and the building reopened a few hours later) FORKLIFT FATALITY Man dies after being struck in forklift accident at Lynnwood business (worker, 69, was struck by a box that fell off a forklift while he was walking through a parking lot of a business at about 10:30 a.m. – the victim, who eventually died in a hospital, was a business customer – worker was driving the forklift through the lot with a box of merchandise on the forks and advised police that he saw a flash of “blue” go in front of the forklift and he slammed on the brakes – the box on the forklift fell forward and landed on the customer) |
