FRONT END LOADER FATALITY
Worker killed in accident outside Pa. coke plant (a front-end
loader struck and killed a worker, 39, outside a coke plant shortly
before noon – he died from blunt-force trauma – the worker
apparently walked in front of the front-end loader and the operator
didn{apos}t see him – he was pronounced dead at a hospital about a
half-hour later)
Contract worker at Boeing killed (a contract worker at a plant
is dead after the car he was working on collapsed, trapping him
underneath – it happened in the plant’s parking lot – the accident
was not related to the construction work being done at the plant –
no word on what caused the car to collapse on the victim)

Fulton man dies after falling through roof (a worker, 41, died
after falling through a roof – he was removing snow and ice from the
onion processing plant when he fell – co-workers saw him lose his
balance while working on the roof and step backwards through a
plastic skylight – he fell three stories before landing on the
concrete floor)

Ministry probes injury at ArcelorMittal Dofasco (Ministry of
Labour is investigating an accident in which a worker was seriously
burned when the worker fell from a platform into a vat – he suffered
third-degree burns over 90 per cent of his body – he is in serious
condition at a burn unit – orders have been issued that the platform
not be used again until it has been cleared by the ministry)

dies in workplace injury (Worksafe officers were at a plant
investigating the death of an employee at the manufacturing plant –
incident occurred at approximately 3 p.m. – no details)

Workers Hand
Amputated in Meat Factory Accident (a meat factory worker had to
have his left hand amputated, after it became trapped in a meat
processing machine – he was feeding meat into the machine when his
arm got dragged into the mechanism, which contained two sets of
rotating blades – he was trapped in the machine for almost ten
minutes, his injuries being so severe that surgeons had to amputate
his left hand – an investigation into the incident revealed that a
supervisor had blocked a safety sensor with the blade of a butcher’s
knife, to speed up the processing operation – supervisor involved
has since lost his job at the company – another incident involving
another employee on the machine severed the tips of two fingers)
FALL from UTILITY POLE
Lineman falls 60 feet after pickup hits pole freak accident on Ala.
24 (a utility worker, 31, was critically injured when a pickup
struck the pole on which he was working – he was performing
subcontractor work stringing line when the vehicle slid off the icy
road – the pole broke when the Ford Sport Trac hit it, causing him
and the section he was on to fall to the ground – firefighters
estimated that the top of the pole, which landed on an incline, fell
about 60 feet – the pole was about 15 to 20 feet from the edge of
the road – a MedFlight helicopter transported him to a Hospital,
where he was in critical condition)

investigating what caused a fire at a refinery that killed one
worker – the fire erupted midday Tuesday from what appears to have
been a ruptured pipeline that carried natural gas – the remains of
the contractor were found late Wednesday, more than 24 hours after
the fire began)

fair condition – three other workers were treated at local hospitals
and two of them were released – plant was evacuated and all

(a 25-year-old man died when a rack of beer kegs collapsed on top of
him – he was pronounced dead of head injuries at the scene –
accident occurred at around 3:45 a.m. at the beverage distribution
center)
CARBON MONOXIDE and FORKLIFTS
Dozen of workers overcome by fumes at Elgin commerce center (38
people were taken to area hospitals and scores were left standing
out in the cold after carbon monoxide began leaking into a building
that houses numerous offices and warehouse operations – none of the
victims seemed to be in critical condition, but four were expected
to be held overnight – firefighters measured elevated levels of
carbon monoxide all over the building, with the highest amount in a
warehouse area – that area contained 309 parts per million of
monoxide – it is believed that two propane-gas-powered forklifts
were being operated in the unit and are the primary cause of the
readings throughout the building – after ventilating the structure,
investigators turned on the forklifts and immediately noted the
carbon monoxide readings were starting to elevate again)
CARBON MONOXIDE and FORKLIFTS
South Brunswick warehouse workers fall ill to carbon monoxide fumes
(several employees at a warehouse felt dizzy, fainted and vomited
when they were overcome by {amp}quot;extreme exposure{amp}quot; to carbon monoxide
because forklifts were being used in the building without
ventilation – they began to treat the employee, who had fainted, and
then realized several others were experiencing headaches, dizziness,
weakness, nausea and vomiting – officers evacuated the warehouse –
there was no ventilation as seven forklifts were being used,
creating carbon monoxide – seventeen employees were exposed to the
gas – sx were treated by EMS workers and paramedics at the scene,
and one was transported to a Hospital in stable condition – some
readings in the warehouse were as high as 750 parts per million)

