2017 Video of the Week #20 (Lack of guarding on winder - fatality)
SERIOUS WARNING!  This video is EXTREMELY GRAPHIC and viewer discretion is advised.  This video shows a worker being pulled into a “winder” and whipped around many times, killing him. FYI:  OSHA just issued (6/12017) citations for a fatality similar to this video.  CLICK HERE for the citations

A MOC makes a great Halloween Costume…
I joke with my HAZMAT students that I am going to dress up as “IDLH” and scare everyone on Halloween and I joke with my PSM students that I am going to dress up as a MOC and scare everyone; it’s just meant to be a joke, but one day someone asked me… “What would you look like if you dressed up as a MOC”? His question got me to thinking… what should a MOC look...

EPA RMP citations @ chemical manufacturing plant (1,3-butadiene & $20K)
Respondent operates a chemical manufacturing plant that has one RMProgram level 3 covered chemical manufacturing process, which stores or otherwise uses 1,3-Butadiene in an amount exceeding its applicable threshold of 10,000 pounds. On November 9, 2015, the EPA conducted an onsite inspection of the RMProgram related records and equipment for the purpose of assessing the respondent’s compliance...

The State of MA revises their "Process Safety" standard "527 CMR 33.00: Hazardous Material Process or Processing"
The state of MA has modified it’s State HAZMAT code, formerly called 527 CMR 33.00: Hazardous Material Process or Processing. The process safety code, now found in the State Fire Cide, Chapter 60, Hazardous Materials, has added a “performance alternative”, as well as providing more specifics and updating definitions. Here is what’s new: NOTE: Still no word from the state Fire...

Do the actions taken under PSM duties/SOPs exempt the facility from the scope of HAZWOPER?
OSHA has two (2) significant LOIs, both aligning, and their HAZWOPER CPL that do a really good job of explaining how one crosses over from PSM SOP actions to actions that would fall under the definition of an “emergency response”, thus moving compliance from 1910.119 to 1910.120. I have presented both of these LOIs and the pertenant portions of the CPL below for discussion. The one point...

Does OSHA’s new GHS for Hazardous Communications require my fixed storage tanks to have Pictograms?
I have to admit that this transition from the old HAZCOM to this Globally Harmonized Hazcom Standard has been a bumpy ride over the past 4-5 years. I have had to correct postings a couple of times with some help from my friends at OSHA and NFPA. But there is one thing that I am 100% certain of, and this has been validated by several OSHA CSHOs and NFPA professionals, is that the NFPA 704...

2017 Video of the Week #19 (Difference from being TRAINED and having read a procedure!)
Every time I watch this video I think back to my days as a lifeguard and realizing that was my very first form of TRAINING.  I was a Boy Scout and was about 14-15 years old and already knew I wanted to be a rescuer and just fell in love with the intensity of my training.  It was then that I learned what “TRAINING” was all about.  For a week, 6 hours each day I had to practice...

When contractors enter a host's PRCSs (1910.146(c)(8)(iii)
What obligations does the host employer have when they hire a contractor to work within one of their Permit-Required Confined Spaces (PRCS)? The answer seems to be all over the map, depending on the industry where the space is located, the size of the company, the company’s written program, etc. This article is intended to help make sense of what should be happening when a contractor is hired...

OSHRC upholds citation for employee(s) reaching under interlocked doors/guards
This is a very good case on machine guarding (1910.212) application.  Employees routinely reached under a set of doors that were interlocked to clear a jam.  One day and employee reached too far and lost his finger tip.  OSHA cited 1910.212(a)(3)(ii) and the commission upheld it.  The reason why this case is so solid for us to use is that the company put forth the very popular machine...

Even the Chemical Safety Board uses UK's HSE Material
Today the U.S. Chemical Safety Board released their report into a 2015 refinery explosion and in their report they reference one of my favorite documents of all time from the U.K.’s Health and Safety Executive (HSE), The safe isolation of plant and equipment. Another EXCELLENT diagram we can incorporate into our energy isolation plans for Line Break/Equipment Opening and...