Gas leak sickens Chrysler workers at Belvidere assembly plant
(about two dozen workers complained of nausea after a pipe carrying
gasoline across the plant began leaking – management cut short the
second shift – an unknown number of workers at the assembly plant
were moved to other parts of the building when the leak was
discovered – one of the gas lines they use for filling up the cars
broke and spilled a couple hundred gallons of fuel – gasoline
spilled out in an area roughly 100 feet by 100 feet, not very big
when you consider the plant is more than 4 million square feet)

Fire Departments Suppress Chlorine Leak at Lawton Valley Water
Treatment Plant (Fire Department quickly contained a dangerous
chlorine leak coming from a tank at a Water Treatment Plant –
workers at the plant called the fire department shortly after
midnight – workers smelled fumes and saw a vapor coming out from
underneath a doorway – a safety valve prevented any further leaks
and managed to completely stop the leak in one of the tanks – crews
stayed on scene for another five and a half hours to make sure the
building was ventilated – the building was empty at the time of the
leak – no nearby neighborhoods were evacuated – the leak occurred at
one of the tanks inside the chlorine storage building – there are
currently six chlorine tanks located at the plant, including one
empty tank)

Construction worker injured in head-first fall off ladder (an
accident at a construction site sent one man, 59, to hospital with
undetermined injuries – he was working at the site shortly after 4
p.m. when he fell roughly three metres off of a ladder – he landed
head-first on the concrete floor -co-workers provided immediate
attention prior to paramedics arriving)
Construction worker{apos}s leg caught in jackhammer (a bizarre
construction accident put paramedics{apos} training to the test – a man
got his leg trapped in a jackhammer on a job site – he was resting
the 45-pound jackhammer on his leg as he was working in a housing
development – when he shut it off, a portion of his thigh got
pinched in the locking mechanism on the tool – he is expected to be
just fine)
ELECTRICAL – CONCRETE BOOM INTO POWER
LINES
Construction Worker Suffers Electric Shock in Work Accident (a
construction worker suffered serious personal injury when a boom
attached to a concrete pump he was operating, touched overhead power
cables resulting in a massive electric shock – he was operating a
concrete pump, which was being used to pour concrete into a mould to
cast sewer chamber lids – as he maneuvered the pump, a twelve metre
long boom attached to the equipment came into contact with overhead
power cables, which were carrying 22,000 volts of electricity – this
resulted in him receiving a massive electric shock which caused
severe internal and external burns, which required skin grafts and
several operations including the removal of part of his skull)
WORKZONE FATALITY
Fellow firefighters shocked by death of former Edgewood chief
(he was working an environmental cleanup on I-71 when he was killed
– they had it clearly marked – even though it was a safe zone on the
highway, a box truck in the fast lane came up on a slow moving
pick-up and veered right into the work area hitting the worker – two
other workers were hurt – one of them told the deceased{apos}s family he
tried to push them out of harm{apos}s way)
Worker Falls Out of Truck and is Run Over in Mission Valley (a
worker weighing an estimated 500 pounds suffered serious leg
injuries when he fell out of the truck he was driving at a
construction-materials yard and was run over by the vehicle – it
took personnel from several engine companies to get the injured 45-
year-old man into an ambulance – medics took him to a trauma center
for treatment of compound fractures to both legs)
Construction Worker Hurt in Tunnel Near Grand Central (a
construction worker was hurt in a tunnel – the man was working on
the tunnel project – the incident took place at roughly 11:18 a.m.
below ground – the worker was driving a payloader used to remove
dirt from the tunnel when another vehicle backed into his – his
injuries did not appear serious)
Worker{apos}s head crushed by hydraulic lift (a 29-year-old worker
suffered severe injuries when his head was squeezed between a crash
barrier and a hoist – police assume that a technical error with the
hydraulic lift was the reason for the incident – the injured road

TREE TRIMMING FATALITY
Tree trimmer killed by falling branch near Belleville (a
38-year-old worker was killed outside a residence after a falling
tree branch struck him in the head – he was pronounced dead at 9:50
a.m. at the scene – incident occurred when a tree trimmer in a
bucket several feet in the air cut a large branch, about 6 inches in
diameter, that fell and struck him in the head – Sheriff{apos}s
Department investigator did not think he was wearing a helmet at the
time of the accident)
Worker injured by tram inside Gateway Arch (a worker, 55, at the
Arch was seriously injured while repairing the tram inside the
monument{apos}s south leg – he was doing electrical work about 11 a.m.
when he was briefly pinned between the moving tram and the internal
structure of the Arch – he was trapped for about 30 seconds before
co-workers freed him – he was in serious condition at a Hospital –
at the time of the accident, the south tram was being controlled by
a person at a work platform below – it{apos}s unclear whether mechanical
or human error caused the accident – he was in radio contact with
the person in control of the tram)
